Description
- Hintergrund der Erfindung
- Die vorliegende Erfindung betrifft einen endoluminalen Stent zur Stützung eines ausgewählten Bereiches eines Körperlumens und eine Vorrichtung zur Ausbildung und Einführung des Stents in situ.
- Atherosklerose wird üblicherweise mittels Angioplastik durch die Verwendung eines Ballonkatheters behandelt. Ballonangioplastik umfaßt perkutanes Einführen eines kleinen Katheters mit Ballonspitze in eine Arterie oder ein Gefäß und Hindurchführen bis in den Obstruktionsbereich. Der Ballon wird dann aufgeblasen, um die Obstruktionsfläche zu dilatieren. Restenoses oder Wiederverschluß des Blutgefäßes im Anschluß an Angioplastik ist jedoch ein häufiges Vorkommnis. Faktoren, die, zumindest teilweise, zu Restenoses beitragen, schließen Proliferation von Glattmuskelzellen im stenosierten Bereich und Rückstoßen der Arterien- oder Gefäßwand ein. Wenn eine luminale Verengung auftritt, können weitere koronare Schwierigkeiten erlitten werden, einschließlich Schlaganfälle, Herzrhythmusstörungen, Infarkte und sogar Tod.
- Ein Ansatz zur Verhinderung von Restenoses im Anschluß an Angioplastik ist gewesen, einen Stützstent in den behandelten Bereich einzuführen. Dies kann unter Verwendung eines herkömmlichen Ballonkatheters durchgeführt werden. Insbesondere wird der Stent in einer zusammengezogenen oder kollabierten Position über den Ballonteil des Katheters gelegt. Der Katheter wird dann in das Blutgefäß benachbart zu einer Stenoses eingeführt und der Ballon wird expandiert, so daß der Stent mit der umgebenden Gefäßwand in Eingriff kommt und diese stützt.
- Typischerweise sind endovaskuläre Stents aus Metallen hergestellt, um die erforderliche Festigkeit zur Aufrechterhaltung der Dilatation des stenosierten Blutgefäßbereiches bereitzustellen. U.S.-Patent Nr. 5,197,978 (Hess et al.) und U.S.-Patent Nr. 5,354,308 (Simon et al.) offenbaren zum Beispiel thermisch verformbare Stents, die aus Nickel und Titan hergestellt sind. U.S.-Patent Nr. 5,366,504 (Andersen et al.) offenbart einen Stent, der aus locker zusammengehaltenen gestrickten Schlaufen, die Metalldraht enthalten, hergestellt ist.
- Nicht-metallische Stents sind ebenfalls für endovaskuläre Stützung verwendet worden. Diese Einrichtungen sind im allgemeinen zylindrische Strukturen, die aus einer Folie oder Hülse aus federndem, elastischen Material hergestellt sind, das im Anschluß an die Einführung des Stents in einen ausgewählten Bereich eines Gefäßes ausgehärtet werden kann. U.S.-Patent Nr. 5,100,429 (Sinofsky) offenbart zum Beispiel einen endovaskulären Stent, der einen röhrenförmigen Körper aufweist, der als eine gerollte Folie aus einem biologisch kompatiblen Material ausgebildet ist, mit einem vernetzbaren Klebermaterial zwischen überlappenden Bereichen der aufgerollten Folie. U.S.-Patent Nr. 5,344,201 (Cowan et al.) offenbart einen endovaskulären Stent, der aus einer expandierbaren Hülse hergestellt ist, die ein vernetzbares Material enthält, die durch Einwirkung von Strahlung ausgehärtet werden kann, und einen Film aus biologisch kompatiblem Material, das die Hülse einkapselt. U.S.-Patent Nr. 5,344,444 (Glastra et al.) und
EP 617 930 EP 649 637 - Viele Stent-Designs aus dem Stand der Technik leiden an dem Nachteil, daß sie einen relativ großen Durchmesser in ihrem nicht-entfalteten Zustand haben, was es schwierig macht, sie einzuführen, und das Risiko mit sich bringt, dem Gefäß unangemessenes Trauma zuzufügen. Zusätzlich können Stents nach dem Stand der Technik etwas unflexibel sein, so daß sie dabei versagen, sich eng an die umgebende Gefäßwand anzulegen, wenn sie an Ort und Stelle sind. Überdies werden diese Stents oft ein permanenter Teil des Körpers und erlauben kein signifikantes Einwachsen von Gewebe oder Einwirken von zirkulierendem Blut auf die Lumenwand, wenn sie an Ort und Stelle sind.
