- Technisches Gebiet
- Die Erfindung betrifft Wirbelsäulenimplantate zum Unterstützen einer Fusion von zwei oder mehr Wirbelkörpern. Die Merkmale des Oberbegriffs in Anspruch 1 sind aus dem Dokument
FR-A-2724312 - Hintergrund
- Es ist bekannt, dass es, wenn eine Bandscheibe degeneriert oder beschädigt ist, häufig zu einer Kompression der Bandscheibe und zu einer Reduktion der normalen Zwischenwirbelhöhe kommt. Typischerweise ergeben sich aus diesen Bedingungen anormale Bewegungen, die eine Quelle für Schmerzen werden.
- Um eine solche Krankheit zu behandeln, wird die Bandscheibe häufig stabilisiert, um die anormalen Bewegungen zu unterbinden, die durch Bandscheiben-Fehlfunktionen oder Verletzungen verursacht werden. Im Allgemeinen ist ein Ansatz, um eine Artikulation zwischen zwei Wirbeln zu verhindern, die neben der beschädigten Bandscheibe angeordnet sind, die Knochenfusion. Diese Fusion fixiert die Wirbel aneinander und eliminiert die relative Mobilität, die Schmerzen verursacht.
- Es wurden verschiedene Wirbelsäulenimplantate vorgeschlagen, die eine Fusion zwischen benachbarten Wirbeln unterstützen. Es wurde vorgeschlagen, die zwei Wirbelkörper durch eine Art starren, U-förmigen Bügel zu verbinden, welcher die Bandscheibenhöhe wieder herstellt, wobei ein Knochen-Transplantat-Material innerhalb des Bügels angeordnet ist. Jedoch ist ein Nachteil dieses Vorschlags die mit der Zeit abnehmende Effektivität.
- Ein Wirbelsäulenkäfig ist ebenfalls bekannt, der aus einem zylindrischen Element besteht, das mit einer Reihe von Öffnungen und mit Ankerpunkten ausgestattet ist. Dieses Implantat wird in einer Aussparung platziert, die in der Bandscheibe ausgebildet ist, und durchdringt die gegenüber liegenden Kortikalplatten der zwei Wirbel, die vorher ausgehöhlt wurden, um das Implantat aufzunehmen. Dieses Durchdringen bildet Öffnungen in den subchondralen Platten, um spongiösen Knochen der Wirbel in Kontakt mit Knochen-Transplantat-Material zu bringen, welches in dem Implantat platziert ist, um eine Knochenfusion zu erleichtern. Das
U.S. Patent Nr. 5,015,247 stellt ein Beispiel für diesen Ansatz bereit. -
FR 2,736,538 - Eine geeignete Ausführung eines Wirbelsäulenimplantats dieser Art erfordert eine Balance zwischen dem Bedarf, eine Fusion zwischen dem spongiösen Knochen zu unterstützen, und dem Bedarf, ein zuverlässiges Belastungsverhältnis mit dem festeren kortikalen Knochen zu bilden. Als ein Ergebnis muss der Wirbelkäfig weder zu weit in die Öffnungen eingreifen, die in den kortikalen Platten vorgesehen sind, um eine ausreichend dichte Belastungsfläche bereitzustellen, noch unzureichend eingesetzt sein, wobei in diesem Fall die Knochenfusion zwischen den zwei Wirbeln aufgrund einer schlechten Verankerung nachteilig beeinflusst würde. Damit gibt es einen Bedarf für Vorrichtungen und Techniken, welche das Erreichen der geeigneten Balance zwischen Fusion und Belastung erleichtert.
- Die Erfindung befriedigt diesen Bedarf und schafft weitere signifikante Vorteile und Nutzen.
- Offenbarung der Erfindung
- Die Erfindung betrifft Wirbelsäulen-Fusionsimplantate. Verschiedene Aspekte der Erfindung sind neu, nicht naheliegend und schaffen verschiedene Vorteile. Während die tatsächliche Natur der hierin offenbarten Erfindung nur unter Bezugnahme auf die hieran angehängten Ansprüche bestimmt werden kann, sind bestimmte Ausführungsformen, welche charakteristisch für die hierin offenbarten bevorzugten Ausführungsformen sind, kurz wie folgt beschrieben.
