- Diese Anmeldung betrifft ein Protein von 18-19 kDa oder ein Derivat oder Fragment davon, das aus Helicobacter pylori erhalten wird, ein Bacterioferritin, das durch Sequenzen in unseren vorherigen Patentanmeldungen PCT IE95/00036 und PCT IE95/00037 definiert ist, eine rekombinante Form dieses Proteins, Verfahren für die Verwendung dieses Proteins als Impfstoff, um einen immunologischen Schutz gegen eine H. pylori-Infektion bereitzustellen, und Verfahren für die Verwendung dieses Proteins in diagnostischen Testansätzen, die H. pylori betreffen.
- HINTERGRUND
- Helicobacter pylori ist ein weit verbreiteter Organismus, der bei Magenbiopsien bei etwa 30% der Bevölkerung unter 40 Jahren gefunden wird, wobei seine Inzidenz danach zunimmt. Der Organismus ist eine der Ursachen für chronische Gastritis beim Menschen (vergl. z. B. Marshall & Warren, 1984*; Glaser, 1990*). Epidemiologische Studien haben gezeigt, dass H. pylori am häufigsten in Verbindung mit Gastritis gefunden wird. Serologische Untersuchungen haben gezeigt, dass Beweise für eine aktuelle oder frühere Infektion bei 30-50% einer zufällig ausgewählten Population an Blutspendern gefunden werden können. Eine direkte ursächliche Beziehung zu Zwölffingerdarm-Geschwüren wurde nicht eindeutig bewiesen. Der Organismus wurde jedoch bei 95% der Patienten mit einem Zwölffingerdarm-Geschwür aufgefunden. Darüber hinaus führt die Ausrottung des Organismus zu einer raschen Abheilung des Geschwürs (vergl. z. B. Rauws & Tygat, 1990*). Diese Daten stellen einen starken Beweis dafür dar, dass H. pylori ein entscheidender Faktor bei der Entwicklung eines Zwölffingerdarm-Geschwürs ist.
- Weitere Hinweise für eine pathogene Beteiligung von H. pylori an diesen Krankheitszuständen lieferten Untersuchungen an gnotobiotischen Ferkeln (Lambert et al., 1987*) und die Erfüllung der Kochschen Postulate bei menschlichen Freiwilligen (Marshall et al., 1985*; Morris & Nicholson, 1987*).
- Darüber hinaus gibt es nun starke Indizienbeweise für eine Beteiligung von H. pylori an der Pathogenese von Magenkrebs (vergl. z. B. Jiang et al., 1987*; Lambert et al., 1986*; Crabtree et al., 1992; 1993; Forman et al., 1990, 1991; Nomura et al., 1991; Parsonnet et al., 1991; Correa et al., 1990). Erst kürzlich zeigte die durch Forman geführte Eurogast-Studiengruppe (1993), dass eine signifikante Beziehung zwischen einer H. pylori-Seropositivität und der Sterblichkeit und der Inzidenz bei Magenkrebs besteht. Tatsächlich gibt es nun eine überzeugende Menge an Literatur, die eine Infektion mit H. pylori in einem beträchtlichen Anteil von oberen Gastrointestinaltrakt-Erkrankungen impliziert. Es wurde eine Reihe an Hypothesen für die pathogenen Mechanismen der durch H. pylori induzierten Magen und Darm-Erkrankungen vorgeschlagen, einschließlich der Produktion von Cytotoxinen und der mechanischen Zerstörung des Epithels (vergl. z. B. Glaser, 1992*). Interessanterweise bleiben jedoch viele infizierte Personen ohne Symptome, obwohl das Pathogen permanent vorhanden ist (Taylor & Blaser, 1991*).
