Consequently, as a physician moves the thumb actuator pad 101 between proximal and distal positions, the slider 14 and the sheath 17 move relative to the rod 91, the cable 90 and the retrieval basked 15 (Fig. 1). Further as the thumb pad 101 moves toward the distal position, the distal end of the sheath 17, as shown in Figs. 2 and 3, advances over and compacts the strands 22 of the retrieval basket 15 due to the axial stiffness and hoop strength of a polyimide tube or tube of similar material.
- an extractor that meets the various objects of this invention.
- the extractor simplifies the capture and release of objects by providing relatively few wires comprising individual strands at the proximal end of the basket.
- the extractor also provides for improved retention of objects captured thereby by providing a basket with a plurality of wires with a plurality of filaments at the distal end.
- spaced, preformed wires each have individual strands in the proximal section and a plurality of spaced filaments at the distal portion.
- the spaced filaments increase the number of contact points with an entrapped calculi at the distal section of the basket. Yet there is no concomitant increase in the contact points in the number of wires or strands in the proximal section of the basket.
- FIGS. 1 and 2 depict a retrieval basket 15 with two prestressed strands 22 and eight filaments 24.
- Each set of four filaments is interconnected only at the ends thereof. Consequently the filaments 24 in each set may be widely spaced where they join their respective strands 22. This opening, in appropriate circumstances, might be reduced by interconnecting adjacent strands from each set by a connector, by joining or by some other means.

Claims (23)
- A surgical extractor for removing an object from a body including a retrieval basket (15) with distal (21) and proximal (19) ends and a retractable sheath (17) which in a first position retains said retrieval basket (15) in a compact condition and in a second position frees said retrieval basket (15) for expansion to form an enlarged basket for retrieving an object, said basket (15) comprising a plurality of wires (28) extending between said distal and proximal ends (21,19) of said basket (15), each of said wires (28) comprising an individual strand (22) extending from one of said distal and proximal ends (21,19) of said basket (15) and a plurality of spaced filaments (24) extending between said strand (22) and the other of said distal and proximal ends (21,19) of said basket (15), said sheath (17) covering both said strands (22) and said filaments (24) when in said first position, characterized in that a diameter of said strands (22) is greater than a diameter of said filaments (24).
- A surgical extractor as claimed in claim 1, wherein said retractable sheath (17) comprises an elongated sheath (17) movable between said first position in which said sheath (17) is axially coextensive with all of said strands (22) and filaments (24) for constraining said strands (22) and filaments (24) in a compact condition wherein said strands (22) and filaments (24) are individually stressed, and said second position for freeing said wires (28) for expansion due to the action of the internal stresses to form said enlarged basket for retrieving the object.
- A surgical extractor as claimed in claim 1 or 2, comprising handle means (11 ) extending along an axis at a proximal portion of said extractor (10) for operating said extractor (10), said handle means (11 ) having base means (13) for being grasped by a physician and slider means (14) for reciprocating along the axis with respect to said base means (13), said basket (15) being connected to said base means (13) and said sheath (17) being connected to said slider means (14), said wires (28) being spring formed and prestressed when retained in said sheath (17).
- A surgical extractor as claimed in claim 3, comprising radially flexible, axially stiff support means (90,91) attached intermediate said base means (13) and said basket (15) and lying within said sheath (17) wherein said strands (22) of said wires (28) additionally have proximal portions that attach to the distal end of said support means (90,91) and means (25,26) at opposite ends of said basket (15) for binding said filaments (24) and strands (22), respectively, together thereby to limit the axial extent of said enlarged basket.
- A surgical extractor as claimed in claim 4, wherein said binding means (25) at the distal end of said basket (15) includes a filiform (49).
- A surgical extractor as claimed in claim 3,4 or 5, wherein said base means (13) comprises a molded plastic, partially hollowed base (13) extending axially.
- A surgical extractor as claimed in claim 4, wherein said support means (90,91) includes a solid stainless steel radially flexible, axially stiff rod (91) attached to said base (13) and extending distally therefrom, a stainless steel cable (90) extending distally from said rod (91) and means for connecting said rod (91) and said cable (90) together axially.
- A surgical extractor as claimed in claim 7, wherein said wires (28) are made of stainless steel and are attached to the distal end of said cable (90).
- A surgical extractor as claimed in any one of claims 3 to 8, wherein said slider means (14) comprises a plastic slider (14) captured in said base means (13) for limited axial movement relative thereto between distal and proximal positions.
- A surgical extractor as claimed in, any one of claims 4 to 9, wherein said sheath (17) comprises a polyimide sheath (17) of a given internal diameter having a proximal end attached to said slider means (14) for overlying said support means (90,91) and said wires (28), said slider means (14), in its distal position, locating said sheath (17) coextensively with said distal portion of said wires (28) thereby to compact and retain said wires (28) at substantially the given diameter and, in its proximal position, retracting said sheath (17) to release said wires (28) to form said enlarged retrieval basket for entrapping the object, such that each of said filaments (24) is spaced from said other filaments (24) and each of said strands (22) is spaced from said other strands (22).
- A surgical extractor as claimed in any one of claims 4 to 10, wherein said sheath (17) comprises a polyimide tube that overlies said support means (90,91) and said wires (28).
- A surgical extractor as claimed in any one of claims 4 to 11, wherein said handle means (11) and said slider means (14) are formed of molded plastic.
- A surgical extractor as claimed in any one of claims 4 to 12, wherein said support means (90,91) and said wires (28) are formed of stainless steel and said sheath (17) is formed of polyimide.
- A surgical extractor as claimed in any one of claim 3 to 13, wherein said slider means (14), in a position intermediate its distal and proximal positions, locates said sheath (17) coextensively with said strands (22) of said wires (28) thereby to compact and retain said strands (22) at substantially the given diameter and to release said filaments (24) to define a second retrieval basket of reduced length and diameter relative to said first retrieval basket.
- A surgical extractor as claimed in any preceding claim, including a filiform (46) extending distally of said wires (28) for facilitating the steering of said extractor.
- A surgical extractor as claimed in any preceding claim, wherein at least one of said strands (22) of said wires (28) comprises extensions of said spaced filaments (24) secured together.
- A surgical extractor as claimed in any preceding claim, wherein each of said filaments (24) is helically wound as a turn in a helix.
- A surgical extractor as claimed in any preceding claim, wherein said plurality of spaced filaments (24) is in the range of two to six.
- A surgical extractor as claimed in any preceding claim, wherein said plurality of wires (28) is in the range of two to four.
- A surgical extractor as claimed in any preceding claim, wherein at least one of said strands (22) is a solid wire secured to said plurality of filaments (24).
- A surgical extractor as claimed in any preceding claim, wherein each of said filaments (24) is helically wound as a turn in a helix and each of said strands (22) extends in substantially a planar fashion.
- A surgical extractor as claimed in any preceding claim, wherein said filaments (24) form a web-like structure.
- A surgical extractor as claimed in any preceding claim, wherein said plurality of filaments (24) is in the range of four to sixteen.
