Description
- Die vorliegende Anmeldung ist eine Continuation der am 27. April 1998 eingereichten vorläufigen U.S.-Patentanmeldung mit der Serial No. 60/083,135.
- Technischer Bereich
- In einem Aspekt betrifft die vorliegende Anmeldung ein Verfahren zur Behandlung von medizinischen Vorrichtungen mit Beschichtungszusammensetzungen, um die Freisetzung von pharmazeutischen Mitteln von der Oberfläche der Vorrichtungen unter physiologischen Bedingungen zur Verfügung zu stellen. In einem anderen Aspekt betrifft die Erfindung die Beschichtungszusammensetzungen per se und Vorrichtungen, die mit solchen Zusammensetzungen beschichtet sind.
- Hintergrund der Erfindung
- Viele chirurgische Eingriffe erfordern die Plazierung einer medizinischen Vorrichtung in den Körper. Während für die Behandlung einer Vielzahl von medizinischen Zuständen nötig und vorteilhaft, verursacht die Plazierung von Metall- oder Polymervorrichtungen im Körper das Auftreten von zahlreichen Komplikationen. Einige dieser Komplikationen schließen ein: erhöhtes Risiko von Infektionen; Auslösen einer Fremdkörperantwort, die zu Entzündung und fibröser Enkapsulierung führt; und Auslösen einer Wundheilungsantwort, die zu Hyperplasie und Restenose führt. Diese und andere Komplikationen müssen berücksichtigt werden, wenn eine Metall- oder Polymervorrichtung in den Körper eingeführt wird.
- Ein Ansatz, um die potentiell schädlichen Effekte von solch einer Einführung zu reduzieren, ist es, zu versuchen eine mehr biokompatible Vorrichtung zur Verfügung zu stellen. Während verschiedene Verfahren erhältlich sind, um die Biokompatibilität von medizinischen Vorrichtungen zu erhöhen, ist es ein Verfahren, das mit begrenztem Erfolg einherging, die Vorrichtung mit der Fähigkeit auszustatten bioaktive Verbindungen in die Umgebung des Implantats zuzuführen. Indem dies getan wird, können einige der schädlichen Effekte, die mit der Implantation mit medizinischen Vorrichtungen assoziiert sind, vermindert werden. So können zum Beispiel Antibiotika von der Oberfläche der Vorrichtung freigesetzt werden, um die Möglichkeit von Infektion zu minimieren und anti-proliferative Wirkstoffe können freigesetzt werden, um eine Hyperplasie zu inhibieren. Ein weiterer Vorteil der lokalen Freisetzung von bioaktiven Mitteln ist die Vermeidung von toxischen Konzentrationen von Wirkstoffen, die manchmal erforderlich sind, wenn systemisch verabreicht, um therapeutische Konzentrationen an der Stelle zu erreichen, wo diese benötigt werden.
- Obwohl die potentiellen Vorteile, die von der Verwendung von medizinischen Vorrichtungen erwartet werden die in der Lage sind pharmazeutische Mittel an ihren Oberflächen freizusetzen, groß sind, war die Entwicklung von solchen medizinischen Vorrichtungen langsam. Diese Entwicklung wurde durch die vielen Herausforderungen behindert, die erfolgreich überwunden werden müssen, wenn diese Entwicklung durchgeführt wird. Einige dieser Herausforderungen sind: 1) Erfordernis, in einigen Fällen, von Langzeit-Freisetzung von bioaktiven Mitteln; 2) der Bedarf für eine biokompatible, nicht-entzündliche Vorrichtungsoberfläche; 3) der Bedarf für signifikante Haltbarkeit, insbesondere bei Vorrichtungen, die eine Flexion und/oder Expansion erfahren, wenn sie in den Körper implantiert oder verwendet werden; 4) Bedenken in Bezug auf die Verarbeitbarkeit, um es zu ermöglichen, die Vorrichtung auf eine wirtschaftlich vernünftige und reproduzierbare Weise herzustellen; und 5) das Erfordernis, daß die fertige Vorrichtung unter der Anwendung von herkömmlichen Verfahren sterilisierbar ist.
- Verschiedenen implantierbare medizinische Vorrichtungen, die in der Lage sind, medizinische Wirkstoffe zuzuführen, wurden beschrieben. Verschiedene Patente sind auf Vorrichtungen gerichtet, die bio-abbaubare oder bio-resorbierbare Polymere als Wirkstoffenthaltende und -freisetzende Beschichtungen verwenden, einschließlich Tang et al., U.S. Patent 4,916,193 und MacGregor, U.S. Patent 4,994,071. Andere Patente sind auf die Bildung eines Wirkstoff-enthaltenden Hydrogels auf der Oberfläche einer implantierbaren medizinischen Vorrichtung gerichtet, diese schließen Amiden et al, U.S. Patent 5,221,698 und Sahatjian, U.S. Patent 5,304,121 ein. Noch weitere Patente beschreiben Verfahren zur Herstellung von beschichteten intravaskulären Stents über die Aufbringung von Polymerlösungen, die dispergiertes therapeutisches Material enthalten, an die Stentoberfläche, gefolgt von Abdampfen des Lösungsmittels. Dieses Verfahren ist in Berg et al, U.S. Patent 5,464,650 beschrieben.
- Jedoch verbleiben signifikante Probleme, die überwunden werden müssen, um eine therapeutisch signifikante Menge einer bioaktiven Verbindung auf der Oberfläche der implantierbaren medizinischen Vorrichtung zur Verfügung zu stellen. Dies ist insbesondere dann wahr, wenn die Beschichtungszusammensetzung auf der Vorrichtung während des Verlaufs einer Flexion und/oder Expansion während der Implantation oder Verwendung gehalten werden muß. Es ist ebenfalls wünschenswert, ein einfaches und leicht durchführbares Verfahren zur Kontrolle der Rate von bioaktiver Freisetzung von der Oberfläche der Vorrichtung zu haben.
- Obwohl eine Vielzahl von hydrophoben Polymeren früher zur Verwendung als Wirkstoff-Freisetzungsbeschichtungen beschrieben wurden, hat die Anmelderin gefunden, daß nur eine kleine Zahl diejenigen physikalischen Charakteristika besitzen, die sie für implantierbare medizinische Vorrichtungen brauchbar machen, die eine Flexion und/oder Expansion während der Implantation erfahren. Viele Polymere, die gute Wirkstoff-Freisetzungscharalcteristika zeigen, wenn sie allein als Wirkstoffzufuhrvehikel verwendet werden, stellen Beschichtungen zur Verfügung, die zu brüchig sind, um auf Vorrichtungen verwendet zu werden, die eine Flexion und/oder Expansion erfahren. Andere Polymere können eine entzündliche Antwort hervorrufen, wenn implantiert. Diese oder andere Polymere zeigen für einen Wirkstoff gute Wirkstoff-Freisetzungscharakteristika, jedoch sehr schlechte für einen anderen.
- Einige Polymere zeigen gute Dauerhaftigkeit- und Flexibilitätscharakteristika, wenn sie auf Vorrichtungen ohne Wirkstoff aufgebracht sind, verlieren jedoch diese bevorzugten Charakteristika, wenn Wirkstoff hinzugefügt wird. Oftmals ist weiterhin, je höher die Konzentration von Wirkstoffen oder je dicker die Aufbringung von Polymer auf der Vorrichtungsoberfläche, desto schlechter werden die physikalischen Charakteristika des Polymers. Es war sehr schwierig, ein Polymer zu identifizieren, das die richtigen physikalischen Charakteristika in Anwesenheit von Wirkstoffen zur Verfügung stellt und in welchem die Wirkstoffzuführrate durch Veränderung der Konzentration des Wirkstoffs in dem Polymer oder der Dicke der Polymerlage kontrolliert werden kann.
- Es verbleibt daher ein Bedarf an einer implantierbaren medizinischen Vorrichtung, die eine Flexion und/oder Expansion nach Implantation erfahren kann, und die ebenfalls in der Lage ist, eine therapeutisch signifikante Menge eines pharmazeutischen Mittels oder Mittel von der Oberfläche der Vorrichtung zuzuführen.

