Description

  • Bereich der Erfindung
  • Die vorliegende Erfindung betrifft eine neue Art eines Stents zur transluminalen Implantierung und insbesondere neue Gefäßstents.
  • Hintergrund der Erfindung
  • Stents zur transluminalen Implantierung sind wohlbekannt. Sie bestehen im wesentlichen aus metallischen Halterungen, die in einen Teil des menschlichen Körpers, beispielsweise die Gallengänge, die Harnwege, den Verdauungskanal und insbesondere über den perkutanen Weg in die Blutgefäße eingeführt werden. gewöhnlich in die Arterien, wobei sie in diesem Fall typischerweise als Gefäßstents bezeichnet werden. Stents sind gewöhnlich im wesentlichen zylindrisch und werden derart konstruiert und angeordnet, daß sie sich in Radialrichtung ausdehnen, sobald sie in dem Körper positioniert sind. Sie werden gewöhnlich eingeführt, während sie einen ersten, verhältnismäßig kleinen Durchmesser aufweisen, und in einem gewünschten Bereich implantiert, beispielsweise in einem Gefäß, und dann wird der Stent in situ ausgedehnt, bis er einen zweiten Durchmesser erreicht, der größer als der erste Durchmesser ist. Um dem Stent die notwendige innere Radialkraft zu verleihen, damit er sich in Radialrichtung ausdehnt, wird gewöhnlich ein dem Katheter zugeordneter Ballon verwendet. Es sind auch selbstausdehnende Stents bekannt, die sich von einem ersten Durchmesser auf einen größeren, zweiten Durchmesser ausdehnen können, ohne daß ein Mittel zum Aufbringen einer inneren Radialkraft auf diese, beispielsweise ein Ballon, verwendet wird. Eine solche Art eines selbstausdehnenden Stents ist ein Stent, der aus einem Metall mit Formgedächtnis besteht und sich bei Einwirkung der Körpertemperatur auf seinen zweiten, größeren Durchmesser ausdehnt. Solche Stents sind ebenfalls bekannt.
  • In US 5,366,504 wird eine rohrförmige Prothese mit einem rohrförmigen Wandungsabschnitt mit locker ineinander hängendem Muster (beispielsweise aus Gestrickmaschen) offenbart, um in einem Körperlumen zu funktionieren. Die Pro these kann mit selbstausdehnenden Verankerungsenden und einem durch den Ballon ausdehnbaren Wandungsabschnitt ausgestattet sein.
  • In US 5,741,325 wird eine selbstausdehnende intraluminale Verbundmaterialprothese offenbart. Die Prothese kann wahlweise gegabelt sein.
  • Zusammenfassung der Erfindung
  • Mit der vorliegenden Erfindung wird eine neue Art eines Stents vorgeschlagen, der einen selbstausdehnenden Teil oder selbstausdehnenden Abschnitt mit einem Teil oder Abschnitt kombiniert, für den zur Ausdehnung eine innere Radialkraft erforderlich ist. Insbesondere umfaßt ein Stent zur transluminalen Implantierung gemäß der vorliegenden Erfindung zylindrische Teile, vorzugsweise nebeneinander liegende, wobei zumindest ein Teil selbstausdehnend ist, wohingegen ein anderer Teil, beispielsweise ein Ballonkatheter oder dergleichen, zu seiner Ausdehnung einer inneren Radialkraft bedarf. Grundsätzlich besteht der Stent aus mindestens zwei der oben genannten Teile, kann jedoch aus mehr als zwei Teilen bestehen. Durch eine solche Anordnung kann der Stent besser angelegt werden, da beim Lösen des Stents von seinem Zuführkatheter für eine sofortige Teilausdehnung gesorgt wird.
  • Der eine Teil, der in Radialrichtung selbstausdehnbar ist, und der andere Teil umfassen eine Vielzahl von Serpentinenbändern. Die Teile sind über Verbindungen, die zwischen den Bändern verlaufen, miteinander verbunden.

  • Ausführliche Beschreibung der bevorzugten Ausführungsformen
  • 1 zeigt einen Stent 10, der Selbstausdehnungseigenschaften und Ausdehnbarkeitseigenschaften durch den kombiniert, d.h. einer Ausdehnung durch Anwendung einer inneren Radialkraft bedarf. Vorzugsweise ist bei dem Stent 10 der selbstausdehnende Teil 12 an einem Ende nebeneinander liegend mit dem körpernahen Ende eines durch den Ballon ausdehnbaren Stents 14 verbunden. Die Teile des Stents können gemäß einem der bekannten, nach dem Stand der Technik vorhandenen Stents ausgebildet sein, auch von Stents, die aus einer Vielzahl von welligen Bändern ausgebildet sind, die durch zwischen benachbarten Bändern verlaufende Elemente miteinander verbunden sind. Der selbstausdehnende Teil 12 des Stents 10 besteht aus einem welligen Band 13, und der durch den Ballon ausdehnbare Stent 14 ist aus einer Vielzahl von welligen Bändern 15 ausgebildet, die durch Verbindungselemente 17 miteinander verbunden sind. Der selbstausdehnende Teil 12 ist durch Verbindungselemente 19 mit dem durch den Ballon ausdehnbaren Teil 14 verbunden. Vorzugsweise jedoch besteht der selbstausdehnende Teil 12 aus einem Metall mit Formgedächtnis, beispielsweise Nitinol, so daß die Selbstausdehnung bei Körpertemperatur beim Lösen des Stents von seinem (nicht gezeigten) Zuführkatheter möglich wird. Durch die sofortige Ausdehnung des Teils 12 wird zur Anlegung des Stents während des Lösens beibetragen, wonach ein Ballon oder dergleichen zum Ausdehnen des Teils 14 verwendet werden kann.