- Die Diagnose von Infektionen mit H. pylori basiert hauptsächlich auf der Histologie und der Kultur von Magen-Biopsie-Proben oder auf indirekten Verfahren, die auf Urease-Aktivität basieren. Es wurden verschiedene seroiogische Testansätze für den Nachweis von Anti-H. pylori-Antikörpern bei epidemiologischen Studien entwickelt, daneben gibt es eher molekular orientierte Ansätze wie z. B. die Clonierung von spezifischen Antigenen aus H. pylori-Arten für die Verwendung bei zum Beispiel auf PCR basierenden serologischen Untersuchungen (vergl. Z. B. Clayton et al., 1989). Die Verwendung von rekombinanten artspezifischen Antigenen hat jedoch noch keine verbreitete Anwendung gefunden, und daher verwendet die Mehrzahl der derzeit gebräuchlichen und auf Immunsorbens basierenden Testansätze verschiedene subzelluläre Fraktionen von H. pylori als Quelle des Antigens. Die Proteinfraktionen, die bei diesen Testansätzen verwendet werden, sind in ihrer Zusammensetzung häufig heterogen, ebenso wie die Verfahren für ihre Herstellung. Interessanterweise haben eine Reihe von Gruppen die Empfindlichkeit und die Spezifität zwischen den Testansätzen verschiedener kommerziell erhältlicher ELISA-Kits, die spezifisch für serologische Studien hergestellt wurden, miteinander verglichen. Es kam nicht überraschend, dass man eine beträchtliche Variation zwischen den Testansätzen beobachtete. Daher muss man vorsichtig sein, bevor ein bestimmtes Proteinpräparat bei einem solchen Immunsorbens-Testansatz verwendet wird, insbesondere im Hinblick auf die signifikante genetische Heterogenität der unterschiedlichen H. pylori-Stämme (vergl. z. B. mit Xia et al., 1994; Owen et al., 1991).
- WO 93/18150 beschreibt das Cytotoxin (CT)-Protein aus Helicobacter pylori. („Cytotoxin associated immunodominant (CAI) antigen and the hegt shock protein (hsp60) for use as vaccine candidates against Helicobacter pylori").
- Wir haben die Prävalenz der Immun-Reaktivität gegen H. pylori sowohl bei infizierten als auch bei nicht infizierten Individuen untersucht und herausgefunden, dass die nicht infizierten Individuen eine starke Antwortreaktion gegen H. pylori sowohl in ihrem B-Zellen- als auch in ihrem T-Zellen-System aufweisen. Bei diesem Ansatz verwendeten wir die Western-Blot-Analyse, um die Antigenspezifität von systemischen Antwortreaktionen gegen H. pylori sowohl bei gesunden als auch bei mit H. pylori infizierten Individuen zu untersuchen, und zeigten, dass die Inzidenz einer Seropositivität bei H. pylori-negativen Individuen wesentlich größer ist, als vorher gezeigt wurde. Weiterhin zeigten wir, dass bei der Mehrzahl der H. pylori-negativen Individuen Antikörper gegen ein Protein von 25 kDa nachweisbar sind. Diese wurden unter Verwendung eines von uns modifizierten Verfahrens nachgewiesen, das wir „Gesteigerte Chemilumineszenz" nennen. Gesteigerte Chemilumineszenz bei der Western-Blot-Analyse offenbart, dass die Mehrzahl der nicht infizierten Individuen Antikörper besitzen, die für H. pylori spezifisch sind, und Antigene erkennen, die in anderen Mikroorganismen nicht vorhanden sind. Eines dieser Antigene, das am häufigsten erkannt wird, ist ein Protein von 18-19 kDa, das für H. pylori spezifisch zu sein scheint. Daher legen diese Daten nahe, dass eine Immunisierung mit dem 18-19 kDa-Protein oder einer Untereinheit davon, das Potenzial aufweisen könnte, Individuen, die entweder mit dem Organismus nicht infiziert sind, oder Individuen, die von dem Organismus durch eine antibakterielle Behandlung befreit wurden, eine schützende Immunität zu verleihen. Wir haben von diesem Protein die N-terminale und interne Aminosäuresequenzen abgeleitet.
