Description
- Die Erfindung betrifft künstliche Wirbelsäulenfusionsimplantate, welche quer über den intervertebralen Raum zu platzieren sind, welcher nach dem Beseitigen einer beschädigten Spinal-Bandscheibe bleibt, und insbesondere ein verbessertes, teilweise zylindrisches Wirbelsäulenfusionsimplantat zur Implantation, wo zwei mit Gewinde versehene, zylindrische Implantate von geforderter Höhe nicht in die Querweite der Wirbelsäule passen würden.
- In der Vergangenheit haben Cloward, Wilterberger, Crock, Viche, Bagby, Brantigan, Michelson und andere, verschiedene Verfahren gelehrt, welche das Bohren von Löchern quer über den Bandscheibenzwischenraum zwischen zwei benachbarten Wirbeln der Wirbelsäule für den Zweck aufweisen, eine Wirbelsäulen-Zwischenkörper-Fusion zu verursachen. Cloward lehrte das Platzieren eines Dübels aus Knochen in das gebohrte Loch zum Zwecke der Überbrückung des Defekts und zur Inkorporierung in der Fusion. Viche lehrte das Einschrauben des Knochendübels. Bagby lehrte das Platzieren des Knochen-Transplantats in einen Metall-Korb hinein, welcher mit Ausnahme von Reihen radial angeordneter Löcher auf seiner Oberfläche glatt ist, welche mit dem Inneren des Korbes und dem Knochen-Transplantat kommunizieren. Von der Bagby-Vorrichtung wurde offenbart, dass sie zur Verwendung in einem Pferd geeignet ist. Brantigan lehrte die Verwendung von inerten Blöcken, welche vorzugsweise aus Metall sind und das Metall an ihrer Außenfläche haben, welche die Porösität des Knochens nachahmt. Brantigan theoretisierte, dass der Knochendübel vollständig von einem Metall-Stopfen ersetzt werden kann, welcher dennoch, obwohl an sich nicht bei der Fusion aktiv, dazu dient, die Wirbel vom Inneren des Bandscheibenzwischenraums zu stützen, wobei das Auftreten der Fusion um ihn herum ermöglicht ist.
- U.S. Patent Nr. 3,848,601, ausgegeben an Ma et al. am 19. November 1974, lehrt ein Verfahren und ein Instrumentarium zum Vorbereiten von rechteckigen Räumen quer über den Bandscheibenzwischenraum in die benachbarten Wirbel hinein und zum Vorbereiten eines rechteckigen Transplantats aus dem Knochen an sich, welches in den rechteckigen Raum eingesetzt wird.
- U.S. Patent Nr. 4,743,256, ausgegeben an Brantigan am 10. Mai 1988, lehrt das Verwenden eines inerten, künstlichen Distanzstücks in der Gestalt eines Rechtecks anstelle der Verwendung eines rechteckigen Knochen-Transplantats, wie von Ma et al. gelehrt wird.
- U.S. Patent Nr. 4,878,915, ausgegeben an Brantigan am 7. November 1989 lehrt das Verwenden eines vollständig zylindrischen, inerten Implantats zum Verwenden bei der Wirbelsäulen-Zwischenkörper-Fusion. Solche Implantate nehmen nicht an dem Knochenfusions-Prozess teil, sondern wirken als inerte Distanzstücke und erlauben das Knochenwachstum zu den Außenflächen der Implantate.
- U.S. Patent Nr. 4,834,757, ausgegeben an Brantigan am 30 Mai 1989, lehrt ein rechteckig gestaltetes, hohles Wirbelsäulenfusionsimplantat, welches, anstelle von einem rechteckigen Knochen-Transplantat oder von Brantignas früherem künstlichen, inerten Distanzstück verwendet wird.
- U.S. Patent Nr. 5,015,247 ausgegeben an Michelson am 14. Mai 1991, lehrt das Verwenden eines dünnwandigen, stark perforierten, mit Gewinde versehenen Hohlzylinder-Implantats, welches an beiden Enden geschlossen oder verschließbar ist zur komprimierten Beladung mit Knochen oder anderen fusions-unterstützenden Materialien.
- Zusätzlich kann die Michelson-Vorrichtung dann mit einem die Knochen-Produktion induzierenden Chemikal wie beispielsweise Hydroxyapatit bedeckt sein. Das Michelson Patent offenbart auch ein verbessertes Verfahren zum Löcher-Bohren quer über den Bandscheibenzwischenraum und in die zwei benachbarten Wirbel hinein und zum sicheren Installieren von zylindrischen Implantaten, sodass das ganze chirurgische Verfahren durch ein hohles, zylindrische Rohr durchgeführt werden kann. Der hohle Zylinder kann während des chirurgischen Verfahrens an Ort und Stelle gelassen werden und dient dazu, die benachbarten Wirbel an Ort und Stelle relativ zueinander zu halten, erlaubt das geschützte Bohren der Löcher quer über den Bandscheibenzwischenraum und erlaubt die Einfügung des Implantats durch das selbe Rohr hindurch in das gebohrte Loch hinein quer über den Bandscheibenzwischenraum und in die benachbarten Wirbel hinein.
