Description

ORTHESE DE POIGNET AMBIDEXTRE ORTHESIS OF WRIST AMBIDEXTRE
The present invention relates to an ambidextrous orthosis adapted to ensure the immobilization of the wrist right or left of a patient while allowing freedom of movement to the phalanges of the hand and improving the comfort of the patient.
In the field of wrist orthoses, ambidextrous orthoses that are able to be positioned both on the right wrist and the patient's left wrist are well known; this is the case, for example, of the European patent EP 0.809.988 which describes a universal wrist orthosis. This orthosis is composed of a flexible envelope comprising several parallel longitudinal pockets juxtaposed in which are able to be inserted ribs of different lengths, said envelope having at least locally velvet webs with loops capable of cooperating with velvet pile with hooks carried by transverse retaining straps. The longitudinal pockets are made in the central part of the envelope adapted to come on the back of the hand. The splints are each provided with a distal bent portion adapted to marry the back or the inside of the hand. In addition, said splints are rigid, removable and retumable in their respective pockets. Furthermore, the flexible envelope is extended beyond the open portion of the longitudinal pockets by a closure flap provided with hooking means on the longitudinal pockets and a flexible hinge able to bend substantially parallel to the axis distribution of metacarpophalangeal joints. This type of orthosis has the disadvantage of being difficult to implement on the hand. Indeed, prior to the installation of the orthosis on the wrist of a patient, it is necessary to introduce the fishplates of different lengths into the appropriate longitudinal pockets, to close the closure flap in the right plane and finally to position the patient. orthotic then immobilize it by means of straps. Thus, given the difficulty of setting up this orthosis, the latter is generally poorly positioned around the wrist of the patient making it totally ineffective. Moreover, after being placed around the wrist of the patient, the orthosis envelops the entire wrist of the patient as well as the distal end of the forearm and the metacarpes which makes the wearing of this type of orthosis particularly uncomfortable especially during hot weather.
In addition, this type of orthosis does not allow immobilization of the thumb of the patient which is necessary for the treatment of certain pathologies such as, for example, rhizarthrosis.
Also known is the international patent application WO 98/05275 which describes a rigid wrist protection. This protection for immobilizing a wrist is in the form of a generally rectangular base element that can adapt as a sleeve to a human wrist and which comprises a longitudinal support element said splint intended to cover the front part of the wrist. a wrist, said splint comprising a distal extension adapted to extend into the area of the palm of the hand. A pair of staple strips are provided on the distal extension, said straps extending in generally opposite directions to wrap around the metacarpophalangeal joints of the hand thereby allowing ambidextrous use of the reinforcement. In addition, a pair of straps surround the reinforcement to improve the immobilization of the wrist.
This type of orthosis also has the inconvenience of discomfort for the patient when it is placed on his wrist, the latter completely enveloping the wrist of the patient, and impossibility to immobilize the thumb of the patient while leaving a freedom of movement at the knuckles of the hand.
One of the aims of the invention is therefore to overcome the disadvantages of the orthoses of the prior art by proposing an ambidextrous wrist orthosis of simple and inexpensive design to improve the comfort of the patient while ensuring immobilization of the wrist and incidentally thumb without hindering the freedom of movement of the knuckles necessary for a good grip. For this purpose, and in accordance with the invention, there is provided an ambidextrous wrist orthosis consisting firstly of a support adapted to be positioned around the wrist of a patient and secondly, of at least one said palmar joint cooperating with said support so as to immobilize the wrist of said patient, the splint consisting of an elongate and rigid piece which comprises a flat proximal portion and a bent distal portion adapted to fit the thenar eminence of the patient's hand, remarkable in that it consists of a central frame adapted to receive the splint extending from one end to the other of the central frame, a first distal positioning strip extending at the end distal from the central frame, a proximal positioning band extending at the proximal end of the central frame and a medial band extending between the proximal and distal bands such that the trapezius of the hand of the patient extends between the median and distal bands, said proximal, distal and medial positioning bands on the one hand comprising at their free ends fastening means adapted to cooperate with complementary fastening means integral with the outer face of the strips and on the other hand, having a length sufficient to wrap the metacarpal of the patient's hand respectively to the lower cubital radio joints, the forearm of the patient and the distal end of the patient's ulna.
It is well understood that, unlike the orthoses of the prior art where the support of the splint completely surrounds the distal end of the forearm, the wrist as well as the hand of the patient to the knuckles to immobilize the wrist, the orthosis according to the invention ensures the immobilization of the wrist by wrapping it only partially to improve the comfort of wearing the orthosis.
To ensure better immobilization of the wrist, the orthosis comprises a second so-called dorsal splint consisting of an elongated and rigid piece adapted to be secured to the positioning strips so that it extends from the proximal band to at the distal band on the dorsal side of the forearm and the patient's hand. Moreover, the orthosis comprises a so-called dorsal sheath capable of accommodating the dorsal splint and provided on one of its faces with fixing means able to cooperate with the complementary fastening means integral with the outer face of the positioning strips of so as to allow the attachment of the dorsal splint on the positioning strips.
In addition, the central frame of the orthosis comprises a so-called palm sleeve adapted to receive the palmar splint.
