Description

The invention relates to a method for controlling light sources, in particular in the ultraviolet range, in which image data of the region to be irradiated by the light sources is recorded and the image data are processed and stored.
The invention further relates to a device for carrying out this method with at least one light source and at least one image recording device for recording a region to be irradiated by the light sources and a computer for processing the image data recorded with the image recording device and a memory for storing the image data.
Irradiation with light, particularly in the ultraviolet range, is frequently used in derology for the treatment of skin diseases. Both ultraviolet lasers and gas discharge lamps in the UVA and UVB range are being used to an increasing extent. Common uses in cosmetics and medicine are e.g. the treatment of neurodermatitis and psoriasis as well as the removal of scar redness and tattoos. The treatments are usually carried out using devices which have a fixed size, for example in the form of a hand or foot device or in the form of a whole-body chamber. As a rule, laser treatment devices cannot be adjusted to the shape and size of the region to be treated. Localized treatment is only achieved by hand guidance. A disadvantage of known devices is that the region that is not to be treated cannot be cut out automatically and is therefore unnecessarily irradiated. Those regions which are involuntarily irradiated during the irradiation with the light sources can have adverse effects due to the irradiation, such as e.g. accelerated aging of the skin or increased risk of skin cancer.
A method and a device for simulating and determining the radiation in the field of radiation therapy is known, for example, from US Pat. No. 5,438,991. An image of the region of the body to be treated is recorded and the selected region is determined using the image data and used for control purposes. projection means, for example in the form of a liquid crystal display. The controlled projection means sends light and thus simulates and visually controls the radiation of the body. After the check, the patient is now exposed to therapeutic radiation, for example using a collimator to cover the regions that are not to be irradiated.
Nachteilig dabei ist, dass die gewünschte Region vor dem Eingriff manuell vom Chirurgen abgegrenzt werden muss.EP 391 376 AI shows a device for computer-controlled laser surgery, an image of the region to be treated being recorded and displayed. In a first simulation operating mode with low energy, the laser is guided over the desired area and these locations are detected. The surgical mode of operation of the laser with high energy is then selected, with activation taking place in accordance with the recorded locations. The disadvantage here is that the desired region must be delimited manually by the surgeon before the intervention.
Eine Änderung der gewählten Region bedarf allerdings hohem Aufwand.US 5 336 217 A shows a method for irradiating body parts by means of a laser using a glass fiber, the end of which is arranged to be movable. Thus, a defined region of the body can be treated with laser light by moving the end of the glass fiber. However, changing the selected region requires a lot of effort.
Infrarotbereich reduziert werden.CH 674 152 A5 describes an irradiation device for medically indicated irradiation in which, with the aid of a liquid crystal plate arranged between the light source and the region to be irradiated, the harmful fractions of the light radiation in the ultraviolet or infrared range are reduced by applying appropriate voltage.
A therapeutic laser device for treating patients, which enables simultaneous treatment of several affected areas of a patient in order to improve the efficiency of the treatment, is known for example from DE 37 20 742 AI. Several laser probes are movably arranged and can be separately supplied with electricity.
Another device for the therapeutic irradiation of Ganische tissue with laser radiation is known from DE 36 03 156 AI, in order to achieve a uniformly distributed power density, a scattering of the intensity is carried out with the aid of a liquid crystal plate.
Schließlich wird in der US 4 950 903 A eine Gasentladungslampe, insbesondere im Ultraviolettbereich beschrieben. Finally, US Pat. No. 4,950,903 A describes a gas discharge lamp, in particular in the ultraviolet range.
The object of the present invention is to provide a method and a device of the type specified, in which irradiation of a selected region and thus local treatment is possible in a simple manner. As a result, regions that are not to be irradiated should have little or no exposure to radiation from the light sources and thus, for example, only the sick regions of the skin should be irradiated. In the other regions, the reduced radiation exposure, for example, is intended to reduce the risk of skin cancer or to accelerate skin aging due to the ultraviolet treatment.
In terms of the method, the object is achieved in that selected regions are determined and stored during the processing of the image data and the light sources are controlled as a function of the selected regions, so that the radiation emitted by the light sources is locally limited to these selected regions.
This creates a method by which local exposure of the radiation to the selected region can take place in a simple manner. The local limitation of the radiation emitted by the light sources is controlled by the image processing, which is usually carried out with the aid of a computer. One or a variety of light sources can be used to emit the radiation to the selected regions.
According to a feature of the invention, it is provided that the radiation emanating from the light sources is partially deflected depending on the selected regions. This partial deflection means that the radiation can only be selected regions.
Alternatively or in addition to the partial deflection mentioned above, the radiation emanating from the light sources can also be partially covered depending on the selected regions. This partial covering can take place by means of mechanical screens or liquid crystals or mirrors, as will be explained in more detail below.
Likewise, the method according to the invention can be carried out by individually controlling the intensity of a multiplicity of light sources as a function of the selected regions.
Furthermore, it is possible to guide the light sources over the region to be irradiated and to control the intensity of the light sources during the guide over the region to be irradiated depending on the selected regions. In this case, the at least one light source itself would experience a change in position.