- In Bezug auf jenes Verfahren zum Durchführen der Wirbelsäulen-Zwischenkörper-Fusion, welche im Wesentlichen zylindrische, mit Gewinde versehene Implantate verwendet, tritt ein spezielles Problem auf (siehe
1 ), wenn ein Versuch unternommen wird, zwei zylindrische Implantate (welche als die bevorzugte Anzahl erachtet wird, da sie eine stabilere Konstruktion ist und mehr Oberfläche als ein einzelnes, mittig platziertes Implantat hat) nebeneinander quer über einen Bandscheibenzwischenraum und in zwei benachbarte Wirbel hinein zu platzieren, wo die Höhe des Bandscheibenzwischenraums derart ist, dass es ein Implantat mit einem so großem Durchmesser erfordert zum Hinein-Penetrieren in die und zum signifikanten Angreifen von jedem der benachbarten Wirbel, dass es nicht mehr möglich ist, zwei solche Implantate nebeneinander zu platzieren und dass sie trotzdem innerhalb der Querweite der Wirbelsäule enthalten sind. Wenn ein Versuch gemacht wird, das Problem durch die Verwendung von nebeneinander platzierten Implantaten mit kleinern Durchmesser abzustellen, sodass dann beide geeignet wären, in die Querweite der Wirbelsäule zu passen, würden die Implantate eine unzureichende Höhe zum adäquaten Ergreifen des Knochens aufweisen. Wenn ein Versuch gemacht wird, das Problem durch das Aufgeben des nebeneinander angeordneten Doppel-Implantats zu Gunsten eines einzelnen, mittig platzierten Implantats abzustellen, wäre dann, wo das Implantat ausreichend groß genug ist, einen ausreichenden Abschnitt von der Querweite des Bandscheibenzwischenraums einzunehmen zum Begünstigen einer sicheren Stabilität, dessen Vertikalhöhe und Exkursion derart heftig, dass, wenn zwei beliebige, aufeinanderfolgende Bandscheibenzwischenräume nacheinander zu operieren sind, die dazwischenliegenden Wirbel in zwei Hälften geschnitten werden würden. - U.S. Patent Nr. 5,055,104, ausgestellt an Ray am 8. Oktober 1991 ("Ray Patent"), offenbart ein Implantat, welches eine schraubenlinienförmige Wicklung ohne Wandelemente aufweist, welches zusammen gebaut wird, nachdem die Wicklungen in den Bandscheibenzwischenraum zwischen den Wirbeln platziert sind, welche angeblich dann beseitigt werden können, wenn die Wirbel miteinander verschmolzen sind. Das Ray-Implantat ist fehlerhaft und nicht bearbeitbar, indem es zur Verwendung auf die beschriebene Weise nicht geeignet ist, da es nicht möglich ist, eine isolierte, schraubenlinienförmige Wicklung ohne irgendwelche Wandelemente zum Stützen von solch einer Wicklung in harten Knochen einzufügen, welche könnte, wäre anlag strukturiert wie ein Slinky, (siehe Ray-Patent,
1 und7 ). Ferner ist das Ray-Implantat überaus komplex, da es die schwierige, wenn nicht sogar die unmögliche Aufgabe der Montage innerhalb des Bandscheibenzwischenraums erfordert.3 des Ray-Patents sagt klar aus, dass Ray nicht das Verwenden von einem kegelstumpfartigen, zylindrischen Implantat lehrt, sondern lediglich das Verwenden einer kegelstumpfartigen, schraubenlinienförmigen Wicklung, welche sich als eine spitzt zulaufende Feder zeigt, wobei irgendein Wandelement vollständig fehlt, welches als zylindrisch angesehen werden könnte. Daher lehrt Ray nur das Verwenden von einem isolierten Gewinde, welches einzig nur durch Rotation eingefügt werden kann und nicht linear vorgerückt werden kann. - Wenn die überwältigenden Hindernisse der Unmöglichkeit vom Einführen eines isolierten Gewindes ohne Wandelemente und das Problem der Montage innerhalb des Bandscheibenzwischenraums überwunden werden könnten, wäre dann das Ray-Implantat immer noch für seinen vorgesehenen Zweck unsicher, da es eine hohes Risiko einer spontanen Demontage und eines mechanischen Versagens geben würde. Ferner würde es nicht ausreichend Platz zum sichern Rotieren zur Einführung von solch einer Vorrichtung geben, da es der große Platz-Mangel an sich ist, der das Verwenden von einer Vorrichtung erfordert, welche eine verringerte Querweite hat.
- Es gibt daher den Bedarf für ein Wirbelsäulenfusionsimplantat, welches zur Einführung in ein Loch geeignet ist, welches quer über den Bandscheibenzwischenraum zwischen zwei benachbarten Wirbeln und teilweise in die zwei benachbarten Wirbel gebohrt ist, sodass das Wirbelsäulenfusionsimplantat zum Einpassen in die Querweite der Wirbelsäule geeignet ist, wenn nebeneinander platziert neben einem zweiten seiner Art.
- EP 0637440A beschreibt ein hohles, rohrförmiges Wirbelsäulenimplantat mit einem im Wesentlichen kreisförmigen Querschnitt. Das Implantat definiert zwei massive Seitenflächen und eine obere Fläche und eine untere Fläche, welche mit Öffnungen versehen sind. Ringförmige Rippen auf der Außenseite verhindern eine Rückwärts-Bewegung und eine Rotation des Implantats, wenn es zwischen benachbarte Wirbel eingesetzt wird.
- Gemäß der Erfindung wird bereitgestellt ein Wirbelsäulenfusionsimplantat, welches aus einem geeigneten Material hergestellt ist und welches zur Humanimplantation zumindest teilweisen quer über die chirurgisch korrigierte Höhe eines Bandscheibenzwischenraumes zwischen benachbarte Wirbelkörper vorgesehen ist, wobei das Implantat aufweist: ein Element mit einer Außenseite und einer Längsachse, wobei die Außenseite ein Vorder-Ende, ein Hinter-Ende und eine Länge dazwischen aufweist, wobei die Außenseite entlang der Längsachse des Implantats einen teilweise kreisförmigen Querschnitt aufweist, wobei der teilweise kreisförmige Querschnitt der Außenseite Bogen-Abschnitte aufweist, welche einander gegenüberliegen und welche Bogen des gleichen Kreises sind und eingerichtet sind, je in einen der benachbarten Wirbelkörper eindringend einzugreifen, wenn sie zwischen den Wirbelkörpern und über die Höhe des Bandscheibenzwischenraumes eingesetzt sind, wobei jeder der gegenüberliegenden Bogen-Abschnitte mindestens eine dort hindurch passierende Öffnung aufweist, um Knochenwachstum vom angrenzenden Wirbelkörper zum benachbarten Wirbelkörper zu ermöglichen, dadurch gekennzeichnet, dass der teilweise kreisförmige Querschnitt einen konkaven Abschnitt aufweist, der eingerichtet ist, um mit einem konvexen Abschnitt eines zweiten Implantats zusammenzupassen, wenn über die Höhe des Bandscheibenzwischenraums implantiert, sodass die kombinierte Breite der beiden Implantate kleiner als das Zweifache des maximalen Durchmessers der beiden Implantate ist.
- Die Erfindung ist ein verbessertes Wirbelsäulen-Zwischenkörper-Fusionsimplantat, welches zur Einsetzung in ein quer über den Bandscheibenzwischenraum gebohrtes Loch zwischen zwei benachbarten Wirbeln und in die beiden benachbarten Wirbel geeignet ist, sodass das Wirbelsäulenfusionsimplantat zur Einpassung in die Querweite der Wirbelsäule geeignet ist, wenn nebeneinander neben einem zweiten von seiner Art platziert. Das Wirbelsäulenfusionsimplantat der Erfindung kann einen dünnwandigen, mehrfach perforierten Zylinder oder Teilzylinder aufweisen, welcher aus einem zur Human-Implantation geeigneten Material ist und vorzugsweise, aber nicht notwendigerweise, ein geschlossenes Ende und ein Ende hat, welches zum Geschlossen-werden geeignet ist, sodass eine Innenkammer mit natürlichem oder künstlichem osteokonduktiven, osteoinduktiven, oder osteogenetischen oder anderen fusionssteigernden Materialien befüllt werden kann und dieselben halten kann. Das Wirbelsäulenfusionsimplantat vertraut auf Unebenheiten der Außenfläche zum Verbessern seiner Stabilität. Das Wirbelsäulenfusionsimplantat der Erfindung inkorporiert an seinem Hinter-Ende ein Eingreifmittel, vorzugsweise an seinem Hinter-Ende, zum Vereinfachen des Einsetzens oder Herausziehens des Implantats. Das Implantat der Erfindung kann sein aus, gefüllt sein mit und/oder ummantelt sein mit fusionsfördernden Substanzen. Ferner ist zur Einführung des Wirbelsäulenfusionsimplantats keine Rotation erforderlich und es ist durch lineares Vorrücken einführbar.