- Die vorliegende Erfindung stellt eine Beschichtungszusammensetzung und verwandte Verfahren zur Verwendung der Zusammensetzung zur Verfügung, um eine implantierbare medizinische Vorrichtung mit einem bioaktiven Mittel auf eine Weise zu beschichten, die es der Oberfläche erlaubt, das bioaktive Mittel über die Zeit hinweg freizusetzen, wenn in vivo implantiert. In einer besonders bevorzugten Ausführungsform ist die Vorrichtung eine solche, die während des Verlaufs der Implantation oder der Verwendung in vivo eine Flexion und/oder Expansion erfährt.
- Die Zusammensetzung umfaßt ein bioaktives Mittel in Kombination mit einer Vielzahl von Polymeren, einschließlich einer ersten Polymerkomponente und einer zweiten Polymerkomponente. Die Polymerkomponenten sind dazu angepaßt, gemischt zu werden, um ein Gemisch zur Verfügung zu stellen, das eine optimale Kombination von physikalischen Charakteristika (z. B. Adhäsion, Dauerhaftigkeit, Flexibilität) und bioaktive Freisetzungscharakteristika zu zeigen, im Vergleich zu den Polymeren, wenn allein oder im Gemisch mit anderen Polymeren verwendet, die vorher bekannt waren. In einer bevorzugten Ausführungsform umfaßt die Zusammensetzung umfaßt mindestens ein Poly(alkyl)(meth)acrylat mit Alkylkettenlängen von 2 bis 8 Kohlenstoffen als eine erste Polymerkomponente und Polyethylen-co-vinylacetat) ("pEVA") als eine zweite Polymerkomponente.
- Die Zusammensetzung und das Verfahren können dazu verwendet werden, um die Menge und Rate Freisetzung des bioaktiven Mittels (z. B. Wirkstoffs) von einer oder mehreren Oberflächen von implantierbaren medizinischen Vorrichtungen zu kontrollieren. In einer bevorzugten Ausführungsform verwendet das Verfahren ein Gemisch von hydrophoben Polymeren in Kombination mit einem oder mehreren bioaktiven Mitteln, wie zum Beispiel einem pharmazeutischen Mittel, so daß die Menge und Rate der Freisetzung von Mittel(n) von der medizinischen Vorrichtung kontrolliert werden kann, z. B. durch Einstellen der relativen Typen und/oder Konzentrationen von hydrophoben Polymeren in dem Gemisch.
- Für eine bestimmte Kombination von Polymeren erlaubt dieser Ansatz, zum Beispiel, daß die Freisetzungsrate durch einfaches Einstellen der relativen Konzentrationen der Polymere in dem Beschichtungsgemisch eingestellt und kontrolliert wird. Dieses beseitigt den Bedarf, die bioaktive Freisetzungsrate durch die Polymerselektion, viele Beschichtungen oder Lagen von Beschichtungen zu kontrollieren und vereinfacht daher die Herstellung von bioaktive Mittel freisetzenden implantierbaren medizinischen Vorrichtungen sehr.
- Ein bevorzugte Beschichtung dieser Erfindung schließt ein Gemisch von zwei oder mehr Polymeren ein, die komplementäre physikalische Charakteristika aufweisen und einem pharmazeutischen Mittel oder Mitteln, die auf die Oberfläche einer implantierbaren medizinischen Vorrichtung aufgebracht werden, die bei der Implantation oder Verwendung eine Flexion und/oder Expansion erfährt. Die aufgebrachte Beschichtung wird gehärtet (z. B. Lösungsmittel evaporiert), um eine zähe und flexible bioaktive Mittel freisetzende Beschichtung auf der Oberfläche der medizinischen Vorrichtung zur Verfügung zu stellen. Die komplementären Polymere werden so ausgewählt, daß ein breiter Bereich von relativen Polymerkonzentrationen verwendet werden kann, ohne daß die wünschenswerten physikalischen Charakteristika der Polymere schädlich beeinträchtigt werden. Durch Verwendung der Polymergemische der Erfindung kann die Rate der Freisetzung des bioaktiven Mittels von einer beschichteten medizinischen Vorrichtung, durch Einstellen der relativen Konzentrationen der Polymere manipuliert werden. Ähnlich kann ein Spektrum von pharmazeutischen Mitteln von der Beschichtung zugeführt werden, ohne den Bedarf ein neues Polymer zu finden oder die Beschichtung auf der Vorrichtung mit einer Lage zu versehen.
- Genaue Beschreibung der Erfindung
- Die vorliegende Erfindung betrifft eine Beschichtungszusammensetzung und ein verwandtes Verfahren zur Beschichtung einer implantierbaren medizinischen Vorrichtung, die nach der Implantation eine Biegung und/oder Expansion erfährt. Die Struktur und Zusammensetzung der unterliegenden Vorrichtung kann von jedem geeigneten und medizinisch akzeptablen Aufbau sein und kann aus jedem geeigneten Material hergestellt sein, das mit der Beschichtung selber kompatibel ist. Die Oberfläche der medizinischen Vorrichtung wird mit einer Beschichtung versehen, die ein oder mehrere bioaktive Mittel enthält.
- Um eine bevorzugte Beschichtung zur Verfügung zu stellen, wird eine Zusammensetzung hergestellt, die ein Lösungsmittel, eine Kombination von komplementären Polymeren, die in dem Lösungsmittel gelöst sind und das bioaktive Mittel oder Mittel, die in dem Polymer/Lösungsmittelgemisch dispergiert sind, einschließt. Das Lösungsmittel ist bevorzugterweise ein solches, in dem die Polymere eine echte Lösung bilden. Das pharmazeutische Mittel selber kann entweder in dem Lösungsmittel löslich sein oder durch das Lösungsmittel hindurch eine Dispersion bilden.
- Die erhaltene Zusammensetzung kann auf die Vorrichtung auf jede geeignete Weise aufgebracht werden, z. B. kann sie direkt auf die Oberfläche der medizinischen Vorrichtung aufgebracht werden oder, alternativ, auf die Oberfläche einer Oberflächen-modifizierten medizinischen Vorrichtung, durch Eintauchen, Aufsprühen oder jegliche herkömmliche Technik. Das Verfahren zur Aufbringung der Beschichtungszusammensetzung auf die Vorrichtung wird typischerweise durch die Geometrie der Vorrichtung und andere Verfahrensüberlegungen bestimmt. Die Beschichtung wird anschließend durch Evaporation des Lösungsmittels gehärtet. Der Härtungsprozeß kann bei Raumtemperatur, erhöhter Temperatur oder mit der Hilfe von Vakuum durchgeführt werden.
- Das Polymergemisch zur Verwendung in dieser Erfindung ist bevorzugterweise biokompatibel, z. B. so daß es zu keiner Induktion von Entzündung oder Irritation führt, wenn es implantiert wird. Zusätzlich muß die Polymerkombination brauchbar unter einem breiten Spektrum von sowohl absoluten Konzentrationen und relativen Konzentrationen der Polymere sein. Dies bedeutet, daß die physikalischen Charakteristika der Beschichtung, sowie Festigkeit, Härte, Flexibilität und Expandierbarkeit typischerweise über einen breiten Bereich von Polymerkonzentrationen geeignet sein werden. Weiterhin kann die Fähigkeit der Beschichtung, die Freisetzungsraten einer Vielzahl von pharmazeutischen Mitteln zu kontrollieren, bevorzugterweise durch die Variation der absoluten und relativen Konzentrationen der Polymere manipuliert werden.
- Eine erste Polymerkomponente diese Erfindung stellt eine optimale Kombination von verschiedenen strukturellen/funktionellen Eigenschaften zur Verfügung, einschließlich Hydrophobizität, Härte, Freisetzungscharakteristika für das bioaktive Mittel, Biokompatibilität, Molekulargewicht und Erhältlichkeit (und Kosten).
