- HINTERGRUND DER ERFINDUNG
- 1. Gebiet der Erfindung
- Die Erfindung betrifft Stents mit verbesserter Konfiguration.
- 2. Kurze Beschreibung des Standes der Technik
- Stents sind in Radialrichtung ausdehnbare Endoprothesen, die typischerweise intravaskulare (innerhalb der Blutgefäße liegende) Implantate sind, die transluminar (in Längsrichtung der lichten Weite) implantiert und in Radialrichtung ausgedehnt werden können, nachdem sie perkutan eingeführt wurden. Sie sind auch in Harnwegen und Gallengängen implantiert worden. Sie werden verwendet, um Gefäße des Körpers zu verstärken und um Restenose (Wiederverengung) nach Angioplastik (Blutgefäßrekonstruktion) im Gefäßsystem zu verhindern. Sie können selbstausdehnend sein oder durch eine innere Kraft in Radialrichtung ausgedehnt werden, wie wenn sie auf einem Ballon montiert sind.
- Früher waren Stents im allgemeinen röhrenförmig, waren jedoch aus vielen Konfigurationen zusammengesetzt und wurden aus vielen Werkstoffen hergestellt, einschließlich Metallen und Kunststoff. Gewöhnliche Metalle wie rostfreier Stahl wurden ebenso verwendet wie Metalle mit Formgedächtnis, wie Nitinol und dergleichen. Stents sind auch aus biologisch abbaubaren Kunststoffen hergestellt worden. Derartige Stents wurden aus Draht, serienmäßigem Röhrenmaterial und so weiter geformt. Einige Stents sind selbstausdehnend und einige werden durch eine innere, in Radialrichtung wirkende Kraft ausgedehnt.

EP 0 734 698 offenbart einen Stent mit Öffnungen, die sich sowohl in der Längsrichtung wie auch in der Umfangsrichtung des Stents wiederholen und Ausdehnung des Stents ermöglichen. Jede Öffnung hat mindestens einen Abschnitt, der sowohl im zusammengedrückten wie auch im ausgedehnten Zustand des Stents schräg zu der Längsachse des Stents angeordnet ist. - ZUSAMMENFASSUNG DER ERFINDUNG
- Die Erfindung stellt neue Konfigurationen der Stents bildenden Segmente bereit, die an alle unterschiedlichen Arten von Stents nach dem Stand der Technik angepasst werden können, die vorstehend beschrieben wurden und/oder früher auf dem Fachgebiet bekannt waren. Die neuen Konfigurationen haben zahlreiche Vorteile. Zum Beispiel begrenzen die erfindungsgemäßen Konfigurationen unter anderem das Zurückfedern und verleihen einem ausgedehnten Stent Widerstandsfähigkeit gegen Zusammendrückung. Außerdem sind die Stents der Erfindung in Längsrichtung biegsam.
- Die erfindungsgemäßen Stents umfassen eine Vielzahl von ringförmigen Elementen, die ausgerichtet sind, um einen zylindrischen Stentkörper zu bilden. Jedes ringförmige Element besteht wiederum aus einer Vielzahl von offenen, im allgemeinen bumerangförmigen Segmenten. Die Segmente sind um jedes der ringförmigen Elemente herum von der Oberseite zur Unterseite miteinander verbunden. Angrenzende ringförmige Elemente sind durch eines oder mehrere Elemente zur Verbindung untereinander verbunden. Jedes Element zur Verbindung untereinander erstreckt sich von einem Ende eines Segmentes in der Form eines Bumerangs in einem ring förmigen Element zu einem Ende eines bumerangförmigen Segmentes in einem angrenzenden ringförmigen Element. In einer bevorzugten Ausführungsform erstreckt sich ein Element zur Verbindung untereinander von jedem bumerangförmigen Segment in einem ringförmigen Element zu einem benachbarten bumerangförmigen Segment in einem angrenzenden ringförmigen Element.
- Elemente zur Verbindung untereinander, die angrenzende ringförmige Elemente aneinanderfügen, sind wünschenswerterweise U-förmig oder zickzackförmig, obwohl auch andere krummlinig und geradlinig begrenzte Elemente verwendet werden können.
- Angrenzende bumerangförmige Segmente in einem ringförmigen Segment können durch ein Verbindungsstück untereinander verbunden sein, das sich von dem Oberteil eines Segments zum Unterteil eines angrenzenden Segmentes erstreckt. Die Verbindungsstücke können sich in der Ausgestaltung von einem kurzen, geraden Verbinder bis zu irgendeiner der nachstehend für die Elemente zur Verbindung untereinander beschriebenen Formen erstrecken.

