Description
- Technisches Gebiet
- Die vorliegende Erfindung betrifft einen Stent mit Flexibilität in axialer Richtung und Spannkraft in seiner expandierten Form.
- Hintergrundtechnik
- Ein Stent wird im Allgemeinen als eine röhrenförmige Struktur verwendet, die innerhalb des Hohlraumes eines Ganges belassen wird, um einer Obstruktion abzuhelfen. Im Allgemeinen werden Stents in den Hohlraum in einer nicht-expandierten Form eingeführt und dann autonom (Tihon et al. (1994) US-Patent Nr. 5,356,423) oder mit der Hilfe einer zweiten Vorrichtung in situ expandiert. Ein typisches Verfahren zum Expandieren erfolgt durch die Verwendung eines an einem Katheter befestigten Angioplastie-Ballons, der innerhalb des verengten Gefäßes oder Körperdurchganges aufgeblasen wird, um die Obstruktionen abzuscheren oder zu zerreißen, die mit den Wandbestandteilen des Gefäßes verbunden sind, und um einen vergrößerten Hohlraum zu erhalten. Bei Abwesenheit eines Stents kann es zu einer Restenose als Ergebnis eines elastischen Zurückspringens der Stenoseläsion kommen. Obwohl eine Vielzahl von Stentkonstruktionen beschrieben worden sind, haben diese Konstruktionen unter einer Vielzahl von Beschränkungen gelitten. Diese umfassen Beschränkungen hinsichtlich der Größe des Stents (Cardon et al. 1995, US-Patent Nr. 5,383,892). Cardon et al. beschreiben einen Stent mit steifen Enden (8 mm) und einem flexiblen mittleren Abschnitt von 7–21 mm. Die Vorrichtung wird aus einer Vielzahl von Teilen gebildet und ist entlang der Längsachse nicht flexibel. Eine weitere Stentkonstruktion, die steife und flexible Segmente aufweist, ist von Pinchasik et al., US-Patent 5,449,373 (1995) beschrieben worden.
- WO-A-9603092, veröffentlicht nach dem Prioritätstag dieser Anmeldung, beschreibt einen Stent, der eine Röhre mit einer mit Mustern versehenen Form umfasst, die erste und zweite Meandermuster mit Achsen in ersten und zweiten Richtungen aufweist.
- Ein weiterer Stent (EP-A-0734698, veröffentlicht nach dem Prioritätstag dieser Anmeldung) weist eine Längsachse auf und stellt Flexibilität in Längsrichtung bereit. Er umfasst eine Vielzahl von in Längsrichtung angeordneten Bändern, wobei ein jedes Band eine im Allgemeinen ungedämpfte Welle mit einer Raumfrequenz entlang einer Strecke parallel zur Längsachse definiert, und eine Vielzahl von Verbindungsstücken, um die Bänder in einer röhrenförmigen Struktur zu halten.
- Andere Stents werden als in der Längsrichtung flexibel beschrieben (Lau et al. (1995), US-Patent Nr. 5,421,955 und EP-Anmeldung 540290 A2, A3), bestehen aber aus einer Vielzahl von zylindrischen Elementen, die durch flexible Stücke verbunden sind. Diese Konstruktion hat wenigstens einen wichtigen Nachteil, dass, z. B., nach dieser Konstruktion vorstehende Kanten auftreten können, wenn der Stent um eine Kurve gebogen wird, was die Möglichkeit eines unbeabsichtigten Hängenbleibens des Stents an Plaques eröffnet, die auf den Arterienwänden abgelagert sind. Dies kann dazu führen, dass der Stent eine Embolie bildet oder aus der Position herausrutscht und der Innenschicht von gesunden Gefäßen Schaden zufügt (siehe
1(a) unten). - Somit umfassen Stents, die in der Technik bekannt sind und die durch Ballon-Angioplastie expandiert werden können, im Allgemeinen eine Flexibilität in axialer Richtung, um eine Expansion zu erlauben und eine strukturelle Gesamtintegrität bereitzustellen.