- Beispiele von geeigneten ersten Polymeren schließen Poly(alkyl)(meth)acrylate, diejenigen mit Alkylkettenlängen von 2 bis 8 Kohlenstoffatomen und mit Molekulargewichten von 50 Kilodalton bis 900 Kilodalton, ein. Ein Beispiel eines besonders bevorzugten ersten Polymers ist Poly-n-butylmethacrylat. Solche Polymere sind käuflich erhältlich, z. B. von Aldrich, wobei die Molekulargewichte von ungefähr 200.000 Dalton bis ungefähr 320.000 Dalton reichen und mit einer sich verändernden inhärenten Viskosität, Löslichkeit und Form (z. B. als Kristalle oder Pulver).
- Eine zweite Polymerkomponente dieser Erfindung stellt eine optimale Kombination von ähnlichen Eigenschaften zur Verfügung und insbesondere wenn im Gemisch mit der ersten Polymerkomponente verwendet. Beispiele von geeigneten zweiten Polymeren sind käuflich erhältlich und schließen Polyethylen-co-vinylacetat) ein, das Vinylacetat-Konzentrationen von zwischen ungefähr 10% und ungefähr 50% aufweist, in der Form von Perlen, Pellets, Granulat, usw. (käuflich erhältlich sind 12%, 14%, 18%, 25%, 33%). pEVA-Copolymere mit geringeren Prozent Vinylacetat werden zunehmend unlöslich in typischen Lösungsmitteln, wohingegen diejenigen mit höheren Prozent Vinylacetat abnehmend dauerhaft werden.
- Ein besonders bevorzugtes Polymergemisch zur Verwendung in dieser Erfindung schließt Gemische von Poly(butylmethacrylat) (pBMA) und Polyethylen-co-vinylacetat) Copolymere (pEVA) ein. Dieses Gemisch von Polymeren hat sich als mit absoluten Polymerkonzentrationen brauchbar (d. h., die gesamten kombinierten Konzentrationen von beiden Polymeren in der Beschichtungszusammensetzung) von zwischen ungefähr 0,25 und ungefähr 70 Gew.% erwiesen. Es hat sich weiterhin als effektiv mit individuellen Polymerkonzentrationen in der Beschichtungslösung von zwischen ungefähr 0,05 und ungefähr 70 Gew.% erwiesen. In einer bevorzugten Ausführungsform schließt das Polymergemisch Poly(n-butylmethacrylat) (pBMA) mit einem Molekulargewicht von zwischen 100 Kilodalton bis 900 Kilodalton und ein pEVA-Copolymer mit einem Vinylacetatgehalt von zwischen 24 bis 36 Gew.% ein. In einer besonders bevorzugten Ausführungsform schließt das Polymergemisch Poly(n-butylmethacrylat) mit einem Molekulargewicht von 200 Kilodalton bis 400 Kilodalton und einem pEVA-Copolymer mit einem Vinylacetatgehalt von 30 bis 40 Gew.% ein. Die Konzentration des bioaktiven Mittels oder der Mittel, die in dem Beschichtungsgemisch gelöst oder suspendiert sind, kann von 0,01 bis 90 Gew.%, basierend auf dem Gewicht der finalen Beschichtungszusammensetzung, reichen.
- Die bioaktiven (z. B. pharmazeutischen) Mittel, die in der vorliegenden Erfindung brauchbar sind, schließen im wesentlichen jede therapeutische Substanz zur Aufbringung an der Implantationsstelle ein, die wünschenswerte therapeutische Charakteristika besitzt. Diese Mittel schließen ein: Thrombin-Inhibitoren, antithrombotischen Mitteln, thrombolytischen Mitteln, fibrinolytischen Mitteln, Vasospasmus-Inhibitoren, Calciumkanalblockern, Vasodilatatoren, blutdruck-senkenden Mitteln, antimikrobiellen Mitteln, Antibiotika, Inhibitoren von Oberflächenglykoproteinrezeptoren, Antithrombozytenmitteln, Antimiotika, Mikrotubulus-Inhibitoren, antisekretorischen Mitteln, Actin-Inhibitoren, Remodellierungs-Inhibitoren, Antisense-Nukleotiden, Antimetaboliten, Antiproliferativa, chemotherapeutischen Antikrebsmitteln, entzündungshemmenden Steroiden oder nicht-steroiden entzündungshemmenden Mitteln, immunsuppressiven Mitteln, Wachstumshormon-Antagonisten, Wachstumsfaktoren, Dopamin-Agonisten, radiotherapeutischen Mitteln, Peptiden, Proteinen, Enzymen, extrazellulären Matrixkomponenten, Inhibitoren, Abfangmitteln für freie Radikale, Chelatbildnern, Antioxidationsmitteln, Antipolymerasen, antiviralen Mitteln, Mitteln für photodynamische Therapie und Mitteln für Gentherapie.
- Eine Beschichtungszusammensetzung dieser Erfindung wird bevorzugterweise dazu verwendet, eine implantierbare medizinische Vorrichtung zu beschichten, die eine Flexion oder Expansion während des Verlaufs ihrer Implantation oder Verwendung in vivo erfährt. Die Worte "Flexion" und "Expansion", wie hier verwendet, in Bezug auf implantierbare Vorrichtungen, werden sich auf eine Vorrichtung oder einen Teil davon beziehen, der gebogen wird (z. B. um mindestens 45 Grad oder mehr) und/oder expandiert wird (z. B. auf mehr als zweimal seiner anfänglichen Dimension), entweder im Verlauf ihrer Plazierung oder danach im Verlauf ihrer Verwendung in vivo.
- Beispiele von geeigneten Kathetern schließen urinäre Katheter ein, die von der Inkorporation von antimikrobiellen Mitteln (z. B. Antibiotika, wie zum Beispiel Vancomycin oder Norfloxacin) in eine Oberflächenbeschichtung profitieren würden und intravenöse Katheter, die von antimikrobiellen Mitteln und von antithrombotischen Mitteln (z. B. Heparin, Hirudin, Cumadin) profitieren würden. Solche Katheter werden typischerweise aus solchen Materialien wie Silikongummi, Polyurethan, Latex und Polyvinylchlorid hergestellt.
- Die Beschichtungszusammensetzung kann ebenfalls dazu verwendet werden, Stents zu beschichten, z. B. entweder selbst-expandierende Stents (wie zum Beispiel die Wallstent-Varietät) oder Ballon-expandierbare Stents (wie erhältlich in einer Vielzahl von Formen, zum Beispiel Gianturco-Roubin, Palmaz-Shatz, Wiktor, Strecker, ACS Multi-Link, Cordis, AVE Micro Stent), die typischerweise aus Materialien wie zum Beispiel rostfreiem Stahl oder Tantal hergestellt sind.
- Eine Beschichtungszusammensetzung der vorliegenden Erfindung kann dazu verwendet werden, eine Implantatoberfläche zu beschichten, unter der Verwendung von jeglichen geeigneten Mitteln, wie z. B. durch Eintauchen, Sprühen oder ähnliches. Die Geeignetheit der Beschichtungszusammensetzung zur Verwendung auf einem bestimmten Material, und im Gegenzug die Geeignetheit der beschichteten Zusammensetzung, kann durch den Fachmann angesichts der vorliegenden Beschreibung ermittelt werden.
- Das Gesamtgewicht der Beschichtung auf der Oberfläche ist typischerweise nicht wichtig. Das Gewicht der Beschichtung, das dem bioaktiven Mittel zugeordnet werden kann, liegt bevorzugterweise im Bereich von ungefähr 0,05 mg bis ungefähr 10 mg von bioaktivem Mittel pro cm2 der Bruttooberflächen der Vorrichtung. Weiter bevorzugt liegt das Gewicht der Beschichtung, das dem Bioaktiva zugeordnet werden kann, zwischen ungefähr 1 mg und ungefähr 5 mg bioaktivem Mittel pro cm2 der Bruttooberflächen der Vorrichtung. Diese Menge von Wirkstoff wird im allgemeinen benötigt, um eine geeignete Aktivität unter physiologischen Bedingungen zur Verfügung zu stellen.
