Description
- HINTERGRUND DER ERFINDUNG
- Die vorliegende Erfindung betrifft den Bereich der Verfahren und Vorrichtungen zur Embolisation von vaskulären Aneurysmen und ähnlichen vaskulären Abnormitäten. Die vorliegende Erfindung betrifft insbesondere ein Gerät zur Embolisation einer Gefäßstelle.
- Die Embolisation von Blutadern ist in einer Reihe von klinischen Situationen erwünscht. Eine vaskuläre Embolisation ist zum Beispiel zur Regelung von vaskulärer Blutung, zum Verschlug der Blutversorgung zu Tumoren und zum Verschluß vaskulärer Aneurysmen, insbesondere intrakranieller Aneurysmen, verwendet worden. In den letzten Jahren hat die vaskuläre Embolisation zur Behandlung von Aneurysmen viel Aufmerksamkeit erhalten. Mehrere verschiedene Behandlungsmodalitäten sind im Stand der Technik verwendet worden. Zum Beispiel wird im US-Patent Nr. 4819637 – Dormandy, Jr. et al., ein vaskuläres Embolisationssystem beschrieben, dass von einem ablösbaren Ballon Gebrauch macht, der mit Hilfe eines intravaskulären Katheters an der Aneurysmastelle bereitgestellt wird. Der Ballon wird in das Aneurysma an der Spitze des Katheters hineingetragen und innerhalb des Aneurysmas mit Hilfe einer verdichtenden Flüssigkeit (typisch ein polymerisierbares Harz oder Gel) aufgeblasen, um das Aneurysma zu verschließen. Der Ballon wird dann durch sanftes Ziehen des Katheters von dem Katheter entfernt. Während die Embolisationsvorrichtung eines Ballontyps eine wirksame Verschlug von vielen Typen von Aneurysmen gewährleisten kann, ist es schwierig, die Vorrichtung nach der Verdichtung der Flüssigkeiten zurückzuholen oder zu verschieben, und sie ist schwierig zu erkennen, es sei denn, dass sie mit einem Kontrastmaterial gefüllt ist. Darüber hinaus gibt es Risiken für eine Zerreißung des Ballons während des Aufblasens und eine verfrühte Ablösung des Ballons von dem Katheter.
- Eine andere Vorgehensweise ist die direkte Injektion eines embolischen Mittels aus flüssigen Polymeren in die Gefäßstelle hinein, die verschlossen werden soll. Ein Typ von flüssigem Polymer, das im direkten Injektionsverfahren verwendet wird, ist eine schnell polymerisierende Flüssigkeit, wie zum Beispiel Cyanoacrylatharz, insbesondere Isobutylcyanoacrylat, das als eine Flüssigkeit an die Zielstelle geliefert wird und daraufhin in situ polymerisiert. Alternativ ist ein flüssiges Polymer, das an der Zielstelle aus einer Trägerlösung ausgefällt wird, verwendet worden. Ein Beispiel für diesen Typ von embolischem Mittel ist ein Zelluloseacetatpolymer, gemischt mit Bismuttrioxid und in Dimethylsulfoxid (DMSO) auf gelöst. Ein anderer Typ ist Ethylenglycolcopolymer, aufgelöst in DMSO. Bei Blutkontakt diffundiert das DMSO heraus, und das Polymer fällt aus und erhärtet schnell zu einer embolischen Masse, die sich der Form des Aneurysmas anpasst. Andere Beispiele für Materialien, die in diesem "direkten Injektions"-Verfahren verwendet werden, sind in den folgenden US-Patenten beschrieben: 4551132 – Pásztor et al.; 4795741 – Leshchiner et al.; 5525334 – Ito et al.; und 5580568 – Greff et al.
- Die direkte Injektion von embolischen Agenten aus flüssigem Polymer hat sich in der Praxis als schwierig erwiesen. Zum Beispiel hat sich die Migration vom Polymermaterial aus dem Aneurysma und in die angrenzende Blutader hinein als ein Problem erwiesen. Darüber hinaus erfordert die Visualisierung des Embolisationsmaterials, dass ein Kontrastmittel mitgemischt wird, und die Auswahl von Embolisationsmaterialien und Kontrastmitteln, die gegenseitig kompatibel sind, kann zu leistungsmäßigen Kompromissen führen, die weniger optimal sind. Darüber hinaus ist die genaue Kontrolle der Entwicklung des polymeren Embolisationsmaterials schwierig, welches ein Risiko für falsche Platzierung und/oder verfrühte Verfestigung des Materials mit sich bringt. Wird das Embolisationsmaterial erst eingesetzt und erhärtet, ist es außerdem schwierig, es zu bewegen oder zurückzuholen.