- Es ist daher eine Aufgabe der vorliegenden Erfindung, einen transformierbaren Stent zur Stützung und Verstärkung eines Körperlumens bereitzustellen, der in seinem vorgeformten Zustand sehr flexibel ist und mit der umgebenden Lumenwand, wenn er an Ort und Stelle expandiert ist, dicht in Eingriff kommt. Es ist auch eine Aufgabe der vorliegenden Erfindung, einen Stent bereitzustellen, der ein sehr kleinen Querschnitt hat, so daß er leicht einzuführen ist und keinen großen Bereich des Lumendurchflußkanals blockiert. Es ist eine weitere Aufgabe der vorliegenden Erfindung, einen Stent bereitzustellen, der, im Anschluß an seine Einführung in einen ausgewählten Bereich eines Körperlumens, extrem stark ist, um die umgebende Lumenwand effektiv zu stützen und zu verstärken, aber trotzdem signifikante Einwirkung von zirkulierendem Blut auf die abgestützte Lumenwand ermöglicht. Es ist noch eine weitere Aufgabe der Erfindung, einen Stent auf Polymerbasis bereitzustellen, der biologisch resorbierbar ist und mit einem Medikament mit gesteuerter Freisetzung imprägniert werden kann.
- Zusammenfassung der Erfindung
- Gemäß der vorliegenden Erfindung werden diese und andere Aufgaben und Vorteile erreicht durch einen Stent aus aushärtbarem Faserverbundmaterial nach Anspruch 1, der, bei Ausbildung in situ innerhalb eines ausgewählten Bereiches eines Körperlumens, dazu dient, das Lumen ohne signifikanten Obstruktion gegenüber dem Blutdurchfluß abzustützen. Der Stent ist aus einem biologischen kompaktiblen Fasermaterial hergestellt, das mit einem aushärtbaren Material beschichtet, imprägniert, gefüllt oder in anderer Weise behandelt ist, so daß das Faserverbundmaterial in geeigneter Weise ausgeformt werden kann, um einen Abschnitt eines Körperlumens abzustützen, und anschließend ausgehärtet, um die Gestalt beizubehalten. In einer Ausführungsform der Erfindung sind das Faserverbundmaterial und . das aushärtbare Material biologisch resorbierbar, so daß der Stent kein permanenter Teil des Körpers wird.
- Der Stent hat eine offene, nicht-kontinuierliche Struktur, die signifikante Einwirkung von zirkulierendem Blut auf die Lumenwand und Endothelüberwachsung ermöglicht. In einer . Ausführungsform der Erfindung besitzt der Stent eine schraubenförmige Konformation. In einer weiteren Ausführungsform umfaßt der Stent eine Reihe von Fasersträngen, die so verflochten sind, daß sie ein poröses Netz bilden. Die Faserstränge sind mit einem aushärtbarem Material behandelt, zum Beispiel durch Sättigen der Stränge mit einem photoaktivierbaren Vernetzungsmittel. Alternativ können die Stränge Hohlfasern sein, die mit einem aushärtbaren Material gefüllt sind. Das Faserverbundmaterial kann anschließend mit einem Film aus einem biologisch kompatiblen Material beschichtet werden, das das aushärtbares Material enthaltende Faserverbundmaterial von der Lumenwand und dem zirkulierenden Blut isoliert. Alternativ kann das aushärtbare Material mikroverkapselt und anschließend mit dem Fasermaterial in Kontakt gebracht werden, wodurch ein biologisch kompatibles aushärtbares Faserverbundmaterial gebildet wird.
- Um den Stent aus Faserverbundmaterial in einen ausgewählten Bereich eines Körperlumens einzuführen, wird der Stent auf dem Kopf eines geleerten Ballonkatheters befestigt, während er in einem flexiblen, nicht-ausgehärteten Zustand ist, an Ort und Stelle geführt und dann durch Aufblasen des Ballons expandiert, so daß der Stent sich fest gegen die umgebende Lumenwand drückt. Der Stent wird anschließend ausgehärtet, so daß er eine starre Stützstruktur bildet, die eng an der Lumenwand anliegt. In einer bevorzugten Ausführungsform der Erfindung wird die Aushärtung durch Einwirkung von UV-Licht, das durch den Ballon über eine Faseroptikspitze zugeführt wird, auf den Stent erreicht. Im Anschluß an das Aushärten des Stents wird der Ballon entleert und entfernt, was einen starren Stent zurückläßt.
