Description

  • Die Erfindung betrifft Vorrichtungen zur Bekämpfung von Fettleibigkeit und insbesondere ein chirurgisch implantierbares verstellbares Band zur Umschlingung des Magens.
  • Vor mehreren Jahren beschrieb Kuzmak et al. in US-A-4 592 339 ein gurtartiges Band zur Umschlingung des Magens, um pathologische Fettleibigkeit zu bekämpfen. Das Band bestand im wesentlichen aus einem Gurt, der in einer Umkreisungsposition um den Magen geführt und dann festgeschnallt werden konnte, um die Stomaöffnung im Magen einzustellen. Weitere Verbesserungen des Magenbandes sind bisher das Vorhandensein eines verstellbaren Abschnitts des Bandes mit einem aufblasbaren Teil, das eine Feineinstellung der Stomaöffnung ermöglicht, nachdem die Größe des Stomas zu Beginn in einem Bandstraffungsvorgang eingestellt worden ist. Der Bandstraffungsvorgang setzt normalerweise die Anordnung einer Kalibriervorrichtung im Magen voraus, um die Stomagröße zu ermitteln. Wenn die Vorrichtung im Magen angeordnet ist, wird das Magenband enger geschnallt, bis sich die Stomaöffnung der gewünschten Größe nähert. Das Band wird in seiner Position festgehalten, normalerweise durch Nähte, und die Stomaöffnung wird schließlich eingestellt, indem man ein Fluid in ein aufblasbares Teil einströmen läßt, das mit dem Abschnitt der inneren Magenkontaktfläche des Bandes deckungsgleich ist. Die Einrichtung zum Einströmen des Fluids in das aufblasbare Teil weist normalerweise einen Füllanschluß auf, der unter der Haut angeordnet ist und der von außerhalb des Körpers durch transkutane Injektion zugänglich ist. Nach der Implantation kann das Magenband also in einem engen Bereich eingestellt werden, um das Stoma nach Bedarf zu vergrößern oder zu verkleinern.
  • Ferner offenbart US-A-5 074 868 eine Magenbandvorrichtung zur Erzeugung einer Stomaöffnung im Magen zur Beschrän kung der Nahrungsaufnahme in den unteren Verdauungsabschnitt des Magens. Das Band wird operativ angelegt, um den Magen zu umschlingen. Wenn es in seiner Position ist, wird es durch Nähte an der Außenseite des Magens festgehalten, wodurch verhindert wird, daß sich die umschlungene Stomaöffnung erweitert. Der Nähschritt wird durch ein Nahtloch erleichtert, das am Bandende der Vorrichtung angeordnet ist. Das Kopfende der Vorrichtung weist einen Schlitz auf, durch den das Bandende einzufügen ist. Der Schlitz am Kopfende ist nicht mehr erforderlich, wenn das Band durch das Anbringen von Nähten oder durch einen Splint im Kreis festgehalten wird, und wird abgeschnitten. Das bekannte Magenband soll entfernbar sein, ohne daß eine größere Operation notwendig ist. Dieses Dokument sagt nichts darüber aus, daß das Band für eine laparoskopische Einpflanzung geeignet ist.
  • EP-A-0 611 561 – was ein Dokument ist, das nur im Sinne des Art. 54(3) EPÜ zu berücksichtigen ist, – offenbart eine Magenbandvorrichtung zur Regulierung der Größe einer Stomaöffnung im Magen eines Patienten. Die Vorrichtung weist ein längliches Band mit einem Abschnitt auf, der den Magen umschlingt, um die Nahrungsaufnahme in den Verdauungsabschnitt des Magens zu begrenzen. Ferner soll diese Vorrichtung unter Verwendung laparoskopischer Verfahren implantiert und wieder entfernt werden können. Ein Verriegelungsteil, das von einer Stelle nahe dem distalen Ende des Umschlingungsbandabschnitts vorsteht, wird in eine Aussparung eingefügt, die in einem Aufnahmesegment an einer mittleren Stelle entlang des Umschlingungsbandabschnitts ausgebildet ist, um einen Kreis oder eine Schlaufe mit festem Durchmesser zu bilden. Ein beweglicher Bolzen arretiert das Verriegelungsteil in der Aussparung. Der Bolzen soll fernverschiebbar sein, wobei sich das Verriegelungsteil löst und angeblich eine laparoskopische Entfernung der Bandvorrichtung ermöglicht. Der Umfang des Magenumschlingungsbandabschnitts wird von einem dehnbaren Teil bedeckt, das mit der Oberfläche des Magens in Kontakt sein soll. Die Größe der Stomaöffnung kann eingestellt werden, indem man ein Fluid in den dehnbaren Teil einströmen oder aus ihm ausströmen läßt.