- Eine andere Vorgehensweise, die sich bewährt hat, ist die Anwendung von thrombogenen Mikrospulen. Diese Mikrospulen können aus einer biologisch verträglichen Metall-Legierung (typisch Platin und Wolfram) oder aus einem geeigneten Polymer hergestellt werden. Wenn aus Metall hergestellt, kann die Spule mit Dacron-Fibern zur Erhöhung der Thrombogenizität versehen werden. Die Spule wird durch einen Mikrokatheter an der Gefäßstelle entwickelt. Beispiele für Mikrospulen sind in den folgenden US-Patenten beschrieben. 4994069 – Ritchart et al.; 5133731 – Butler et al.; 5226911 – Chee et al.; 5312415 – Palermo; 5382259 – Phelps et al.; 5382260 – Dormandy, Jr. et al.; 5476472 – Dormandy, Jr. et al.; 5578074 – Mirigian; 5582619 – Ken; 5624461 – Mariant; 5645558 – Horton; 5658308 – Snyder; und 5718711 – Berenstein et al.
- Die Mikrospulen-Vorgehensweise hat einen gewissen Erfolg in Bezug auf die Behandlung kleiner Aneurysmen mit engen Einschnürungen gezeigt, aber die Spule muss im Aneurysma dicht zusammengepackt werden, um eine Verschiebung zu vermeiden, die zur Rekanalisierung führen kann. Mikrospulen haben einen geringeren Erfolg in Verbindung mit der Behandlung größerer Aneurysmen, insbesondere denjenigen mit relativ weiten Einschnürungen, gehabt. Ein Nachteil von Mikrospulen ist, dass sie nicht leicht rückholbar sind; wenn eine Spule aus dem Aneurysma auswandert, ist es notwendig, es durch ein zweites Verfahren zurückzuholen und es an die richtige Stelle zurückzuverschieben. Darüber hinaus kann es schwierig sein, ein komplettes Verpacken eines Aneurysmas unter Anwendung von Mikrospulen in der Praxis umzusetzen.
- Ein spezifischer Typ von Mikrospulen, der erheblichen Erfolg erzielt hat, ist das "Guglielmi Detachable Coil" ("GDC"). GDC verwendet eine Spule aus Platindraht, die durch einen Lötanschluss an einem Leitdraht aus rostfreiem Stahl befestigt ist. Nachdem die Spule innerhalb des Aneurysmas angebracht ist, wird elektrischer Strom am Leitdraht angelegt, der ausreichend erhitzt wird, um die Lötverbindung zu zerschmelzen, wobei die Spule von dem Leitdraht gelöst wird. Die Verwendung des Stroms erzeugt auch eine positive elektrische Ladung auf der Spule, die negative geladene Blutzellen, Plättchen und Fibrinogene anzieht, wobei die Thrombogenizität der Spule erhöht wird. Mehrere Spulen mit unterschiedlichen Durchmessern und Längen können in ein Aneurysma gepackt werden, bis das Aneurysma völlig gefüllt ist. Innerhalb des Aneurysmas bilden die Spulen somit einen Thrombus und halten ihn fest, wobei dessen Verlagerung und dessen Fragmentierung blockiert werden.
- Die Vorteile des GDC-Verfahrens sind die Fähigkeit, die Spule zurückzuziehen und zu verschieben, wenn es aus der gewünschten Stelle abwandert, und die erhöhte Fähigkeit, die Bildung eines stabilen Thrombus innerhalb des Aneurysmas zu fördern. Dennoch ist die erfolgreiche Verwendung des GDC-Verfahrens, wie in üblichen Mikrspulen-Verfahren, auf kleinere Aneurysmen mit engen Einschnürungen erheblich begrenzt worden.
- Eine weitere Vorgehensweise zur Embolisation einer abnormen Gefäßstelle ist die Injektion eines biologisch verträglichen Hydrogels in die Stelle hinein, wie zum Beispiel Poly-(2-hydroxyethylmethacrylat) ("pHEMA" oder "PHEMA"); oder eines Polyvinylalkoholschaums ("PAF"). Siehe z.B. Horák et al., "Hydrogels in Endovascular Embolization. II. Neuroradiol., Band 18, S. 61–69 (1991); Latchaw et al., "Polyvinyl Foam Embolization of Vascular and Neoplastic Lesions of the Head, Neck, and Spine", Radiology, Band 131, S. 669–679 (Juni 1979). Diese Materialien werden als Mikropartikel in einer Trägerflüssigkeit, die in die Gefäßstelle eingespritzt wird, geliefert, ein Verfahren, wo es sich herausgestellt hat, dass es schwierig zu kontrollieren ist.