The radiation emanating from the light sources can be focused. Such focusing can take place both before the control of the light sources and after the control before hitting the selected regions.
Furthermore, the radiation emanating from the light sources can be automatically directed to the region to be irradiated. The beam path of the light source can be directed either by changing the position of the light source itself or by guiding the beam using mirrors or lenses.
According to a further feature of the invention, new image data of the region to be irradiated can be recorded during the irradiation and selected regions can be newly determined and stored from this image data and the light sources can be locally controlled depending on the new selected regions. In this way, in the case of treatment of a diseased area of the skin, an improvement or change in the skin's appearance can be dealt with as treatment progresses, and the local limitation can be readjusted or corrected. -In addition, by recording and Storage of the following image data of the course of treatment or therapy are documented and darüberh inaus done 'in the presence of a plurality of image data, a selection of which image is to be used to be irradiated region for controlling the light sources. The image processing thus enables an automatic readjustment of the local limitation of the radiation emitted by the light sources, even during the irradiation.
The image processing or the determination of the selected regions from the image data can take place, for example, taking into account the color intensity of the image data. The consideration of the color intensity is particularly advantageous in the case of skin diseases, since these often have special color differences. It is advantageous if the tolerance value for color recognition can be set in the course of image processing.
The second object according to the invention is achieved by a device of the type specified above, in which a device is provided for controlling the radiation emitted by the light source as a function of selected regions determined from the stored image data by means of the computer. The device for controlling the radiation emanating from the light source ensures that only the selected regions are contaminated with radiation.
The device for controlling the radiation emitted by the light source can be arranged between the light source and the region to be irradiated.
The device arranged between the light source and the region to be irradiated can either be formed by a device for location-dependent shading, which can be formed, for example, " by a matrix-like arrangement of mirrors, the mirrors being arranged to be movable, for example pivotable, and the movement of the mirrors in Depending on the selected regions, the control of the matrix-shaped mirrors can ensure that the light radiation is reflected from the mirrors at selected locations and is guided to the desired region, while no reflection of the light beams takes place at undesired regions. finds and therefore no pollution of the corresponding region with radiation.
According to a further feature of the invention, at least one focusing device can be provided for focusing the radiation emanating from the light source. Such focusing devices can be arranged, for example, between the light source and the device for controlling the light source or between the device for controlling the light source and the region to be irradiated.
As an alternative to designing the device for controlling the light sources by a device for location-dependent shading, this can also be formed by mechanical shutters which can be controlled as a function of the selected regions. However, a large number of diaphragms and necessary stepper motors or the like are required for high shading accuracy. necessary, which is why the effort for the mechanical solution is relatively high.
Likewise, the device for location-dependent shading of the light sources can be formed by a liquid crystal display which can be controlled depending on the selected regions.
The light source can be formed by a laser or by a gas discharge lamp, in particular in the ultraviolet range. The gas discharge lamps can be used for both UVA and UVB light treatment.
A device for changing the wavelength of the light emanating from the light source is advantageously provided in order to be able to optimally adapt the device to the respective applications.
If the light source itself is arranged to be movable, it can easily be oriented towards the regions to be treated. As an alternative or in addition to this, at least one device for deflecting the radiation emanating from the light source, for example in the form of mirrors or lenses, can also be provided, by means of which a change in the beam path can be achieved. that can.
According to a further feature of the invention, there is at least one database for storing the image data together with, for example, personal data of patients, the type of disease and the type of radiation form or the like. intended. The data and images of the size and position of regions to be treated can thus be stored in the database in order to ensure that after possible changes, for example the skin appearance upon recovery, the treatment can be continued on the basis of the areas of skin recorded and stored in the database.

The device according to the invention for controlling light sources for irradiating the human body according to FIG. 1 has a computer 1 which processes and stores the image data of the region to be irradiated, recorded with a camera 4. The camera 4 is preferably a digital camera, which provides an image that is immediately readable for the computer 1. The image data recorded with the camera 4 contain the region 3 to be irradiated of the patient 2, for example a diseased skin area. The image data are processed further in the computer 1 and a selected region 3, which should correspond as closely as possible to the region of the patient 2 to be irradiated, is determined and stored, for example taking into account the color intensity of the image data. In front of the light source 6, which can also be connected to the computer 1, a device 5 for controlling the radiation emanating from the light source 6 can be arranged, which is also connected to the computer 1. The radiation of the light sources 6 can thus be controlled as a function of the regions 3 selected from the image data. The light source 6 for irradiating the human body can be formed by an ultraviolet lamp or an ultraviolet laser.