7 ist eine Ansicht des Stents der3 , nachdem er gebogen wurde. - BESCHREIBUNG DER BEVORZUGTEN AUSFÜHRUNGSFORMEN
IN EINZELHEITEN - Für die Zwecke der Erfindung wird der Ausdruck Bumerang verwendet, um die Form bestimmter Stentsegmente zu beschreiben, und wird in dem Sinn wie im Websters New Collegiate Dictionary, mit Bezug auf die
1 davon, verwendet:
„Bumerang 1: ein gebogener oder winkelförmiger Wurfkolben, der so geworfen werden kann, dass er in die Nähe des Ausgangspunktes zurückkehrt". - Eine Ausführungsform eines allgemein zylindrischen, erfindungsgemäßen Stents wird im flachgelegten Zustand bei
110 in2 oder4 veranschaulicht. Der Stent kann aus einer Röhre aus einem Metall wie Nitinol oder vorzugsweise rostfreiem Stahl geformt sein, die zu der in der ebenen Grundrisszeichnung der2 oder4 gezeigten Konfiguration geätzt oder vorzugsweise lasergeschnitten wurde. Die Konfiguration kann in einem ebenen Blech ausgebildet und zu einem Zylinder mit einer verschweißten Naht oder dergleichen gerollt werden, indem die Ränder112 und114 zusammengefügt werden, oder die Konfiguration kann unmittelbar in einer kleinen Röhre wie einem Kanülenrohr gebildet werden. Eine röhrenartige Form des Stents wird in allgemeiner Form bei210 in3 gezeigt. - Die in den
2 –4 gezeigten Konfigurationen sind aus eine Vielzahl von ausgerichteten ringförmigen Elementen114 hergestellt, die wie gezeigt ausgerichtet sind, um einen allgemein zylindrischen Körper des Stents bereitzustellen. Jedes ringförmige Element114 besteht aus einer Reihe von allgemein bumerangförmigen Segmenten, die bei118 angedeutet sind (siehe das dunkler gemachte Segment in den Abbildungen zur Verdeutlichung), mit einer offenen Struktur, wobei das Oberteil120 mit dem Unterteil122 an der Segmentverbindungsstelle124 aneinandergefügt ist. Jedes Bumerangsegment umfasst einen ersten, im allgemeinen in Längsrichtung angeordneten, oberen Teil119 und einen zweiten, im Allgemeinen in Längsrichtung angeordneten, unteren Teil121 mit einer Öffnung dazwischen. Die Segmente118 sind wie in den Abbildungen gezeigt angeordnet oder zu einem Netz verflochten, wobei die Enden126 von benachbarten Segmenten an angrenzenden ringförmigen Elementen mittels Elementen zur Verbindung untereinander128 zusammengefügt sind. In den2 –4 ist das Element zur Verbindung untereinander128 ein U-förmiges Element, das eine teilweise offene Kurve ist. Alternative Elemente zur Verbindung untereinander beinhalten das zickzackförmige Element228 wie in6 gezeigt, das an Stelle des U-förmigen Elementes128 dazu verwendet werden kann, angrenzende ringförmige Elemente114 aneinander zu fügen. - Die Konfigurationen der
2 und4 sind im Wesentlichen zueinander ähnlich, wobei sie sich grundsätzlich durch das Vorhandensein einer Vertiefung130 in jeder Unterseite122 jedes Segmentes118 in der Konfiguration der4 unterscheiden. Ohne durch eine besondere Theorie gebunden zu sein, wird angenommen, dass das Vorhandensein der Vertiefung das Ausmaß begrenzt, bis zu dem der Stent sich bei Ausdehnung aus der Ebene herausbeult. - Es ist wünschenswert, dass die bumerangförmigen Segmente mindestens im Wesentlichen symmetrisch zu einer sich von dem oberen Teil
120 zu dem unteren Teil122 des Segmentes erstreckenden Mittellinie138 sind. Die Mittellinie138 befindet sich in der Mitte zwischen den Enden126 des Segmentes. - Wenn der Stent der
2 ausgedehnt wird, wie allgemein bei310 in5 an einem Ballon als Beispiel gezeigt wird, nehmen die bumerangförmigen Segmente118 des unausgedehnten Stents eine neue Konfiguration ein. Die Segmente318 nehmen die Form von abgerundeten Dreiecken mit ausbauchenden Unterteilen322 ein. - Es ist wünschenswert, dass die Elemente zur Verbindung untereinander U-förmig, wie in den
2 und4 gezeigt, oder zickzackförmig, wie in6 gezeigt, sind. Jedoch zieht die Erfindung in einem allgemeineren Sinn sowohl die Verwendung von krummlinig wie von geradlinig begrenzten Elementen zur Verbindung in Erwägung, einschließlich gerader Elemente. Beispiele anderer Verbindungsstücke werden in WO 97/32543, erteilt für Divysio Solutions LTD., und WO 97/40780, erteilt für David G. Jang, offenbart. Natürlich können angrenzende bumerangförmige Segmente auch Seite an Seite, mit einem Überlappungsgebiet zwischen angrenzenden bumerangförmigen Segmenten, aneinandergefügt werden. - Es ist auch wünschenswert, dass untereinander verbundene Elemente biegsam sind, so dass Krümmen des Stents ohne wesentliche Verwindung von bumerangförmigen Elementen leicht möglich ist.