- Eine weitere Entwicklung ist die Formulierung der Hydrogelmaterialien in ein vorgeformtes Implantat oder einen vorgeformten Stöpsel gewesen, die durch Mittel, wie zum Beispiel einen Mikrokatheter, an die Gefäßstelle eingebaut werden. Siehe z.B. Patent-Nr. 5258042 – Mehta und 5456693 – Conston et al. Diese Typen von Stöpseln oder implantaten sind primär zur Blockierung der Durchblutung durch ein röhrförmiges Gefäß oder die Einschnürung eines Aneurysmas vorgesehen, und sie können an eine präzise Implantation innerhalb einer sackförmigen Gefäßstruktur, wie zum Beispiel eines Aneurysmas, nicht leicht angepasst werden, um so im wesentlichen das ganze Volumen der Struktur zu füllen.
- US-A-5750585 beschreibt eine wasserquellbare Schaummatrix mit der Form eines makroporösen Feststoffes, umfassend ein schaumstabilisierendes Mittel und ein Polymer oder Copolymer eines frei radikalen polymerisierbaren hydrophilen Olefinmonomers, das mit ungefähr 0,1 bis ungefähr 10 Gew.-% eines multiolefin-funktionellen Querverbindungsmittels querverbunden ist. Die Schaummatrix ist durch rasches Quellen und hohe Quellensanteile gekennzeichnet.
- WO 9804198 A beschreibt ein Verfahren, das die Einführung eines embolischen Materials in ein Aneurysma umfasst. Das embolische Material wird angepasst, um Gewebeeinwachstum innerhalb der Region, die durch das Aneurysma verformt wird, zuzulassen, welches zur Behandlung des Aneurysmas führt. Bevorzugte embolische Materialien sind diejenigen mit einer offenen Zellenstruktur, wie zum Beispiel Polyvinylalkoholschäume. Es wird auch ein Katheter beschrieben, der zur Einführung eines embolischen Materials in ein Aneurysmus verwendet werden kann.
- Es hat folglich einen seit Jahren empfundenen, aber bisher unerfüllten, Bedarf an einer Vorrichtung zur Behandlung von Aneurysma und einem Verfahren, das Aneurysma mit großen Größenbereichen, Konfigurationen und Einschnürungsbreiten mit einem thrombogenen Medium mit einem minimalen Risiko für versehentlichen Bruch des Aneurysmas oder Beschädigung der Blutaderwand im wesentlichen füllen kann. Es hat einen weiteren Bedarf an einem solchen Verfahren und einer solchen Vorrichtung gegeben, das auch die präzise stellenmäßige Entwicklung des Mediums bei gleichzeitiger Minimierung des Abwanderungspotentials von der Zielstelle weg berücksichtigt. Darüber hinaus sollten ein Verfahren und eine Vorrichtung, die diesen Kriterien nachkommen, auch in einem klinischen Rahmen relativ leicht zu verwenden sein. Solch eine erleichterte Anwendung sollte zum Beispiel auch vorzugsweise eine Einrichtung für gute Visualisierung der Vorrichtung während des Einsatzes und nach dem Einsatz eines Aneurysmas umfassen.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Die vorliegende Erfindung stellt eine Vorrichtung, wie in Anspruch 1 definiert, bereit.
- In einer bevorzugten Ausführungsform ist das Implantat aus einem hydrophilen, makroporösen, polymeren Hydrogelschaummaterial hergestellt, insbesondere einer wasserquellbaren Schaummatrix, die als ein makroporöser Feststoff gebildet ist, der ein schaumstabilisierendes Mittel und ein Polymer oder Copolymer eines frei radikalen polymerbaren hydropfilen Olefinmonomers umfasst, verbunden mit bis zu etwa 10 Gew.-% eines multiolefinfunktionellen Vernetzungsmittels. Das Material ist modifiziert oder enthält Zusatzstoffe, um das Implantat mit Hilfe von herkömmlichen Bildgebungsverfahren erkennbar zu machen.
- Das Gefäßimplantat kann durch ein Verfahren, das folgende Schritte umfasst, hergestellt werden. (a) Abbildung einer Gefäßstelle durch Abtasten der Gefäßstelle zur Gestaltung eines digitalisierten Abtastungsdatensatzes; (b) Anwendung der Abtastungsdaten zur Gestaltung eines dreidimensionalen digitalisierten virtuellen Modells der Gefäßstelle; und (c) Bildung einer Gefäßimplantatvorrichtung in Form eines physischen Modells der Gefäßstelle unter Anwendung des virtuellen Modells, wobei das Implantat aus einem komprimierbaren und ausdehnbaren biologisch verträglichen Schaummaterial gebildet wird. In einer spezifischen Ausführungsform umfasst die Formbildungsstufe (c) die Unterstufen von: (c)(1) Anwendung des virtuellen Modells zur Erschaffung einer verkleinerten dreidimensionalen physischen Form der Gefäßstelle; und (c)(2) Anwendung der Form zur Gestaltung eines Gefäßimplantats in Form eines verkleinerten physischen Modells der Gefäßstelle.
