Definitions
- the invention relates to the technical sector of orthopedic implants for the hip joint.
- An acetabular implant essentially comprises a metallic cup, of general hemispherical shape, intended to be impacted in the acetabular cavity of the iliac bone, according to different arrangements, in combination or not with cement.
- the cup receives a polyethylene, ceramic or metal core intended to cooperate, with articulation capacity, with a femoral head of a complementary femoral implant.
- the core can be mounted in the internal cavity of the cup, in a fixed manner or with displacement or multidirectional capacity.
- the movable or fixed nature of the core with respect to the metal cup requires different arrangements. In other words, it is not possible to use the same metal cup for mounting a mobile core or mounting a fixed core.
- a cotyloid implant comprising a cup which can receive a fixed insert or a fixed mobile insert on a mobile core, while a mobile insert receives a fixed core.
- the solution described in this patent only allows the mounting of a core with mobility capacity or the mounting of a core fixed to an insert mounted with mobility capacity in the hemispherical cavity of the cup. It results that the nucleus cooperating with this femoral head is, in all cases, mounted with mobility capacity relative to the cup.
- the problem which the invention proposes to solve is to be able, depending on the type of arthroplasty to be treated, to use the same metal cup with the possibility of obtaining double mobility (mobile core -mobile head), or simple mobility (fixed insert-movable head), it being recalled that, in all cases, the femoral head is mounted with articulation capacity relative to the nucleus of the acetabular implant.
- a cotyloid implant for hip prosthesis has been designed and developed which is remarkable in that it comprises: a first type of insert has a spherical internal cavity for mounting with mobility capacity of a hemispherical nucleus having an internal cavity capable of cooperating, with articulation capacity, with a femoral head; a second type of insert has an internal cavity capable of cooperating, with articulation capacity, with the femoral head; characterized in that the center of rotation of the femoral head is different from the axis of rotation of the above first type of insert and / or of the above second type of insert.
- this task was solved by placing a bipolar cup between the head and the acetabulum, which allows the head to rotate in the bipolar cup, and the bipolar cup to rotate in the acetabular cavity. This double possibility of rotation greatly reduces dislocations and subluxations.
- This system is also called a dual mobility system or three-pole system.
- the implant according to the invention will comprise a metallic cup of hemispherical shape capable of being fixed in the bottom of the acetabular cavity of the hipbone.
- the above cup will imitate an internal cavity having arrangements for the assembly at will, in a fixed manner, of the aforesaid two first and second types of insert.
- the first type of insert makes it possible to obtain double mobility (mobility of the nucleus and of the femoral head), while the second type of insert makes it possible to obtain a single mobility ( mobility of the femoral head).
- 1 'of the second type and the movable core insert have fixtures for the assembly of a ring capable of ensuring the retentiveness of a femoral head.
- This ring is split in order to be spaced apart by elasticity in order to be positioned in arrangements of complementary shapes which have the opening of the insert or of the core.
- the ring delimits a concave internal bearing in relation to the hemispherical external bearing of the femoral head.
- the arrangements of the internal layer of the cavity cooperate with complementary arrangements presented by the external surface of the two types of insert to ensure a fixing by clipping effect in particular.
- the arrangements are constituted by a series of frustoconical circular surfaces.
- the first type of insert is ceramic, polyethylene or metal
- the insert of the second type is either ceramic, polyethylene or metal
- the core meanwhile, is either ceramic, polyethylene or metal.
- the inserts, the core and the femoral head are made of ceramic.
- the inserts of the first type, of the second type, and the core will be either metal, polyethylene or ceramic or a combination of these materials, the femoral head being either metal or ceramic, or a combination of these materials.
- the inserts of the first type, of the second type, and the core as well as the femoral head are either of metal, or of polyethylene or of ceramic, or of a combination of these materials.