  • WO 86/04 498 betrifft eine chirurgische Vorrichtung zum Bandagieren des Magens eines Patienten, z. B. eines Patienten, der an Fettleibigkeit leidet, wobei die Vorrichtung ein Band aufweist, das so angeordnet ist, daß es zu einem Ring um den Magen gefügt wird, und das mit einem Hohlraum versehen ist, der mit einem Fluid gefüllt ist, wobei die Menge des Fluids in dem Hohlraum durch einen Einströmanschluß, der unter der Haut des Patienten angeordnet ist, geändert werden kann, so daß die Querschnittsfläche des Rings verändert werden kann. Die Verbindung erfolgt durch Überlappung und Zusammennähen der freien Bandenden.
  • US-A-5 207 694 betrifft ein Verfahren, einen Ausrüstungssatz und einen Applikator zur Durchführung eines chirurgischen Okklusion und wird anstelle herkömmlicher Metallklammern verwendet. Ein röhrenförmiger Applikator, der an beiden Enden offen ist, wird mit dem ersten Ende, das an das frei präparierte Gewebe angrenzt, eingeführt. Mindestens ein Bündelgurtband wird in den Applikator eingeführt. Ein Bandabschnitt, der aus dem ersten Ende des Applikators vorsteht, wird erfaßt, um das Band an seiner richtigen Stelle um das Gewebe herum zu positionieren. Danach wird aus dem Band eine verriegelte Umschlingungsschleife um das Gewebe. Die Umschlingungstechnik soll sowohl für Laparotomie als auch für Laparoskopie verwendbar sein.
  • Einer der Nachteile der bekannten Magenbänder ist die Schwierigkeit, das Band um den Magen herum zu spannen, um dem gewünschte Stoma nahezukommen. Dies ist besonders schwierig, wenn das Band laparoskopisch eingesetzt werden soll. Wenn das Band laparoskopisch eingesetzt wird, sind spezielle Instrumente erforderlich, um das Band vor der Feineinstellung des Stomas zu spannen. Solche Instrumente, die erforderlich sind, das Band zu erfassen und in eine Kreisposition um den Magen zu ziehen und dann zusammenzuziehen, sind durch eine laparoskopische Kanüle schwer manipulierbar. Es wäre daher wünschenswert, ein Band mit einem aufblasbaren Element bereitzustellen, das auf einfache Weise in einer Umschlingungsposition um den Magen festgehalten werden kann und bei dem die Einstellung der Stomaöffnung nur durch das Einströmen eines Fluids in das aufblasbare Element reguliert werden kann.
  • Eine Aufgabe der Erfindung ist es, ein Magenband bereitzustellen, das laparoskopisch eingesetzt und nichtverstellbar in einer Umschlingungsposition um den Magen so festgehalten werden kann, daß ein Ring mit einem Innendurchmesser entsteht.
  • Es ist noch eine weitere Aufgabe der Erfindung, ein Magenband bereitzustellen, bei dem das Band auf einfache Weise mit herkömmlichen laparoskopischen Instrumenten in einer Umschlingungsposition um den Magen befestigt werden kann.
  • Es ist noch eine weitere Aufgabe der Erfindung, ein Magenband bereitzustellen, das für eine laparoskopische Einpflanzung um den Magen herum geeignet ist und das danach nichtverstellbar zu einem Kreis um den Magen herum geschlossen wird, um den umschlungenen Abschnitt des Magens zusammenzudrücken, um sein Stoma zu verengen, wobei die Verengung nur mit einem aufblasbaren Teils weiter verstellt werden kann.