- Der Stent aus aushärtbarem Faserverbundmaterial der Erfindung kann verwendet werden, um jedes Körperlumen aufzuweiten und abzustützen, einschließlich, aber nicht hierauf beschränkt, Gefäß-, urologische, Gallen-, Oesophagus-, reproduktive, endobronchiale, gastrointestinale und Prostata-Lumina. Der Stent kann auch verwendet werden, um Zellen einer Lumenwand Therapeutika zuzuführen. In einer Ausführungsform der Erfindung ist das Therapeutikum in den Stent eingearbeitet, so daß es bei Expansion des Stents in die umgebende Lumenwand hinein gedrückt wird. In einer weiteren Ausführungsform der Erfindung ist das Therapeutikum durch Licht freisetzbar mit der Oberfläche des Stents verknüpft, so daß das Mittel, bei Einwirkung von Licht, aus der Stentoberfläche in die umgebende Lumenwand freigesetzt wird. Der Stent der Erfindung kann auch verwendet werden, um akuten Gefäßverschluß, Dissektion einer Arterie oder sub-optimale Angioplastik zu behandeln.
- Die vorliegende Erfindung stellt den Vorteil eines endovaskulären Stents zur Verfügung, der einen relativ kleinen Querschnitt hat, so daß er leicht durch ein Körperlumen geführt werden kann und den Blut- oder Luftdurchfluß, wenn er an Ort und Stelle ist, nicht substantiell versperrt. Der Stent ist bei Einführung auch sehr flexibel, so daß er sich dicht an die umgebende Wand anlegt, wenn er an Ort und Stelle expandiert wird. Einmal an Ort und Stelle ausgebildet, ist der Stent starr, so daß er die Lumenwand abstützt und verstärkt.
- Kurze Beschreibung der Zeichnungen

- Gemäß einer Ausführungsform der vorliegenden Erfindung, die in
1(a) und1(b) dargestellt ist, wird ein Stent zur Stützung eines ausgewählten Bereiches eines Körperlumens7 bereitgestellt. Der Stent besteht aus einem Fasermaterial, das mit aushärtbarem Material behandelt und anschließend zu einer Helix ausgeformt ist, die mit der umgebenden Lumenwand7 in Eingriff kommt. Der Stent wird in situ gebildet, zum Beispiel innerhalb eines stenosierten Bereiches eines Blutgefäßes, wodurch das Risiko der Verletzung der Gefäßwand verringert wird, das aus der Einführung eines vorgeformten Stents resultiert. - Der Stent
4 wird unter Verwendung eines herrkömmlichen Ballonkatheters, wie dargestellt in2 , ausgebildet und eingeführt. Solche Katheter sind im Stand der Technik gut bekannt und schließen im allgemeinen einen länglichen flexiblen Schlauch12 mit einem expandierbaren Ballon5 , der am distalen Ende angebracht ist, ein. Der Katheter schließt auch wenigstens eine optische Faser8 zur Zuführung von Strahlung in den Ballon5 ein. An ihrem proximalen Ende ist die optische Faser mit einer Strahlungsquelle9 , wie etwa einem Laser, verbunden. Geeignete Laser zur Zuführung von Strahlung sind zum Beispiel im US-A-5161834 beschrieben. Die optische Faser8 erstreckt sich durch den Katheterkörper12 hindurch in den Ballon5 hinein, der am distalen Ende angebracht ist. Die Spitze der Faser ist vorzugsweise so konstruiert, daß Licht durch den Ballon nach außen diffundiert, zum Beispiel durch Abschrägen des Endes oder durch Verwendung eines diffusiven strahlungsundurchlässigen Materials, wie es aus dem Stand der Technik gut bekannt ist. - Um Stent