- Zusammenfassung der Erfindung
- Die vorliegende Erfindung überwindet einige der erkannten Unzulänglichkeiten von Stents nach dem Stand der Technik, indem ein Stent mit Flexibilität in axialer Richtung bereitgestellt wird. In einer bevorzugten Ausführungsform weist der Stent ein erstes Ende und ein zweites Ende mit einem Zwischenabschnitt zwischen den beiden Enden auf. Der Stent weist weiter eine Längsachse auf und umfasst eine Vielzahl von in Längsrichtung angeordneten Bändern, wobei ein jedes Band im Allgemeinen eine ungedämpfte Welle entlang einer Strecke parallel zur Längsachse definiert. Eine Vielzahl von Verbindungsstücken hält die Bänder in einer röhrenförmigen Struktur, wobei ein jedes in Längsrichtung angeordnete Band des Stents mit einer Vielzahl von periodischen Stellen durch ein kurzes Umfangsverbindungsstück mit einem benachbarten Band verbunden ist. Die Raumfrequenz der mit einem jeden der Bänder verbundenen Welle ist in einer ersten Endregion, die unmittelbar am ersten Ende liegt, und in einer zweiten Endregion, die unmittelbar am zweiten Ende liegt, verglichen mit der Raumfrequenz der Welle in dem Zwischenabschnitt verringert. In einer Ausführungsform der Erfindung weist die mit einem jeden der Bänder verbundene Welle in dem Zwischenabschnitt in etwa die gleiche Raumgrundfrequenz auf und die Bänder sind so angeordnet, dass die mit ihnen verbunden Wellen räumlich so ausgerichtet sind, dass sie im Allgemeinen miteinander in Phase sind.
- Insbesondere bei einer jeden aus einer ersten Gruppe von gemeinsamen axialen Stellen gibt es ein Umfangsverbindungsstück zwischen einem jeden von einem ersten Satz von benachbarten Paaren von Bändern.
- Bei einer jeden von einer zweiten Gruppe von gemeinsamen axialen Stellen gibt es ein Umfangsverbindungsstück zwischen einer jeden von einem zweiten Satz von benachbarten Reihen von Bändern, wobei, entlang der Längsachse, eine gemeinsame axiale Stelle alternierend in der ersten Gruppe und der zweiten Gruppe auftritt, und der erste und der zweite Satz so ausgewählt sind, dass ein gegebenes Band mit einem benachbarten Band nur an einer der ersten und zweiten Gruppe von gemeinsamen axialen Stellen verbunden ist.
- In einer weiteren Ausführungsform der Erfindung ist die Raumfrequenz der Bänder in der ersten und zweiten Endregion um 20% verglichen mit der Raumfrequenz der Bänder in dem Zwischenabschnitt verringert. Die erste Endregion kann zwischen dem ersten Ende und einem Satz aus Umfangsverbindungsstücken angeordnet sein, die am nächsten zu dem ersten Ende liegen, und die zweite Endregion liegt zwischen dem zweiten Ende und einem Satz aus Umfangsverbindungsstücken, die am nächsten zu dem zweiten Ende liegen. Die Weiten von korrespondierenden Abschnitten der Bänder in diesen Endregionen, gemessen in einer Umfangsrichtung, sind in der ersten und zweiten Endregion größer als in dem Zwischenabschnitt. Ein jedes Band umfasst ein Ende an einem jeden des ersten und zweiten Endes und die benachbarten Paare von Bändern sind an ihren Enden miteinander verbunden, um eine geschlossene Schleife zu bilden.