- Gemäß einer Ausführungsform der Erfindung weist das Wirbelsäulen-Implantat einen Körper mit einem mittleren Teil, der so angeordnet ist, dass er eine Arthrodese ermöglicht, und zumindest einem Endteil zum Anliegen an dem kortikalen Knochen der Wirbel-Endplatte. Der mittlere Teil ist so angepasst, dass er die Wirbel-Endplatten durchdringt, wobei er quer von dem End-Trageteil absteht. Damit erzielt die Erfindung eine Separation zwischen den Endteilen, welche die Belastungsabstützung bilden, und der dazwischen liegende Teil des Implantates unterstützt eine Fusion. Zusätzlich kann der mittlere Teil zumindest einen Hohlraum zum Aufnehmen eines Knochen-Transplantations-Materials aufweisen.
- In einer anderen Ausführungsform der Erfindung weist ein Implantat zum Einsetzen zwischen einem ersten Wirbelkörper mit einer ersten kortikalen Knochenplatte und einem zweiten Wirbelkörper mit einer zweiten kortikalen Knochenplatte zwei Endteile auf. Der erste Endteil definiert eine erste Tragefläche, die gegen die erste kortikale Knochenplatte anliegt, und eine zweite Tragfläche gegenüber der ersten Fläche, die gegen die zweite kortikale Knochenplatte anliegt. Der zweite Endteil liegt gegenüber dem ersten Endteil und definiert eine dritte Tragfläche, die gegen die erste kortikale Knochenplatte anliegt, und eine vierte Tragfläche gegenüber der dritten Tragfläche, die gegen die zweite kortikale Knochenplatte anliegt. Das Implantat weist einen länglichen mittleren Teil auf, der einen oberen Vorsprung, der sich hinter die erste und die dritte Fläche erstreckt, und einen unteren Vorsprung auf, der sich hinter die zweite und die vierte Fläche erstreckt. Diese Vorsprünge passieren jeweils durch Öffnungen in der ersten und der zweiten kortikalen Knochenplatte, wenn die erste und die dritte Fläche gegen die erste kortikale Knochenplatte und die zweite und vierte Fläche gegen die zweite kortikale Knochenplatte anliegen. Die Endteile sind so dimensioniert, dass sie eine Restauration der natürlichen Geometrie des Zwischenwirbelraums erleichtern (Lordose, Cyphose und parallel Bandscheiben). Damit können die erste und die zweite Fläche mit einem ersten Abstand voneinander separiert sein und die dritte und die vierte Fläche können mit einem zweiten Abstand voneinander separiert sein, der größer als der erste Abstand ist, um an eine natürliche Krümmung der Wirbelsäule anzupassen.
- In einer weiteren Ausführungsform der Erfindung weist ein Implantat mit zwei Endteilen auch einen länglichen mittleren Teil auf, welcher ein Paar von Längswänden aufweist, die einen Hohlraum definieren. Die Wände definieren einen ersten Rand, der hinter der ersten und der dritten Fläche vorsteht, und einen zweiten Rand, der hinter der zweiten und vierten Fläche vorsteht. Der erste und der zweite Rand durchdringen jeweils die erste beziehungsweise die zweite kortikale Knochenplatte, wenn die erste und die dritte Fläche gegen die erste kortikale Knochenplatte anliegt und die zweite und die vierte Fläche gegen die zweite kortikale Knochenplatte anliegt.
- Gemäß einer anderen Ausführungsform sind die Trageflächen der Endteile durch Flansche gebildet, die sich von gegenüberliegenden Enden des Implantats entlang seiner Längsachse erstrecken. Vorzugsweise sind die Tragflächen zum Anliegen gegen den kortikalen Knochen der Wirbel-Endplatten der zwei benachbarten Wirbelkörper im Allgemeinen flach. Es ist ebenfalls bevorzugt, dass Öffnungen in die kortikalen Platten in ihrem mittleren Bereich geschnitten sind, der zu der Länge eines mittleren Teils des Implantats entlang der Längsachse korrespondiert, und dass sie einen Bereich der kortikalen Knochenplatte entlang des Umfangs der Öffnungen belasten. Die Länge der verbleibenden umgebenden Platte korrespondiert zu der Länge der Trageflächen entlang der Längsachse. Wenn das Implantat in Position platziert ist, greifen die Ränder der Wände des mittleren Teils in die Öffnungen, die in die kortikalen Platten geschnitten sind, und liegen konsequenterweise im Wesentlichen nicht an den verbleibenden Umfangsbereich der Platten an. Ein Hohlraum kann von dem mittleren Teil gebildet sein, welcher Knochen-Implantations-Material in Kontakt mit dem spongiösen Knochen der zwei Wirbelkörper hält. Im Gegensatz dazu sind die Trageflächen der Flansche benachbart zu den Rändern der Öffnungen der kortikalen Platten angeordnet und liegen gegen die verbleibenden Bereiche der Platten an, um ein starkes Belastungsverhältnis zu begründen. Damit werden sowohl eine Knochenfusion als auch eine Abstützung durch die unterschiedlichen Teile der Implantatstruktur bereitgestellt, welches das Erzielen einer befriedigenden Abstützung der Wirbelkörper an dem Implantat und eine exzellente Arthrodese ergibt.