4 auszubilden und einzuführen, kann ein Faserstrang mit einem aushärtbarem Material gesättigt und an anderer Weise behandelt werden, wodurch ein aushärtbares Faserverbundmaterial gebildet wird. Das Faserverbundmaterial wird dann um einen zylindrischen Dorn oder eine ähnliche Vorrichtung gewickelt, um eine Helix zu bilden. Das schraubenförmige Faserverbundmaterial wird anschließend über dem Kopf eines entleerten Ballonkatheters5 angebracht, wie dargestellt in1(a) , bevor es ausgehärtet wird. Der Katheter wird anschließend unter Verwendung von Führungsdraht6 durch ein Körperlumen in einen ausgewählten Bereich7 , wie etwa ein verstopftes Blutgefäß, vorgeschoben. Wenn er erst einmal benachbart zu diesen Bereich ist, wird der Ballon5 expandiert, so daß der flexible Stent fest gegen die umgebende Lumenwand7 gedrückt wird, wie dargestellt in2 . Die Strahlung wird dann über die optische(n) Faser(n)8 oder andere energieleitende Mittel zum Ballon zugeführt, was bewirkt, daß das aushärtbare Material im Stent vernetzt oder polymerisiert und eine starre Stützstruktur bildet, die eng an der umgebenden Lumenwand7 anliegt, wie dargestellt in1(b) . Der Ballon wird anschließend entleert und der Katheter entfernt. - In einer weiteren Ausführungsform der Erfindung besteht der Stent aus einer Hohlfaser, wie etwa einem Polyethylenterephthalat(PET)-Schlauch, der ein aushärtbares Material einkapselt. Der gefüllte Faserschlauch kann auf einem Ballonkatheter angebracht und zu einem ausgewählten Bereich eines Körperlumens zugeführt werden, wie in der ersten Ausführungsform der Erfindung beschrieben. Wenn das aushärtbare Material jedoch von dem Faserverbundmaterial eingekapselt ist, wird das Toxizitätsrisiko vom aushärtbaren Material in großem Umfang verringert.
- In noch einer weiteren Ausführungsform der Erfindung, die dargestellt ist in
3(a) ,(b) und(c) , besteht der Stent13 aus einer Anzahl von Fasersträngen11 , die verflochten sind, um ein poröses Netz zu bilden. In einem Beispiel sind ungefähr 16 Stränge aus einem Faserverbundmaterial, wie etwa Polyethylenterephthalat (Dacron®), Seide oder Kohlenstoff, vorgeflochten und anschließend mit einem UV-aushärtbaren Material gesättigt. Um Stent13 . zu einem ausgewählten Bereich eines Körperlumens zuzuführen, wird das flexible Flechtwerk in Längsrichtung über einen entleerten Ballonkatheter5 gezogen, wie dargestellt in3(a) . Der Stent wird anschließend durch das Lumen zur Zielstelle geführt und über Ballon5 expandiert, wie zuvor in der ersten Ausführungsform der Erfindung beschrieben. Im Anschluß an die Expansion des Ballons wird Strahlung über die optische Faser8 in sein Inneres zugeführt, was bewirkt, daß das Flechtwerk ausgehärtet wird, während es die Form der umgebenden Lumenwand7 annimmt, wie dargestellt in3(b) . Die Einwirkung von Licht bewirkt, daß das aushärtbare Material in Kontakt mit den Fasern des Flechtwerkes das Flechtwerk versteift, was zu einem steifen, offenmaschigen Stent führt, wie dargestellt in3(c) . - Fasermaterialien, die zur Verwendung im Stent der vorliegenden Erfindung geeignet sind, schließen eine Vielzahl von Multifilamentgeweben und Hohlfasern ein. Der Begriff "Fasermaterial", wie hierin verwendet, umfaßt alle solchen Gewebe und Fasern, einschließlich zum Beispiel Baumwolle, Polyester, Polyethylenterephthalat (Dacron®), Kohlenstoff und Metall. Die Fasern, die das Verbundmaterial bilden, haben einen sehr kleinen Durchmesser und können sich frei übereinander bewegen, wodurch in einem nichtausgehärteten Zustand Flexibilität bereitgestellt wird. Nach dem Binden der Fasern mit einem aushärtbaren Material durch zum Beispiel Einwirkung von UV-Strahlung bilden die Fasern jedoch eine sehr feste, steife Stützstruktur.