  • Es ist noch eine weitere Aufgabe der Erfindung, ein Magenband mit einem aufblasbaren Element bereitzustellen, das sich auf einer Innenfläche befindet, wobei die Innenfläche eine glatte, kontinuierliche Oberfläche bietet, wenn das Band in einer Umschlingungsposition festgehalten wird und das aufblasbare Teil aufgeblasen ist.
  • Diese Aufgaben werden mit den Merkmalen der Ansprüche gelöst, die nunmehr deutlich werden, wenn wir uns den bevorzugten Ausführungsformen zuwenden, die in der folgenden Zeichnung dargestellt sind, in der:
  • 1 eine perspektivische Ansicht des erfindungsgemäßen Magenbandes; und
  • 2 das erfindungsgemäße Magenband ist, das in einer Umschlingungsposition festgehalten wird und teilweise aufgeblasen ist.
  • Wie bereits erwähnt, haben bekannte Magenbänder zwei Einstellschritte oder -stufen: Im ersten Schritt wird das Magenband in einer Position grob befestigt, so daß der Magen umschlungen ist, wobei sich die Stomaöffnung der gewünschten Größe nähert. Diese erste Einstellung entspricht dem Festschnallen eines Gürtels um die Taille, bis die Schnallenzunge durch eines von mehreren Löchern paßt, wobei eine angenehme Spannung entsteht. Der nächste Schritt bei der Einstellung erfordert das Einströmen eines Fluids in das aufblasbare Element, um die Stomaöffnung auf die gewünschten Größe zu bringen.
  • Bekannte Magenbänder, die keinen großen Bereich für eine aufblasbare Einstellung aufweisen, erfordern eine anfängliche "Grobeinstellung". Die "Grobeinstellung", die den ersten Schritt im Magenstomaverengungsprozeß umfaßt, erfordert das Spannen des bekannten Bandes und das Halten der Spannung im Band, während Nähte gesetzt werden, um das Band in der Umschlingungsposition zu arretieren. Dies kann nur mit Mühe laparoskopisch durchgeführt werden, und die erfindungsgemäße Ausführung überwindet diese Schwierigkeiten durch das Spannen des Bandes.
  • Wenn man nunmehr 1 betrachtet, sieht man ein Magenband, das insgesamt mit dem Bezugszeichen 10 bezeichnet ist und das einen Körperabschnitt 11 mit einer dem Magen zugewandten Innenfläche 15 aufweist. Der Körperabschnitt 11 hat ein Kopfende 12 und ein Schwanzende 13 mit einem oder mehreren Nahtlöchern 13(a). Ein Füllschlauch 14, der im allgemeinen ein Schlauch mit einem einzigen koextensiven Hohlraum bzw. Lumen 14(a) ist, ist in Fluidkommunikation mit einem erweiterbaren bzw. aufblasbaren Element 16 auf der Innenfläche 15 des Bandkörpers 11. Es ist ein wichtiges Merkmal der Erfindung, daß der aufblasbare Abschnitt 16 im wesentlichen deckungsgleich bzw. koextensiv mit der Innenfläche 15 des Körperabschnitts 11 ist, um eine im wesentlichen durchgehende Oberfläche zu bieten, die frei von abrupten Konturen ist, wenn das Band den Magen umschlingt und der aufblasbare Abschnitt 16 aufgeblasen ist. Der mittige Hohlraum 14(a) des Füllschlauchs 14 tritt an der Hohlraumöffnung 17 in das aufblasbare Element 16 ein. Der Kopf 12 des Körperabschnitts 11 hat eine Schnalle 19 mit einer Zuglasche 18 mit einem integrierten Nahtloch 18(a). Beide Abschnitte, die eine Naht aufnehmen, d. h. die Zuglasche 18 und das Endstück 13, sind vorzugsweise medizinisch verwendbares Silikon, das mit Dacron verstärkt ist.