- Es wird vorhergesehen, dass die Implantate der Erfindung mit anderen Werkzeugen und Prozeduren verwendet werden können, dass die Werkzeuge der Erfindung mit anderen Implantaten und Prozeduren verwendet werden können und dass die Prozeduren der Erfindung mit anderen Implantaten und Werkzeugen verwendet werden können, je nachdem, wie es für einen Fachmann sinnvoll erscheint.
- Gemäß der Aufgabe der Erfindung wird ein Wirbelsäulen-Implantat geschaffen, welches eine einwandfreie Fusion von zwei oder mehr Wirbelkörpern erleichtern kann. Das Implantat kann zumindest einen strukturellen Teil, der eine Knochenfusion durch Kontakt mit spongiösem Wirbelknochen unterstützt, und einen oder mehrere andere strukturelle Teile für einen Kontakt mit dem kortikalen Knochen aufweisen, um eine Lastabstützung bereitzustellen.

- Zum Unterstützten des Verständnisses der Prinzipien der Erfindung wird nun auf die Ausführungsformen Bezug genommen, die in den Zeichnungen illustriert sind, und eine spezielle Sprache wird verwendet werden, um dieselben zu beschreiben. Es wird nichtsdestotrotz verstanden sein, dass dadurch keine Beschränkung des Umfangs der Erfindung beabsichtigt ist. Irgendwelche Änderungen und weitere Modifikationen der beschriebenen Vorrichtung und irgendwelche weiteren Anwendungen der Prinzipien der Erfindung, wie hierin beschrieben, werden so betrachtet, wie sie normalerweise einen Fachmann erscheinen würden, welchen die Erfindung betrifft.
- Die
1 ,6 und7 zeigen ein Wirbelsäulen-Käfigimplantat1 . Das Implantat1 ist so angepasst, dass es in einen Hohlraum eingesetzt werden kann, der in einem beschädigten Zwischen-Wirbel-Bandscheibenraum2 (7 ) bereitgestellt wird, um die normale Höhe des Zwischen-Wirbelraums zwischen zwei Wirbelkörper V1 und V2 benachbart zu dem Bandscheibenraum2 wiederherzustellen, zum Beispiel zwischen den Lendenwirbeln L3, L4, wie in7 dargestellt.1 zeigt das Implantat1 entlang seiner Längsachse YY angeordnet. Das Implantat1 weist einen hohlen Körper3 mit einer länglichen Form auf, der einen mittleren Teil3c , der von zwei parallelen Längswänden4 gebildet wird, um eine Arthrodese zu erlauben, und an den zwei gegenüberliegenden Enden des mittleren Teils3c in Richtung der Implantation des Implantats1 zwei Endteile5 ,6 zum Anliegen gegen den kortikalen Knochen14 der vertebralen kortikalen Knochenplatten15 ,16 der zwei Wirbel (V1, V2) benachbart zu dem Bandscheibenraum2 aufweist (siehe z.B.6 ). - Die Längswände
4 definieren dazwischen einen Hohlraum7 und sind mit Öffnungen11 ausgestattet. Die Endteile5 ,6 sind entlang der Achse YY entgegengesetzt zueinander angeordnet und jeder weist eine Endfläche auf, in der ein zentral angeordnetes Gewindeloch11a definiert ist. Die Löcher11a sind für einen Eingriff mit Gewindeschäften eines Einsetz-/Manipulations-Werkzeugs (nicht dargestellt) angepasst, um ein Platzieren des Implantats1 zwischen den Wirbelkörpern V1, V2 zu erleichtern. Der Hohlraum7 ist bereitgestellt, um ein Transplantat G aus spongiösem Knochen oder irgendein anderes Material aufzunehmen, das eine Knochenfusion unterstützt, zum Beispiel eine synthetische Keramik. - Das Endteil
