Abstract

Kniebandagen unterstützen durch ihre elastische Uirkung den Band- und Muskelapparat, während die Knieorthesen mit nicht-elastischen, knochenfunk t ionsähnlich starren Elementen Band- und Muskelapparat des Knies noch zusätzlich festigen und unterstützen sollen. Scharnierstellen von Orthesen sind jedoch schwierig zu plazieren und vor allen Dingen in den Bewegungsablauf einzubringen. Ein sich beugendes Knie verhält sich jedoch nicht wie ein einfaches Scharnier mit einem festen Drehpunkt, vielmehr verschiebt sich dieser kontinuierlich beim Abrollen des Condylus, was biomechanisch sehr schwierig nachbildbar ist. So bewähren sich immer noch die elastischen Unterstützungsmechanismen in Form von Bandagen aller Art. Während die steiler gekreuzten Kreuzbänder im Bereich zwischen den - Kondylen angeordnet sind, umschlingen die schwächer gekreuzten Menisken die Kondylen im Aussenbereich. Die kollateralen Bänder sowie das Ligamentum patallae mit dem Tendo quadricipitis (Kniescheibensehne) verbinden die Femur mit der Tibia und Fibula in direkter Verbindung. Alle diese schräg bis gerade verlaufenden Verbindungen werden in praktisch jedem Belastungsfall, Zug-, Druck-, Rotationsbelastung Lind Mischformen davon, in ihrem Zusammenspiel optimal belastet. Der diagonale Verlauf der Di agonalbänder unterstützt die Haltewirkung des Band- und Muskelapparat des Kniegelenks bei Rotationsbewegungen zwischen Femur und Tibia: bei den Kreuzbändern entlastet das Bandage- Di agonal band 22 bspw. das hintere Kreuzband 11 und das Bandage- Diagonalband 23 das vordere Kreuzband 10. Direkt werden auch die kollateralen Bänder 15 in ihrer torsionshemmenden Wirkung unterstützt. Ferner bewirken die rund um Ober- und Unterschenkel laufenden Schenkelmanschetten, durch Druck auf die Schenkel- Muskeln eine indirekte aktive Unterstützung der muskulären Verbindungsstellen im Kniegelenk. Die Längs-Elastizität des Bandes ist bspw. an den Aussense iten 31,32 geringer, damit sich die Manschetten dem konischen Verlauf der Schenkel entsprechend anpassen. Ebenso kann man mit den Diagonalbändern 22,23 verfahren, die übereinandergelegt gekreuzt an den Stellen 28 mit den Sehen- ke lmansche t ten vernäht sind und bspw. an den Seiten 33 einer geringere Elastizität aufweisen als an der anderen Parallelseite. Auf diese Weise schmiegen sich die Diagonalbänder von unten her sanft an die Kniescheibe ohne sie hoch zu drücken.FIG. 5 shows the knee brace of FIG. 4 in its open form. Mit einem im Bereich der Oeffnung P angeordneten Ring 35, bspw. aus Gummi, Silikon o.ä. bestehend, der mit, hier an der Ober- und Unterschenkelmanschette befestigten, Bändern gehalten ist, lässt sich die verstärkte Patellaführung recht einfach realisieren. Figur 6 zeigt noch eine vorteilhafte Ausgestaltung der Kniebandage mit einer längeren Oberschenkelmanschette und einer kürzeren Unterschenkelmanschette, wie dies bei den üblichen anatomischen Gegebenheiten des Beines angebracht ist. Als Verschlussmittel sind hier Oesen 38 gezeichnet, in welche die Bandenden 39 eingeschlauft werden können. Die Bemessungen der Länge der Diagonalbänder und der Kreuzungswinkel richten sich nach anatomischen Erfordernissen aus. Auch ist es möglich neben einer gemäss statistischen anatomischen Abmessungen ausgelegten "Standard-Kniebandage", die mehr zur Prophylaxe gedacht ist, mit Hilfe der gezeigten Projektionsmethode auch spezielle posttraumatische Laxitäten individuell auszugleichen. Die Führung von weiteren Bändern zwischen den Schenkelmanschetten, bewirkt zusätzliche Hilfs- funktionen und die spezielle Ausgestaltung der verwendeten Bänder bezüglich Schnitt und bezüglich Elastizitätsgradienten im Band, öffnet weitere Möglichkeiten zur verfeinerten Ausgestaltung der Kniebandage gemäss Erfindung.