- Im Gegenzug wird die Beschichtungsdicke einer im Augenblick bevorzugten Zusammensetzung typischerweise im Bereich von ungefähr 5 Mikrometern bis ungefähr 100 Mikrometern liegen. Dieser Level von Beschichtungsdicke wird im allgemeinen benötigt, um eine geeignete Dichte von Wirkstoff zur Verfügung zu stellen, um eine geeignete Aktivität unter physiologischen Bedingungen zur Verfügung zu stellen.
- Die Erfindung wird nun mit Bezug auf die folgenden nicht-begrenzenden Beispiele weiter beschrieben werden. Es wird dem Fachmann ersichtlich sein, daß viele Veränderungen in den beschriebenen Ausführungsformen gemacht werden können, ohne sich vom Bereich der vorliegenden Erfindung zu entfernen. Daher sollte der Bereich der vorliegenden Erfindung nicht auf die in dieser Anmeldung beschriebenen Ausführungsformen begrenzt werden, sondern nur durch die Ausführungsformen, die durch die Sprache der Ansprüche beschrieben werden und die Äquivalente dieser Ausführungsformen. Wo nicht anders angegeben, sind alle Prozentsätze in Gewichtsprozent.
- Beispiele
- Testverfahren
- Die potentielle Geeignetheit von bestimmten beschichteten Zusammensetzungen zur Verwendung in vivo kann durch eine Vielzahl von Verfahren bestimmt werden, einschließlich Haltbarkeits-, Flexibilitäts- und Freisetzungs-Tests, von denen von jedem hier Beispiele beschrieben werden.
- Probenherstellung
- Ein Millimeter-Durchmesser rostfreie Stahldrähte (z. B. 304 Grad) werden in 5 Zentimeter Längen geschnitten. Die Drahtsegmente können Parylen-behandelt oder mit keiner Behandlung evaluiert werden. Die Drahtsegmente werden auf einer Mikro-Waage gewogen.
- Bioaktives Mittel/Polymergemische werden in einem Bereich von Konzentrationen in einem geeigneten Lösungsmittel, auf die hier beschriebene Weise hergestellt. Die Beschichtungsgemische werden auf die jeweiligen Drähte oder Teile davon, durch Eintauchen oder Besprühen aufgebracht, und den beschichteten Drähten wird es erlaubt, durch Lösungsmittelevaporation zu härten. Die beschichteten Drähte werden nochmals gewogen. Von diesem Gewicht wird die Masse der Beschichtung berechnet, was im Gegenzug erlaubt, die Masse der/des beschichteten Polymer(s) und des bioaktiven Mittels zu bestimmen. Die Beschichtungsdicke kann unter der Verwendung geeigneter Mittel, z. B. durch die Verwendung eines Mikroprozessor-Beschichtungsdicke-Meßgeräts (Minitest 4100) gemessen werden.
- Die Dauerhaftigkeit und Flexibilität der beschichteten Zusammensetzung kann auf die folgende Weise bestimmt werden.
- Dauerhaftigkeitstest
- Ein geeigneter Dauerhaftigkeitstest schließt ein Verfahren ein, bei dem eine beschichtete Probe (z. B. Draht) wiederholten Reibungskräften unterzogen wird, die dazu gedacht sind, die Art von Abnutzung zu simulieren, welcher die Probe bei aktueller Verwendung ausgesetzt würde, wie zum Beispiel eine implantierbare Vorrichtung, die eine Biegung und/oder Expansion im Verlauf ihrer Implantation oder Verwendung erfährt.
- Der unten beschriebene Test verwendet eine wiederholte 60-Zyklusbehandlung und wird verwendet, um zu bestimmen, ob irgendeine Veränderung in den Kräftemessungen zwischen den ersten 5 Zyklen und den letzten 5 Zyklen auftritt oder ob irgendein durch Scanning-Elektronenmikroskopie ("SEM") Analyse nachweisbares Abblättern oder Verkratzen auftritt. Regenerierte Cellulosemembran wird hydriert und um einen 200 Gramm-rostfreien Stahlschlitten gewickelt. Die Zellulosemembran wird auf der gegenüberliegenden Seite des Schlittens eng festgeklammert. Der Schlitten mit einem rotierbaren Arm wird dann an ein 250 Gramm digitales Kraftmeßgerät mit Computerschnittstelle angebracht. Die Testoberfläche wird auf einen Schienentisch mit Mikro-Schrittmotorkontrolle montiert. Die Drähte werden auf die Testoberfläche geklammert. Der Zellulose bedeckte Schlitten wird auf die Oberfläche der Kabel plaziert. Anfangs werden Kraftmessungen genommen, während sich der Schlitten bei 0,5 cm/Sek. über einen 5 cm Teil für 5 Drücken/Ziehen-Zyklen bewegt. Der Schlitten fuhr dann fort, über die beschichteten Proben für 50 Drücken/Ziehen-Zyklen bei 5 cm/Sek. zu kreisen, um Abrieb zu simulieren. Die Geschwindigkeit wird dann auf 0,5 cm/Sek. verringert und die finalen Kraftmessungen werden über weitere 5 Druck/Zieh-Zyklen genommen.
- SEM-Mikrographien werden von abgeriebenen und nicht abgeriebenen beschichteten Drähten genommen, um die Effekte des Abriebs auf die Beschichtung zu evaluieren.
- Flexibilitätstest
- Ein geeigneter Flexibilitätstest kann im Gegenzug dazu verwendet werden, um Fehler nachzuweisen (wenn durch Scanning-Elektronenmikroskopie untersucht), die sich während der Biegung einer beschichteten Probe entwickeln und, insbesondere, Anzeichen von Rissen an oder nahe dem Bereich einer Biegung.
- Eine Drahtprobe wird erhalten und auf die oben beschriebenen Weise beschichtet. Ein Ende des beschichteten Drahts (1,0 cm) wird in einem Schraubstock eingespannt. Das freie Ende des Drahts (1,0 cm) wird mit einer Zange gehalten. Der Draht wird gebogen, bis der Winkel, den er mit sich selber bildet, weniger als 90 Grad beträgt. Der Draht wird aus dem Schraubstock entfernt und durch SEM untersucht, um den Effekt der Biegung auf die Beschichtung zu bestimmen.
- Test zur Freisetzung von bioaktivem Mittel
- Ein geeigneter Test zur Freisetzung von bioaktivem Mittel, wie hier beschrieben, kann dazu verwendet werden, um das Ausmaß und die Rate von Wirkstoffreisetzung unter physiologischen Bedingungen zu bestimmen. Im allgemeinen ist es wünschenswert, daß weniger als 50% der Gesamtmenge des freizusetzenden Wirkstoffs in den ersten 24 Stunden freigesetzt werden. Es ist häufig für Mengen von Wirkstoff wünschenswert, über eine Dauer von mindestens 30 Tagen freigesetzt zu werden. Nachdem der gesamte Wirkstoff freigesetzt wurde, sollte die SEM-Evaluierung eine kohärente und defektfreie Beschichtung zeigen.
- Jede beschichtete Draht wird in einem Teströhrchen mit 5 ml PBS plaziert. Die Röhrchen werden auf eine Halterung in einem Umgebungs-Orbitalschüttler plaziert und bei 37°C geschüttelt. Zu bestimmten Zeitintervallen wird das PBS aus dem Röhrchen entfernt und durch frisches PBS ersetzt. Die Wirkstoffkonzentration in jeder PBS-Probe wird unter Verwendung des geeigneten Verfahrens bestimmt.
- Nachdem aller meßbarer Wirkstoff von dem beschichteten Bereich freigesetzt wurde, wird der Draht mit Wasser gewaschen, getrocknet, nochmals gewogen, die Beschichtungsdicke nochmals gemessen und die Beschichtungsqualität durch SEM-Analyse untersucht.
- Beispiel 1
- Freisetzung von Hexachlorphen von beschichteten Drähten aus rostfreiem Stahl.