5 weist zwei gegenüberliegende Trageflächen12 quer zur Achse YY auf, die so dimensioniert sind, dass sie mit dem Abstand D1 voneinander separiert sind. Der Endteil6 weist zwei gegenüberliegende Trageflächen13 quer zur Achse YY auf, welche so dimensioniert sind, dass sie mit dem Abstand d2 voneinander separiert sind. Die Abstände d1, d2 sind angepasst, um die Geometrie des Zwischenwirbelraums wiederherzustellen. Wenn d2 größer als d1 ist, wie dargestellt, wird das Implantat1 vorzugsweise für eine Fusion der Wirbelkörper in einem Bereich der Wirbelsäule mit einer natürlichen Krümmung verwendet. Der Abstand1 korrespondiert zu einer maximalen Weite des Körpers3 . In einer Ausführungsform ist die Weite des Körpers3 im Allgemeinen konstant entlang der Achse YY und ist in etwa der gleiche wie der Abstand d1, um eine zuverlässige Passung zwischen den Wirbelkörpern V1, V2 zu erleichtern, was im Folgenden ersichtlicher werden wird. - Von den Endteilen
5 ,6 erstrecken sich Last-Trageflansche8 ,9 , das heißt zwei Last-Trageflansche8 von Teil5 und zwei Last-Trageflansche9 von Teil6 . In der dargestellten Ausführungsform bilden die Flansche8 ,9 parallele Leisten senkrecht zu den Längswänden4 und definieren im Allgemeinen flache Flächen12 ,13 zum Tragen und Anliegen gegen die kortikalen Knochen14 der Wirbel-Endplatten15 ,16 . - In der illustrierten Ausführungsform weisen die Längswände
4 eine im Wesentlichen trapezoide Kontur auf, von welcher die kleine Basis zu dem Endteil5 korrespondiert und die große Basis zu dem Endteil6 korrespondiert. Die Wände4 , welche den mittleren Teil3c des Implantats1 bilden, sind so geformt, dass sie quer von den End-Trageteilen5 ,6 vorstehen, wie es in der1 und der6 gesehen werden kann. Damit können die Wände4 jeweils Öffnungen19 ,21 der Wirbelplatten15 ,16 aufweisen, deren Ränder an. den Trageflächen12 ,13 der Flansche8 ,9 anliegen (6 ). Die Flansche8 ,9 sind so geformt, dass sie Stufen-Vorsprünge17a ,18a bilden, um ein Durchdringen der Öffnungen19 ,21 zu erleichtern. Die Vorsprünge17a ,18a stellen ebenfalls eine Lippe bereit, um ein Ausrichten der Trageflächen12 ,13 mit den kortikalen Knochenplatten15 ,16 um den Umfang der Öffnungen herum beizubehalten, wie in6 dargestellt. Die Wände4 weisen ebenfalls abgeschrägte Ränder4a auf, um das Durchdringen zu erleichtern. Entsprechend weist der mittlere Teil3c obere Vorsprünge3a und untere Vorsprünge3b auf, die von den Rändern4a gebildet werden. Es wird angemerkt, dass die Ränder4a und die Vorsprünge3a ,3b quer von den Flächen12 ,13 vorstehen. - Das Implantat
1 kann in Position in dem Zwischenwirbel-Bandscheibenraum2 nach der Präparation mit der Schneidwerkzeug-Baugruppe22 platziert werden, welche nun unter Bezug auf die2 ,4 und5 beschrieben werden wird. Vorzugsweise ist das Werkzeug22 aus einem metallischen Material hergestellt, das für eine chirurgische Verwendung geeignet ist. Das Werkzeug22 weist einen Fräs-Schneider23 mit einem mittleren Schneidbereich24 und zwei nicht-schneidende Bereichen31 ,36 auf, die an gegenüberliegenden Enden des mittleren Schneidbereichs24 angeordnet sind. Die nicht-schneidenden Bereiche31 ,36 weisen eine Höhe h korrespondierend zu dem intersomatischen Raum auf und ermöglichen ein gleichmäßiges, symmetrisches Schneiden einer vorbestimmten Länge durch einen mittleren Bereich von beiden Wirbelplatten15 ,16 . Vorzugsweise ist die Geometrie der Bereiche24 ,31 ,36 zum Präparieren des intersomatischen Raums mit der Geometrie des Implantats1 bestimmt, um die natürliche Lordose des Zwischenwirbelraums wiederherzustellen und entsprechend die Abstände, die durch h und d1 repräsentiert werden, sind in etwa gleich. - Der mittlere Schneidbereich
