Abstract

An oral appliance for reducing the occurrence of snoring, comprises a generally U-shaped platform (10) formed with flanges (e.g. 12, 16) extending along its inner and outer edges and projecting from either side of the platform to define upper and lower U-shaped channels for receiving the teeth of the upper and lower jaws, respectively. At least one vent passage (20, 22) is formed through the platform (10).

Description

Appliance for reducing snoring
The present invention relates to an appliance for reducing the occurrence of snoring.
It has been proposed to reduce the occurrence of snoring by inserting an oral appliance between the jaws, the appliance being arranged to displace the lower jaw forwardly: this has the effect of enlarging the airway in the throat, and also the effect of bringing the tongue forward from the back of the throat. This allows the air passing through the throat to slow down and not vibrate the loose tissues which are present, with the result that snoring is stopped.
Appliances of this type have hitherto been of a generally U-shape form in plan, comprising a U-shape platform or bite stop, with flanges extending along the inner and outer edges of this platform and projecting both upwardly and downwardly from it. The appliance therefore has U-shaped channels either side of the bite stop, to receive the opposed sets of teeth of the upper and lower jaws: these two channels are generally aligned with each other, such that, in use, the lower jaw is displaced forwardly from its normal position. US patent No. 5,462,066 discloses such an appliance, which is made of a plastics material which may be heated to a temperature higher than body temperature, then inserted into the mouth and deformed to conform to the contours of the subject's teeth and gums. However, I have found that the appliance prevents the user breathing through the mouth, and therefore gives rise to difficulties particularly if the user is suffering from a stuffy or blocked nose.
I have now devised an appliance which overcomes the above-outlined problem. In accordance with the present invention, as seen from a first aspect, there is provided an oral appliance for reducing the occurrence of snoring, the appliance comprising a generally U-shaped platform formed with flanges extending along its inner and outer edges and projecting from either side of the platform to define upper and lower U-shaped channels for receiving the teeth of the upper and lower jaws, respectively, the appliance being formed with at least one vent passage extending through said U-shaped platform from its outer edge to its inner edge.
Preferably a pair of vent passages are formed through the U-shaped platform, positioned either side of the front centre of the appliance. Preferably each vent extends from approximately the mid-line of the respective central incisor to approximately the mid-line of the respective first pre- molar . It will be appreciated that the vent passages allow significant air flow and so enable the user to breathe through the mouth, where this is desired. Nevertheless, I find that the appliance still reduces the occurrence of snoring because of its action in displacing the lower jaw slightly forwardly from its normal position.
In the appliance of US patent No. 5,462,066, the U- shaped platform or bite stop is of uniform depth, and can only be fully gripped if the crests of the upper teeth lie in a plane parallel to the crests of the lower teeth. However, in the natural opening movement of the mouth, the mandible rotates around a fixed horizontal axis in the tempero mandibular joint
(the terminal hinge axis) , which lies rearwardly of both sets of teeth: thus, as a result of a natural opening movement, the upper and lower teeth lie on diverging planes; the requirement for the upper and lower teeth to try and adopt a parallel disposition, to fully grip the bite stop of the appliance of US patent No. 5,462,066, places a strain on the jaws which leads to discomfort. I have now devised an appliance which overcomes this problem. In accordance with the present invention, as seen from a second aspect, there is provided an oral appliance for reducing the occurrence of snoring, the appliance comprising a generally U-shaped platform formed with flanges extending along its inner and outer edges and projecting from either side of the platform to define upper and lower U-shaped channels for receiving the teeth of the upper and lower jaws, respectively, the platform increasing in depth from its rearward ends to its front centre. With the platform or bite stop of the appliance varying in depth in this manner, the upper and lower sets of teeth will engage the opposite surfaces of the platform in appropriate manner when the jaws are opened slightly in the normal manner
(i.e. with simple rotation around the terminal hinge axis) . Also in the appliance of US patent No. 5,462,066, each of the flanges diminish in depth, relative to the bite platform, towards the rear ends of the platform. The flanges are accordingly of uncomfortably large depth towards the front of the appliance. I have now devised an appliance which overcomes this problem.
Thus, in accordance with the present invention as seen from a third aspect, there is provided an oral appliance for reducing the occurrence of snoring, the appliance comprising a generally U-shaped platform formed with flanges extending along its inner and outer edges and projecting from either side of the platform to define upper and lower U-shaped channels for receiving the teeth of the upper and lower jaws, respectively, at least the outer flanges being each of substantially uniform depth relative to the respective surface of the platform. Preferably each of the outer flanges is arranged to overlap the gums, beyond the gingival line, by a substantially uniform amount, preferably by about 2mm. Typically, each flange is of a depth in the range of 10 to 15mm relative to the corresponding surface of the platform. An embodiment of the present invention will now be described by way of example only and with reference to the accompanying drawings, in which:
FIGURE 1 is a diagrammatic plan view of an appliance in accordance with the present invention, for reducing the occurrence of sleep apnoea;
FIGURE 2 is a side view of the appliance shown in Figure 1 ; and
FIGURE 3 is a front view of the appliance shown in Figures 1 and 2.
Referring to the drawings, there is shown an oral appliance for reducing the occurrence of snoring, the appliance being formed in one-piece from a thermoplastics material. The appliance comprises a platform or bite stop 10 which is generally U-shaped in plan view: flanges 12,14 extend along the U-shaped outer edge of the bite stop 10 and project upwardly and downwardly, respectively, whilst flanges 16,18 extend along the U-shaped inner edge of the bite stop 10 and also project upwardly and downwardly, respectively. The appliance is thus formed with upper and lower U-shaped channels, to receive the sets of teeth of the upper and lower jaws of the user: when worn, the appliance acts to displace the lower jaw forwardly from its normal position, by approximately Vβth inch. It will be noted that the bite stop 10 increases progressively in depth, from the molar region to the incisors
(where the depth may be in the range 5 to 10mm, most typically
8mm) . In this way, the bite stop is suited to the diverging planes on which the upper and lower sets of teeth lie when the jaws are slightly open.
Further, it will be noted that the buccal flanges 12,14 are of generally uniform depth relative to the bite stop 10, this depth being generally in the range of 10 to 15mm. In this way, the flanges 12,14 overlap the gingival line by a uniform amount, typically 2mm, to ensure a comfortably fit.
A pair of vents 20,22 are formed through the bite stop 10, either side of the centre front. In particular, each vent extends from approximately the mid- line of the respective central incisor, around to approximately the mid-line of the first pre-molar. The vents 20,22 allow substantial air flow through them and so allow the user to breathe through the mouth when this is desired.
It will also be noted that the lower, outer flange 14 is formed at the front, centre region (lower labial flange) with a curved recess 24, to accommodate the user's lower labial frenum, which has a relatively high attachment in some individuals .
The appliance is formed of a thermoplastics material which may be heated to a temperature above body temperature, preferably by immersing in hot water: the appliance can then be inserted into the mouth of the intended user, who then bites firmly and presses the inner flanges against the teeth and gums, using the tongue, and also presses the outer flanges against the teeth and gums, to mould the appliance to the contours of the user's mouth. Conveniently, the appliance can be gripped by a pair of tongs, the opposed jaws of which are engaged into the respective vents 20,22 to facilitate handling of the appliance without interfering with the manner in which it fits into the user's jaws.