- Das Wirbelsäulenfusionsimplantat ist allgemein wirksam und ist sicherer und wirksamer als die zylindrischen Implantate des Stands der Technik für den speziellen Fall, wenn es wünschenswert ist, zwei Implantate nebeneinander in vorbereitete Kanäle einzuführen und wo die Höhe des Bandscheibenzwischenraums zwischen zwei benachbarten Wirbeln so groß relativ zur Querweite der Wirbelsäule ist, dass zwei Implantate von der erforderlichen Höhe nicht in die Querweite der Wirbelsäule passen. Der Stand der Technik lehrt jenen, welche in der Wirbelsäulen-Chirurgie sachkundig sind, dass die Wahrscheinlichkeit zum Erzielen einer Wirbelsäulenfusion von drei Faktoren proportioniert ist: 1) der Oberfläche des Implantats 2) der Qualität und Quantität des Transplantationsmaterials und 3) der Stabilität der Fusions-Konstruktion. Das Wirbelsäulenfusionsimplantat der Erfindung vergrößert jeden dieser drei Faktoren, indem es ermöglicht wird, zwei Implantate nebeneinander quer über einen Bandscheibenzwischenraum zu verwenden, welchem andernfalls zum Akzeptieren von mehr als einem Implantat die ausreichende Weite fehlt.
- Das Wirbelsäulenfusionsimplantat der Erfindung ist auf Basis eines individuellen Implantats aus folgenden Gründen wirkungsvoller als der Stand der Technik:
- 1. Vergrößerte Oberfläche. Das Wirbelsäulenfusionsimplantat der Erfindung hat wegen seiner Flächen-Unebenheiten eine größere Oberfläche zum Ergreifen der benachbarten Wirbel als ein Implantat mit glatten Außenflächen. Die Anwesenheit oder Abwesenheit von Löchern beeinflusst dies nicht wesentlich, so weit wie die mit Material gefüllten Löcher an dem Kontaktbereich an der Fläche wirksam beitragen. Die gebogenen Abschnitte des teilweise zylindrischen Implantats der Erfindung stehen in Kontakt mit den benachbarten Wirbeln und stellen eine größere Oberfläche bereit, als es mit einem geraden Abschnitt von einem nicht zylindrischen Implantat möglich ist.
- 2. Die Quantität und die Qualität des präsentierten Transplantations-Materials. Da das Wirbelsäulenfusionsimplantat der Erfindung nicht eingeschraubt wird, braucht es nicht so konstruiert sein, dass es der damit zugehörigen Drehmoment-Aufbringung standhält. Daher kann das Implantat der Erfindung dünnwandiger und dadurch bei einem gegebenen Durchmesser ein größeres Innenvolumen haben. Das Wirbelsäulenfusionsimplantat der Erfindung hat gebogene Abschnitte, welche das Implantat druckfester machen als ein Implantat, welchem die obere und die untere gekrümmte, tragende Fläche fehlen, sodass die Wand des Implantats relativ dünner sein kann als wie bei solchen Implantaten. Eine dünnere Wand ist für den Knochen einfacher zum Hindurchwachsen. Auch können die Zwischenporen-Brücken kleiner sein, was eine größere Porosität und dadurch größere Bloßlegung des inneren Transplantations-Materials erlaubt. Ferner kann das Wirbelsäulenfusionsimplantat der Erfindung aus/mit einer Vielzahl von Materialien und/oder chemischen Substraten, welche bekannt sind, sich aktiv in dem Knochenfusionsprozess zu beteiligen, konstruiert sein und/oder ummantelt sein und/oder beladen sein. Da das Wirbelsäulenfusionsimplantat der Erfindung eine größere Oberfläche und für seinen Außendurchmesser ein größeres Innenvolumen bereitstellt, biete es die Möglichkeit, von Fusionsmaterialien eine größere jenen Oberfläche und ein größeres Volumen zu präsentieren.
- 3. Das Implantat der Erfindung biete eine größere Stabilität als die Stand-der-Technik-Implantate. Die am wenigsten stabilsten Implantate sind die Implantate mit fehlenden Flächen-Unebenheiten. Flächenlöcher vergrößern die Implantat-Stabilität durch Vergrößern der Überlagerung des Implantats zu den gegenüberliegenden Flächen. Das Wirbelsäulenfusionsimplantat der Erfindung ist gegenüber dem Stand der Technik eine weitere Verbesserung, indem die Oberflächen-Unebenheiten des Wirbelsäulenfusionsimplantats der Erfindung Bewegungen in allen Richtungen abfangen. Ferner sind alle Implantate der Möglichkeit ausgesetzt, hinausgezogen zu werden, indem sie entlang der Bahn, mit der sie eingesetzt wurden, zurückgewunden werden. Jedoch kann das Wirbelsäulenfusionsimplantat der Erfindung eine Fläche haben, welche zum Vorwärts-Zwingen des Wirbelsäulenfusionsimplantats konfiguriert ist, um einen vergrößerten Widerstand gegen solch eine ungewünschte Rückwärts-Wanderung zu bieten. Ferner stellen die gebogenen Abschnitte der Erfindung einen größeren Tragebereich bereit zum besseren Verteilen der Drückkräfte durch die Wirbel hindurch.
- Das Wirbelsäulenfusionsimplantat der Erfindung ist einfacher zu verwenden, da es weniger Platz belegt, kein vorheriges Anbohren benötigt und nicht den Bedarf hat, dass ein Instrument in den geschlossenen Rändern der Wirbelsäulenwunde rotiert wird. Ferner ist das Wirbelsäulenfusionsimplantat der Erfindung leichter einzuführen als Implantate, welchen eine obere und eine untere Tragefläche fehlen, welche Bögen des gleichen Kreises sind, und welche Implantate quer über den Bandscheibenzwischenraum und in die benachbarten Wirbel einzuführen sind, da es einfacher ist, ein rundes Loch vorzubereiten als ein quadratisches Loch, da eine rundes Loch in einem einzelnen Schritt gebohrt werden kann.
- Es ist eine Aufgabe der Erfindung, ein Wirbelsäulen-Zwischenkörper-Fusionsimplantat bereitzustellen, sodass es möglich ist, zwei solche Implantate nebeneinander quer über einen Bandscheibenzwischenraum und in zwei benachbarte Wirbel, welche zueinander sehr nahe sind, in die Querweite der Wirbelsäule zu platzieren, wo die Querweite der Wirbelsäule ansonsten gegenüber der erforderlichen Implantat-Höhe unzureichend gewesen sein würde, welche für die Unterbringung von zwei zylindrischen mit Gewinde versehenen Stand-der-Technik-Implantate zulässig ist.