- Verbesserungen, die durch Ausführungsformen der vorliegenden Erfindung geboten werden, umfassen (a) erhöhte Flexibilität in zwei Ebenen des nicht-expandierten Stents, während die radiale Stärke und der hohe Prozentsatz an offener Fläche nach der Expansion beibehalten wird; (b) gleichmäßiger Druck auf den expandierenden Stent, der konsistenten und durchgehenden Kontakt des expandierten Stents mit der Arterienwand gewährleistet; (c) Vermeiden von vorstehenden Teilen während des Biegens; (d) Entfernen von bestehenden Einschränkungen bezüglich der maximalen Länge von Stents; und Verringern von irgendwelchen Verkürzungswirkungen während der Expansion des Stents.
- In einer bevorzugten Ausführungsform wird ein expandierbarer zylindrischer Stent mit einer mit Fenstern versehenen Struktur zur Anordnung in einem Blutgefäß, Gang oder Hohlraum bereitgestellt, um das Gefäß, den Gang oder den Hohlraum offen zu halten, insbesondere, um einen Arterienabschnitt vor Restenose nach Angioplastie zu schützen. Der Stent kann in Umfangsrichtung expandiert werden und in einer expandierten Konfiguration gehalten werden, die in Umfangsrichtung fest ist. Der Stent ist in Axialrichtung flexibel und wenn an einem Band gebogen, vermeidet der Stent jegliche nach außen vorspringenden Komponententeile.
1 zeigt, was mit einem Stent geschieht, der eine ähnliche Konstruktion aufweist wie eine bevorzugte Ausführungsform, aber stattdessen eine Serie von in Umfangsrichtung angeordneten Bändern verwendet, wenn er in einer Art und Weise gebogen wird, wie es wahrscheinlich innerhalb eines Hohlraums des Körpers erfolgen wird. Ein Stent mit einer in Umfangsrichtung erfolgenden Anordnung von Bändern (1 ) erfährt eine Wirkung analog einer Reihe von Eisenbahnwagen auf einer Schiene. Während die Reihe der Eisenbahnwagen um eine Kurve biegt, wird die Ecke eines jeden Wagens, der um die Kurve biegt, nach dem Koppeln über die Kontur des Schienenstrangs vorstehen. In ähnlicher Weise weisen die schlängelnden Umfangsbänder Vorsprünge (2 ) über die Oberflächen des Stents hinaus auf, wenn sich der Stent biegt. Im Gegensatz dazu vermeidet die neue Konstruktion der Ausführungsform, wie sie in den1(c) und1(d) und7 gezeigt ist, bei der die Bänder (3 ) in Axialrichtung flexibel sind und entlang der Längsachse angeordnet sind, den Güterwageneffekt, wenn der Stent gebogen wird, so dass die gebogenen Bänder (4 ) nicht über das Profil der Kurve des Stents vorstehen. Weiterhin wird jegliches Ausstellen an den Enden des Stents, das bei einem Stent mit einer gleichmäßigen Struktur auftreten kann, im Wesentlichen durch Einführen einer Modifikation an den Enden des Stents eliminiert. Diese Modifikation umfasst die Verringerung der Raumfrequenz und Erhöhung der Weite der korrespondierenden Bänder in Umfangsrichtung (LA und A) verglichen mit derjenigen des Zwischenabschnittes (LB und B). In einer Ausführungsform der Erfindung kann die Raumfrequenz LA um 0–50% bezüglich LB verringert sein und die Weite A kann im Bereich von 0–150% bezüglich B erhöht sein. Andere Modifikationen am Ende des Stents können die Erhöhung der Dicke der Wand des Stents und selektives Elektropolieren umfassen. Diese Modifikationen schützen die Arterie und einen jeglichen Plaque vor dem Abreißen, was durch die Stentenden während des Einführen des Stents geschehen kann. Die Modifikation kann auch an den Enden des Stents eine erhöhte Strahlenundurchlässigkeit bereitstellen. Entsprechend kann es möglich sein, den Stent genauer zu lokalisieren, wenn er einmal im Körper platziert ist. - Die in
