Abstract

A target nerve or nerve plexus registration point is located utilizing a nerve stimulator needle (18) positioned in a bore (14) of a carrier (12). The carrier (12) is configured with an electrically nonconductive smooth curved surface (16) at a distal end of the bore. The needle tip (28) is fixed in an operative cutaneous electrode position and the smooth curved surface (16) of the carrier (12) is forced against and compresses the epidermis, corium and subcutaneous tissue and traverses cutaneous surfaces to locate a target nerve or nerve plexus without physical penetration by the needle tip (28). The needle (18) is then inserted at the located point.

AMENDED CLAIMS

c) initiating current flow through the patient from the tip by noninvasively contacting dermal layers of the patient with the tip, d) varying the point of contact between the tip and the dermal layers, e) ascertaining a point of contact in registration with a target nerve or nerve plexus by eliciting a neurological response to current flow during step d) and f) advancing the needle into the patient at the ascertained point of contact.
19. A method of administering an anesthetic blockade or other medical procedure to a patient in accordance with claim 1 S wherein step d) is practiced in the absence of a coupling gel.
20. Λ method of administering an anesthetic blockade in accordance with claim 12 wherein step £) is practiced in the absence of a coupling gel

A positioning system for cutaneously locating a target nerve or nerve plexus of a subject having cutaneous dermal layers including a corium, the system comprising a carrier, the carrier having a smooth blunt shield, the shield being electrically nonconductive, an electrode including a tip, the tip having an operative position relative to the shield, the tip being selectively coupled to a current supply for eliciting a neurological response to current flow, the operative position being such that when the shield is pressed against the dermal layers, the tip contacts the dermal layers without invasive penetration of the corium.
22. A positioning system for locating a target nerve or nerve plexus of a subject having cutaneous dermal layers including a corium, the system comprising a carrier, the carrier having a shield with a smooth contour, the shield being electrically nonconductive, an electrode, the electrode having a tip, the tip being selectively coupled to a current supply, the tip having an operative position relative to the shield such that when the shield is pressed against the dermal layers , the tip contacts the dermal layers without invasive penetration of the corium and when current is supplied to the tip, current flows througli the subject for eliciting a neurologic response which varies as a function of the lateral position of the tip on the dermal layers relative to the target nerve or nerve plexus.
23. A method of cutaneously locating a target nerve or nerve plexus of a patient having dermal layers including a corium for the administration of an anesthetic blockade or other medical procedure, the method comprising the steps of: a) providing a carrier having an electrically nonconductive smooth blunt shield, b) providing an electrode having a lip, c) positioning the tip relative to the shield such that when the shield is pressed against the dermal layers, the tip contacts the dermal layers without penetration of the corium, 28
d) coupling a current supply to the tip, e) pressing the shield against the dermal layers so that the tip contacts the dermal layers without penetrating the corium, f) laterally moving the shield relative to the dermal layers while continuing to press the shield against the dermal layers without penetrating the corium, and g) ascertaining a position of the tip in registration with the target nerve or nerve plexus by eliciting a neurological response to current flow during step f).