- Es ist eine andere Aufgabe der Erfindung ein Wirbelsäulenfusionsimplantat bereitzustellen, welches einfacher einzuführen ist und kein Gewindeschneiden vor der Implantation benötigt.
- Es ist noch eine andere Aufgabe der Erfindung, ein Wirbelsäulenfusionsimplantat bereitzustellen, welches sicherer ist, bei welchem es keinen Bedarf gibt, scharfe Gewinde nahe empfindlichen Strukturen zu verlegen.
- Es ist noch eine andere Aufgabe der Erfindung, ein Wirbelsäulenfusionsimplantat bereitzustellen, welches schneller zwischen zwei benachbarten Wirbeln über lineares Vorrücken anstatt über Rotations-Vorrücken einzuführen ist.
- Diese und andere Aufgaben der Erfindung werden aus einer Besprechung der beigefügten Zeichnungen und der folgenden, detaillierten Beschreibung der Zeichnungen verständlich.

- Die bisherigen Vorrichtungen
- Mit Bezug auf
1 ist eine schematische Darstellung eines Segments der menschlichen Wirbelsäule gezeigt, welche allgemein mit dem Buchstaben S bezeichnet ist. Das Segment der Wirbelsäule S weist mehrere Wirbel V und einen Bandscheibenzwischenraum D zwischen zwei benachbarten Wirbeln V auf. Verschiedene zylindrische, mit Gewinde versehene Fusionsimplantate, wobei jedes unterschiedliche Durchmesser hat, sind quer über den Bandscheibenzwischenraum D eingesetzt gezeigt. - Wenn die Höhe Hs des Bandscheibenzwischenraums D so groß ist, dass zwei zylindrische Implantate, wie beispielsweise die Wirbelsäulenfusionsimplantate
10a und10b , welche jeweils einen ausreichenden Durchmesser zum Quer-Überbrücken des Bandscheibenzwischenraums D haben und in den Knochen von benachbarten Wirbeln V ausreichend hineingreifen, quer über den Bandscheibenzwischenraum D platziert sind, überschreitet die kombinierte Gesamtweite der beiden Wirbelsäulenfusionsimplantate10a und10b die Querweite Ws der Wirbelsäule S. Als Ergebnis steht ein Abschnitt von jedem Implantat10a und10b aus den Seiten der Wirbelsäule S vor und könnte ernsthaften und möglicherweise tödlichen Schaden an dem Patienten verursachen, da empfindliche und lebenswichtige Strukturen zu jenem Bereich der Wirbelsäule S benachbart liegen, sodass das Verwenden von zwei zylindrischen Wirbelsäulenfusionsimplantaten10a und10b nicht wünschenswert wäre. - Wenn anstatt von zwei Wirbelsäulenfusionsimplantaten
10a und10b eine einziges Implantat, wie beispielsweise das Wirbelsäulenfusionsimplantat12a , verwendet werden würde, welches einen ausreichenden Durchmesser zum Bereitstellen von Stabilität und von Fusion hat, würde dann das Implantat in die benachbarten Wirbel V tief eindringen. Das Wirbelsäulenfusionsimplantat12a würde einen Durchmesser haben, welcher wesentlich größer als die Höhe Hs des Bandscheibenzwischenraums D ist, sodass die Wirbel V wesentlich aufgebohrt werden müssten zum Unterbringen des großen Durchmessers des Wirbelsäulenfusionsimplantats12a . Als Ergebnis würde ein großer Teil der Wirbel V beseitigt werden und daher würde die gesamte Struktur-Integrität der Wirbel V wesentlich geschwächt werden. Dies ist insbesondere ein Problem wenn ein zweites Wirbelsäulenfusionsimplantat12b , welches zu dem Wirbelsäulenfusionsimplantat12a identisch ist, quer über den Bandscheibenzwischenraum D auf der anderen Seite desselben Wirbel V platziert ist, sodass zwei Wirbelsäulenfusionsimplantate12a und12b quer über den Bandscheibenzwischenraum D auf beiden Seiten der Wirbel V platziert sind. Als Ergebnis ist der Wirbel V in eine "Schmetterlings"-Konfiguration, wie in1 gezeigt, gespalten und die strukturelle Integrität und Festigkeit der Wirbel V ist weiter herabgesetzt, sodass die Wirksamkeit des Wirbelsäulenfusions-Verfahrens wesentlich reduziert ist und die Wirbel V ein hohes Devaskularisierungs und Fraktur-Risiko aufweisen. - Umgekehrt, wenn zwei zylindrische Implantate, wie beispielsweise die Wirbelsäulenfusionsimplantate
14a und14b , jeweils einen ausreichend großen Durchmesser haben, sodass, wenn in dem Bandscheibenzwischenraum D nebeneinander platziert, die kombinierte Gesamtweite der Wirbelsäulenfusionsimplantate14a und14b gerade die Querweite Ws der Wirbelsäule S füllt, ist der Durchmesser von jedem der Wirbelsäulenfusionsimplantate14a und14b nicht ausreichend zum Quer-Überbrücken des Bandscheibenzwischenraums D zum Ergreifen der Wirbel V. Daher können, obwohl die Wirbelsäulenfusionsimplantate14a und14b nicht von den Seiten der Wirbelsäule S vorstehen, die Wirbelsäulenfusionsimplantate14a und14b den Knochen der Wirble V nicht erreichen und angreifen und können daher nicht zum Stabilisieren der benachbarten Wirbel V funktionieren. - Mit Bezug auf
2 ist eine Draufsicht, welche entlang Linie 2-2 aus1 ist, wobei die oberen Wirbel V beseitigt sind, von zwei zylindrischen, mit Gewinde versehenen Implantaten10a und10b gezeigt, welche quer über den Bandscheibenzwischenraum D platziert sind. Die mit Gewinde versehenen Implantate10a und10b haben ein Außengewinde11a und11b , welche eine minimale Höhe haben müssen, welche zu der Größe des mit Gewinde versehenem Implantat proportional ist, dass es wirksam ist. Das Gewinde11a und11b von den mit Gewinde versehenen Implantaten10a und10b wandelt ein Drehmoment in lineare Bewegung um, sodass die Gewinde11a und11b eine ausreichende Gesamthöhe benötigen zum Überwinden des Widerstands des Materials, wie beispielsweise Knochen, in welches die mit Gewinde versehenen Implantate10a und10b eingeführt werden, wobei solch ein Widerstand proportional zur Oberfläche und dem Durchmesser von jedem der mit Gewinde versehenen Implantate10a und10b ist. Daher ist der Unterschied zwischen dem Hauptdurchmesser (einschließlich der Gewinde) und dem Kerndurchmesser (abzüglich der Gewinde) von jedem mit Gewinde versehenen Implantaten10a und10b , derart dass, wenn zwei mit Gewinde versehene Implantate10a und10b quer über den Bandscheibenzwischenraum D in die benachbarten Wirbel V hinein