- Das Faserverbundmaterial wird zu einer steifen Stützstruktur ausgebildet, indem die Fasern mit einem Material gebunden werden, das durch zum Beispiel Einwirkung von Strahlung ausgehärtet werden kann, wie etwa Urethane, Polyurethan-Oligomermischungen, Acrylat-Monomere, aliphatische Urethan-Acrylat-Oligomere, Acrylamide, UV-aushärtbare Epoxyharze und andere UV-aushärtbare Monomere. Alternativ kann das aushärtbare Material ein Material sein, das chemisch ausgehärtet werden kann, wie etwa Verbindungen auf Silikonbasis, die Raumtemperatur-Vulkanisation (RTV) durchlaufen. Materialien, die über die Zeit aushärten, können ebenfalls verwendet werden, wie etwa Epoxyharze. Der Begriff "aushärtbares Material", wie hierin verwendet, ist so gedacht, daß er jedes Material einschließt, das vernetzt oder polymerisiert werden kann. In einer Ausführungsform wird eine UV-aushärtbare Polyurethan-Oligomermischung wie etwa Dymax (erhältlich von Dymax Corp., Torrington, CT) verwendet und unter Verwendung einer Dymax®-UV-Härtungsmaschine Modell PC-3 vernetzt. In einer weiteren Ausführungsform wird eine UVpolymerisierbare Mischung aus Acrylat-Monumeren, aliphatischen Urethan-Acrylat-Oligomeren und Acrylamiden, wie etwa Loctite® 3301 (erhältlich von Loctite Corp., Newington, CT), verwendet.
- Wenn Materialien verwendet werden, die durch Einwirkung von Strahlung ausgehärtet werden, wird die An und Dauer der Strahlung entsprechend dem besonderen verwendeten Material variieren. Obgleich UV-Strahlung bevorzugt ist, kann auch sichtbare, Infrarot- und Wärmestrahlung verwendet werden, um die Vernetzung der Fasern, die den Stent bilden, zu fördern. In einer bevorzugten Ausführungsform der Erfindung wird UV-Strahlung mit einer Wellenlänge im Bereich von 240–400 nm verwendet, die von zum Beispiel einer nichtkohärenten UV-Lichtquelle oder einer Excimerquelle, wie etwa einem KrF-Excimerlaser, der bei 248 Nanometer arbeitet, bereitgestellt wird. Alternativ kann ein Neodym-dotierter YAG-Festkörperlaser mit einer vierfachen Frequenz oder dergleichen, der bei 266 nm arbeitet, verwendet werden oder er kann ein Argonionenlaser, der bei 257 oder 275 oder 360 nm arbeitet, verwendet werden.
- Um das Risiko der Toxizität von einem aushärtbaren Material vor dem Aushärten zu verringern, kann der Stent weiter eine Schicht aus einem biologisch kompatiblen Material einschließen, das das aushärtbare Faserverbundmaterial einkapselt und als eine Sperrschicht zwischen dem Stent und jedlichem zirkulierenden Blut und zwischen dem Stent und der Lumenwand dient. Geeignete biologisch kompatible Materialien, die für diesen Zweck verwendet werden können, schließen, ohne Beschränkung hierauf, Silikone, Wachse, Polyacrylamide, Polyethylene, Polystyrole, Polypropylene, Polyolefine, Polyurethane und andere thermoplastische elastische Polymere ein. In einer Ausführungsform der Erfindung wird eine Multifilamentfaser (z. B. Baumwolle oder Kohlenstoff) mit UV-aushärtbarem Material gesättigt und anschließend in einem Polyethylenterephthalat(PET)-Schrumpfschlauch eingekapselt.
- Der Stent kann auch ein Therapeutikum einschließen, das in die Fasermatrix des Stents eingearbeitet wird oder in die biologisch kompatible Beschichtung, die den Stent einkapselt, so daß es nach Expansion und Aushärten des Stents innerhalb eines Körperlumens auf die Zellen der umgebenden Lumenwand freigesetzt werden kann. Das Therapeutikum kann zum Beispiel durch Druck von der Expansion des Stents in die Lumenwand imprägniert werden. Alternativ kann das Therapeutikum durch Lichteinwirkung freisetzbar mit der Oberfläche des Stents verknüpft sein, so daß, bei Kontakt mit der umgebenden Lumenwand, das Mittel auf die Zellen der Wand durch Einwirkung von Strahlung freigesetzt wird, die über die optische Faser in den Ballon zugeführt wird.