Claims

Claims
1) An oral appliance for reducing the occurrence of snoring, the appliance comprising a generally U-shaped platform formed with flanges extending along its inner and outer edges and projecting from either side of the platform to define upper and lower U-shaped channels for receiving the teeth of the upper and lower jaws, respectively, the appliance being formed with at least one vent passage extending through said U-shaped platform from its outer edge to its inner edge.
2) An appliance as claimed in claim 1, in which a pair of said vent passages are formed through said U-shaped platform, said vent passages being positioned either side of a front centre point of the appliance.
3) An appliance as claimed in claim 2, m which each vent extends from a point corresponding approximately with the mid- line of the respective central incisor to a point corresponding approximately to the mid- line of the respective first pre- molar.
4) An appliance as claimed in any preceding claim, in which said platform increases in depth from its rearward ends to its front centre .
5) An appliance as claimed in any preceding claim, in which said flanges which extend along said outer edge of said platform are each of substantially uniform depth relative to the respective surface of said platform.
6) An appliance as claimed in any preceding claim, in which said flanges which extend along said inner edge of said platform are each of substantially uniform depth relative to the respective surface of said platform.
7) An appliance as claimed in any preceding claim, in which the edge of the lower, outer flange is formed with a curved recess at its front centre, to accommodate the lower labial frenum of the user.
8) An oral appliance for reducing the occurrence of snoring, the appliance comprising a generally U-shaped platform formed with flanges extending along its inner and outer edges and projecting from either side of the platform to define upper and lower U-shaped channels for receiving the teeth of the upper and lower jaws, respectively, the platform increasing in depth from its rearward ends to its front centre.
9) An oral appliance for reducing the occurrence of snoring, the appliance comprising a generally U-shaped platform formed with flanges extending along its inner and outer edges and projecting from either side of the platform to define upper and lower U-shaped channels for receiving the teeth of the upper and lower jaws, respectively, at least the outer flanges being each of substantially uniform depth relative to the respective surface of the platform.
10) An oral appliance as claimed in claim 9, in which each of the outer flanges is arranged to overlap the gums, beyond the gingival line, by a substantially uniform amount.