implantiert sind, es einen minimalen Anstand zwischen den zwei mit Gewinde versehenen Implantaten10a und10b geben muss, dass sie für die Höhe der Gewinde11a und11b zulässig ist. Dies würde selbst dann zutreffen, wenn die Gewinde11a und11b ineinandergreifen würden, die mit Gewinde versehenen Implantate10a und10b würden zumindest immer noch um die Höhe des Gewindes von mindestens einem der mit Gewinde versehenen Implantate10a und10b versetzt sein. Solch ein minimaler Abstand zwischen den beiden mit Gewinde versehenen Implantaten10a und10b vergrößert die kombinierte Gesamtweite der beiden mit Gewinde versehnen Implantate10a und10b , wenn sie eingesetzt sind. - Daher ist es für ein im Wirbelsäulenfusions-Verfahren zu verwendendes, zylindrisches Implantat, wo die Höhe Hs des Bandscheibenzwischenraums D zwischen zwei benachbarten Wirbeln V im Vergleich zu ihrer Weite Ws groß ist, notwendig ein Implantat zu haben, welches benachbart zu einem zweiten seiner Art in engeren Kontakt implantierbar ist, als es mit Gewinde versehen Implantaten möglich ist, wobei dennoch eine Implantat-Fläche bereitgestellt wird, welche beim Eingriff an die benachbarten Wirbel V mechanische Stabilität bereitstellt. Das Verwenden eines zylindrischen Implantats ist wünschenswert, da es einfach ist, die Empfangs-Stelle vorzubereiten durch Bohren eines zylindrischen Lochs quer über den Bandscheibenzwischenraum D und in die benachbarten Wirbel V hinein. Die gekrümmte Fläche der zylindrischen Löcher, welche in die Wirbel V hineingebohrt sind, hat im Vergleich zu einer ebenen Fläche eine größere Oberfläche und stellt auch die Möglichkeit von enger Kongruenz bereit, wenn in das zylindrisches Loch ein Implantat eingepasst wird, welches korrespondierende, zylindrische Abschnitte mit passendem Durchmesser hat.
- Die Erfindung
- Mit Bezug auf
3 –10 ist eine Ausführungsform des Wirbelsäulenfusionsimplantats gezeigt und allgemein mit dem Bezugszeichen100 bezeichnet. Obwohl jenes nicht erfindungsgemäß ist, weist es viele Merkmale auf, welche es damit gemeinsam hat. Das Wirbelsäulenfusionsimplantat100 hat eine im Wesentlichen zylindrische Konfiguration, welches eine dünne Außenwand112 , welche eine Innenkammer114 umgibt, und eine zentrale Längsachse L hat. Das Äußerer des Wirbelsäulenfusionsimplantat100 weist Flächen-Unebenheiten auf, welche eine Fläche bereitstellen, welche zum Ergreifen der Wirbel V geeignet ist zum Stabilisieren des Wirbelsäulenfusionsimplantats100 quer über den Bandscheibenzwischenraum D und in die benachbarten Wirbel V hinein, sobald chirurgisch implantiert. In einer Ausführungsform des Wirbelsäulenfusionsimplantats100 weisen die Oberflächenunebenheiten eine Mehrzahl von Sperrteilen120 entlang des Umfangs des Wirbelsäulenfusionsimplantats auf. Jedes der Vielzahl von Sperrteilen120 weist einen Knochen-Eingriffs-Rand122 und ein abgewinkeltes Segment124 auf. - Jedes der Vielzahl von Sperrteilen
120 hat eine Höhe, welche wesentlich geringer als die Höhe von einem notwendigen Gewinde für ein zylindrisches, mit Gewinde versehenes Implantat derselben Größe ist. Da ein Gewinde eine einfache Vorrichtung zum Umwandeln eines Drehmoments in ein lineares Vorrücken ist, ist die notwendige Höhe des Gewindes proportional zu der Oberfläche und dem Durchmesser des Implantats und muss zum Ziehen eines zylindrischen Implantats ausreichend sein, welches einen Durchmesser hat, welcher zum Querüberbrücken des Bandscheibenzwischenraums D durch ein Material so dicht wie Knochen ausreichend sein muss. Im Gegensatz dazu haben die Sperrteile120 eine Höhe, welche wesentlich geringer als die notwendige Höhe von einem Gewinde von einem gleich großen mit Gewinde versehenen Implantat ist, da das Wirbelsäulenfusionsimplantat100 quer über den Bandscheibenzwischenraum D und in die benachbarten Wirbel V hinein durch lineares Vorrücken implantiert wird. Das Wirbelsäulenfusionsimplantat100 kann in den zylindrischen Bandscheibenzwischenraum D durch direktes, lineares Vorrücken hineingedrückt werden, da es kein Gewinde benötigt, um es vorwärts durch die Wirbelsäule hindurch zu ziehen. Da kein Drehmoment zum Vorrücken des Wirbelsäulenfusionsimplantats100 benötigt wird, gibt es keine minimal notwendige Höhe der Flächenunebenheiten. Das einzig notwendige Flächenmerkmal ist jenes, welches dem Wirbelsäulenfusionsimplantat100 Stabilität gibt, sobald es implantiert ist. - Außerdem können die Sperrteile
120 einer einzigen Richtung zugewandt sein, der Richtung, in welcher das Wirbelsäulenfusionsimplantat100 eingeführt wird, und funktionieren zum Verhindern, dass das Wirbelsäulenfusionsimplantat100 aus dem Bandscheibenzwischenraum D in einer zur Einsetzrichtung entgegengesetzten Richtung zurückwandert, sobald es zwischen die beiden benachbarten Wirbel V eingeführt ist. Die Sperrteile120 zwingen das Wirbelsäulenfusionsimplantat100 vorwärts gegen den unbeseitigten Knochen der Wirbel V. Da Implantate im Allgemeinen entlang dem gleichen Pfad, in welchen sie eingeführt sind, zurückwandern wollen, so wie beispielsweise bei wiederholter Bewegung des Körpers des Patienten über die Zeit, und welche bei einem etwas anderen Implantat-Design ein Löslösen bewirken würde (zum Beispiel bewirken, dass ein mit Gewinde versehenes, zylindrisches Implantat möglicherweise sich herausschraubt), tendieren die Sperrteile120 dazu, das Wirbelsäulenfusionsimplantat100 vorwärts gegen den massiven, unbeseitigten Knochen zu zwingen, wobei ein Widerstehen gegen weiteres Loslösen und ein gesteuertes Bewegen eine außerordentlich, stabile Implantation ergeben. - Die Knochen-Eingreif-Ränder