- Therapeutika, die über den Stent der Erfindung zugeführt werden können, schließen jedes Mittel oder jede Kombination von Mitteln ein, die die Zellen in der Gefäßwand beeinflussen können, einschließlich Wirkstoffen, Chromophoren und Nukleinsäuren. Therapeutika schließen auch Diagnostika ein, die bei späterer Behandlung helfen werden, wie etwa strahlungsundurchlässige Verbindungen, die ermöglichen, daß das Gefäß durch Fluoroskopie oder ähnliche Methoden visualisiert werden kann. Therapeutika können weiter antimikrobielle Mittel einschließen, wie antibakterielle oder antivirale Mittel.
- Für restenose Inhibition ist es typischerweise wünschenswert, die Profileration von Glattmuskelzellen zu stoppen. Medikamente, die Thrombozytenaggregation und – adhäsion verhindern, können demgemäß verwendet werden, wie etwa Antithrombozytika, Antithrombogenesemittel und Antikoagulationsmittel. Zusätzlich können Rezeptorblocker, Wachstumsfaktoren und andere Hormone verwendet werden, um die normale Reparaturreaktion zu beschränken. Das folgende sind Gruppen von bestimmten Medikamenten, die verwendet werden können, um Gefäßerkrankungen zu behandeln, wie etwa Atherosklerose und Restenoses: Antikoagulationsmittel, einschließlich Heparin, Hirudin, Hirulog, Gewebeplasminogenaktivator und Fibrinogen; entzündungshemmende Mittel, wie etwa Steroide, Ibuprofen, Aspirin, Somatostatin, Angiopeptin und entzündungshemmendes Peptid 2; Cytotoxine, einschließlich Colchicin, Dexamethason, Doxorubicin, Methotrexat und Psoralen; Antibiotika; und Enzyme und Enzyminhibitoren einschließlich Urokinase, 2,4-Dinitrophenol und Thiolproteaseinhibitor.
- Ein weiterer Aspekt der vorliegenden Erfindung stellt einen aushärtbaren Stent aus Faserverbundmaterial zu Verfügung, der biologisch resorbierbar ist. Die Faserkomponente des Stents kann zum Beispiel aus Polyglykolsäure und anderen Materialien hergestellt werden, die in biologisch resorbierbaren Nähten verwendet werden. Aufgrund der offenen, nicht-kontinuierlichen Struktur des Stents, die signifikante Einwirkung von zirkulierendem Blut auf die Lumenwand ermöglicht, wird der Stent durch Endothelialgewebe der Lumenwand durch Wachstums des Gewebes über die Fasern eingekapselt. Die resultierende Gewebeschicht verstärkt und stützt das Gewebe weiter. Der Stent wird jedoch absorbiert werden, bevor das einkapselnde Gewebe sich bis zu einem unerwünschten Niveau verdickt.
Claims (13)
- Stent zur Stützung eines ausgewählten Abschnittes eines Körperlumens, der wenigstens ein expandierbares, flexibles Faserverbandmaterial (
4 ) umfaßt, das so ausgebildet ist, daß es mit der Wand eines Körperlumens (7 ) in Eingriff kommt, wobei das Faserverbandmaterial (4 ) eine Mehrzahl von Fasern und ein aushärtbares Material in Kontakt mit den Fasern umfaßt, wobei besagter Stent innerhalb des Körperlumens (7 ) positioniert und expandiert werden kann und die Fasern nach Aushärtung des aushärtbaren Materials, zusammengebunden werden, um eine starre Stützstruktur zu bilden, die eng an der Lumenwand (7 ) anliegt. - Stent nach Anspruch 1, dadurch gekennzeichnet, daß die Fasern mit dem aushärtbaren Material beschichtet sind.
- Stent nach Anspruch 1 oder Anspruch 2, dadurch gekennzeichnet, daß das Faserverbandmaterial eine Hohlfaser umfaßt, die das aushärtbare Material einkapselt.
- Stent nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß das Faserverbandmaterial weiter in Kontakt gebracht wird mit einem Therapeutikum, zum Beispiel einem Antithrombogenesemittel.
- Stent nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß das Faserverbandmaterial: (a) eine Schraubenkonformation besitzt; oder (b) eine Mehrzahl von Strängen umfaßt, die so verflochten sind, daß sie ein poröses Netz bilden.