- FIG. 1 is a view in longitudinal section before mounting of the components of the acetabular implant, in the case where the implant is of the first type for mounting with mobility capacity of a core intended to receive the femoral head, and in the case of an alignment of the different centers relative to one axis of symmetry of the cup;
- FIG. 1a is a view similar to Figure 1 in which the internal hemispherical cavity of the movable core is eccentric along the axis of symmetry;
- Figure 2 is a longitudinal sectional view before mounting of the main components of the acetabular implant, in the case of an insert of the second type intended to receive the femoral head directly, and in the case of an alignment of the different centers with respect to the axis of symmetry of the cup;

- the acetabular implant comprises a common metallic cup (1) of generally hemispherical shape.
- This cup (1) has all types of arrangements to be fixed in the acetabular cavity of the iliac bone with or without cement.
- This cup (1) defines an internal cavity (la) having arrangements for mounting at will, at the operator's choice of two types of insert (2) or (3).
- the insert of the first type (2) has a spherical internal cavity (2a) for mounting, with mobility capacity, a hemispherical core (4) which itself has a concave hemispherical internal cavity (4a) for mounting , with articulation capacity, of a femoral head (T) that has the corresponding femoral implant.
- dual mobility is obtained by the ability to multidirectional movement of the core (4) in the cavity (2a) of the insert (2) and of the femoral head (T) being in the internal cavity (4a) of the core (4).
- the insert of the second type (3) has an internal cavity (3a) able to cooperate with capacity for articulation with the femoral head (T). According to this embodiment, shown in Figures 2, 5 and 6, mobility is provided by the multidirectional movement of the head (T) in the cavity (3a) of one insert (3).
- the insert (3) of the second type (and possibly the insert (2)), as well as the movable core (4) may have arrangements (3d) and (4b), for mounting a ring. (5) capable of ensuring the retentivity of the femoral head (T).
- a stepped circular bearing is formed concentrically with the cavities (3a) and (4a), the circular bearing being of diameter greater than that of said cavities.
- the ring (5) has an external profile in accordance with that illustrated in FIG. 13 and is split in (5a) so as to be spaced apart by elasticity so as to be put in place at the level of the stepped circular bearings (3d) and (4b). Holes (5b) allow the introduction of beaks that a clamp can have.
- the internal surface (5c) of the ring (5) is concave in relation to the external hemispherical surface of the femoral head (T).
- the ring provides a retentive effect, that is to say keeps the femoral head in the inserts, between the femoral head and the bottom of the cavity of the movable core (4).
- the diameters of the cavities (3a) and (4a) are identical to receive the same femoral head (T) of determined diameter.
- the diameters of the cavities (3a) and (4a) are established according to a dimensional range corresponding to the diameters of the femoral heads (T).
- the internal cavity (la) of the cup (1) has arrangements for the fixed mounting of any of the types of inserts (2) and (3).
- these arrangements cooperate with complementary arrangements that the external face of the insert (2) and the insert (3) has, in particular for fixing by clipping effect.
- these arrangements are constituted, for an insert (2) by a series of frustoconical circular bearings (2b) and (2c) and for the insert (3), by a series of bearings tapered circulars (3b) and (3c).
- (1) may be killed by other additional assembly sformes and by any known and appropriate means.
- the insert of the second type (3) is ceramic
- the insert of the second type (3) is ceramic
- the core (4) is also ceramic
- the metal cup (1) may have any coating and / or external surface treatment. From this basic concept, in a preferred embodiment, the insert, the movable core and the femoral head are made of ceramic.
- An all-metal construction of the acetabular implant is also planned (first or second type insert, nucleus and femoral head if necessary).
- the centers (01) of the first type insert (2), (02) of the core (4) and (03) of the second type insert (3) are aligned with each other and along the axis of symmetry (x-x ') of the cup (1) ( Figures 1 to 6).
- the centers (01) of the first type insert (2), (02) of the core (4) and (03) of the second type insert (3 ) are aligned and offset relative to the axis of symmetry (xx 1 ) of the cup (1) ( Figures 7 to 12).
- the main aim of the present invention is to improve the risk of dislocations in hip prostheses of the systems of the prior art. Furthermore, the invention also aims as advantages the use of two types of insert to obtain a dual mobility assembly with an intermediate core or single mobility, without an intermediate core. In addition, this dual mobility reduces the risk of dislocation and avoids any conflict by ensuring automatic refocusing.