122 der Sperrteile120 , welche eine Höhe an der höchsten Stelle haben, gemessen vom Kerndurchmesser des Wirbelfusionsimplantats100 aus, welche ungefähr 0,35 mm ist. Auf diese Weise kann das Wirbelsäulenfusionsimplantat100 neben einem zweiten seiner Art mit einem Abstand von ungefähr 0,7 mm abseits davon oder wenn versetzt sogar näher platziert werden, was wesentlich die kombinierte Querweite der zwei Wirbelsäulenfusionsimplantate100 reduziert, sobald sie chirurgisch implantiert sind. Die Sperrteile120 können eine Höhe im Bereich von 0,25–1,5 mm haben, wobei die bevorzugte Höhe im Bereich von 0,35–0,75 mm ist. - Mit Bezug auf
5 sind zwei Wirbelsäulenfusionsimplantate100a und100b quer über den Bandscheibenzwischenraum D eingesetzt gezeigt, welche die gleichen Abmessungen von dem in2 gezeigten Bandscheibenzwischenraum D haben. Die beiden Wirbelsäulenfusionsimplantate100a und100b haben im Vergleich zu den zwei mit Gewinde versehenen Wirbelsäulenfusionsimplantaten eine verminderte kombinierte Gesamtweite, welche wie vorher beschreiben und in2 gezeigt nebeneinander platziert sind. Die verkleinerte, kombinierte Gesamtweite der beiden Wirbelsäulenfusionsimplantate100a und100b ist der Unterschied zwischen dem Kern- und dem Hauptdurchmesser des Wirbelsäulenfusionsimplantats100a und100b und wird durch Anwendung von Flächen-Unebenheiten erzielt, wie beispielsweise Sperrteilen120 zur Stabilisierung. Die Flächen-Unebenheiten ermöglichen, dass die beiden Wirbelsäulenfusionsimplantate100a und100b in eine wesentlich engere Nähe zueinander kommen und eine geringere Gesamtquerweite für ihre Einführung benötigen als es für zwei mit Gewinde versehene, zylindrische Implantate möglich ist, welche identische Kerndurchmesser haben, wegen der notwendigen Gewindehöhe von solchen mit Gewinde versehenen Implantaten. Das Reduzieren der Versetzung zwischen Implantaten erlaubt das Verwenden von Implantaten mit größerem Durchmesser, welche dann dennoch in die Querweite Ws der Wirbelsäule passen und einen wesentlicheren Eingriff in die benachbarten Wirbel V erzielen. - Mit Bezug auf
7 ist ein Querschnitt des Wirbelsäulenfusionsimplantats100 gezeigt, wobei die Wand112 Öffnungen128 aufweist, welche dort hindurch passieren zum Kommunizieren mit der Innenkammer114 . Die Innenkammer114 kann mit einem Knochenmaterial oder jedem natürlichen oder künstlichen Knochenwachstums-Material oder fusions-fördernden Material gefüllt sein, sodass Knochenwachstum aus den Wirbeln V durch die Öffnungen128 hindurch zu dem Material innerhalb der Kammer114 auftritt. Obwohl die Öffnungen128 in den Zeichnungen kreisförmig gezeigt worden sind, ist verständlich, dass die Öffnungen128 jede Gestalt, Größe oder Form haben können, welche für das Verwenden in einem Wirbelsäulefusionsimplantat geeignet ist, ohne das vom Umfang der Erfindung abgewichen wird. Auch kann die Anzahl von Öffnungen variiert werden oder keine Öffnungen können auf dem Wirbelsäulenfusionsimplantat vorhanden sein. - Mit Bezug auf
8 und9 weist das Wirbelsäulenfusionsimplantat100 eine Kappe130 mit einem Gewinde132 auf, welches an ein Ende des Wirbelsäulenfusionsimplantats100 schraubend angebracht wird. Sobald die Kappe130 an dem Wirbelsäulenfusionsimplantat100 angebracht ist, wirkt der Rand136 als ein zusätzliches Sperrteil120 zur weitern Stabilisierung des Wirbelsäulenfusionsimplantats100 , sobald es quer über den Bandscheibenzwischenraum D implantiert ist. - Die Kappe
130 ist ablösbar zur Zutritts-Bereitstellung in die Innenkammer114 , sodass die Innenkammer114 mit jedem Material gefüllt und jedes Material enthalten kann, welches natürlich oder künstlich osteokonduvtiv, osteoinduktiv, oteogenetisch oder ein anderes fusionssteigerndes Material ist. Einige Beispiele von solchen Materialien sind Knochen, welcher vom Patienten entnommen ist, oder knochenwachstum-induzierende Materialien, wie beispielsweise, aber nicht darauf beschränkt, Hydroxyapatit, Hydroxyapatit-Tricalciumphosphat oder knochenmorphogene Proteine. Die Kappe130 und/oder das Wirbelsäulenfusionsimplantat100 an sich sind/ist aus einem Material, welches zur Human-Implantation geeignet ist, wie zum Beispiel Titan und/oder könne aus einem sein, und/oder gefüllt sein mit, und/oder ummantelt sein von einem knochen-hineinwachs-induzierendem Material, wie beispielsweise, aber nicht darauf beschränkt, Hydroxyapatit, Hydroxyapatit-Tricalciumphosphat oder jedem anderen osteokonduvtiven, osteoinduktiven, oteogenetischen oder anderen fusionssteigernden Material. - Mit Bezug auf
4 kann alternativ die Kappe130a in der Gestalt einer "Kugel" sein zum Vereinfachen der Einführung. Die Kappe130a hat an ihrem größten Durchmesser einen Durchmesser, welcher gleich dem Kerndurchmesser des Wirbelsäulenfusionsimplantats100 ist, sodass keine zusätzlichen Sperrteile120 ausgebildet sind. - Mit Bezug auf
10 weist das Wirbelsäulenfusionsimplantat100 an einem Ende ein Engriffs-Mittel in der Form eines rechteckigen Schlitzes140 auf zum Eingreifen eines Eintreib-Instruments, welches ein ablösbares Eingriffsmittel hat zum intensiven Ergreifen des rechteckigen Schlitzes140 . Ein mit Gewinde versehener Abschnitt des Eintreib-Instruments, welches sich in einer Ausführungsform wie ein Stab durch das hohle, rohrförmige Element erstreckt und rotationsweise steuerbar ist, wird in eine mit Gewinde versehene Öffnung142 geschraubt. Sobald es an dem Implantat-Eintreibinstrument befestigt ist, kann das Wirbelsäulenfusionsimplantat100 durch ein hohles, zylindrisches Rohr hindurch eingesetzt und in das zylindrische Loch hinein eingetrieben werden, welches quer über den Bandscheibenzwischenraum D gebohrt worden ist. Das Implantat-Eintreibinstrument kann dann mittels eines Hammers oder einer ähnliche Vorrichtung eingestoßen werden zum linearen Vorrücken des Wirbelsäulenfusionsimplantats100 quer über den Bandscheibenzwischenraum D. Sobald das Wirbelsäulenfusionsimplantat100 quer über den Bandscheibenzwischenraum D eingesetzt ist, ergreifen die Sperrteile120 den Knochen der Wirbel V und das Implantat-Eintreibinstrument100 wird aus dem Wirbelsäulenfusionsimplantat100 gelöst. Das Verfahren zum Bohren der Löcher quer über den Bandscheibenzwischenraum D und das zugehörige Instrumentarium dazu sind in WO 94/28824 beschrieben. - Mit Bezug auf