- Ein ein Millimeter-Durchmesser rostfreier Stahldraht (304 Grad) wurde in 2 Zentimeter Segmente geschnitten. Die Segmente wurden mit Parylen C Beschichtungszusammensetzung behandelt (Parylen ist eine Marke der Union Carbide Corporation). Diese Behandlung hinterläßt eine dünne konformale Polymerbeschichtung auf den Drähten.
- Vier Lösungen wurden zur Verwendung bei der Beschichtung der Drähte hergestellt. Die Lösungen enthielten Gemische von: pEVA (33 Gew.% Vinylacetat von Aldrich Chemical Company, Inc); Poly(butylmethacrylat) ("pBMA") (337.000 durchschnittliches Molekulargewicht, von Aldrich Chemical Company, Inc.) und Hexachlorophen ("HCP") von Sigma Chemical Co., gelöst in Tetrahydrofuran. Die Lösungen wurden wie folgt hergestellt:
- 1) 10 mg/ml pEVA//60 mg/ml pBMA//100 mg/ml HCP
- 2) 35 mg/ml pEVA//35 mg/ml pBMA//100 mg/ml HCP
- 3) 60 mg/ml pEVA//10 mg/ml pBMA//100 mg/ml HCP
- 4) 0 mg/ml pEVA//0 mg/ml pBMA//100 mg/ml HCP
- Neun Drahtsegmente wurde mit jeder Beschichtungslösung beschichtet. Das folgende Protokoll wurde zur Beschichtung der Drahtsegmente befolgt. Die Parylen-behandelten Drahtsegmente wurden mit einem mit Isopropylalkohol befeuchteten Tuch vor der Beschichtung abgewischt. Die Drahtsegmente wurden in die Beschichtungslösung unter Anwendung einer 2 cm/Sek. Eintauchgeschindigkeit eingetaucht. Die Drahtsegmente wurden sofort aus der Beschichtungslösung bei einer Rate von 1 cm/Sek. herausgezogen, wonach die beschichteten Segmente bei Raumtemperatur Luft-getrocknet wurden.
- Einzelne Drahtsegmente wurden in Röhrchen plaziert, die 2 ml Phosphat-gepufferte Kochsalzlösung ("PBS", pH 7,4) enthielten. Die Röhrchen wurden bei 37°C auf einem Umgebungs-Orbitalschüttler bei 100 Umdrehungen/Minute inkubiert. Das PBS wurde bei 1 Stunde, 3 Stunden und 5 Stunden am ersten Tag gewechselt und danach täglich. Die PBS-Proben wurden auf die HCP-Konzentration durch Messen der Absorption der Proben bei 298 nm auf einem UV/sichtbares Licht-Spektrophotometer analysiert und mit einer HCP-Standardkurve verglichen.
- Die Ergebnisse sind in
1 zur Verfügung gestellt, welche die Fähigkeit darstellt, die Elutionsrate eines pharmazeutischen Mittels von einer beschichteten Oberfläche zu kontrollieren, indem die relativen Konzentrationen eines Polymergemischs variiert werden, wie durch diese Erfindung beschrieben. - Beispiel 2
- Die in dieser Offenbarung beschriebenen Polymere wurden unter der Verwendung eines Testprotokolls, wie oben angegeben, evaluiert. Die evaluierten Polymergemische reichten von 100% pBMA zu 100%p EVA. Repräsentative Ergebnisse dieser Evaluierungen sind unten zusammengefaßt.
- Kontrollbeschichtungen, die vollständig aus pBMA hergestellt sind, sind sehr dauerhaft und zeigen keine Anzeichen von Abnutzung in dem Dauerhaftigkeitstest. Wenn dem Flexiblitätstest unterzogen, entwickeln diese Beschichtungen jedoch Risse, insbesondere in der Anwesenheit von signifikanten Konzentrationen von Wirkstoff. Diese Beschichtungen setzen Wirkstoff auch sehr langsam frei.
- Die Kontrollbeschichtungen, die vollständig aus pEVA hergestellt sind, sind im Gegensatz dazu weniger dauerhaft und zeigen keine Anzeichen von Rissen in dem Flexibilitätstest, entwickeln jedoch signifikante Beschädigung in dem Dauerhaftigkeitstest. Diese Beschichtungen setzten Wirkstoffe relativ schnell frei, wobei normalerweise mehr als 50% des Gesamten innerhalb von 24 Stunden freigesetzt werden.
- Beschichtungen der vorliegenden Erfindung, die ein Gemisch von beiden Polymeren enthalten, sind sehr dauerhaft, mit keinem Anzeichen von Abnutzung in dem Dauerhaftigkeitstest und keinen Rissen in dem Flexibilitätstest. Die Wirkstoffreisetzung von diesen Beschichtungen kann durch Variieren der relativen Konzentrationen der Polymere beeinflußt werden. Zum Beispiel kann die Rate von Wirkstoffreisetzung durch Erhöhen der relativen Konzentration von pEVA kontrollierbar erhöht werden.
- Bioaktive Mittel enthaltende Beschichtungen, die keine Anzeichen von Beschädigungen im Dauerhaftigkeitstest und keine Risse im Flexibilitätstest zeigen, besitzen die nötigen Charakteristika zur Aufbringung auf medizinische Vorrichtungen, die eine Flexion und/oder Expansion während des Verlaufs der Implantation und/oder Verwendung erfahren.
Claims (74)
- Stent, der mit einer Zusammensetzung beschichtet ist, wobei die Zusammensetzung einen oder mehrere bioaktive Wirkstoffe in Kombination mit mehreren Polymeren umfasst, die eine erste Polymerkomponente, die wenigstens ein Poly(alkyl)(meth)acrylat umfasst, und eine zweite Polymerkomponente, die Poly(ethylen-co-vinylacetat) umfasst, einschließen, wobei die erste Polymerkomponente ausgewählt ist aus der Gruppe, die aus Poly(alkyl)(meth)acrylaten mit Alkylkettenlängen von 2 bis 8 Kohlenstoffen besteht.
- Stent nach Anspruch 1, dadurch gekennzeichnet, daß die Zusammensetzung ermöglicht, daß die Menge und Rate der Freisetzung von bioaktive(m)(n) Wirkstoffen) aus der medizinischen Einheit gesteuert wird, indem die relativen Typen und/oder Konzentrationen von Polymeren in der Mischung eingestellt werden.
- Stent nach Anspruch 2, dadurch gekennzeichnet, daß die Zusammensetzung die Menge und Rate der Freisetzung von bioaktive(m)(n) Wirkstoffen) aus der medizinischen Einheit während des ersten Verwendungstages auf 50% der anfänglichen Konzentration des (der) bioaktiven Wirkstoff(s)(e) oder weniger einregelt.
- Stent nach Anspruch 1, dadurch gekennzeichnet, daß die erste Polymerkomponente ein Molekulargewicht von 50 Kilodaltons bis 900 Kilodaltons aufweist.
- Stent nach Anspruch 4, dadurch gekennzeichnet, daß die erste Polymerkomponente Poly-n-butylmethacrylat umfasst.
- Stent nach Anspruch 1, dadurch gekennzeichnet, daß die zweite Polymerkomponente ausgewählt ist aus der Gruppe, die aus Polyethylen-co-vinylacetat)-Polymeren mit Vinylacetat-Konzentrationen von zwischen etwa 10 und etwa 50 Gew.-% besteht.
- Stent nach Anspruch 6, dadurch gekennzeichnet, daß die Vinylacetat-Konzentrationen zwischen etwa 24 und etwa 36 Gew.-% liegen.
- Stent nach Anspruch 7, dadurch gekennzeichnet, daß die Vinylacetat-Konzentrationen zwischen etwa 30 und etwa 34 Gew.-% liegen.
- Stent nach Anspruch 1, dadurch gekennzeichnet, daß die Zusammensetzung eine Mischung aus Poly(n-butylmethacrylat) und Polyethylen-co-vinylacetat) umfasst.
- Stent nach Anspruch 9, dadurch gekennzeichnet, daß die kombinierten Gesamtkonzentrationen beider Polymere in der Beschichtungszusammensetzung zwischen etwa 0,25 und etwa 70 Gew.-% liegen.