7 zeigt den Stent von3 , nachdem er gebogen wurde. Wie in7 gezeigt, öffnen sich unter Zugspannung stehende Elemente zur Verbindung untereinander, wenn der Stent gekrümmt wird, während Elemente zur Verbindung untereinander, die zusammengedrückt werden, sich schließen. - Obwohl, wie in den Abbildungen gezeigt, sich ein Element zur Verbindung untereinander von jedem bumerangförmigen Segment in einem ringförmigen Element zu einem nächsten benachbarten bumerangförmigen Segment in einem angrenzenden ringförmigen Element erstreckt, zieht die Erfindung ferner die Möglichkeit eines Elementes zur Verbindung untereinander in Erwägung, das sich von jedem bumerangförmigen Segment in einem ringförmigen Element zu einem übernächsten benachbarten bumerangförmigen Segment in einem angrenzenden ringförmigen Element erstreckt. In dem letzteren Fall wären das erste Ende
140 und das zweite Ende144 jedes Elementes zur Verbindung untereinander128 in Umfangsrichtung entlang des Stents gegeneinander versetzt. - In einem allgemeineren Sinn zieht die Erfindung ferner einen Stent in Erwägung, bei dem jedes angrenzende ringförmige Element mittels einem oder mehreren Elementen zur Verbindung untereinander verbunden ist, und sich jedes Element zur Verbindung untereinander von dem Ende eines bumerangförmigen Segmentes in einem ringförmigen Element zu einem Ende eines bumerangförmigen Segmentes in einem angrenzenden ringförmigen Element erstreckt. Als solches muss sich ein Element zur Verbindung untereinander nicht von jedem bumerangförmigen Segment aus erstrecken. Ein Beispiel davon ist ein Stent, bei dem sich Elemente zur Verbindung untereinander von jedem zweiten oder dritten bumerangförmigen Segment in einem ringförmigen Element erstrecken.
- Die Erfindung zieht auch die Möglichkeit in Erwägung, die Ausrichtung einiger der ringförmigen Elemente zu verändern. In einer solchen Ausführungsform werden in dem flach ausgebreiteten Muster angrenzende ringförmige Elemente um 180° gegeneinander verdreht, so dass angrenzende ringförmige Elemente in entgegengesetzt Richtungen weisen.
- Obwohl die Enden von nächstbenachbarten Segmenten in angrenzenden ringförmigen Elementen in den Abbildungen als entlang dem Umfang des Stents miteinander ausgerichtet gezeigt werden, zieht die Erfindung ferner Ausführungsformen des Stents in Erwägung, bei denen nächstbenachbarte Segmente in angrenzenden ringförmigen Elementen in Umfangsrichtung in Bezug zueinander verschoben sind.
- In noch einer anderen Reihe von Ausführungsformen können angrenzende (oder nicht-angrenzende) ringförmige Elemente aus bumerangförmigen Segmenten unterschiedlicher Größe gebildet werden. Als solche können angrenzende (oder nicht-angrenzende) ringförmige Elemente unterschiedliche Längen überbrücken. Alternativ können angrenzende (oder nicht-angrenzende) ringförmige Elemente unterschiedliche Anzahlen von bumerangförmigen Segmenten umfassen.
- Obwohl in den Ausführungsformen von
2 -4 die Verbindungsstelle der Segmente124 als ein kleines, gerades Verbindungsstück gezeigt wird, das sich von der Oberseite eine Segmentes zu der Unterseite eines angrenzenden Segmentes erstreckt, zieht die Erfindung auch die Möglichkeit in Erwägung, dass angrenzende bumerangförmige Segment innerhalb eines ringförmigen Elementes durch U-förmige Verbindungsstücke, zickzackförmige Verbindungsstücke oder durch irgendeine der vorstehend für die Elemente zur Verbindung untereinander offenbarten Formen verbunden sind. Zusätzlich können Segmente mit anderer Form mit den bumerangförmigen Segmenten vermischt sein. - Der erfindungsgemäße Stent kann selbstausdehnend oder mechanisch, wie durch einen Ballon, ausdehnbar sein. Er kann aus einer Legierung mit Formgedächtnis, wie Nitinol oder dergleichen, bestehen.
