Abstract

A surgical clip(10) includes a U-shaped configuration with first and second arms (12,14), and a bridge portion (16) therebetween. The first arm (12) is provided with a tip (20) preferably having a catch (28,29), and the second arm (14) extends into a deformable retainer (22) having a tissue-piercing end (24) and preferably also a hook (30). During application, tissue is clamped, and the clip is forced over the clamped tissue and the retainer of the second arm is bent and may be pierced through the tissue. The retainer is toward and around or adjacent the tip of the first arm preferably until the hook is engaged about the catch to secure the clip to the tissue and prevent the clip and tissue from separating. The clip is provided with structure that facilitates the stacking of a plurality of clips in a clip chamber of a clip applier.

Claims

AMENDED CLAIMS

26. A method according to claim 25, further comprising: e) providing a clip applier device, wherein said compressing the tissue is performed by said clip applier device.
27. A method according to claim 24, further comprising: d) piercing the retainer through the tissue.
28. A method according to claim 24, wherein: the tissue is non-tubular.
29. A method according to claim 24, wherein: said folding the retainer includes plastically deformably bending said retainer substantially 180°.
30. a surgical clip applier, comprising: a) a clip storage chamber; b) at least one clip in said chamber, each of said at least one clip having first and second substantially parallel extending arms, said first and second arms being coupled together at one end, and said second arm including a retaining element extending therefrom substantially in alignment with said second arm and beyond said first arm; c) an end effector assembly including first and second jaws each having a groove sized to at least partially receive one of the arms of a clip, said second jaw including a distal anvil, at least one of said first and second jaws being rotatable relative to the other between open and closed positions; d) actuation means for rotating said jaws between said open and closed positions; and e) advancement means for advancing a clip from said chamber through said jaws when said first and second jaws are in the closed position and for forcing said retaining element against said anvil such that said retaining element is plastically deformed.
31. A surgical clip applier according to claim 30, wherein: said anvil is configured to plastically deform said retaining element such that said retaining element bends toward said first arm.
32. A surgical clip applier according to claim 31, wherein: said anvil is configured to plastically deform said retaining element such that said retaining element bends about said first arm.
33. A surgical clip applier according to claim 30, wherein: said chamber includes a plurality of clips in a stacked configuration.
34. A surgical clip applier according to claim 33, further comprising: means for retracting remaining clips in said stacked configuration of clips after a distalmost one of said clips is deployed.
35. A surgical clip applier according to claim 30, wherein: said first and second arms of said clip remain substantially parallel during advancement through said jaws by said advancement means .
36. A surgical clip applier according to claim 30, wherein: said advancement means includes an arm coupled to one of a pushwire, a coil and a tube that is longitudinally movable relative to said chamber.
37. A surgical clip applier according to claim 30, further comprising: a flexible tubular member, wherein said actuation means includes a handle and a control element extending from said handle to said end effector assembly, and said flexible tubular member extends over said
18 control element and between said handle and said end effector assembly.
38. A surgical clip applier according to claim 30, wherein: said first jaw of said end effector assembly also includes a distal anvil .
39. A surgical clip applier according to claim 38, wherein: said first arm of said clip also includes a retaining element extending therefrom.
40. A surgical clip applier for use with at least one substantially U-shaped clip having first and second substantially parallel extending arms which are coupled together at one end and uncoupled at the opposite ends, comprising: a) an elongate flexible shaft including proximal and distal ends ; b) a clip storage chamber at said distal end of said shaft, said chamber adapted to store a plurality of clips; c) an end effector assembly including first and second jaws each having a groove sized to at least partially receive one of the arms of a clip, said second jaw including a distal anvil, at least one of said first and second jaws being rotatable relative to the other between open and closed positions; d) actuation means for rotating said jaws between said open and closed positions; e) clip control means for advancing a clip from said chamber through said jaws when said first and second jaws are in the closed position and for the uncoupled ends of one of the arms of a clip against said anvil such that the uncoupled ends is plastically deformed; and f) means for retracting remaining clips in said stacked configuration of clips after a distalmost one of said clips is deployed.
41. A surgical clip applier according to claim 40, wherein: said chamber includes a plurality of the clips in a stacked configuration.