- Die Abbildungsstufe kann durch ein Abtastungsverfahren, wie zum Beispiel Computertomographie (im allgemeinen als "CT" oder "CAT" bezeichnet), Bildgebung durch Magnetresonanz (MRI), magnetische Resonanzangiographie (MRA) oder Ultraschall, vorgenommen werden. Kommerziell erhältliche Software, typisch mit der Abtastungsvorrrichtung verpackt und eingesetzt, rekonstruiert den Abtastungsdatensatz, der durch Abbildung im dreidimensionalen digitalisierten Modell der Gefäßstelle gestaltet wird. Das digitalisierte Modell wird dann durch im Handel erhältliche Software auf eine Form übertragen, die in einem im Handel erhältlichen CAD/CAM-Programm zur Gestaltung der verkleinert physischen Form mit Hilfe von Stereolithographie geeignet ist. Ein angemessenes Implantatmaterial, vorzugsweise ein makroporöses Hydrogelschaummaterial, wird in einen flüssigen oder halbflüssigen Zustand in die Form hineingespritzt. Nach der Erhärtung wird das Hydrogelschaummaterial als ein Implantat in Form eines verkleinert physischen Modells der Gefäßstelle aus der Form entfernt.
- Ein Verfahren zur Embolisation einer Gefäßstelle kann folgende Stufen aufweisen: (a) Intravaskuläre Durchführung eines Mikrokatheters, so dass sein distales Ende in einer Gefäßstelle ist; (b) Bereitstellung eines Gefäßimplantats in Form eines verkleinert physischen Modells der Gefäßstelle, wobei das Implantat aus einem komprimierbaren und ausdehnbaren biologisch verträglichen Schaummaterial gebildet wird; (c) Komprimierung des Implantats in eine komprimierte Konfiguration, die so dimensioniert ist, dass sie durch einen Mikrokatheter durchgeführt werden kann; (d) Durchführung des Implantats, während es in seiner komprimierter Konfiguration vorkommt, durch den Mikrokatheter, so dass das Implantat aus dem distalen Ende des Mikrokatheters in die Gefäßstelle erscheint; und (e) das Expandieren des Implantats in situ, um im wesentlichen die Gefäßstelle zu füllen.
- Die Implantatvorrichtung in ihrer bevorzugten Ausführungsform ist aus einem geformten, hydrophil expandierbaren biologisch verträglichen Schaummaterial gebildet, das eine Anfangskonfiguration in Form eines verkleinert physischen Modells der Gefäßstelle hat, das in eine komprimierte Konfiguration komprimierbar ist, die in den Mikrokatheter passt, und das in eine expandierte Konfiguration hydrophil expandierbar ist, worin es im wesentlichen so dimensioniert ist, dass es sich der Gefäßstelle anpasst und sie auffüllt. Alternativerweise kann die Implantatvorrichtung aus einem nicht-hydrophilen Schaummaterial gebildet sein, das eine Anfangskonfiguration hat, die im wesentlichen die gleiche Größe und Form wie die Gefäßstelle hat, und die sich selber zu dessen Anfangskonfiguration wiederherstellt, nachdem sie aus ihrer komprimierten Konfiguration freigegeben wird.
- Das Retentionselement ist vorzugsweise ein flexibler Draht mit einem distalen Ende, das so konfiguriert ist, dass es das Implantat lösbar eingreift, während die Implantatvorrichtung in ihrer komprimierten Konfiguration vorkommt, um so mit die Implantatvorrichtung im distalen Ende des Mikrokatheters zu bewahren, während das distale Ende des Mikrokatheters an der Gefäßstelle eingesetzt wird. Der Draht kann mit dem Einsatzelement in die distale Richtung axial bewegt werden, um das Implantat von dem distalen Ende des Mikrokatheters auszusetzen, und kann gegenüber dem Einsatzelement proximal bewegt werden, um die Implantatvorrichtung gegen das distale Ende des Einsatzelements anzutreiben und dabei die Implantatvorrichtung vom Draht abzudrücken. Wenn die Implantatvorrichtung somit in die Gefäßstelle freigegeben ist, kann sie in eine expandierte Konfiguration expandieren, wo sie sich im wesentlichen der Gefäßstelle anpasst und sie auffüllt.