2 und6 gezeigte Ausführungsform hat den einzigartigen Vorteil, dass sie wirksame „Federn" sowohl in Umfangs- als auch Längsrichtung aufweist, als Punkt (5 ) bzw. (6 ) gezeigt. Diese Federn verleihen dem Stent die notwendige Flexibilität, um sowohl Blutgefäße in dem Körper mit verringerter Reibung zu durchfahren als auch an der ausgewählten Stelle in einer An und Weise zu expandieren, die die endgültigen notwendigen expandierten Größen ohne ungebührliche Kraft bereitstellt, während die strukturelle Spannkraft der expandierten Struktur beibehalten wird. - Wie sowohl in den
2 ,4 als auch6 gezeigt ist, onduliert ein jegliches Längsband durch in etwa zwei Zyklen bevor ein Umfangsverbindungsstück mit einem benachbarten Band gebildet wird. Vor der Expansion kann die mit einer jeden der Bande verbundene Welle in etwa die gleiche Raumgrundfrequenz aufweisen und die Bänder sind so angeordnet, dass die mit ihnen verbundenen Wellen räumlich ausgerichtet sind, so dass sie im Allgemeinen wie in6 gezeigt miteinander in Phase sind. - Die ausgerichteten Bänder auf der Längsachse sind an einer Vielzahl von periodischen Stellen durch kurze Umfangsverbindungsstücke zu einem jeden Band verbunden. Man betrachte eine erste gemeinsame Achsenstelle, wie sie durch die Linie X-X in
4 und6 gezeigt ist. Hier wird ein benachbartes Paar von Bändern durch Umfangsverbindungsstück7 verbunden. In ähnlicher Weise werden auch andere Paare von Bändern an dieser gemeinsamen Achsenstelle miteinander verbunden. An einer zweiten gemeinsamen Achsenstelle, gezeigt in6 durch die Linie Y-Y, wird ein benachbartes Paar von Bändern durch das Umfangsverbindungsstück8 verbunden. Ein jegliches gegebene Paar von Bändern, das an X-X miteinander verbunden ist, ist jedoch nicht an Y-Y miteinander verbunden und umgekehrt. Das X-X-Muster aus Verbindungen wiederholt sich an der gemeinsamen Achsenposition Z-Z. Im Allgemeinen gibt es somit zwei Gruppen von gemeinsamen Achsenstellen. In einer jeden der Achsenstellen von einer Gruppe gibt es Verbindungsstücke zwischen den gleichen Paaren von benachbarten Bändern und die Gruppen alternieren entlang der Längsachse der Ausführungsform. Auf diese Weise werden die Umfangsfeder5 und die Längsfeder6 bereitgestellt. - Ein Merkmal des Expansionsvorgangs besteht darin, dass das Muster aus offenen Räumen im Stent der Ausführungsform von
2 vor der Expansion verschieden ist von dem Muster des Stents nach der Expansion. Insbesondere ist in einer bevorzugten Ausführungsform das Muster aus offenen Räumen des Stents vor der Expansion schlangenförmig, während sich nach der Expansion das Muster einer Diamantenform (3a, 3b) annähert. In Ausführungsformen der Erfindung kann die Expansion durch Anwendung von Druck durch einen expandierenden Ballon oder durch andere mechanische Mittel erreicht werden. - Im Verlauf der Expansion neigen, wie in