- AUSSAGEN DER ERFINDUNG
- Die Erfindung wird in den Ansprüchen definiert.
- Der durch ein Verfahren der Erfindung hergestellte Impfstoff kann einen pharmazeutisch verträglichen Träger enthalten.
- Der Impfstoff kann mit einem geeigneten Adjuvans wie z. B. Interleukin 12 oder einem Hitzeschockprotein oder mit beiden kombiniert werden.
- Der Impfstoff kann mindestens ein anderes pharmazeutisches Produkt umfassen wie Z. B. ein Antibiotikum und/oder ein antibakterielles Mittel wie Wismutsalze. Typischerweise wird das Antibiotikum ausgewählt aus einem oder mehreren von Metronidazol, Amoxycillin, Tetracyclin, Erythromycin, Clarithromycin oder Tinidazol.
- Der Impfstoff kann in einer Form für eine orale, intranasale, intravenöse oder intramuskuläre Verabreichung vorliegen.

- GENAUE BESCHREIBUNG DER ERFINDUNG
- Wir haben DNA-Sequenz-Information erzeugt, bei der das 18-19 kDa-Protein als ein Bacterioferritin identifiziert wurde. Wir haben ebenfalls ein rekombinantes 18 kDa-Protein erzeugt und dieses in E. coli exprimiert. Wir fanden heraus, dass dieses rekombinante Protein durch Antiseren aus Individuen, die für Antikörper gegen das 18 kDa große Bacterioferritin aus Helicobacter positiv waren, immunologisch erkannt wird. Dieses rekombinante Protein soll die Grundlage für einen potenziellen Impfstoff gegen H. pylori bilden.

- Clonierung und Expression des Gens für das 18 kDa-Protein aus Helicobacter pylori
- Die Desoxyribonucleinsäure (DNA) wurde wie bei Silhavy et al. (1984) beschrieben aus Helicobacter pylori extrahiert.
- Es wurden Oligonucleotide (oder „Primer") erzeugt, die für das 5'- und das 3'-Ende des Gens für das 18 kDa-Protein spezifisch waren. Das Vorwärts- oder 5'-Oligonucleotid (bezeichnet als HP18CF) wurde durch den Einbau einer NdeI-Restriktionsendonuclease-Schnittstelle modifiziert. Es wurden weitere Modifizierungen vorgenommen, um die Stabilität der Bindung des Oligonucleotids an seine Zielsequenz zu erhöhen, und um intramolekulare Sekundärstrukturen zu verhindern. Die Sequenz des Oligonucleotids HP18CF ist (von 5' nach 3'):
GAAGGACTTCATATGAAGACATTTG (SEQUENZ ID NO. 1) - Das reverse oder 3'-Oligonucleotid (bezeichnet als HP18CR) wurde durch den Einbau einer BamHI-Restriktionsendonuclease-Schnittstelle und eines 5'-Schwanzes umfangreich modifiziert. Die 15 am 3'-Ende liegenden Nucleotide dieses Oligonucleotids entsprechen der Gensequenz des 18 kDa-Proteins aus Helicobacter pylori. Die Sequenz des Oligonucleotids HP18CR ist (von 5' nach 3'):
CGTGAATGGATCCTCATGCTGACTTCT (SEQUENZ ID NO. 2) - Diese Oligonucleotide wurden in einer Polymerasekettenreaktion (PCR) verwendet, um die Gensequenz des 18 kDa-Proteins aus Helicobacter pylori zu amplifizieren. Die Reaktionsbedingungen waren wie folgt: es wurden zwischen 50 und 100 ng Helicobacter pylori-DNA zu 75 pmol jedes Primers und 0,4 mM jedes Desoxyribonucleotidtriphosphats (dNTP) zugegeben, eine Endkonzentration von 4 mM MgSO4, 1-fach „ThermoPol"-Reaktionspuffer (New England Biolabs) (Zusammensetzung: 10 mM KCl, 