- Die vorliegende Erfindung stellt eine Reihe von bedeutsamen Vorteilen bereit. Die vorliegende Erfindung stellt insbesondere ein wirksames Gefäß-Embolisationsimplantat bereit, das im Verhältnis zu den Implantatvorrichtungen im Stand der Tecknik in einer Gefäßstelle mit hervorragender stellenmäßiger Kontrolle und mit niedrigerem Risiko für Gefäßbruch, Gewebeschaden oder Abwanderung als Implantatvorrichtungen im Stand der Technik eingesetzt werden kann. Darüber hinaus bewirkt die Implantatvorrichtung, indem sie auf der tatsächlichen Gefäßstelle, an der sie implantiert werden soll, geformt wird, eine konforme Passform innerhalb der Stelle, die eine wirksame Embolisation fördert, und dennoch erleichtert deren Fähigkeit, in einer hoch komprimierten Konfiguration, an die Stelle geliefert zu werden, einen präzisen und hoch kontrollierbaren Einsatz mit einem Mikrokatheter. Das Verfahren zur Herstellung der Implantatvorrichtung, indem sie auf jeder individuellen Stelle geformt wird, erlaubt außerdem die Herstellung von Implantatvorrichtungen, die Gefäßstellen, die eine Reihe von Größen, Konfigurationen und (insbesondere im Falle von Aneurysmen) Einschnürungsbreiten haben, effektiv embolisieren können. Diese und andere Vorteile können der folgenden ausführlichen Beschreibung leicht entnommen werden.

- Verfahren zur Herstellung eines Gefäßimplantats. Die Stufen eines bevorzugten Herstellungsverfahrens sind als eine Sequenz von beschreibenden Boxen im Flussdiagramm der
1 gezeigt. - Die erste Stufe, in Box
10 der1 gezeigt, ist die Stufe der Erschaffung eines Bildes einer Gefäßstelle, wie zum Beispiel eines Aneurysmas, in dem ein Embolisationsimplantat eingesetzt werden soll. Die Abbildungsstufe wird durch Abtasten der Stelle unter Anwendung von irgendeinem vieler herkömmlicher Abbildungsverfahren, wie zum Beispiel Computertomographie, Bildgebung durch Magnetresonanz (MRI), magnetischer Resonanzangiographie (MRA) oder Ultraschall vorgenommen. - Das Ergebnis der Abbildungsstufe ist ein digitalisiertes Abtastungsdatensatz, der in einem Computerspeicher gelagert wird, aus dem der Datensatz zur Ausführung der nächsten Stufe abgerufen wird: computergesteuerte Rekonstruktion eines dreidimensionalen digitalisierten virtuellen Modells der Gefäßstelle (Box
12 der1 ). Diese Stufe der Erschaffung eines dreidimensionalen digitalen Modells wird typisch mit Hilfe von Software, die dafür vorgesehen ist, vorgenommen, die mit der Abbildungsvorrichtung verpackt ist und verwendet wird. - Das digitalisierte, dreidimensionale virtuelle Modell wird dann auf eine Form übertragen, wo es in einem kommerziell erhältlichen CAD/CAM-Programm (Box
14 ) verwendet werden kann, das ein stereolithographisches Verfahren (Box16 ) kontrolliert, um eine Form für die Formung der Implantatvorrichtung zu erschaffen. Die Übertragung des virtuellen Modells wird mit Hilfe von Software vorgenommen, die kommerziell erhältlich ist, wie zum Beispiel von Cyberform International, Inc., of Richardson, Texas, und von Stratasys, Inc., of Minneapolis, Minnesota. Die Form (nicht gezeigt) wird vorzugsweise von den Dimensionen der Gefäßstelle mit einer Skala von ungefähr 1:2 bis ungefähr 1:6, vorzugsweise 1:4, verkleinert. Die Fom kann wahlweise in "Lebensgröße" (dh. 1:1) hergestellt werden; das ist eine Kopie der Gefäßstelle in natürlicher oder fast natürlicher Größe. Die Form wird zur Herstellung einer Gefäßimplantatvorrichtung mit Hilfe von herkömmlichen Formungsverfahren (Box18 ) verwendet. - Die Implantatvorrichtung. Nach der vorliegenden Erfindung ist in
2 eine Gefäßimplantatvorrichtung20 in ihrer unkomprimierten oder vorkomprimierten Anfangskonfiguration nach Entzug aus der Form gezeigt. Die Implantatvorrichtung20 wird vorzugsweise direkt auf den distalen Endteil eines länglichen, flexiblen, rohrförmigen Retentionselements, wie zum Beispiel eines Retentionsdrahts22 , zum unten beschriebenen Zweck geformt. Der Retentionsdraht22 hat vorzugsweise ein distales Ende, das für bessere Retention der Implantatvorrichtung20 daraufhin in einen Knopf24 (5 ) endet. - In einer bevorzugten Ausführungsform ist die Implantatvorrichtung