3 gezeigt, die wellenförmigen Bänder dazu, gerader zu werden. Wenn die Bänder gerader werden, werden sie steifer und widerstehen dadurch vergleichsweise großen Radialkräften.3 zeigt, wie die radial Expansion des Stents dazu führt, dass sich die Fenster in eine Diamanten-ähnliche Form öffnen, wobei die maximale Belastung an den Spitzen des Diamanten entlang der Längsachse aufgewandt wird. Als eine finite Elementeanalyse einschließlich Dehungsstudien am Stent durchgeführt wurde, fand man, dass die maximale Dehnung an den Bändern und Verbindungsstücken anlag und unterhalb des Maximums lag, das als notwendig identifiziert wurde, um die strukturelle Integrität beizubehalten. - Die Optimierung der Dehnung des Stents wird durch Erzeugen eines Wenderadiuses, der so groß wie möglich ist, in der Welle erreicht, die mit einem jeden Band im nicht-expandierten Stent verbunden ist. Dies wird ausgeführt, während eine ausreichende Anzahl von Bändern und Verbindungsstücken beibehalten wird, um die strukturelle Integrität des Stents nach Expansion beizubehalten. In einer Ausführungsform der Erfindung kann der Stent weniger als 0,57 für 316 L rostfreien Stahl sein. Der Expansionsdruck kann 1,0–7,0 Atmosphären betragen. Die Anzahl von Bändern und die Raumfrequenz der Welle entlang der Längsachse beeinflusst ebenfalls die Anzahl der Umfangsverbindungstücke. Die Umfangsverbindungsstücke tragen zur strukturellen Integrität während der Anwendung von Radialkraft bei, die bei der Expansion des Stents und bei der Beibehaltung der expandierten Form aufgewandt wird. Obwohl nicht auf einen einzelnen Satz an Parametern beschränkt, kann ein Beispiel für einen Stent gemäß der Erfindung mit einer Längsachse und einer Flexibilität in Längsrichtung von der in
6 gezeigten Form einen Stent mit einem expandierten Durchmesser von 4 mm und einer Länge von 30 mm umfassen, der beispielsweise 8 bis 12 Reihen, genauer 10 Reihen, und etwa 6 bis 10 Schlitze, genauer 8 Schlitze (ein Schlitz ist in6 als sich zwischen X und Z erstreckend dargestellt), mit einer Wellenamplitude von etwa 1/4–1/10 einer Schlitzlänge, genauer 1/8 einer Schlitzlänge, umfassen. - Der Stent kann mittels bekannter Verfahren hergestellt werden. Zum Beispiel kann der Stent aus einer Hohlröhre oder einer geformten Röhre aus rostfreiem Stahl hergestellt werden, die mittels Laser, funkenerosivem Fräsen (EDM), chemischem Ätzen oder anderen Mitteln ausgeschnitten werden kann. Der Stent wird in den Körper eingesetzt und an der erwünschten Stelle in nicht-expandierter Form plaziert. In einer bevorzugten Ausführungsform wird die Expansion des Stents in einem Blutgefäß mittels eines Ballonkatheters bewirkt, wo der Enddurchmesser des Stents eine Funktion des Durchmessers des verwendeten Ballonkatheters ist. Im Gegensatz zu Stents nach dem Stand der Technik kann der Stent gemäß der Erfindung mit einer jeglichen erwünschten Länge, am bevorzugtestens mit einer Nominallänge von 30 mm hergestellt werden, die in Inkrementen, beispielsweise 1,9 mm Inkrementen, verlängert oder verkürzt werden kann.
- Es wird anerkannt werden, dass ein Stent gemäß der vorliegenden Erfindung aus einem Formgedächnismaterial hergestellt sein kann, einschließlich, z. B. einer geeigneten Legierung aus Nickel und Titan; oder rostfreiem Stahl. In dieser Ausführungsform kann der Stent, nachdem er geformt worden ist, komprimiert werden, um einen Raum einzunehmen, der ausreichend klein ist, um seine Einführung in ein Blutgefäß oder ein anderes Gewebe durch Einführungsmittel zu erlauben, wobei die Einführungsmittel einen geeigneten Katheter oder einen flexiblen Stab umfassen. Beim Austritt aus dem Katheter kann der Stent so konfiguriert werden, dass er in die erwünschte Konfiguration expandiert, wobei die Expansion automatisch erfolgt oder durch eine Druckänderung, Temperaturänderung oder Elektrostimulation ausgelöst wird.