10 mM (NH)4SO4, 20 mM Tris-HCl [pH-Wert 8,8 bei 25°C], 2 mM MgSO4, 0,1% Triton X-100), und dann wurde deionisiertes Wasser zugegeben, um das Reaktionsvolumen auf 50 μl einzustellen. Das Reaktionsgemisch wurde mit 50 μl Paraffinöl überschichtet und bei 90°C in ein Thermozyklusgerät von Perkin Elmer gestellt. Dann wurde 1 Einheit VentR-DNA-Polymerase (New England Biolabs) zugegeben. Es wurde ein „touchdown"-PCR-Verfahren verwendet (Don et al., 1989)*. Die Zyklusbedingungen waren wie folgt: die DNA wurde 2,5 Minuten bei 94°C denaturiert. Darauf folgten 2 Zyklen mit 30 Sekunden bei 94°C (Denaturierungsschritt), 50 Sekunden bei 65°C (Anlagerungsschritt) und 20 Sekunden bei 72°C (Verlängerungsschritt). Darauf hin erfolgten 2 Zyklen unter den gleichen Bedingungen mit der Ausnahme, dass die Anlagerungstemperatur auf 64°C abgesenkt wurde. Nach 2 Zyklen bei 64°C wurde die Anlagerungstemperatur für weitere 2 Zyklen auf 63°C abgesenkt und nach diesem Muster wurde weiter verfahren, bis die Anlagerungstemperatur auf 60°C über 28 Zyklen abgesenkt worden war.
- Die Reaktionsprodukte wurden auf einem 4%-igen Agarosegel mit niedriger Schmelztemperatur (NuSieve GTG, FMC BioProducts) gereinigt. Das DNA-Fragment wurde aus dem Gel ausgeschnitten und die Agarose wurde unter Verwendung von β-Agarase 1 (New England Biolabs) verdaut, dann wurde die DNA durch Fällung mit Isopropanol entsprechend der durch den Hersteller bereitgestellten Vorschrift gewonnen.
- Das gereinigte DNA-Fragment, das dem das 18 kDa-Protein codierenden Gen entsprach, wurde anschließend mit den Restriktionsenzymen NdeI und BamHI (Boehringer Mannheim) verdaut, deren Erkennungssequenzen jeweils nur einmal in dem amplifizierten Fragment vorhanden waren. Es wurden 10 Einheiten jedes Enzyms zu etwa 3 μg DNA bei der 1-fachen Endkonzentration des durch den Hersteller mitgelieferten Restriktionspuffers B in 40 μl Reaktionsvolumen zugegeben. Das Reaktionsgemisch wurde 3,5 Stunden bei 37°C inkubiert.
- Der verwendete Expressionsvektor war pET16b (Novagen). Unter Verwendung von NdeI und BamHI wurden 1,6 μg des Vektors unter den gleichen Bedingungen verdaut, wie sie für das amplifizierte Fragment beschrieben wurden. Die dabei entstehenden 5'-Phosphatgruppen wurden unter Verwendung von alkalischer Phosphatase aus Kälberdarm (CIAP, New England Biolabs) gemäß den Angaben des Herstellers entfernt. Das Enzym wurde durch Inkubation des Reaktionsgemisches in Gegenwart von 5 mM EDTA bei 65°C für 1 Stunde inaktiviert, darauf hin erfolgte eine Extraktion mit Phenol/Chloroform/Isoamylalkohol (25:24:1) und dann eine Extraktion mit Chloroform/Isoamylalkohol (24:1).
- Sowohl das verdaute Fragment als auch der verdaute Vektor wurden über ein 3%-iges Agarosegel mit niedrigem Schmelzpunkt (NuSieve GTG, FMC BioProducts) gereinigt, und die Agarose wurde unter Verwendung von β-Agarase 1 (New England Biolabs) gemäß der Vorschrift des Herstellers verdaut. Die DNA-Fragmente verblieben in dem so erhaltenen Reaktionsgemisch ohne weitere Reinigungsschritte.