  • Der Teil 12 kann einen größeren Durchmesser aufweisen, oder er kann konisch aufgeweitet sein, um für den frühestmöglichen Kontakt mit dem Körper abschnitt zu sorgen, in den er eingeführt wird, beispielsweise in ein Gefäß. Das ist beispielsweise am besten in den 2 und 3 zu sehen, die einen Stent 10 ähnlich dem gemäß 1 an einem Zuführkatheter zeigen. 2 zeigt den Stent 10 mit den zwei Teilen 12 bzw. 14. Der Teil 12 ist der selbstausdehnende Teil, während der Teil 14 einer getrennten Kraft zum Ausdehnen bedarf, beispielsweise eines Ballons 16 eines Zuführkatheters, der allgemein bei 18 im Innern des Stents angezeigt ist. In 2 ist der Stent vor dem Lösen oder dem Entfalten gezeigt, der selbstausdehnende Teil ist von einer zurückziehbaren Hülle 20 umschlossen. In 3 ist die Hülle zurückgezogen, wodurch der selbstausdehnende Teil 12 konisch nach außen aufgeweitet werden kann. Anschließend kann der Ballon 16 in der bekannten Weise zum Ausdehnen des Teils 14 verwendet werden.
  • Da die meisten Stentzuführkatheter eine zurückziehbaren Hülle enthalten, die den Stent zum Implantieren und zum Entfernen aus dem Katheter freilegt, wird der selbstausdehnende Teil 12, wenn er in der gezeigten Weise an dem körpernahen Ende des Stents angelegt ist, sehr wahrscheinlich den Stent an Ort und Stelle festklemmen können, bis der Rest des Stents beispielsweise durch Ausdehnen des Ballons ausgedehnt ist. Diese Anlegung ist zu bevorzugen, obwohl die Anlegung in anderen Teilen des eigentlichen Stents ebenso angewandt werden kann.
  • Wenn der selbstausdehnende Abschnitt an dem körpernahen Ende des Stents angelegt wird, eignet sich ein solcher Stent besonders beim Einführen von Stents in Gefäße, bei dem das Gefäß eine sich gabelnde oder verzweigende Art ist. Ein solcher Stent ist schematisch bei 10 in den 4 und 5 gezeigt. 4 zeigt den Stent mit einem körpernahen Endteil 12, der selbstausdehnend ist, während der Rest des Stentkörpers 14 durch den Ballon 16 an dem Katheter 18 ausdehnbar ist. In 4 ist die zurückziehbare Hülle entfernt, wodurch sich der selbstausdehnende Teil an dem Ostium 22 in einem Gefäß 24 ausdehnen kann, das von einem Hauptgefäß abzweigt, beispielsweise einer Aorta 26, was zu einer genaueren Anlegung des Stents beiträgt. 5 zeigt den Stent nach der Ausdehnung des Teils 14 durch den Ballonkatheter und dem Zurückziehen des Katheters. Mit einem Stent gemäß der vorliegenden Erfindung kann der Stent definitiv nahe dem Ostium der Gabelung angelegt werden.
  • Stents gemäß der Erfindung können auch mehr als einen selbstausdehnenden und einen durch den Ballon ausdehnbaren Teil umfassen. Jede Kombination dieser Teile wird als innerhalb des Umfangs der Erfindung liegend betrachtet. Beispielsweise ist in 6 ein Stent 110 gezeigt, der an jedem Ende eines durch den Ballon ausdehnbaren Teils 114 einen selbstausdehnenden Teil 112 aufweist. Der selbstausdehnende Teil 112 des Stents 110 besteht aus einem welligen Band 113, und der durch den Ballon ausdehnbare Stent 114 ist aus einer Vielzahl von welligen Bändern 115 ausgebildet, die durch Verbindungselemente 117 miteinander verbunden sind. Der selbstausdehnende Teil 112 ist durch Verbindungselemente 119 mit dem durch den Ballon ausdehnbaren Teil 114 verbunden.