24 weist eine trapezoide Form mit zwei im Allgemeinen glatten Längsflächen25 auf, die einander gegenüberliegen. Die Flächen25 sind so konfiguriert, dass sie ein Einsetzen in den intersomatischen Raum erleichtern und die im Allgemeinen mit dem Abstand h voneinander separiert sind. Der mittlere Schneidbereich24 weist ebenfalls Schneidflächen26 auf, die sich zwischen den Flächen25 erstrecken. Die Schneidflächen26 definieren eine Anzahl von gleichmäßig im Abstand voneinander befindlichen Schneidzähnen28 . Die Zähne28 erstrecken sich jeweils entlang eines im Allgemeinen geraden Pfades, der mit einem geeigneten Winkel relativ zur Längsachse XX des Werkzeugs22 geneigt ist. Vorzugsweise ist der mittlere Schneidbereich24 aus einem geeigneten metallischen Material hergestellt, welches bearbeitet wurde, um die Zähne28 auszubilden. - Der nicht-schneidende Bereich
31 ist an dem distalen Ende des mittleren Schneidbereichs24 fixiert. Der Bereich31 erstreckt sich von dem mittleren Schneidbereich24 , um einen distalen Kopf in der Form von parallelen Rohren bereitzustellen. Der Bereich31 weist eine erste Dimension auf, die im Wesentlichen die gleiche wie der Abstand h ist, um im Allgemeinen flächengleich mit den Flächen25 des mittleren Schneidbereichs24 zu sein. Der Bereich31 weist auch gegenüberliegende Flächen32 auf, die mit einem Abstand H voneinander separiert sind, wie in5 dargestellt. - Vorzugsweise ist der Abstand H in etwa gleich dem Abstand d2, wenn das Werkzeug
22 verwendet wird, um das Implantat1 zu installieren. - Das Werkzeug
22 weist ebenfalls einen Schaft oder Schenkel33 auf, der mit einem proximalen Betätigungshandgriff34 verbunden ist. Der Schaft33 ist an dem mittleren Schneidbereich24 und dem nicht-schneidenden Bereich31 fixiert. Der Schaft33 erstreckt sich von der kleinen Endfläche27 entfernt von dem nicht-schneidenden Kopf31 und endet in dem Handgriff34 , was ein Rotieren des Schneidbereichs24 um die Längsachse XX des Werkzeugs22 ermöglicht. - Der nicht-schneidende Bereich
36 weist eine rechteckige Form mit im Allgemeinen planaren Flächen37 ,38 auf. Der Bereich36 kann zwischen zwei aufeinanderfolgende Wirbel während einer Rotation des mittleren Schneidbereichs24 eingesetzt werden. Der Bereich36 erstreckt sich in Richtung des Handgriffs34 durch einen röhrenförmigen Teil40 und durch einen Block40a , welcher in der Nähe des Handgriffs34 endet. Der nicht-schneidende Bereich36 ist mit seitlichen Anschlägen39 ausgestattet, die geeignet sind, an den Seiten der Wirbel (V1, V2) nach einem Einsetzen dazwischen anzuliegen. Der nicht-schneidende Bereich36 umschließt den Schaft33 . Der Schaft33 ist so konfiguriert, dass er relativ zu dem Bereich36 rotiert. Entsprechend, wenn der Handgriff34 gedreht wird, dreht sich der Schaft33 , der mittlere Schneidbereich24 und der nicht-schneidende Bereich31 zusammen um die Achse XX, wobei der nicht-schneidende Bereich36 vorzugsweise stationär bleibt. Es sollte angemerkt sein, dass die Teilansicht der4 und5 nicht den Handgriff34 zeigt, sondern eine abgeschnittene Ansicht des nicht- schneidenden Bereichs36 mit dem Schaft33 , der von diesem vorsteht. - Verschiedene nicht-beschränkende Ausführungsformen einer Wirbelsäulen-Fixation gemäß der Erfindung werden als nächstes beschrieben. Eine Prozedur ist gekennzeichnet durch:
- a) Schneiden der Wirbel V1, V2 und der Bandscheibe
2 mit dem Werkzeug22 , um eine Implantation vorzubereiten und - b) Einsetzen des Implantats
1 zwischen die Wirbel V1, V2. - Andere detailliertere Prozeduren zum Fusionieren von zwei Wirbelkörpern wird in Bezug auf die prozeduralen Stufen a) bis h) wie folgt beschrieben:
- a) Ein Chirurg separiert zunächst die
durale Hülse,
welche die Ausdehnung des Knochenmarks bildet, wenn die Prozedur
im Lendenbereich ist, und führt
anschließend
eine Diskektomie durch, um einen Raum für das Implantat
1 in dem Bandscheibenraum2 bereitzustellen. - b) Der Chirurg setzt zwischen die zwei Wirbel V1, V2 von hinten
(posterior) zwei Lordosedistraktoren
41 ein, wie in3 dargestellt. Jeder Distraktor41 besteht aus einem Teil mit parallelen Röhren, von dem sich ein keilförmiger Teil43 eines dreieckigen Abschnitts erstreckt. Der Teil42 ist mit einer Schlagplatte44 ausgestattet. Die Distraktoren42 werden lateral in Bezug auf den Hohlraum eingesetzt, welcher durch die Diskektomie des Schritts a) bereitgestellt wird. Jeder Distraktor41 wird anfänglich mit seinem engeren Aspekt zwischen die Wirbel V1 und V2 eingesetzt und anschließend um 90° gedreht, um die Wirbelkörper voneinander wegzuspreizen und die Bandscheibenhöhe und die Höhe des betrachteten Wirbelsegments wiederherzustellen. Als Nächstes wird einer der Distraktoren41 entfernt. - c) Der Chirurg setzt das Werkzeug
22 zwischen die Wirbelkörper V1, V2 ein, wie in4 dargestellt, so dass die größeren glatten Flächen25 im Kontakt mit den Wirbelplatten15 ,16 sind. Wenn der Schneidbereich24 richtig in dem mittleren Bereich der kortikalen Platten positioniert ist, liegen die Anschläge39 an den äußeren Flächen der Wirbel V1, V2 an und der nicht-schneidende Bereich36 ist teilweise zwischen die Platten15 ,16 eingesetzt. - d) Als Nächstes
dreht der Chirurg den Handgriff
34 und verursacht, dass der Schneidbereich24 sich um die Achse XX dreht. Typischerweise wird der Bereich24 mehrere Male rotiert. Wenn die Rotation fortgeführt wird, greifen die Scheidzähne28 in den mittleren Teil der kortikalen Platten15 ,16 ein (5 ) und entfernen graduell kortikalen Knochen14 . Am Ende des Aushöhlens dieser Platten haben die Schneidflächen26 des mittleren Schneidbereichs24 und die Schneidzähne28 durch die Platten15 ,16 hindurchgeschnitten und im Allgemeinen zentral angeordnete Öffnungen19 ,21 ausgebildet. Die verbleibenden Bereiche der Platten15 ,16 tragen dann die gegenüberliegenden Flächen32 des nicht-schneidenden Bereichs31 und liegen an dem nicht-schneidenden Bereich36 an. - e) Dann entfernt der Chirurg das Werkzeug
22 zwischen den Wirbeln V1, V2. - f) Als Nächstes
setzt der Chirurg das Implantat
1 , welches vorher mit Knochentransplantat G befüllt ist, von hinten zwischen den Platten15 ,16 bis in eine geeignete Position ein, mit dem Endteil6 und den Trageflanschen9 am vorderen Ende. Das Implantat1 ist flach, so dass die Längswände4 im Wesentlichen parallel zu den kortikalen Platten15 ,16 sind und anfänglich während des Einsetzens in Kontakt mit diesen kommen. - g) Anschließend
dreht der Chirurg das Implantat
1 um eine Vierteldrehung um seine Längsachse YY, um es in Position zu platzieren, die in6 dargestellt ist, wobei die Wände4 im Wesentlichen senkrecht zu den kortikalen Platten und ihre flachen Flächen12 ,13 im Allgemeinen parallel zu den kortikalen Platten15 ,16 sind. Das Implantat1 kann mittels des Eingriffslochs11a mit einem korrespondierenden Gewindeschaft eines Werkzeugs (nicht dargestellt) eingesetzt und gedreht werden. Die Ränder4a der Längswände4 , welche die Vorsprünge3a ,3b definieren, werden in die Öffnungen19 ,21 eingesetzt, wodurch sie durch die Platten15 ,16 hindurchpassieren, während die verbleibenden Bereiche der Platten15 ,16 die flachen Flächen12 ,13 tragen und an diesen anliegen. Das Implantat1 ist dann in seiner endgültigen Position, in welcher es stabilisiert ist. Das Knochen-Transplantat G ist im Kontakt mit dem spongiösen Teil S1, S2 und unterstützt eine Knochenfusion. - h) Der Chirurg entfernt den zweiten Distraktor