- Dann wurde das amplifizierte Fragment mit der Vektor-DNA wie folgt ligiert. Es wurden etwa 200 ng Vektor mit etwa 100 ng Insert-DNA in 1 × Ligierungs-Reaktionspuffer und 3 Einheiten T4-DNA-Ligase (Boehringer Mannheim) in einem Reaktionsvolumen von 30 μl bei 20 °C über 16 Stunden ligiert.
- Die Produkte dieser Reaktion wurden verwendet, um kompetente E. coli-Zellen des Stammes XL1-blue (Bullock et al., 1987) unter Verwendung des Standard-CaCl2-Transformationsverfahrens (Sambrook et al., 1989) zu transformieren. Die transformierten XL1-blue-Zellen wurden auf LG-Medium selektiert (Sambrook et al., 1989), das mit 50 μg/ml Ampicillin ergänzt worden war, und bei 37 °C angezogen. Geeignete Kolonien wurden herausgepickt und verwendet, um 10 ml LB-Nährmedium anzuimpfen, das mit 50 μg/ml Ampicillin ergänzt worden war, und dann unter Schütteln bei 37°C angezogen. Aus diesen Kulturen wurden die Plasmide unter Verwendung des Standard-Plasmidpräparationsverfahrens mit alkalischer Lyse (Sambrook et al., 1989) gereinigt, und ein Aliquot wurde gemäß den Angaben des Herstellers (Boehringer Mannheim) mit NdeI und HindIII verdaut, um die Anwesenheit der Insertion im Vergleich mit einem Größenstandard und dem Vektor pET16b ohne Insertion zu überprüfen.
- Zwei Plasmide, von denen gezeigt werden konnte, dass sie die passende Insertion enthielten (bezeichnet als pET16b-18.1 und pET16b-18.2) wurden dann verwendet, um die E. coli-Expressionswirte BL21 DE3 (Studier und Moffat, 1986) und Novablue DE3 (Novagen) zu transformieren, dies erfolgte unter Verwendung des Standard-CaCl2-Transformationsverfahrens (Sambrook et al., 1989), ergänzt mit 50 μg/ml Ampicillin (Novablue DE3) oder mit 50 μg/ml Ampicillin und 34 μg/ml Chloramphenicol (ΒL21 DE3) und Wachstum bei 37°C. Die transformierten Zellen wurden durch Ausplattieren auf festem LB-Medium selektiert. Eine Kolonie jedes Wirts, die jeweils ein Plasmid-Isolat repräsentierte, wurde nach 16 Stunden Inkubation herausgepickt und verwendet, um 50 ml LB-Nährmedium anzuimpfen, das mit den Antibiotika wie vorstehend beschrieben ergänzt worden war, und anschließend erfolgte die Anzucht, bis die optische Dichte bei 600 nm etwa 0,6 betrug. Die Expression des 18 kDa-Proteins durch den Expressionsvektor wurde dann durch Zugabe von Isopropyl-β-D-thiogalactopyranosid (IPTG) in einer Endkonzentration von 1 mM induziert, und die Inkubation wurde weitere 2,5 Stunden bei 37°C unter Schütteln fortgesetzt. Dann wurden die Zellen durch 10 Minuten Zentrifugation bei 4000 × g geerntet und in 12 ml 50 mM Tris-HCl (pH-Wert 8,0 bei 25°C) resuspendiert, danach erfolgte eine weitere Zentrifugation über 10 Minuten bei 4000 × g.
- Sequenzierung der gereinigten DNA-Sequenz
- Das gereinigte DNA-Fragment, das dem 18 kDa-Protein entsprach, wurde unter Verwendung von universellen Vorwärts- und Reverse-Sequenzierungsprimern sequenziert. Die DNA wurde in der Vorwärts- und in der reversen Orientierung sequenziert. Die Sequenzierung wurde unter Verwendung eines automatisierten Sequenzierungsgeräts von ABI und des auf einer PCR basierenden Fluoreszenz-Didesoxy-Kettenterminations-Termini-Sequenzierungs-Kits von Genpak durchgeführt.