- Stent nach Anspruch 9, dadurch gekennzeichnet, daß das Poly(n-butylmethacrylat) ein Molekulargewicht von 100 Kilodaltons bis 900 Kilodaltons aufweist und das Polyethylen-co-vinylacetat) einen Vinylacetat-Gehalt von 24 bis 36 Gew.-% liefert.
- Stent nach Anspruch 11, dadurch gekennzeichnet, daß das Poly(n-butylmethacrylat) ein Molekulargewicht von 200 Kilodaltons bis 400 Kilodaltons aufweist und das Polyethylen-co-vinylacetat) einen Vinylacetat-Gehalt von 30 bis 34 Gew.-% liefert.
- Stent nach Anspruch 1, dadurch gekennzeichnet, daß die Zusammensetzung weiter ein Lösemittel umfasst, in dem die Polymere eine echte Lösung bilden.
- Stent nach Anspruch 1, dadurch gekennzeichnet, daß der bioaktive Wirkstoff in der Beschichtungszusammensetzung in einer Konzentration von 0,01 bis 90 Gew.-% gelöst oder suspendiert ist.
- Stent nach Anspruch 14, dadurch gekennzeichnet, daß der bioaktive Wirkstoff ausgewählt ist aus der Gruppe, die aus Thrombin-Inhibitoren, antithrombotischen Mitteln, thrombolytischen Mitteln, fibrinolytischen Mitteln, Vasospasmus-Inhibitoren, Calciumkanalblockern, Vasodilatatoren, blutdruck-senkenden Mitteln, antimikrobiellen Mitteln, Antibiotika, Inhibitoren von Oberflächenglykoproteinrezeptoren, Antithrombozytenmitteln, Antimiotika, Mikrotubulus-Inhibitoren, antisekretorischen Mitteln, Actin-Inhibitoren, Remodellierungs-Inhibitoren, Antisense-Nukleotiden, Antimetaboliten, Antiproliferativa, chemotherapeutischen Antikrebsmitteln, entzündungshemmenden Steroiden oder nicht-steroiden entzündungshemmenden Mitteln, immunsuppressiven Mitteln, Wachstumshormon-Antagonisten, Wachstumsfaktoren, Dopamin-Agonisten, radiotherapeutischen Mitteln, Peptiden, Proteinen, Enzymen, extrazellulären Matrixkomponenten, Inhibitoren, Abfangmitteln für freie Radikale, Chelatbildnern, Antioxidationsmitteln, Antipolymerasen, antiviralen Mitteln, Mitteln für photodynamische Therapie und Mitteln für Gentherapie besteht.
- Stent nach Anspruch 1, dadurch gekennzeichnet, daß die zweite Polymerkomponente ausgewählt ist aus der Gruppe, die aus Polyethylen-co-vinylacetat)-Polymeren mit Vinylacetat-Konzentrationen von zwischen etwa 10 und etwa 50 Gew.-% besteht.
- Stent nach Anspruch 16, dadurch gekennzeichnet, daß die kombinierten Gesamtkonzentrationen beider Polymere in der Beschichtungszusammensetzung zwischen etwa 0,25 und etwa 70 Gew.-% liegen und die Zusammensetzung weiter ein Lösemittel umfasst, in dem die Polymere eine echte Lösung bilden.
- Stent nach Anspruch 17, dadurch gekennzeichnet, daß das Poly(n-butylmethacrylat) ein Molekulargewicht von 100 Kilodaltons bis 900 Kilodaltons aufweist und das Polyethylen-co-vinylacetat) einen Vinylacetat-Gehalt von 24 bis 36 Gew.-% liefert und der bioaktive Wirkstoff in der Beschichtungszusammensetzung in einer Konzentration von 0,01 bis 90 Gew.-% gelöst oder suspendiert ist.
- Stent nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß der Stent ein selbstexpandierender Stent oder ein mit einem Ballon expandierbarer Stent ist.
- Stent nach Anspruch 19, dadurch gekennzeichnet, daß der selbstexpandierende Stent oder der mit einem Ballon expandierende Stent aus rostfreiem Stahl oder Tantal hergestellt ist.
- Verfahren zur Herstellung eines Stents nach einem der vorangehenden Ansprüche, wobei das Verfahren die Schritte umfasst, daß eine in einem der vorangehenden Ansprüche angegebene Zusammensetzung bereitgestellt und die Zusammensetzung auf den Stent aufgebracht wird.
- Verfahren nach Anspruch 21, dadurch gekennzeichnet, daß die Beschichtung durch Eintauchen der Einheit in die Zusammensetzung oder Besprühen der Einheit mit der Zusammensetzung bereitgestellt wird.
- Verfahren nach Anspruch 22, dadurch gekennzeichnet, daß die Beschichtungszusammensetzung ein Lösemittel einschließt und die Beschichtung auf dem Stent durch Verdampfen des Lösemittels ausgehärtet wird.
- Verfahren nach Anspruch 21, dadurch gekennzeichnet, daß die erste Polymerkomponente ausgewählt wird aus der Gruppe, die aus Poly(alkyl)(meth)acrylaten mit Alkylkettenlängen von 2 bis 8 Kohlenstoffen besteht, und die zweite Polymerkomponente ausgewählt wird aus der Gruppe, die aus Poly(ethylen-co-vinylacetat)-Polymeren mit Vinylacetat-Konzentrationen von zwischen etwa 10 und etwa 50 Gew.-% besteht.
- Verfahren nach Anspruch 24, dadurch gekennzeichnet, daß die Zusammensetzung eine Mischung aus Poly(n-butylmethacrylat) und Polyethylen-co-vinylacetat) umfasst.
- Verfahren nach Anspruch 24, dadurch gekennzeichnet, daß die kombinierten Gesamtkonzentrationen beider Polymere in der Beschichtungszusammensetzung zwischen etwa 0,25 und etwa 70 Gew.-% liegen.
- Verfahren nach Anspruch 26, dadurch gekennzeichnet, daß der eine oder die mehreren bioaktive(n) Wirkstoffe) in der Beschichtungszusammensetzung in einer Konzentration von 0,01 bis 90 Gew.-% gelöst oder suspendiert wird (werden).
- Verfahren nach Anspruch 21, dadurch gekennzeichnet, daß das Gewicht der Beschichtung, das dem einen oder den mehreren bioaktiven Wirkstoffen) zuschreibbar ist, im Bereich von etwa 0,05 mg bis etwa 10 mg bioaktiver Wirkstoff pro cm2 der Bruttooberfläche des Stents liegt.
- Verfahren nach Anspruch 28, dadurch gekennzeichnet, daß das Gewicht der Beschichtung, das dem einen oder den mehreren bioaktiven Wirkstoffen) zuschreibbar ist, zwischen etwa 1 mg bis etwa 5 mg bioaktiver Wirkstoff pro cm2 der Bruttooberfläche des Stents liegt und die Beschichtungsdicke der Zusammensetzung im Bereich von etwa 5 Mikrometern bis etwa 100 Mikrometern liegt.
- Stent nach einem der Ansprüche 1 bis 20 zur Verwendung in einem Behandlungsverfahren, welches die Schritte umfasst: a) Implantieren des Stents in vivo unter Bedingungen, bei denen der Stent Flexion oder Expansion durchläuft, indem er um wenigstens 45 Grad oder mehr gebogen und/oder auf mehr als das Zweifache seiner ursprünglichen Abmessung expandiert wird, entweder im Verlauf seiner Platzierung oder danach im Verlauf seiner Verwendung in vivo, und b) Zulassen, daß der Stent implantiert bleibt und den bioaktiven Wirkstoff in situ freisetzt.
- Stent nach Anspruch 30, dadurch gekennzeichnet, daß die Zusammensetzung eine Mischung aus Poly(n-butylmethacrylat) und Polyethylen-co-vinylacetat) umfasst.