- Eine Ausführungsform des verbesserten Katheters kann nicht nur in Blutgefäßen, wie oben beschrieben, verwendet werden, sondern in einem jeglichen Gefäßsystem des Körpers, wie dem Gallengang, dem Urinsystem, dem Verdauungstrakt und den Gängen des reproduktiven Systems sowohl bei Männern als auch bei Frauen.
Claims (9)
- Stent, der erste und zweite Enden mit einem dazwischenliegenden Zwischenabschnitt umfasst, wobei der Stent weiter eine Längsachse aufweist und axiale Flexibilität bereitstellt, umfassend: eine Vielzahl von in Längsrichtung angeordneten Bändern (
5 ), wobei ein jedes Band eine im allgemeinen ungedämpfte Welle mit einer Raumfrequenz entlang einer Strecke parallel zur Längsachse aufweist; und eine Vielzahl von Verbindungsstücken (8 ) zum Halten der Bänder in einer röhrenförmigen Struktur, wobei jedes Band, an einer Vielzahl von periodischen Stellen, durch ein kurzes umfangsmäßiges Verbindungsstück mit einem benachbarten Band verbunden ist, dadurch gekennzeichnet, dass: die Raumfrequenz der mit einem jeden der Bänder (5 ) verbundenen Welle in einer ersten Endregion, die proximal zu dem ersten Ende liegt, und in einer zweiten Endregion, die proximal zum zweiten Ende liegt, verglichen mit der Raumfrequenz der Welle in dem Zwischenabschnitt verringert ist. - Stent nach Anspruch 1, wobei die mit einem jeden der Bänder verbundene Welle etwa die gleiche Grundraumfrequenz für den Zwischenabschnitt aufweist.
- Stent nach Anspruch 2, wobei die Bänder so angeordnet sind, dass die mit ihnen verbundenen Wellen räumlich so ausgerichtet sind, dass sie im allgemeinen miteinander in Phase sind.
- Stent nach Anspruch 3, wobei an einer jeden von einer ersten Gruppe aus gemeinsamen axialen Positionen ein umfangsmäßiges Verbindungsstück zwischen einem jeden eines ersten Satzes von benachbarten Paaren von Bändern existiert.
- Stent nach Anspruch 4, wobei an einer jeden von einer zweiten Gruppe aus gemeinsamen axialen Positionen ein umfangsmäßiges Verbindungsstück zwischen einer jeden eines zweiten Satzes von benachbarten Reihen von Bändern existiert, wobei, entlang der Längsachse, eine gemeinsame axiale Position abwechselnd in der ersten Gruppe und in der zweiten Gruppe auftritt, und die ersten und zweiten Sätze so ausgewählt sind, dass ein gegebenes Band mit einem benachbarten Band an nur einer der ersten und zweiten Gruppe von gemeinsamen axialen Positionen verbunden ist.
- Stent nach Anspruch 1, wobei die Raumfrequenz um etwa 20% verglichen mit der Raumfrequenz der Welle in dem Zwischenabschnitt verringert ist.
- Stent nach Anspruch 1 oder 6, wobei die erste Endregion zwischen dem ersten Ende und einem Satz von umfangsmäßigen Verbindungsstücken, die am nächsten zu dem ersten Ende liegen, und die zweite Endregion zwischen dem zweiten Ende und einem Satz von umfangsmäßigen Verbindungsstücken, die am nächsten zu dem zweiten Ende liegen, liegt.
- Stent nach Anspruch 1 oder 7, wobei Weiten korrespondierender Abschnitte der Bänder, gemessen in einer Umfangsrichtung, in den ersten und zweiten Endregionen größer sind als in dem Zwischenabschnitt.
- Stent nach Anspruch 1, wobei jedes Band (
5 ) einen Endpunkt an einem jeden ersten und zweiten Ende aufweist und die benachbarten Bänderpaare an ihren Endpunkten verbunden sind, um eine geschlossene Schleife zu bilden.