Beispielsweise wird das aufgenommene Bild in der Bildverarbeitungseinheit 9 in zwei Teile zerlegt, wobei in einem Teil die gesunden Hautstellen und im anderen Teil die kranken Hautstellen abgebildet werden, welche für die Ansteuerung der Lichtquellen 6 von Bedeutung sind.2 shows a functional diagram of the image processing in the computer 1, the personal data of the patient and the type of disease or type of radiation, such as by means of laser or radiation lamp, being entered via the control element 7 of the computer, for example when registering a patient, and in a database 8 can be saved. After the data have been entered via the control element 7, the image data originating from the camera 4 are processed in an image processing unit 9 and, for example, likewise stored in the database 8. The processed image data are forwarded to the control unit 10, which either generates signals for the device 5 for controlling the radiation emanating from the light sources 6 or corresponding signals for controlling the light sources 6 themselves. Depending on the embodiment of the device 5 for controlling the radiation emanating from the light sources 6, the corresponding signals for the movement of mechanical shutters, the alignment of the crystals in liquid crystal displays or the movement of stepper motors for mirrors or the like are, for example, in the control unit 10. generated. In the image processing unit 9, the image data are evaluated by special algorithms and forwarded to the control unit 10. For example, the captured image is broken down into two parts in the image processing unit 9, the healthy part being in one part Skin areas and in the other part the diseased skin areas are shown, which are important for the control of the light sources 6.
Während der Bestrahlung kann in einem einstellbaren Zeitintervall eine automatische Kalibrierung für die Bewegung der gesamten Bestrahlungsvorrichtung erfolgen, welche die Bewegungen des Patienten 2 ausgleichen soll.3 schematically shows a device according to the present invention, in which a device 5 for controlling the radiation emitted by the light sources 6 is arranged between the light source 6 and the patient 2. The light source 6 can be formed, for example, a UV lamp, which obtains its light from an ionized gas. The light from the light source 6 can be directed to the control device 5 by means of a first focusing device 11, which achieves a location-dependent shading of the radiation emitted by the light source 6 as a function of the data supplied by the computer 1. The partially shaded radiation 12 emerges after the control device 5 and can be directed onto the selected region 3 of the patient 2 by means of a further focusing device 13. During the irradiation, an automatic calibration for the movement of the entire irradiation device, which is intended to compensate for the movements of the patient 2, can take place in an adjustable time interval.
Durch eine zusätzliche Steuerung, beispielsweise der Intensität der Lichtquelle 6 über den Rechner 1, kann ein Ausschalten der Lichtquelle, wenn diese über eine nicht-ausgewählte Region gleitet, durchgeführt werden.FIG. 4 schematically shows a further embodiment of the device, in which a laser is provided as the light source 6, the light beam 14 of which is guided to the selected region 3 of the patient 2 via two movable mirrors 15, 16. The mirror 15 is responsible, for example, for beam deflection in the horizontal direction, while the mirror 16 is responsible for beam deflection in the vertical direction. The two mirrors 15, 16 are moved with the aid of motors 17, 18, which are controlled by the computer 1. The control of the motors 17, 18 can thus be controlled as a function of the selected regions 3 ascertained and stored in the computer 1, so that the radiation emitted by the light source 6 is locally limited to these selected regions 3. By means of an additional control, for example the intensity of the light source 6 via the computer 1, the light source can be switched off when it glides over a non-selected region.
Somit wird erzielt, dass nur die ausgewählte Region 3 von der Strahlung der Lichtquelle 6 erfasst und somit behandelt wird.5 schematically shows a further embodiment of a pre Direction for controlling light sources 6, in which the light source 6 strikes a matrix-shaped arrangement of mirrors 19, the mirrors 19 being arranged movably and the movement of the mirrors 19 depending on the selected regions 3 being controllable by the computer 1. The radiation emanating from the light source 6 is partially reflected by the mirror matrix 19 in accordance with the control by the computer 1 and, after having passed through a further mirror or a focusing device 20, is directed onto the selected region 3 of the patient 2. For example, the mirror matrix 19 can consist of a multiplicity of round, movably arranged mirror plates, which can be reversed by a current, after which they turn around. A light-absorbing material is applied to one side of the mirror and a light-reflecting film is attached to the other side. Each individual mirror of the mirror matrix 19 can be activated via the computer 1. It is thus achieved that only the selected region 3 is detected by the radiation from the light source 6 and is therefore treated.
Finally, FIG. 6 shows the sequence in the control of a mirror matrix 19 according to FIG. 5 depending on the selected regions. The left image I shows an image of a region to be irradiated which is recorded by a camera 4, for example a digital camera and which contains diseased skin areas which are noticeable due to color differences. After the image processing carried out by the computer 1, selected regions 21 are determined from the image data, for example taking into account the color intensity of the image data, and are emphasized by contours. In the following step III, an image mask is generated from the selected regions 21 and, by linking the image mask in accordance with image III to a black image, the image mask in accordance with image IV is revised. This image mask is now used to control the mirrors of the mirror matrix 19, the image mask IV being divided into a number of regions corresponding to the number of mirrors of the mirror matrix 19. In image V, the mirrors of the mirror matrix 19 are shown in white, which are arranged with their reflecting side facing upwards, while the remaining mirrors of the mirror matrix 19 are shown in black, since their non-reflecting side is oriented upward. Thus, only the white regions of the mirror matrix 19 reflect the rays emanating from the light source 6 and fed to the selected region 3 of the patient 2 to be treated.