41 und wiederholt anschließend die vorangegangenen Schritte a bis g, um ein zweites Wirbelsäulen-Käfigimplantat1 durch Platzieren in einer Position im Allgemeinen parallel zu dem ersten Käfigimplantat1 an der anderen Seite der Achse der Wirbelsäule einzusetzen, was die Konfiguration ergibt, die in7 dargestellt ist (die Implantate1 sind gestrichelt dargestellt). - In anderen Ausführungsformen kann angenommen werden, dass die beschriebenen Schritte geändert, entfernt, kombiniert, wiederholt oder in einer anderen Reihenfolge gebracht werden können, je nach Bedarf eines Fachmanns. Als nicht-beschränkendes Beispiel kann die Prozedur ein oder mehrere unterschiedliche Werkzeuge verwenden, um die Wirbelsäule für eine Fixation durch das Implantat gemäß der Erfindung zu präparieren.
- Sicherlich ist der Umfang der Erfindung nicht beabsichtigt, durch die beschriebene Ausführungsform beschränkt zu werden, und kann ebenfalls Varianten innerhalb des Umfangs der anhängenden Ansprüche einschließen. Zum Beispiel können die End-Trageflächen
12 ,13 für die Wirbelplatten irgendeine Form aufweisen, wie etwa eine gekrümmte oder zylindrische Form, wobei die Platten15 ,16 entsprechend geschnitten sind, um ein Platzieren der Trageflächen in einer geeigneten Position zu erlauben. Ferner können diese Trageflächen paarweise miteinander verbunden sein und ein einzelnes Element50 bilden, wie in6 dargestellt. - Genauso kann der mittlere Teil des Implantats gemäß der Erfindung irgendeine Form aufweisen, vorzugsweise Rückhalteränder, welche von den End-Trageteilen vorstehen. Insbesondere kann der Körper eine Vielzahl von Zellen aufweisen. Ebenfalls sollte im Allgemeinen angemerkt sein, dass das Implantat gemäß der Erfindung an eine Geometrie der Wirbelsäule in Bezug auf eine Lordose, Zyphose oder auf parallele Wirbel-Endplatten angepasst werden kann. Damit umfasst die Erfindung auch eine Anwendung zwischen benachbarten Wirbelkörpern, anderen als L3, L4. Entsprechend können das Implantat und der Schneidbereich des Werkzeugs eine unterschiedliche Form aufweisen, wie etwa eine zylindrische Geometrie, anders als die dargestellte im Allgemeinen konische Geometrie. Ebenso kann anstelle der Verwendung des offenbarten Implantats
1 der spinale Raum, der durch das Werkzeug22 präpariert ist, mit irgendeinem anderen Material gefüllt werden, je nach Vorstellung eines Fachmanns. - Entsprechend der anderen Ausführungsformen kann das Implantat teilweise oder vollständig durch poröse rehabilitierbare oder resorbierbare Materialien ersetzt werden, die eine Osteointegration begünstigen. Solche Ausführungsformen umfassen: a) ein Implantat gemäß der oben dargestellten Geometrie, welches vollständig aus einem resorbierbaren oder rehabilitierbaren Material hergestellt ist, b) ein Implantat, bei welchem der gesamte mittlere Teil aus einem resorbierbaren oder rehabilitierbaren Material hergestellt ist, oder c) ein Implantat, bei welchem der Umfang des mittleren Teils aus einem metallischen oder einem anderen Material hergestellt ist und der innere Teil aus einem Material hergestellt ist, das eine Osteointegration begünstigt, welches anfänglich in einem festen, zähflüssigen oder flüssigen Zustand ist.