  • In den 710 sind nunmehr gegabelte Stents beschrieben, bei denen die Erfindung genutzt wird. Im wesentlichen bestehen gegabelte Stents aus drei Segmenten oder Bestandteilen, einem Hauptrumpf und zwei Zweigen. Bei der Nutzung der Erfindung kann jedes der drei Teile bei Bedarf durch den Ballon ausdehnbar oder selbstausdehnbar gestaltet werden. Beispielsweise zeigt 7 einen nicht ausgedehnten gegabelten Stent 10 mit einer Konfiguration in schematischer Form mit einem Rumpf 30, einem ersten Zweig 32 und einem zweiten Zweig 34. Wie gezeigt ist, ist der Rumpf 30 selbstausdehnend, beispielsweise der Teil 12 in den vorhergehenden Figuren, und die Zweige 32 und 34 sind durch den Ballon ausdehnbar, beispielsweise der Teil 14 in den vorhergehenden Figuren.
  • 8 zeigt den Stent gemäß 7 in implantierter Lage in einem sich verzweigenden Gefäß 36.
  • 9 zeigt ein anderes Beispiel für einen nicht ausgedehnten, gegabelten Stent 10 mit einer anderen Konfiguration in schematischer Form, der aus einem Rumpf 30 und Zweigen 32 und 34 besteht. In diesem Fall sind der Rumpf 30 und der Zweig 32 beide selbstausdehnend, und der Zweig 34 ist durch den Ballon ausdehnbar.
  • 10 zeigt den Stent gemäß 9 ausgedehnt in einer implantierten Stelle in einem sich verzweigenden Gefäß 36.
  • Der Rumpf 30 bei einer anderen Ausführungsform kann durch den Ballon ausdehnbar sein, während die Zweige 32 und 34 selbstausdehnbar sein können.
  • Die obigen Beispiele und die Offenbarung sollen veranschaulichend und nicht erschöpfend sein. In diesen Beispielen und der Beschreibung werden dem Fachmann viele Variationen und Alternative vorgeschlagen. Alle diese Alternativen und Variationen sollen innerhalb des Umfangs der beigefügten Ansprüche enthalten sein. Mit der Technik vertraute Personen können weitere Äquivalente zu den speziellen hier beschriebenen Ausführungsformen erkennen, wobei diese Äquivalente ebenfalls durch die hieran angefügten Ansprüche umfaßt sein sollen.

Claims (13)

  1. Stent (10) zur transluminalen Implantierung, bestehend aus einem zylindrischen Aufbau mit mindestens zwei verbundenen zylindrischen Teilen, wobei ein Teil (12) in Radialrichtung selbstausdehnbar ist und der andere Teil (14) nur mit Hilfe einer inneren Radialkraft in Radialrichtung ausdehnbar ist, wobei der eine Teil (12), der in Radialrichtung selbstausdehnbar ist, und der andere Teil (14) einstückig verbunden sind, dadurch gekennzeichnet, daß die Teile eine Vielzahl von Serpentinenbändern umfassen, wobei benachbarte derselben über zwischen den Bändern verlaufende einstückig ausgebildete Verbindungen miteinander verbunden sind.
  2. Stent nach Anspruch 1, wobei sich die zwei Teile in Nebeneinanderlage befinden.
  3. Stent nach Anspruch 1 oder 2, wobei der andere Teil (14) durch einen Ballon ausdehnbar ist.
  4. Stent nach einem der Ansprüche 1 – 3, wobei der eine Teil (12) aus Nitinol besteht.
  5. Stent nach einem der Ansprüche 1 – 4, wobei der eine Teil (12) einen proximalen Abschnitt des Stents umfaßt.
  6. Stent nach Anspruch 5, wobei der eine Teil (12) in expandiertem Zustand in bezug auf den Rest des Stents konisch aufgeweitet ist.
  7. Stent nach Anspruch 6, wobei der selbstausdehnbare Teil (12) konisch aufgeweitet ist.
  8. Stent nach einem der Ansprüche 1 – 7, wobei der selbstausdehnbare Teil (12) einen größeren Gesamtdurchmesser als der Rest des Stents aufweist.
  9. Stent nach einem der Ansprüche 1 – 8, mit einem selbstausdehnbaren Teil an jedem Ende des anderen Teils.
  10. Stent nach einem der Ansprüche 1 – 9, dadurch gekennzeichnet, daß der Steht aus drei Teilen mit einem Stammteil (30) und zwei Zweigteilen (32, 34) besteht, und bei dem mindestens ein Teil selbstausdehnbar ist und mindestens ein Teil mit Hilfe einer inneren Radialkraft in Radialrichtung ausdehnbar ist.
  11. Stent nach Anspruch 10, wobei der Stammteil (30) und ein Zweigteil selbstausdehnbar sind und der andere Zweigteil durch einen Ballon ausdehnbar ist.
  12. Stent nach Anspruch 10, wobei der Stammteil selbstausdehnbar ist und die Zweigteile durch einen Ballon ausdehnbar sind.
  13. Stent nach einem der Ansprüche 1 – 12, wobei der selbstausdehnbare Teil ein Serpentinenband umfaßt, das länger als die Serpentinenbänder in dem Teil ist, das nur mit Hilfe einer inneren Radialkraft in Radialrichtung ausdehnbar ist.