20 aus einem biologisch verträglichen, makroporösen, hydrophilen Hydrogelschaummaterial hergestellt, insbesondere einer wasserquellbaren Schaummatrix, die als ein makroporöser Feststoff gebildet ist, der ein schaumstabilisierendes Mittel und ein Polymer oder Copolymer eines frei radikalen polymerbaren hydropfilen Olefinmonomers, verbunden mit bis zu etwa 10 Gew.-% eines multiolefinfunktionellen Verbindungsmittels, umfasst. Ein angemessenes Material diesen Typs ist in US-Patent Nr. 5570585 – Park et al. beschrieben. Ein anderes angemessenes Material ist ein poröses, hydriertes Polyvinylalkoholschaum (PAF)-Gel, das aus einer Polyvinylalkohollösung in einem gemischten Lösungsmittel, die aus Wasser und aus einem wassermischbaren Lösungsmittel besteht, hergestellt ist, wie beschrieben in beispielsweise US-Patent Nr. 4663358 – Hyon et al., deren Beschreibung hierin unter Bezug inkludiert ist. Ein weiteres angemessenes Material ist PHEMA, wie in den oben zitierten Referenzen diskutiert. Siehe z.B. Horák et al., oben, und Rao et al., oben. Das Schaummaterial hat vorzugsweise einen Hohlraumanteil von mindestens ungefähr 90%, und seine hydrophilen Eigenschaften sind so, dass es einen Wassergehalt von mindestens ungefähr 90% bei voller Hydrierung hat. - In einer bevorzugten Ausführungsform wird die Implantatvorrichtung
20 in ihrer vorkomprimierten Anfangskonfiguration die gleiche Konfiguration wie die Gefäßstelle haben, aber sie wird um einen Faktor von ungefähr zwei bis ungefähr sechs kleiner sein. Das Material der Implantatvorrichtung20 und dessen Anfangsgröße sind so ausgewählt, dass die Implantatvorrichtung20 quellbar oder expan dierbar ist zu ungefähr der Größe der Gefäßstelle, primär durch die hydrophile Absorption von Wassermolekülen vom Blutplasma und sekundär durch das Füllen ihrer Poren mit Blut. Das Ergebnis ist eine expandierte Konfiguration für die Implantatvorrichtung20 , wie in4 gezeigt, die eine ausreichende Größe hat, um im wesentlichen die Gefäßstelle zu füllen. - Alternativerweise kann die Implantatvorrichtung
20 geformt werden, so dass sie in ihrer vorkomprimierten Anfangskonfiguration ihre "Lebensgröße" ist, d.h. ungefähr die gleiche Größe wie die Gefäßstelle hat. In diesem Fall ist das bevorzugte Material ein komprimierbares, nicht-hydrophiles Polymerschaummaterial, wie zum Beispiel Polyurethan. In derzeitiger klinischer Praxis würde eine nicht-hydrophile Implantatvorrichtung20 vorteilhaft ein wenig kleiner als die tatsächliche Lebensgröße hergestellt werden, um aufgrund des Füllens der Poren das Quellen anzupassen. - Das Schaummaterial der Implantatvorrichtung
20 , ob hydrophil oder nicht-hydrophil, wird vorteilhaft modifiziert oder enthält Zusatzstoffe, um das Implantat20 durch herkömmliche Abbildungsverfahren erkennbar zu machen. Zum Beispiel kann der Schaum mit einem wasserunauflöslichen Kontrastmittel, wie zum Beispiel Bariumsulfat, imprägniert werden, wie von Thanoo et al., "Radiopaque Hydrogel Microspheres", J. Microencapsulation, Band 6, Nr. 2, S. 233–244 (1989) beschrieben. Alternativerweise können die Hydrogelmonomere mit Kontrastmittel polymerisiert werden, wie in Horák et al., "New Radiopaque PolyHEMA-Based Hydrogel Particles", J. Biomedical Materials Research, Band 34, S. 183–188 (1997) beschrieben. - Egal, aus welchem Material die Implantatvorrichtung
20 hergestellt ist, muss die Implantatvorrichtung20 mit einer Fraktion ihrer Anfangsgröße komprimierbar sein, vorzugsweise in einer im wesentlichen zylindrischen oder pastillengeformten Konfiguration, wie in3 gezeigt. Eine Kompression der Imlantatvorrichtung20 kann so ausgeführt werden, dass sie mit irgendeiner Spannung oder irgendeinem Werkzeug (nicht gezeigt) gedrückt oder gequetscht wird und dann durch Erhitzen und/oder Trocknen in ihre komprimierte Konfiguration "gesetzt" wird, was wohl bekannt ist. Der Zweck dieser Kompression wird unten in Verbindung mit dem Verfahren hinsichtlich der Anwendung der Implantatvorrichtung20 zur Embolisation einer Gefäßstelle beschrieben. - Das Verfahren und die Vorrichtung zur Embolisation einer Gefäßstelle. Das Verfahren zur Embolisation einer Gefäßstelle unter Anwendung der Implan tatvorrichtung