- In einer anderen alternativen Ausführungsform ist ein Implantat mit nur einem Trage-Endteil ausgestattet, wobei der Endbereich gegenüber dem Trage-Endteil offen ist, und bildet eine U-Form zum Aufnehmen eines Knochen-Transplantats oder eines rehabilitierbaren oder resorbierbaren Materials.

- Implantat zum Einsetzen zwischen einem ersten Wirbelkörper (V1) und einem zweiten Wirbelkörper (V2), wobei der erste Wirbelkörper eine erste kortikale Knochenplatte aufweist, der zweite Wirbelkörper eine zweite kortikale Knochenplatte aufweist und das Implantat aufweist: ein erstes Ende (
5 ), das eine erste Lagerungsfläche (12 ), die gegen die erste kortikale Knochenplatte (14 ) anliegt, und eine zweite Lagerungsfläche (12 ) entgegengesetzt zu der ersten Lagerungsfläche definiert, die gegen die zweite kortikale Knochenplatte anliegt, wobei die erste Fläche von der zweiten Fläche mit einem ersten Abstand (d1) separiert ist, ein zweites Ende (6 ) entgegengesetzt zu dem ersten Ende (5 ), wobei das zweite Ende eine dritte Lagerungsfläche (13 ), die gegen die erste kortikale Knochenplatte (15 ) anliegt, und eine vierte Lagerungsfläche (13 ) entgegengesetzt zu der dritten Fläche (13 ), die gegen die zweite kortikale Knochenplatte (16 ) anliegt, wobei die dritte Fläche von der vierten Fläche mit einem zweiten Abstand (d2) separiert ist, und einen länglichen Teil (3c ), der eine Längsachse (YY) definiert und eine erste Wand (4 ) und eine gegenüberliegende zweite Wand (4 ) aufweist, wobei jede Wand (4 ) das erste Ende (5 ) mit dem zweiten Ende (6 ) verbindet und sich an der ersten Fläche (12 ), der zweiten Fläche (12 ), der dritten Fläche (13 ) und der vierten Fläche (13 ) vorbei erstreckt, wobei die erste und die zweite Wand so konfiguriert sind, dass sie durch die erste und die zweite kortikale Knochenplatte hindurch passieren, wenn die erste und die dritte Fläche die erste kortikale Knochenplatte tragen und die zweite und die vierte Fläche die zweite kortikale Knochenplatte tragen, sodass die Enden (5 ,6 ) Trageflächen (12 ,13 ) definieren, um die kortikalen Knochenplatten zu tragen, während der mittlere Teil sich durch die kortikalen Knochenplatten hindurch erstreckt und eine Arthrodese ermöglichen, dadurch gekennzeichnet, dass das erste Ende (5 ) einen ersten Flansch (8 ), der die erste Fläche (12 ) definiert, und einen zweiten Flansch (8 ) aufweist, der die zweite Fläche (12 ) definiert, und dass das zweite Ende (6 ) einen dritten Flansch (9 ), der die dritte Fläche (13 ) definiert, und einen vierten Flansch (9 ) aufweist, der die vierte Fläche (13 ) definiert. - Implantat nach Anspruch 1, wobei der mittlere Teil (
3c ) einen Hohlraum (7 ) zum Aufnehmen von Knochen-Transplantatmaterial (G) aufweist. - Implantat nach Anspruch 1 oder 2, wobei der mittlere Teil (
3c ) eine Querwand aufweist, die Stufen-Vorsprünge (17a ,18a ) bilden. - Implantat nach einem der Ansprüche 1 bis 3, wobei der mittlere Teil (
3c ) ein Paar von Wänden (4 ) aufweist, die jeweils eine im Allgemeinen trapezoide Kontur aufweisen. - Implantat nach einem der Ansprüche 1 bis 4, wobei die erste (
12 ), die zweite (12 ), die dritte (13 ) und die vierte (13 ) Fläche im Allgemeinen flach sind. - Implantat nach einem der Ansprüche 1 bis 5, wobei der zweite Abstand (d2) größer als der erste Abstand (d1) ist.