- Stent nach Anspruch 29, dadurch gekennzeichnet, daß das Gewicht der Beschichtung, das dem einen oder den mehreren bioaktiven Wirkstoffen) zuschreibbar ist, im Bereich von etwa 0,05 mg bis etwa 10 mg bioaktiver Wirkstoff pro cm2 der Bruttooberfläche des Stents liegt und die Beschichtungsdicke der Zusammensetzung im Bereich von etwa 5 Mikrometern bis etwa 100 Mikrometern liegt.
- Katheter, der mit einer Zusammensetzung beschichtet ist, wobei die Zusammensetzung einen oder mehrere bioaktive Wirkstoffe in Kombination mit mehreren Polymeren umfasst, die eine erste Polymerkomponente, die wenigstens ein Poly(alkyl)(meth)acrylat umfasst, und eine zweite Polymerkomponente, die Polyethylen-co-vinylacetat) umfasst, einschließen und wobei die erste Polymerkomponente ausgewählt ist aus der Gruppe, die aus Poly(alkyl)(meth)acrylaten mit Alkylkettenlängen von 2 bis 8 Kohlenstoffen besteht.
- Katheter nach Anspruch 33, dadurch gekennzeichnet, daß die Zusammensetzung ermöglicht, daß die Menge und Rate der Freisetzung, von bioaktive(m)(n) Wirkstoffen) aus der medizinischen Einheit gesteuert wird, indem die relativen Typen und/oder Konzentrationen von Polymeren in der Mischung eingestellt werden.
- Katheter nach Anspruch 34, dadurch gekennzeichnet, daß die Zusammensetzung die Menge und Rate der Freisetzung von bioaktive(m)(n) Wirkstoff(en) aus der medizinischen Einheit während des ersten Verwendungstages auf 50% der anfänglichen Konzentration des (der) bioaktiven Wirkstoff(s)(e) oder weniger einregelt.
- Katheter nach Anspruch 35, dadurch gekennzeichnet, daß die erste Polymerkomponente ein Molekulargewicht von 50 Kilodaltons bis 900 Kilodaltons aufweist.
- Katheter nach Anspruch 36, dadurch gekennzeichnet, daß die erste Polymerkomponente Poly(-n-butylmethacrylat) umfasst.
- Katheter nach Anspruch 32, dadurch gekennzeichnet, daß die zweite Polymerkomponente ausgewählt ist aus der Gruppe, die aus Polyethylen-co-vinylacetat)-Polymeren mit Vinylacetat-Konzentrationen von zwischen etwa 10 und etwa 50 Gew.-% besteht.
- Katheter nach Anspruch 38, dadurch gekennzeichnet, daß die Vinylacetat-Konzentrationen zwischen etwa 24 und etwa 36 Gew.-% liegen.
- Katheter nach Anspruch 39, dadurch gekennzeichnet, daß die Vinylacetat-Konzentrationen zwischen etwa 30 und etwa 34 Gew.-% liegen.
- Katheter nach Anspruch 33, dadurch gekennzeichnet, daß die Zusammensetzung eine Mischung aus Poly(n-butylmethacrylat) und Polyethylen-co-vinylacetat) umfasst.
- Katheter nach Anspruch 41, dadurch gekennzeichnet, daß die kombinierten Gesamtkonzentrationen beider Polymere in der Beschichtungszusammensetzung zwischen etwa 0,25 und etwa 70 Gew.-% liegen.
- Katheter nach Anspruch 41, dadurch gekennzeichnet, daß das Poly(n-butylmethacrylat) ein Molekulargewicht von 100 Kilodaltons bis 900 Kilodaltons aufweist und das Polyethylen-co-vinylacetat) einen Vinylacetat-Gehalt von 24 bis 36 Gew.-% liefert.
- Katheter nach Anspruch 43, dadurch gekennzeichnet, daß das Poly(n-butylmethacrylat) ein Molekulargewicht von 200 Kilodaltons bis 400 Kilodaltons aufweist und das Polyethylen-co-vinylacetat) einen Vinylacetat-Gehalt von 30 bis 34 Gew.-% liefert.
- Katheter nach Anspruch 33, dadurch gekennzeichnet, daß die Zusammensetzung weiter ein Lösemittel umfasst, in dem die Polymere eine echte Lösung bilden.
- Katheter nach Anspruch 33, dadurch gekennzeichnet, daß der eine oder die mehreren bioaktive(n) Wirkstoffe) in der Beschichtungszusammensetzung in einer Konzentration von 0,01 bis 90 Gew.-% gelöst oder suspendiert ist (sind).
- Katheter nach Anspruch 46, dadurch gekennzeichnet, daß der der eine oder die mehreren bioaktive(n) Wirkstoffe) ausgewählt ist (sind) aus der Gruppe, die aus Thrombin-Inhibitoren, antithrombotischen Mitteln, thrombolytischen Mitteln, fibrinolytischen Mitteln, Vasospasmus-Inhibitoren, Calciumkanalblockern, Vasodilatatoren, blutdrucksenkenden Mitteln, antimikrobiellen Mitteln, Antibiotika, Inhibitoren von Oberflächenglykoproteinrezeptoren, Antithrombozytenmitteln, Antimiotika, Mikrotubulus-Inhibitoren, antisekretorischen Mitteln, Actin-Inhibitoren, Remodellierungs-Inhibitoren, Antisense-Nukleotiden, Antimetaboliten, Antiproliferativa, chemotherapeutischen Antikrebsmitteln, entzündungshemmenden Steroiden oder nicht-steroiden entzündungshemmenden Mitteln, immunsuppressiven Mitteln, Wachstumshormon-Antagonisten, Wachstumsfaktoren, Dopamin-Agonisten, radiotherapeutischen Mitteln, Peptiden, Proteinen, Enzymen, extrazellulären Matrixkomponenten, Inhibitoren, Abfangmitteln für freie Radikale, Chelatbildnern, Antioxidationsmitteln, Antipolymerasen, antiviralen Mitteln, Mitteln für photodynamische Therapie und Mitteln für Gentherapie besteht.
- Katheter nach Anspruch 47, dadurch gekennzeichnet, daß der eine oder die mehreren bioaktive(n) Wirkstoffe) ausgewählt ist (sind) aus der Gruppe, die aus Vancomycin, Norfloxacin, Heparin, Hirudin und Cumadin besteht.
- Katheter nach Anspruch 33, dadurch gekennzeichnet, daß die erste Polymerkomponente ausgewählt ist aus der Gruppe, die aus Poly(alkyl)(meth)acrylaten mit Alkylkettenlängen von 2 bis 8 Kohlenstoffen besteht, und die zweite Polymerkomponente ausgewählt ist aus der Gruppe, die aus Poly(ethylen-co-vnylacetat)-Polymeren mit Vinylacetat-Konzentrationen von zwischen etwa 10 und etwa 50 Gew.-% besteht.
- Katheter nach Anspruch 49, dadurch gekennzeichnet, daß die kombinierten Gesamtkonzentrationen beider Polymere in der Beschichtungszusammensetzung zwischen etwa 0,25 und etwa 70 Gew.-% liegen und die Zusammensetzung weiter ein Lösemittel umfasst, in dem die Polymere eine echte Lösung bilden.
- Katheter nach Anspruch 50, dadurch gekennzeichnet, daß das Poly(n-butylmethacrylat) ein Molekulargewicht von 100 Kilodaltons bis 900 Kilodaltons aufweist und das Polyethylen-co-vinylacetat) einen Vinylacetat-Gehalt von 24 bis 36 Gew.-% liefert und der eine oder die mehreren bioaktive(n) Wirkstoffe) in der Beschichtungszusammensetzung in einer Konzentration von 0,01 bis 90 Gew.-% gelöst oder suspendiert ist (sind).
- Katheter nach einem der Ansprüche 33 bis 51, dadurch gekennzeichnet, daß der Katheter ein Urinkatheter oder ein intravenöser Katheter ist.
- Katheter nach Anspruch 52, dadurch gekennzeichnet, daß der Urinkatheter oder der intravenöse Katheter hergestellt ist aus einem oder mehreren Materialien, die ausgewählt sind aus der Gruppe, die aus Silikon, Gummi, Polyurethan, Latex und Polyvinylchlorid besteht.