11 –14 ist eine alternative Ausführungsform, welche allgemein mit dem Bezugszeichen200 bezeichnet ist, gezeigt. Obwohl jene Ausführungsform nicht erfindungsgemäß ist, hat sie viele Merkmale mit der Erfindung gemein. Das Wirbelsäulenfusionsimplantat200 ist dem Wirbelsäulenfusionsimplantat100 ähnlich, außer dass die Öffnungen228 mittels des Eingreif-Rands222 von der Vielzahl von Sperrteilen220 halbiert sind. Auf diese Weise sind die Knochen-Eingreif-Ränder von den Öffnungen228 unterbrochen zum Bereitstellen eines "zahn-artigen" Randes, welcher den Knochen des Wirbels V ergreift und einen Presssitz erzeugt zum Verhindern des Zurück-Wanderns des einmal eingesetzten Implantats200 . Es ist verständlich, dass die Anzahl der Öffnungen228 in jeder Orientierung relativ zu den Sperrteilen220 oder anderen Flächen-Unebenheit platziert werden können, ohne vom Umfang de Erfindung abzuweichen. - Mit Bezug auf
15 –22 ist eine alternative Ausführungsform des Wirbelsäulenfusionsimplantats gezeigt, welches allgemein mit dem Bezugszeichen300 bezeichnet ist. Wieder hat diese Ausführungsform, obwohl sie nicht erfindungsgemäß ist, viele Merkmale damit gemeinsam. Das Wirbelsäulenfusionsimplantat300 hat eine im Wesentlichen zylindrische Konfiguration mit Flächen-Unebenheiten zur Stabilisierung des Implantats300 innerhalb des intervertebralen Zwischenraums D. Wie in18 gezeigt, weisen die Flächen-Unebenheiten eine Oberflächen-Riefelung320 auf, wie beispielsweise, aber nicht darauf beschränkt, das gezeigte diamant-artige, knochen-ergreifende Muster. Das Wirbelsäulenfusionsimplantat300 kann die Oberflächen-Riefelung320 überall an der gesamten Außenfläche des Wirbelsäulenfusionsimplantats300 , überall nur an einem Abschnitt der Außenfläche, oder eine Kombination davon haben, ohne vom Umfang der Erfindung abzuweichen. In jenen Umständen, wo es kein ungebohrtes Loch in dem Bandscheibenzwischenraum D vorn an dem Wirbelsäulenimplantat300 gibt, wird zum Widerstehen einer weitern Vorwärts-Vorrückung des Implantats, die Oberflächen-Riefelung320 bevorzugt, da sie einen ausgezeichneten Presssitz mit dem Knochen der Wirbel V produziert und Bewegung in allen Richtungen gleichermaßen und ohne die Tendenz sich selbst vorwärts zu drücken widersteht. - Mit Bezug auf
17 sind zwei Wirbelsäulenfusionsimplantate300a und300b nebeneinander quer über den Bandscheibenzwischenraum D platzierbar, welche die gleichen Abmessungen des in2 gezeigten Bandscheibenzwischenraums D haben, sodass die beiden Wirbelsäulenfusionsimplantate300a und300b einander berühren und daher die kombinierte Gesamtweite der beiden Wirbelsäulenfusionsimplantate300a und300b auf den minimal möglichen Abstand reduzieren, wobei ein im Wesentlichen zylindrisches Implantat eine aufgeraute Oberfläche hat. Auf diese Weise können zwei zylindrische Wirbelsäulenfusionsimplantate300a und300b mit einem ausreichenden Durchmesser zum Überbrücken der Höhe Hs des Bandscheibenzwischenraums D quer über den Bandscheibenzwischenraum D platziert werden, ohne dass sie die Querweite Ws der Wirbelsäule S überschreiten. Die Wirbelsäulenfusionsimplantate300a und300b werden durch lineares Vorrücken wie oben für das Wirbelsäulenfusionsimplantat100 beschrieben eingeführt. - Daher braucht, da kein Gewindeschneiden für die Einführung der Wirbelsäulenfusionsimplantate
300a und300b notwendig ist, kaum oder kein Platz zwischen den Wirbelsäulenfusionsimplantaten300a und300b vorhanden sein, wie im Vergleich zu dem Platz, welcher für eine Gewinde notwendig wäre, wenn mit Gewinde versehene Implantate verwendet werden. Daher können die Wirbelsäulenfusionsimplantate300a und300b zum wesentlichen Reduzieren der kombinierten Gesamtweite näher aneinander platziert werden. - Mit Bezug auf
23 ist ein Segment der Wirbelsäule S gezeigt mit einer alternativen Ausführungsform von zwei Wirbelsäulenfusionsimplantaten900a und900b , welche quer über den Bandscheibenzwischenraum D1 eingesetzt sind. Das Wirbelsäulenfusionsimplantat900a hat eine konkave Fläche902 , welche zum Empfangen der konvexen Fläche900b des Wirbelsäulenfusionsimplantats904 korrespondierend gestaltet ist. Wenn die beiden Wirbelsäulenfusionsimplantate900a und900b nebeneinander platziert sind, paart sich die konkave Fläche902 mit der konvexen Fläche904 , sodass die kombinierte Gesamtweite der beiden Wirbelsäulenfusionsimplantate kleiner als zweimal der maximale Durchmesser M von jenen Implantaten ist. Als Ergebnis können die Vorteile zum Platzieren von zwei Implantaten nebeneinander quer über den Bandscheibenzwischenraum D, welche bezüglich des Abschnitts zum Ergreifen der Wirbel V teilweise zylindrisch sind, erzielt werden, ohne die Weite Ws der Wirbelsäule S zu überschreiten. - Immer noch mit Bezug auf
23 ist eine alternative Ausführungsform des Wirbelsäulenfusionsimplantats der Erfindung gezeigt, welches ein Wirbelsäulenfusionsimplantat1000 aufweist, welches quer über den Bandscheibenzwischenraum D2 der Wirbelsäule S eingesetzt ist. Das Wirbelsäulenfusionsimplantat1000 weist einen ersten zylindrischen Abschnitt1010 und einen zweiten zylindrischen Abschnitt1012 auf und kann jede der mit Bezug auf die oben dargelegten Ausführungsformen beschriebenen Oberflächen-Unebenheiten aufweisen. In der bevorzugten Ausführungsform wird das Wirbelsäulenfusionsimplantat1000 durch lineares Vorrücken in zwei sich überlappende, zylindrische Löcher eingeführt, welche quer über den Bandscheibenzwischenraum D2 gebohrt sind. - Während die Erfindung detailliert mit Bezug auf die bevorzugten Ausführungsformen beschrieben worden ist, ist es verständlich, dass andere Variationen der Erfindung ersonnen werden können, welche nicht vom Umfang der Erfindung, wie von den beigefügten Ansprüchen definiert, abweichen.