- Western-Blot-Analyse des clonierten Produkts (18 kDa-Protein)
- Die beiden transformierten E. coli-Expressions-Wirte (BL21 DE3 und Novablue DE3) wurden einer SDS-PAGE (12,5% T) und anschließend einer Western-Blot-Analyse unterworfen. Die Western-Blots wurden mit Serum ausgetestet, das aus nicht mit H. pylori infizierten Kindern erhalten worden war, und mittels der „Gesteigerten Chemilumineszenz" entwickelt. Wie
1 zeigt, erkannten zwei von drei Seren das rekombinante 18 kDa-Protein nach der Induktion der Expression des Proteins durch IPTG. Darüber hinaus erkannten die drei Seren eine Reihe weiterer Proteine aus E. coli. - Man wird verstehen, dass das rekombinante Proteinmaterial der Anmeldung als Impfstoff gegen eine H. pylori-Infektion und insbesondere als therapeutisches Immunogen für die Ausrottung einer H. pylori-Infektion verwendet werden kann.
- Der Impfstoff kann das Proteinmaterial der Anmeldung in Kombination mit anderen Bestandteilen umfassen, wie z. B. einem pharmazeutisch verträglichen Träger, einem geeigneten Adjuvans, wie z. B. Interleukin 12 oder einem Hitzeschockprotein, sowie einem Antibiotikum und/oder einem antibakteriellen Mittel wie z. B. Wismutsalzen. Der Impfstoff kann über eine Reihe unterschiedlicher Wege verabreicht werden, nämlich oral, intranasal, intravenös oder intramuskulär.
- Die Erfindung ist nicht auf die hierin vorstehend beschriebenen Ausführungsformen beschränkt, sondern diese können im Einzelnen variiert werden.

- Verfahren zur Herstellung eines Impfstoffs für die Behandlung oder Prophylaxe einer Helicobacter pylori-Infektion oder mit Helicobacter pylori in Zusammenhang stehender(n) Krankheit(en), wobei der Impfstoff ein rekombinantes Helicobacter pylori-Protein oder Fragment davon beinhaltet, umfassend den Schritt der Expression des rekombinanten Helicobacter pylori-Proteins von einem Vektor, umfassend eine rekombinante Nucleinsäuresequenz, die das gesamte oder einen Teil eines Helicobacter pylori-Proteins codiert, für das Immunreaktivität in Helicobacter pylori-negativen Personen nachgewiesen wird, umfassend die folgende Sequenz von Nucleotiden: 5'-GATCGTGTTATTTATGAAAGTGCATAACTTCCATTGGAATGTG AAAGGCACCGATTTTTCAAT-3'
- Verfahren wie in Anspruch 1 beansprucht, wobei der Impfstoff einen pharmazeutisch verträglichen Träger beinhaltet.
- Verfahren wie in Anspruch 1 oder 2 beansprucht, wobei der Impfstoff in Kombination mit einem pharmakologisch geeigneten Adjuvans, bevorzugt Interleukin 12 oder einem Hitzeschockprotein vorliegt.
- Verfahren wie in einem der Ansprüche 1 bis 3 beansprucht, wobei der Impfstoff mindestens ein weiteres pharmazeutisches Produkt beinhaltet, wobei das pharmazeutische Produkt bevorzugt ein Antibiotikum ist, wobei das Antibiotikum bevorzugt ausgewählt ist aus einem oder mehreren von Metronidazol, Amoxycillin, Tetracyclin oder Erythromycin, Clarithromycin oder Tinidazol, wobei alternativ oder zusätzlich das pharmazeutische Produkt einen antibakteriellen Wirkstoff wie Wismutsalze beinhaltet.
- Verfahren wie in einem der Ansprache 1 bis 4 beansprucht, wobei der Impfstoff ein Peptid-Verteilungssystem beinhaltet.