- Verfahren zur Herstellung eines Katheters nach einem der Ansprüche 33 bis 53, wobei das Verfahren die Schritte umfasst, daß eine in einem der Ansprüche 33 bis 53 angegebene Zusammensetzung bereitgestellt und die Zusammensetzung auf den Katheter aufgebracht wird.
- Verfahren nach Anspruch 54, dadurch gekennzeichnet, daß die Beschichtung durch Eintauchen der Einheit in die Zusammensetzung oder Besprühen der Einheit mit der Zusammensetzung bereitgestellt wird.
- Verfahren nach Anspruch 55, dadurch gekennzeichnet, daß die Beschichtungszusammensetzung ein Lösemittel einschließt und die Beschichtung auf dem Katheter durch Verdampfen des Lösemittels ausgehärtet wird.
- Verfahren nach Anspruch 54, dadurch gekennzeichnet, daß die erste Polymerkomponente ausgewählt wird aus der Gruppe, die aus Poly(alkyl)(meth)acrylaten mit Alkylkettenlängen von 2 bis 8 Kohlenstoffen besteht, und die zweite Polymerkomponente ausgewählt wird aus der Gruppe, die aus Poly(ethylen-co-vinylacetat)-Polymeren mit Vinylacetat-Konzentrationen von zwischen etwa 10 und etwa 50 Gew.-% besteht.
- Verfahren nach Anspruch 57, dadurch gekennzeichnet, daß die Zusammensetzung eine Mischung aus Poly(n-butylmethacrylat) und Polyethylen-co-vinylacetat) umfasst.
- Verfahren nach Anspruch 57, dadurch gekennzeichnet, daß die kombinierten Gesamtkonzentrationen beider Polymere in der Beschichtungszusammensetzung zwischen etwa 0,25 und etwa 70 Gew.-% liegen.
- Verfahren nach Anspruch 59, dadurch gekennzeichnet, daß der eine oder die mehreren bioaktive(n) Wirkstoffe) in der Beschichtungszusammensetzung in einer Konzentration von 0,01 bis 90 Gew.-% gelöst oder suspendiert wird (werden).
- Verfahren nach Anspruch 54, dadurch gekennzeichnet, daß das Gewicht der Beschichtung, das dem einen oder den mehreren bioaktiven Wirkstoffen) zuschreibbar ist, im Bereich von etwa 0,05 mg bis etwa 10 mg bioaktiver Wirkstoff pro cm2 der Bruttooberfläche des Katheters liegt.
- Verfahren nach Anspruch 61, dadurch gekennzeichnet, daß das Gewicht der Beschichtung, das dem einen oder den mehreren bioaktiven Wirkstoffen) zuschreibbar ist, zwischen etwa 1 mg und etwa 5 mg bioaktiver Wirkstoff pro cm2 der Bruttooberfläche des Katheters liegt und die Beschichtungsdicke der Zusammensetzung im Bereich von etwa 5 Mikrometern bis etwa 100 Mikrometern liegt.
- Katheter nach einem der Ansprüche 33 bis 53 zur Verwendung in einem Behandlungsverfahren, welches die Schritte umfasst: a) Implantieren des Katheters in vivo unter Bedingungen, bei denen der Katheter Flexion oder Expansion durchläuft, indem er um wenigstens 45 Grad oder mehr gebogen und/oder auf mehr als das Zweifache seiner ursprünglichen Abmessung expandiert wird, entweder im Verlauf seiner Platzierung oder danach im Verlauf seiner Verwendung in vivo, und b) Zulassen, daß der Katheter implantiert bleibt und den bioaktiven Wirkstoff in situ freisetzt.
- Katheter nach Anspruch 63, dadurch gekennzeichnet, daß die Zusammensetzung eine Mischung aus Poly(n-butylmethacrylat) und Polyethylen-co-vinylacetat) umfasst.
- Katheter nach Anspruch 63, dadurch gekennzeichnet, daß das Gewicht der Beschichtung, das dem einen oder den mehreren bioaktiven Wirkstoffen) zuschreibbar ist, im Bereich von etwa 0,05 mg bis etwa 10 mg bioaktiver Wirkstoff pro cm2 der Bruttooberfläche des Katheters liegt und die Beschichtungsdicke der Zusammensetzung im Bereich von etwa 5 Mikrometern bis etwa 100 Mikrometern liegt.
- Kombination, die eine medizinische Einheit umfasst, die mit einer Zusammensetzung beschichtet ist, die einen oder mehrere bioaktive Wirkstoffe in Kombination mit mehreren Polymeren umfasst, die eine erste Polymerkomponente, die wenigstens ein Poly(alkyl)(meth)acrylat umfasst, und eine zweite Polymerkomponente, die Polyethylen-co-vinylacetat) umfasst, einschließen, wobei der eine oder die mehreren bioaktive(n) Wirkstoffe) ausgewählt ist (sind) aus der Gruppe, die aus Thrombin-Inhibitoren, antithrombotischen Mitteln, thrombolytischen Mitteln, fibrinolytischen Mitteln, Vasospasmus-Inhibitoren, Calciumkanalblockern, Vasodilatatoren, blutdrucksenkenden Mitteln, antimikrobiellen Mitteln, Antibiotika, Inhibitoren von Oberflächenglykoproteinrezeptoren, Antithrombozytenmitteln, Antimiotika, Mikrotubulus-Inhibitoren, antisekretorischen Mitteln, Actin-Inhibitoren, Remodellierungs-Inhibitoren, Antisense-Nukleotiden, Antimetaboliten, Antiproliferativa, chemotherapeutischen Antikrebsmitteln, entzündungshemmenden Steroiden oder nicht-steroiden entzündungshemmenden Mitteln, immunsuppressiven Mitteln, Wachstumshormon-Antagonisten, Wachstumsfaktoren, Dopamin-Agonisten, radiotherapeutischen Mitteln, Peptiden, Proteinen, Enzymen, extrazellulären Matrixkomponenten, Inhibitoren, Abfangmitteln für freie Radikale, Chelatbildnern, Antioxidationsmitteln, Antipolymerasen, antiviralen Mitteln, Mitteln für photodynamische Therapie und Mitteln für Gentherapie besteht.
- Kombination, die eine medizinische Einheit umfasst, die mit wenigstens zwei Schichten einer Zusammensetzung beschichtet ist, die einen oder mehrere bioaktive Wirkstoffe in Kombination mit mehreren Polymeren umfasst, die eine erste Polymerkomponente, die wenigstens ein Poly(alkyl)(meth)acrylat umfasst, und eine zweite Polymerkomponente, die Polyethylen-co-vinylacetat) umfasst, einschließen.
- Kombination nach Anspruch 67, dadurch gekennzeichnet, daß die Zusammensetzung der Schichten ermöglicht, daß die Menge und Rate der Freisetzung von Wirkstoffen) aus der medizinischen Einheit gesteuert wird, indem die relativen Typen und/oder Konzentrationen von Polymeren in den Schichten eingestellt werden.
- Kombination nach einem der Ansprüche 66 bis 68 zur Verwendung in der Behandlung von Komplikationen, die aus der Implantation einer medizinischen Einheit resultieren, welche die Schritte umfasst: a) Implantieren der Einheit in vivo und b) Zulassen, daß die Einheit implantiert bleibt und den einen oder die mehreren bioaktiven Wirkstoff e) in situ freisetzt.
- Kombination nach einem der Ansprüche 66 bis 68 zur Verwendung in der Behandlung von Entzündung.
- Kombination nach einem der Ansprüche 66 bis 68 zur Verwendung in der Behandlung von fibröser Enkapsulierung.
- Kombination nach einem der Ansprüche 66 bis 68 zur Verwendung in der Behandlung von Infektion.
- Kombination nach einem der Ansprüche 66 bis 68 zur Verwendung in der Behandlung von Hyperplasie.
- Kombination nach einem der Ansprüche 66 bis 68 zur Verwendung in der Behandlung von Restenose.