  • In der Praxis ist das Magenband in einer Umschlingungsposition um den Magen angeordnet, wie in 2 gezeigt. (In
  • 2 ist der Magen zur besseren Übersicht weggelassen.) Dies erfolgt, indem der Füllschlauch 14 durch eine in den Unterleib des Patienten gelegte laparoskopische Kanüle (nicht dargestellt) geschoben wird. Die laparoskopische Einpflanzung besteht aus einer stumpfen Sektion um die größere Krümmung des Magens herum. Das Ende des Füllschlauchs 14 wird um den Magen geführt, und der Schwanz 13 wird an der Schnalle 19 befestigt, so daß die Schnalle und das Endstück nichtverstellbar und irreversibel aneinander befestigt sind. In diesem Sinne ist das Band eine Vorrichtung, die "für alle Größen paßt". Das heißt, bei einem bestimmten Band gibt es nur eine einzige Position, in der das Endstück und die Schnalle aneinander befestigt werden können. Die weitere Einstellung des Stoma – die durch das Band erzeugte enge Öffnung im Magen – erfolgt nur durch Aufblasen oder Entleeren des aufblasbaren Elements 16, nachdem das Band in dieser einzigen Position arretiert worden ist.
  • Bekannte Magenbänder verwenden einen verstellbaren Ballonabschnitt, der bei Bedarf für eine kleine postoperative Einstellung des Stoma verwendet wird. Das laparoskopische hydraulische Magenband, das hier beschrieben ist, hat einen größeren Ballon, der einen größeren Flächenbereich der Innenfläche des Magenbandes abdeckt als nach dem Stand der Technik. Das aufblasbare Element oder der Ballon 16 ist vorzugsweise koextensiv mit der dem Magen zugewandten Innenfläche 15 des Bandes zwischen dem Kopfende 12 und dem Schwanzende 13. Dieser größere Ballon 16 wird verwendet, um das Stoma während einer Operation einzustellen. Das Innere des verstellbaren Ballons 16 ist durch einen mittigen Hohlraum 14(a) des Füllschlauchs 14 in Fluidkommunikation mit einem Fluidreservoir (nicht dargestellt), wie bei bekannten verstellbaren Magenbändern. Das aufblasbare Element 16 wird über das Fluidreservoir (nicht dargestellt) allmählich mit Salzlösung gefüllt, so daß das aufblasbare Element 16, das, wie oben ausgeführt, mit der Innenfläche 15 des Bandes 11 zwischen dem Kopfende 12 und dem Bandende 13 koextensiv ist, auf die Magenwand drückt, die unter dem Band liegt, und diese zusammendrückt. Dies führt zu einer Verkleinerung der Öffnung (Stoma) im Magen direkt unter dem Umschlingungsband. Eine durchgehende Oberfläche, die das vergrößerte aufblasbare Element dem darunter liegenden Gewebe bietet, verhindert ein Zusammenquetschen des Gewebes, das zur Bildung von Falten, Erhebungen oder ähnlichen Unebenheiten führt, die auf der Innenfläche von bekannten Bändern auftreten, und verhindert dadurch Nekrose.
  • Ein elektronischer Druckwandler, der als elektronischer Gastrostenometer-Sensor bekannt ist, der mit einer Kalibriersonde gekoppelt ist, die unter dem Oberbegriff "Kalibrierschlauch" bekannt ist, wird verwendet, um die Größe des Stomas zu messen. Der Kalibrierschlauch wird durch den Mund und die Speiseröhre des Patienten in den Magen eingeführt. Das Band wird aufgeblasen, wobei es sich um das Ende des Kalibrierschlauchs herum spannt. Der elektronische Gastrostenometer-Sensor zeigt den Stomadurchmesser an, der entsteht, während das Band gespannt wird.