20 wird unter Anwendung einer Implantationsvorrichtung30 ausgeführt, wobei eine bevorzugte Ausführungsform in5 gezeigt ist. Die Implantationsvorrichtung30 umfasst das Retentionselement oder den Draht22 , einen Mikrokatheter32 und ein längliches, flexibles, hohles, rohrförmiges Ele ment34 (vorzugsweise eine Spule), das als ein Implantat-Einsatzelement funktioniert, welches unten beschrieben wird. Mit der Implantatvorrichtung20 am distalen Ende des Retentionsdrahtes22 befestigt, wird das proximale Ende des Retentionsdrahtes22 in das distale Ende des Implantat-Einsatzelements34 eingeführt und durch das Implantat-Einsatzelement34 axial eingefädelt, bis das proximale Ende der Implantatvorrichtung20 in Sitz an dem distalen Ende des Implantat-Einsatzelements34 ist oder dicht damit verbunden ist. Das Implantat-Einsatzelement34 ist so dimensioniert, dass es axial durch den Mikrokatheter32 durchgeführt werden kann. Das Implantat-Einsatzelement34 , mit der die Implantatvorrichtung20 , das aus ihrem proximalen Ende ausgedehnt ist, kann somit in das proximale Ende (nicht gezeigt) des Mikrokatheters32 eingeführt und axial dadurch durchgeführt werden, bis die Implantatvorrichtung20 aus dem distalen Ende des Mikrokatheters32 erscheint, wie in5 gezeigt. - Die Implantatvorrichtung
20 in ihrer komprimierten Konfiguration, hat einen maximalen Außendurchmesser, der kleiner ist als der Innenurchmesser des Mikrokatheters32 , so dass die Implantatvorrichtung20 durch den Mikrokatheter32 durchgeführt werden kann. Die Implantatvorrichtung20 wird vorzugsweise komprimiert und "gesetzt", wie oben beschrieben, bevor sie in den Mikrokatheter32 eingeführt wird. -
6 bis10 illustrieren die Stufen, die im Verfahren zur Embolisation einer Gefäßstelle40 unter Anwendung der Implantatvorrichtung20 verwendet werden. Die Gefäßstelle40 , die aus der Zeichnung hervorgeht, ist ein typisches Aneurysma, aber die Erfindung ist nicht auf irgendeinen spezifischen Typ von Gefäßstelle begrenzt, die embolisiert werden soll. - Zuerst wird, wie in
6 gezeigt, der Mikrokatheter32 mit Hilfe von herkömmlichen Mitteln intravaskulär eingefädelt, bis sein distales Ende sich innerhalb der Gefäßstelle40 befindet. Dieser Einfädelvorgang wird typisch dadurch ausgeführt, dass zuerst ein Katheterleitdraht (nicht gezeigt) entlang dem erwünschten Mikrokatheterweg eingeführt wird, wonach der Mikrokatheter32 über den Katheterleitdraht bewirtet wird, bis der Mikrokatheter32 im wesentlichen so posi tioniert ist, wie es aus6 hervorgeht. Der Katheterleitdraht wird dann entfernt. - Das Implantat-Einsatzelement
34 mit der Implantatvorrichtung20 , die aus ihrem distalen Ende ausdehnt, wird dann durch den Mikrokatheter32 durchgeführt, wie oben beschrieben, bis die Implantatvorrichtung20 aus dem distalen Ende des Mikrokatheters32 in die Gefäßstelle40 erscheint, wie in7 und8 gezeigt. Bei Einführung der Implantatvorrichtung20 in den Mikrokatheter32 kann eine biologisch verträgliche nicht-wässrige Flüssigkeit, wie zum Beispiel Polyethylenglycol, in den Mikrokatheter32 eingesprizt werden, um eine vorzeitige Expansion der Implantatvorrichtung20 aufgrund von Hydrierung zu verhindern und um die Friktion mit dem Inneren des Mikrokatheters32 zu reduzieren. Hinsichtlich der Implantatvorrichtung20 , die somit aus dem Mikrokatheter32 in das Innere der Gefäßstelle40 ausgesetzt wird, fangen die Poren der Implantatvorrichtung20 damit an, eine wässrige Flüssigkeit aus dem Blut innerhalb der Gefäßstelle40 aufzunehmen, um ihre Menge freizugeben und es ihnen zu ermöglichen, ihre expandierte Konfiguration anzunehmen, wie in9 gezeigt. Wenn die Implantatvorrichtung aus einem hydrophilen Material besteht, wird sie weiterhin aufgrund hydrophiler Hydrierung des Implantatmaterials expandieren, sowie aus dem Fülle nderen Poren mit Blut. Sofern die Implantatvorrichtung20 aus einem nicht-hydrophilen Material besteht, ist ihre Expansion nur auf den letzteren Mechanismus zurückzuführen. - Schließlich, wenn die Expansion der Implantatvorrichtung