42. A surgical clip applier according to claim 40, wherein: said first jaw of said end effector assembly also includes a distal anvil.
43. A surgical clip applier, comprising: a) an outer tubular member having proximal and distal ends and defining a longitudinal axis; b) a clip-advancing element extending through said tubular member, and having proximal and distal ends, said distal end defining a clip-pushing end; c) a jaw mount located at said distal end of said tubular member; d) first and second jaws mounted on said jaw mount, at least one of said jaws being rotatable on said jaw mount relative to the other of said jaws, said first jaw including a first clamping surface and a curved or bent distal anvil directed toward said second jaw, at least one of said first and second jaws having a longitudinally extending clip guide groove, and each of said jaws having said clip guide groove has said clip guide groove recessed relative to its respective clamping surface; e) at least one flexible control element having proximal and distal ends and extending within said tubular member, said distal end of each said at least one control element being coupled to at least one said jaws; and f) a handle assembly coupled to said proximal ends of said tubular member, said clip-advancing element, and said at least one control element and adapted
(i) to move said clip-advancing element relative to said tubular member, and
(ii) to move said at least one control element relative to said tubular member to effect clamping of said jaws.
44. A surgical clip applier according to claim 43, wherein: said first jaw includes a first longitudinally extending clip guide groove recessed relative to said first clamping surface and said distal anvil is curved or bent relative to said first clip guide groove, and
20 said second jaw includes a second longitudinally extending clip guide groove recessed relative to said second clamping surface.
45. A surgical clip applier according to claim 44, further comprising: a clip chamber adapted to store at least one surgical clip, said clip pusher extending into said clip chamber and adapted to advance a clip from said clip chamber into said first and second clip guide grooves between said jaws.
46. A surgical clip applier according to claim 45, further comprising: at least one clip in said clip chamber having first and second arms and a bridge portion therebetween to together define a generally U-shaped construct, the first arm extending into a deformable retainer, wherein when said clip pusher is advanced, said first and second arms of one of said at least one clip are moved through said first and second clip guide grooves, and said deformable retainer of said one of said clips is bent by the anvil toward said second arm.
47. A surgical clip applier according to claim 46, wherein: said at least one clip comprises a plurality of clips.
48. A surgical clip applier according to claim 47, wherein: said plurality of clips each coupled in a manner such that when a proximally located clip is retracted, said proximally located clip pulls a distally located clip backwards.
49. A surgical clip applier according to claim 44, wherein: said first clamping surface includes two surfaces separated by said first clip guide groove, and said second clamping surface includes two surfaces separated by said second clip guide groove.

50. A surgical clip applier according to claim 43, wherein: said first and second clamping surfaces include a plurality of teeth.
51. A surgical clip applier according to claim 50, wherein: said teeth are proximally directed.
52. A surgical clip applier according to claim 43, wherein: said outer tubular member' is flexible and sized to be received through a lumen of an endoscope.
53. A surgical clip applier, comprising: a) a flexible shaft having proximal and distal ends; b) clip storage chamber at said distal end of said shaft adapted to store a plurality of clips; c) an end effector assembly including first and second jaws each having a groove sized to at least partially receive one of the arms of a clip, said second jaw including a distal anvil, at least one of said first and second jaws being rotatable relative to the other between open and closed positions; d) a cli -advancing element adapted to engage a rearmost clip in said chamber; e) at least one flexible control element having proximal and distal ends and extending within said shaft, said distal end of said at least one control element being coupled to at least one said jaws; and f) a handle assembly coupled to said proximal ends of said shaft, said clip-advancing element, and said at least one control element and adapted
(i) to move said clip-advancing element proximally and distally relative to said tubular member, and
(ii) to move said at least one control element relative to said tubular member to effect clamping of said jaws.

54. A surgical clip applier according to claim 53, further comprising: a plurality of clips in said chamber, said clips arranged in a stacked configuration in which each clip is engaged with an adjacent clip.
55. A surgical end effector assembly for use on an endoscopic instrument, comprising: first and second jaws, at least one of said jaws rotatable relative to the other between open and closed positions, each of said jaws having a longitudinal groove, and second jaw including a distal anvil,
56. A surgical end effector assembly according to claim 55, wherein: said first jaw of said end effector assembly also includes a distal anvil .
57. A surgical end effector assembly according to claims 55 or 56, further comprising: a clip having first and second substantially parallel extending arms, said first and second arms being coupled together at one end, and said second arm including a retaining element extending therefrom substantially in alignment with said second arm, wherein said first arm is positioned in said groove of said first jaw, said second arm is positioned in said groove of said second jaw, and said anvil is adapted to bend said retaining element of said second arm toward said first arm.
58. A surgical end effector assembly according to claim 57, wherein: said anvil is adapted to bend said retaining element of said second arm about said first arm.
59. A surgical end effector assembly according to claim 57, wherein:
23 said first arm of said clip also includes a retaining element extending therefrom.
60. An end effector assembly for a surgical clip applier, comprising: a) a jaw mount; b) a first jaw mounted on said jaw mount; and c) a second jaw mounted on said jaw mount, wherein at least one of said jaws is rotatable on said jaw mount relative to the other of said jaws, said first jaw including a first clamping surface and a curved or bent distal anvil directed toward said second jaw, and said second jaw including a second clamping surface, at least one of said jaws having a longitudinally extending clip guide groove, and each of said jaws having said clip guide groove has said clip guide groove recessed relative to its respective clamping surface.
61. An end effector assembly according to claim 60, wherein: said first jaw includes a first longitudinally extending clip guide groove recessed relative to said first clamping surface, and said second jaw includes a second longitudinally extending clip guide groove recessed relative to said second clamping surface.
62. An end effector assembly according to claim 60, wherein: said first clamping surface includes two surfaces separated by said first clip guide groove, and said second clamping surface includes two surfaces separated by said second clip guide groove.
63. An end effector assembly according to claim 60, wherein: said first and second clamping surfaces include a plurality of teeth.
64. An end effector assembly according to claim 63, wherein: said teeth are proximal1^ directed.