- Die vorstehenden Beispiele und die Offenbarung sind als veranschaulichend und nicht erschöpfend gedacht. Diese Beispiele und die Beschreibung legen Fachleuten mit den üblichen Fähigkeiten auf dem Gebiet viele Veränderungen und Alternativen nahe. Alle diese Alternativen und Veränderungen sollen in dem Umfang der angehängten Ansprüche beinhaltet sein. Die mit dem Fachgebiet Vertrauten können andere, mit den hierin beschriebenen spezifischen Ausführungsformen gleichwertige Gegenstücke erkennen, welche Gegenstücke ebenso in den hieran angehängten Ansprüchen inbegriffen sein sollen.
Claims (15)
- Ein im Wesentlichen zylindrischer, in Radialrichtung ausdehnbarer Stent (
110 ), der im unausgedehnten Zustand besteht aus einer Vielzahl von ringförmigen Elementen (114 ), die ausgerichtet sind um einen zylindrischen Stentkörper zu bilden, wobei jedes ringförmige Element (114 ) aus einer Vielzahl von Segmenten (118 ) in der Form eines Bumerangs besteht, wobei jedes Segment in der Form eines Bumerangs einen ersten, im Allgemeinen in Längsrichtung angeordneten oberen Teil (119 ) und einen zweiten, im Allgemeinen in Längsrichtung angeordneten unteren Teil (121 ) hat, mit einer Öffnung in der Form eines Bumerangs dazwischen, wobei angrenzende Segmente in Form eines Bumerangs mit dem oberen Teil und dem unteren Teil um jedes der ringförmigen Elemente herum verbunden sind, und wobei angrenzende ringförmige Elemente an einer oder mehreren Stellen untereinander verbunden sind. - Stent nach Anspruch 1, wobei angrenzende ringförmige Elemente (
114 ) durch eines oder mehrere Elemente zur Verbindung untereinander (128 ) untereinander verbunden sind, wobei sich jedes Element zur Verbindung untereinander von einem Ende eines Segmentes in der Form eines Bumerangs in einem ringförmigen Element zu einem Ende eines Segmentes in der Form eines Bumerangs in einem angrenzenden ringförmigen Element erstreckt. - Stent nach Anspruch 1, wobei sich ein Element zur Verbindung untereinander (
128 ) von jedem Segment in der Form eines Bumerangs (118 ) in einem ringförmigen Element (114 ) zu einem Segment in der Form eines Bumerangs in einem angrenzenden ringförmigen Element erstreckt. - Stent nach Anspruch 3, wobei sich ein Element zur Verbindung untereinander (
128 ) von jedem Segment in der Form eines Bumerangs (118 ) in einem ringförmigen Element zu einem benachbarten Segment in der Form eines Bumerangs in einem angrenzenden ringförmigen Element erstreckt. - Stent nach Anspruch 1, wobei jedes Element zur Verbindung untereinander (
128 ) eine oder mehrere Krümmungen darin aufweist. - Stent nach Anspruch 5, wobei jedes Element zur Verbindung untereinander (
128 ) ein U-förmiges Segment ist. - Stent nach Anspruch 5, wobei jedes Element zur Verbindung untereinander (
128 ) ein zickzackförmiges (228 ) Segment ist. - Stent nach Anspruch 1, wobei mindestens ein Teil jedes Elementes zur Verbindung untereinander (
128 ) krummlinig ist. - Stent nach Anspruch 1, wobei jedes Segment in der Form eines Bumerangs (
114 ) symmetrisch zu einer sich von dem oberen Teil (119 ) des Segmentes in der Form eines Bumerangs zu dem unteren Teil (121 ) des Segmentes in der Form eines Bumerangs erstreckenden Mittellinie (138 ) ist, wobei die Mittellinie sich in der Mitte zwischen den Enden des Segmentes in der Form eines Bumerangs befindet. - Stent nach Anspruch 1, wobei jede Unterseite (
122 ) eine Vertiefung (130 ) darin aufweist. - Stent nach Anspruch 1, der aus Metall hergestellt ist.
- Stent nach Anspruch 11, der aus rostfreiem Stahl hergestellt ist.
- Stent nach Anspruch 11, wobei das Metall ein Metall mit Formgedächtnis ist.
- Stent nach Anspruch 1, der aus einem biologisch verträglichen Material hergestellt ist.
- Stent nach Anspruch 1, wobei angrenzende Segmente in der Form eines Bumerangs in einem kreisförmigen Element durch eine U-förmige oder zickzackförmige Verbindung untereinander verbunden sind.