  • Die Erfindung erlaubt die laparoskopische Implantation einer therapeutischen chirurgischen Vorrichtung ohne das Risiko, das mit größeren Unterleibsschnitten bei fettleibigen Patienten verbunden ist. Eine schnellere Heilung, weniger Komplikationen, kürzerer Krankenhausaufenthalt und drastisch verringerter Schmerz gehören zu den Vorteilen der laparoskopischen Einpflanzung. Das erfindungsgemäße Magenband erleichtert die laparoskopische Einpflanzung und erlaubt außerdem eine breitere Anwendung des Magenbandsystems, um einen Magenverengungsvorgang zur Behandlung pathologischer Fettleibigkeit hervorzurufen. Das Band ermöglicht es, daß mehr Chirurgen, die der laparoskopischen Chirurgie fähig sind, diese Operation vornehmen, ohne daß hochspezialisierte und schwer zu verwendende Spanninstrumente verwendet werden müssen. Da nur eine einzige, nichtverstellbare Verriegelungsposition für das Band um den Magen besteht, eignet sich das Verfahren gut für eine rigorosere Standardisierung. Außerdem werden weitere Patienten erwägen, wegen der Vorteile der laparoskopischen Chirurgie gegenüber traditionellen Unterleibslaparotomien diese Operation durchführen zu lassen. Diese Vorteile, gekoppelt mit den Vorteilen der bekannten Magenbänder, die eine postoperative Feineinstellung des Stoma ermöglichen, ermöglichen die Durchführung einer leichteren Operation.
  • Im allgemeinen gilt: Je glatter und durchgängiger die Innenfläche 15 des Bandes 10 ist, um so weniger wahrscheinlich ist eine Nekrose. Die eine Schließ- oder Feststellposition des vorliegenden Magenbandes stellt sicher, daß das aufblasbare Element 16 im wesentlichen mit der Innenfläche 15 des Bandes 10 koextensiv ist und mit der Schnalle 19 zusammenhängt. Das Band mit dem festen Durchmesser ermöglicht es, daß die Innenfläche 15 dem umschlungenen Gewebe eine durchgängige Fläche bietet, während das Band aufgeblasen wird, wobei Nekrose verhindert wird. Bekannte Magenbänder haben keine Vorkehrung, die sicherstellt, daß das aufblasbare Element an den Schnallenabschnitt angrenzt und mit diesem zusammenhängt, wenn das Band in einer Umschlingungsposition geschlossen ist. Dadurch kann ein Spalt oder Zwischenraum zwischen dem aufblasbaren Element und der Schnalle entlang der Innenfläche entstehen, wenn das aufblasbare Element aufgeblasen wird. Das Gewebe kann in einem solchen Spalt zusammengequetscht werden, was zu einer Nekrose führt. Die Verringerung und/oder Beseitigung solcher Ungleichmäßigkeiten auf der Innenfläche des (aufgeblasenen) Bandes, wie oben beschrieben, verringert die Möglichkeit einer Nekrose.
  • Während bestimmte Ausführungsformen der Erfindung dargestellt und beschrieben worden sind, ist den Fachleuten klar, dass verschiedene andere Änderungen und Modifikationen möglich sind, ohne den Schutzbereich der Erfindung zu verlassen. Daher sollen alle diese Änderungen und Modifikationen, die im Schutzbereich der Erfindung liegen, mit den beigefügten Ansprüchen abgedeckt sein.

Claims (2)

  1. Magenband (10) zur Behandlung von pathologischer Fettleibigkeit, umfassend einen Körperabschnitt (11) mit einem Kopfende (12) und einem Schwanzende (13) und dazwischen einer dem Magen zugewandten Innenfläche (15), wobei das Schwanzende (13) ein längliches, röhrenförmiges Element (14) mit der Fähigkeit zu einer fluiddichten Verbindung mit einem Injektionsreservoir aufweist, das Kopfende (12) auf ihm eine Schnalle (19) zum Aufnehmen des Schwanzendes (13) und zum nichtverstellbaren Schließen des Magenbandes (10) zu einem Kreis mit einem Innenumfang aufweist, ein aufblasbares Element (16) auf der Innenfläche (15) mit dem röhrenförmigen Element (14) in Fluidkommunikation ist, das aufblasbare Element (16) mit der Innenfläche (15) des Körperabschnitts (11) koextensiv ist, wenn das Magenband (10) nichtverstellbar zu dem Kreis geschlossen ist, das Magenband (10) zur laparoskopischen Anordnung um den Magen eines Patienten geeignet ist.
  2. Magenband (10) nach Anspruch 1, wobei das aufblasbare Element (16) sich nicht knittert oder faltet, wodurch eine im Wesentlichen glatte Kontur entlang des Innenumfangs bereitgestellt wird.