Claims (20)
- Wirbelsäulenfusionsimplantat, welches aus einem geeigneten Material hergestellt ist und welches zur Humanimplantation zumindest teilweisen quer über die chirurgisch korrigierte Höhe eines Bandscheibenzwischenraumes (D1) zwischen benachbarte Wirbelkörper (V) vorgesehen ist, wobei das Implantat aufweist: ein Element (
900a ) mit einer Außenseite und einer Längsachse, wobei die Außenseite ein Vorder-Ende, ein Hinter-Ende und eine Länge dazwischen aufweist, wobei die Außenseite entlang der Längsachse des Implantats einen teilweise kreisförmigen Querschnitt aufweist, wobei der teilweise kreisförmige Querschnitt der Außenseite Bogen-Abschnitte aufweist, welche einander gegenüberliegen und welche Bogen des gleichen Kreises sind und eingerichtet sind, je in einen der benachbarten Wirbelkörper (V) eindringend einzugreifen, wenn sie zwischen den Wirbelkörpern (V) und über die Höhe des Bandscheibenzwischenraumes (D1) eingesetzt sind, wobei jeder der gegenüberliegenden Bogen-Abschnitte mindestens eine dort hindurch passierende Öffnung (128 ;228 ;328 ) aufweist, um Knochenwachstum vom angrenzenden Wirbelkörper zum benachbarten Wirbelkörper zu ermöglichen, dadurch gekennzeichnet, dass der teilweise kreisförmige Querschnitt einen konkaven Abschnitt (902 ) aufweist, der eingerichtet ist, um mit einem konvexen Abschnitt (904 ) eines zweiten Implantats (900b ) zusammenzupassen, wenn über die Höhe des Bandscheibenzwischenraums (D1) implantiert, sodass die kombinierte Breite der beiden Implantate kleiner als das Zweifache des maximalen Durchmessers der beiden Implantate ist. - Wirbelsäulenfusionsimplantat gemäß Anspruch 1, wobei das Element ein hohles Inneres aufweist, welches ein fusionsbeschleunigendes Material aufnehmen kann.
- Wirbelsäuleninfusionsimplantat gemäß Anspruch 2, wobei das Implantat mindestens eine abnehmbare Kappe (
130 ;230 ;330 ) zum Verschließen zumindest eines Endes des hohlen Inneren aufweist. - Wirbelsäulenfusionsimplantat gemäß einem der vorhergehenden Ansprüche, ferner aufweisend eine Mehrzahl von Ring-Sperrteilen (
120 ;220 ) an der Außenseite des Elements (900a ) zum Eingreifen in die Wirbelkörper (V), um das Implantat an Ort und Stelle zu halten, wobei die Ring-Sperrteile am Umfang des Elements sind. - Wirbelsäulenfusionsimplantat gemäß Anspruch 4, wobei die Sperrteile (
120 ;220 ) einer einzigen Richtung zugewandt sind. - Wirbelsäulenfusionsimplantat gemäß Anspruch 4 oder 5, wobei die Sperrteile (
120 ;220 ) ein abgewinkeltes Segment (124 ;224 ) aufweisen, welches in einem knochen-eingreifenden Rand (122 ;222 ) am Umfang des Elements endet. - Wirbelsäulenfusionsimplantat gemäß einem der Ansprüche 4, 5 oder 6, wobei die mindestens eine Öffnung (
228 ) die Sperrteile (220 ) unterbricht. - Wirbelsäulenfusionsimplantat gemäß einem der Ansprüche 4, 5, 6 oder 7, ferner aufweisend eine Mehrzahl von Öffnungen (
128 ), die zumindest teilweise zwischen den Sperrteilen angeordnet ist. - Wirbelsäulenfusionsimplantat gemäß einem der Ansprüche 1 bis 3, ferner aufweisend eine Riefelung (
320 ) an der Außenseite des Elements zum Eingreifen in die Wirbelkörper (V), um das Implantat an Ort und Stelle zu halten. - Wirbelsäulenfusionsimplantat gemäß einem der vorhergehenden Ansprüche, wobei das Implantat ein Knochenhineinwachs-Material aufweist.
- Wirbelsäulenfusionsimplantat gemäß Anspruch 10, wobei das Implantat ein Knochenhineinwachs-Material aufweist, welches ein anderes als Knochen ist.
- Wirbelsäulenfusionsimplantat gemäß einem der vorhergehenden Ansprüche, wobei die mindestens eine Öffnung (
128 ;228 ;328 ) ein fusionsbeschleunigendes Material aufbewahren kann. - Wirbelsäulenfusionsimplantat gemäß einem der vorhergehenden Ansprüche, wobei das Implantat in Kombination mit einem fusionsbeschleunigenden Material ist.
- Wirbelsäulenfusionsimplantat gemäß Anspruch 13, wobei das fusionsbeschleunigende Material zumindest entweder Knochen, Knochenmorphogenese-Protein, Hydroxylapatit oder Hydroxylapatit-Tricalciumphosphat aufweist.
- Wirbelsäulenfusionsimplantat gemäß einem der vorhergehenden Ansprüche, wobei das Implantat aus einem künstlichen Material hergestellt ist.
- Wirbelsäulenfusionsimplantat gemäß Anspruch 15, wobei das künstliche Material ein Implantatqualitätsmetall aufweist.
- Wirbelsäulenfusionsimplantat gemäß einem der vorhergehenden Ansprüche in Kombination mit einem zweiten Fusionsimplantat (
900b ). - Wirbelsäulenfusionsimplantat gemäß einem der Ansprüche 1 bis 12 in Kombination mit einem Transplantat-Material.
- Wirbelsäulenfusionsimplantat gemäß einem der vorhergehenden Ansprüche, in Kombination mit einem Implantat-Eintreiber, der konfiguriert ist, das Wirbelsäulenfusionsimplantat in die Wirbelsäule einzusetzen.
- Wirbelsäulenfusionsimplantat gemäß einem der vorhergehenden Ansprüche in Kombination mit einer Hohlröhre, die eine Öffnung zum Bereitstellen eines geschützten Zugangs zu dem Bandscheibenzwischenraum (D) und den benachbarten Wirbelkörpern (V) und zum Führen des Einsetzens des Wirbelsäulenfusionsimplantats in die Wirbelsäule aufweist.