- Stent nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß er weiter eine Schicht aus einem biologisch kompatiblen Material umfaßt, die das Faserverbundmaterial einkapselt, wobei fakultativ: (a) das biologisch kompatible Material ausgewählt ist aus der Gruppe, die aus Silikonen, Wachsen, Polyacrylamiden, Polyethylenen, Polystyrolen, Polypropylenen, Polyolefinen und Polyurethanen besteht; und/oder (b) das biologisch kompatible Material weiter ein Therapeutikum umfaßt.
- Stent nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß das Faserverbundmaterial biologisch resorbierbar ist.
- Stent nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß das Faserverbundmaterial ein Multifilamentgewebe umfaßt, das ausgewählt ist aus der Gruppe, die aus Polyester, Baumwolle, Polyethylentherephthalat und Kohlenstoff besteht.
- Stent nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß das aushärtbare Material: (a) UV-aushärtbar ist; oder (b) ausgewählt ist aus der Gruppe, die aus Epoxyharzen, Silikonen, Urethanen, Acrylamiden und Polyurethan-Oligomermischungen besteht; oder (c) thermisch aushärtbar ist; oder (d) chemisch aushärtbar ist.
- Vorrichtung zur Ausbildung eines Stents in einem Körperlumen, welche umfaßt: einen flexiblen länglichen Schlauch mit einem lichtdurchlässigen expandierbaren Ballon (
5 ), der an seinem distalen Ende angebracht ist; wenigstens ein expandierbares, flexibles Faserverbandmaterial (4 ), das um den expandierbaren Ballon (5 ) herum gelegt ist, wobei das Faserverbandmaterial eine Mehrzahl von Fasern und ein aushärtbares Material in Kontakt mit den Fasern umfaßt; Kathetermittel, die mit besagtem flexiblen Schlauch und besagtem Ballon zum Aufblasen des Ballons verbunden sind; und energieleitende Mittel zur Übertragung von Strahlung von einer äußeren Quelle zum Inneren des Ballons, wobei die Strahlung eine ausreichende Energie aufweist, um die Aushärtung des aushärtbaren Materials zu bewirken, wodurch die Fasern des Faserverbandmaterials (4 ) in einem expandierten Zustand zusammengebunden werden, was zur Bildung einer starren Stützstruktur führt, die eng an der Wand des Körperlumens (7 ) anliegt, wobei fakultativ: (a) das energieleitende Mittel eine optische Faser ist; oder (b) die Strahlung UV-Strahlung ist. - Stent nach einem der vorangehenden Ansprüche 1 bis 9 zur Verwendung in der Therapie und ausgebildet mit einem Verfahren, welches die Schritte umfaßt: Aufbringen eines aushärtbaren Materials auf eine Mehrzahl von expandierbaren, flexiblen Fasern, wodurch ein aushärtbares Faserverbandmaterial gebildet wird; Ausformen des Faserverbandmaterials zu einer Form, die zur Einführung in einer Körperlumen geeignet ist; Anbringen des ausgeformten Faserverbandmaterials über einem expandierbaren Teil; Anordnen des expandierbaren Teils in einem nicht-expandierten Zustand innerhalb eines Körperlumens; Positionieren des expandierbaren Teils benachbart zu einem ausgewählten Abschnitt . des Körperlumens; Aufblasen des expandierbaren Teils, um das Faserverbundmaterial zu expandieren und das Faserverbandmaterial in Kontakt mit der umgebenden Lumenwand zu bringen; und Aushärten des aushärtbaren Materials, um die Fasern des Faserverbandmaterials in einem expandierten Zustand zusammenzubinden, wodurch eine starre Stützstruktur gebildet wird.
- Stent nach Anspruch 11, wobei besagtes Verfahren zur Ausbildung des Stents weiter umfaßt: (a) den Schritt des Beschichtens des Faserverbandmaterials mit einem biologisch kompatiblen Material; und/oder (b) den Schritt des Flechtens des Faserverbandmaterials, um ein poröses Netz zu bilden; und/oder (c) den Schritt des Imprägnierens eines Therapeutikums in das Faserverbandmaterial hinein.
- Stent nach Anspruch 11 oder 12, dadurch gekennzeichnet, daß der Schritt des Aushärtens des aushärtbaren Materials das Vernetzen oder Polymerisieren des aushärtbaren Materials durch Strahlung (z. B. UV-Strahlung) umfaßt.