20 in vollem Gange ist (und nicht notwendigerweise, wenn sie abgeschlossen ist), wird der Retentionsdraht22 in Bezug auf das Implantat-Einsatzelement34 proximal gezogen, wobei die Implantatvorrichtung durch den Druck, der vom distalen Ende des Implantat-Einsatzelements34 an dieser angewandt wird, aus dem Ende des Installationsdrahtes22 abgestoßen wird. Die Implantatvorrichtung20 , die jetzt frei von der Implantationsvorrichtung30 ist, wie in10 gezeigt, kann weiterhin expandieren, bis sie im wesentlichen die Gefäßstelle40 auffüllt. Die Implantationsvorrichtung30 wird dann entfernt, wobei die Implantatvorrichtung20 in einer Position belassen wird, wo sie die Gefäßstelle40 embolisieren kann. - Während eine bevorzugte Ausführungsform der Erfindung oben beschrieben worden ist, mögen eine Reihe von Variationen und Modifikationen für Fachleute der entsprechenden technischen Bereiche sich selbst andeuten. Anstatt der maßgefertigten Herstellung der Implantatvorrichtung für jeden Patient, können zum Beispiel Implantatvorrichtungen in einer Reihe von standardgemäßen Größen und Formen hergestellt werden, und eine spezifische Implantatvorrichtung kann dann auf der Grundlage der Abbildung der Gefäßstelle für einen Patient ausgewählt werden. In diesem Fall würde das Herstellungsverfahren, das aus
1 hervorgeht, modifiziert werden, indem zuerst ein dreidimensionales digitales Modell für jedes standardmäßiges Implantat (Box12 ) erschafft wird, wonach mit den nachfolgenden Stufen, die in Box14 ,16 und18 gezeigt sind, fortgefahren wird. Abbildung (Box10 ) würde als eine frühe Stufe im Embolisationsverfahren ausgeführt werden, gefolgt von der Auswahl einer der standardmäßigen Implantatvorrichtungen.

32 ) mit einem distalen Ende und einem proximalen Ende; eine Gefäßimplantatvorrichtung (20 ), die aus einem makroporösen Hydrogelschaummaterial gebildet ist und eine Anfangskonfiguration hat, die so dimensioniert ist, dass sie durch den Mikrokatheter von dessen proximalen Ende und aus dem distalen Ende heraus durchgeführt werden kann, wobei die Vorrichtung von der Anfangskonfiguration zu einer expandierten Konfiguration hauptsächlich durch hydrophile Wirkung expandierbar ist; ein Retentionselement (22 ), das in dem Mikrokatheter enthalten ist und ein distales Ende aufweist, das mit der Implantatvorrichtung ablösbar verbunden ist und ein Einsatzelement (34 ), das mit dem Retentionselement funktionell verbunden ist und in die Implantatvorrichtung so eingreifen kann, dass die Implantatvorrichtung von dem Retentionselement getrennt werden kann, wenn die Implantatvorrichtung aus dem distalen Ende des Mikrokatheters ausgetreten ist, wobei das Einsatzelement so dimensioniert ist, dass es axial durch den Mikrokatheter vom proximalen Ende zum distalen Ende davon durchgeführt werden kann, wobei das Einsatzelement ein distales Ende hat, das in die Implantatvorrichtung (20 ) eingreifen kann; wobei das Retentionselement (22 ) so ausgebildet ist, dass es sich mit dem Einsatzelement (34 ) bewegen kann, wenn das Einsatzelement durch den Mikrokatheter (32 ) geführt wird und sich auch zwischen einer ersten und einer zweiten Position zum distalen Ende des Einsatzelements bewegen kann, wodurch die Implantatvorrichtung (20 ) aus dem distalen Ende des Mikrokatheters geschoben wird, wenn das Einsatzelement durch den Mikrokatheter geführt wird und wobei die Implantatvorrichtung von dem Retentionselement getrennt wird, wenn sich das Retentionselement von der ersten Position zur zweiten Position bewegt wird. - Vorrichtung nach Anspruch 1, wobei das Einsatzelement (
34 ) ein längliches, flexibles, rohrförmiges Element aufweist. - Vorrichtung nach Anspruch 2, wobei das Retentionselement (
22 ) ein längliches, flexibles, faserförmiges Element aufweist, das axial durch das rohrförmige Element angeordnet ist und zwischen einer ersten und einer zweiten Position bewegbar ist. - Vorrichtung nach einem der Ansprüche 1 bis 3, wobei die Implantatvorrichtung (
20 ) strahlenundurchlässig ist.