Other advantages and features will become more apparent from the following description of several alternative embodiments given as non-limiting examples of the ambidextrous wrist orthosis according to the invention, with reference to the appended drawings in which:

With reference to FIG. 1, the wrist orthosis according to the invention is composed on the one hand of a support 1 able to be positioned around the wrist of a patient and, on the other hand, of at least one splint The support 1 consists of a central armature 3 comprising a longitudinal sheath said palm sheath 4 adapted to receive the palmar splint 2 which extends from one end to the other of the central armature 3, and three said positioning strips extending transversely on either side of the central frame 3 and comprising at their free ends fastening means adapted to cooperate with complementary fastening means integral with the outer face of the strips. Said positioning strips consist of a first proximal positioning band 5 extending at the proximal end of the central frame 3 and having a length sufficient to wrap the forearm of the patient, a distal positioning band 6 extending at the distal end of the central frame 3 and having a length sufficient to wrap the metacarpals from the patient's hand to the lower cubital radio joints and a median band 7 extending between the proximal and distal bands 6 and of sufficient length to wrap the wrist at the distal end of the patient's ulna, the trapezius of the patient's hand extending between the median 7 and distal 6 bands.
Each proximal positioning band 5, distal 6 and median 7 is secured in its central part to the central frame 3 by seam lines 8 along the longitudinal edges of the central frame 3. In addition, each positioning strip 5 7 advantageously has an inner face respectively 5a, 6a and 7a, that is to say the face of the strip 5, 6 or 7 in contact with the skin of the patient, padded non-allergenic soft fabric and an outer face respectively 5b , 6b and 7b comprising loops adapted to cooperate with a Velcro ® (registered trademark) fastened to a hook fabric band 9 secured to one of the free ends of each positioning strip 5, 6 and 7, when the latter are placed respectively around the forearm of the patient, metacarpals of the patient's hand to the lower cubital radio joints and wrist at the distal end of the patient's ulna.
It is obvious that the positioning strips 5, 6 and 7 can be secured to the central frame 3 by any suitable means such as by heat sealing or heat sealing for example.
Moreover, it goes without saying that said positioning strips 5 to 7 may respectively consist of a pair of strips without departing from the scope of the invention.
The central armature 3 is obtained in flexible fabric and has an inner face 3a, that is to say the face of the central frame 3 coming into contact with the patient's skin, padded hypoallergenic tissue and an outer face 3b having loops adapted to cooperate hooks of a hook fabric of the Velcro ® type (trademark). The palmar sheath 4 of the central frame 3 adapted to receive the palmar splint 2 extends from the proximal end of the central armature 3 where it is open to the distal end of said armature 3 where it is closed. Said central armature further comprises, at its proximal end, a flap 10 for closing the palmar sheath 4 comprising, on its inner face, hooking means 11, such as a strip of hook fabric capable of cooperating with the loops of the outer face 3b of the central frame 3 to allow the closure of the palmar sheath 4 by applying the flap 10 on the outer face 3b of the central frame 3.
With reference to FIGS. 1 to 3, the palmar splint 2 consists of an elongated and rigid piece which comprises a flat proximal portion 2a and a bent distal portion 2b adapted to fit the thenar eminence of the patient's hand, as shown in FIG. 3. Said palmar splint 2 is introduced into the palmar sheath 4 of the central armature 3 in such a way that the concavity of the bent distal portion 2a is oriented in the direction of the internal face 3a of the central armature 3 and that said bent distal portion 2a extends between the distal positioning band 6 and the median positioning band 7. The wrist orthosis according to the invention furthermore comprises, with reference to FIG. 1, a second said rib joint dorsal 12 consisting of an elongated and rigid piece adapted to be secured to the positioning strips 5 to 7 so that it extends from the proximal strip 5 to the distal band 6 on the dorsal side of the forearm and the patient's hand. This dorsal splint 12 is introduced into a so-called dorsal sheath 13 obtained in a flexible fabric. Said dorsal sheath 13 is closed at a first end and open at its opposite end, which then comprises a closure flap 14 provided on its inner face with hooking means 15 such as Velcro ® type hooks adapted to cooperate with means of locking. complementary hooking consisting of loops secured to one of the faces of the back sheath 13 to allow the closure of said sheath 13 by applying the flap 14 on the ringlets. Said dorsal sheath 13 has, moreover, on one of its faces, and more precisely on the face coming to the right of the dorsal surface of the hand and forearm of the patient, strips of hook fabric 16 of the Velcro ® type capable of hooking on the loops of the outer faces 5b, 6b and 7b of the positioning strips 5, 6 and 7. Thus, the back sheath 13 comprises three strips of fabric with hooks 16, a strip of fabric each ends of the dorsal sheath 13 and a strip of tissue in the middle portion of said sheath 13.
In order to ensure perfect immobilization of the dorsal splint 12 fixed by means of the dorsal sheath 13 on the positioning strips 5, 6 and 7, the wrist orthosis according to the invention comprises three so-called proximal compression bands 17, median 18 and distal 19, of width slightly less than the width of the proximal positioning strips 5, median 7 and distal 6 respectively, extending on either side of the central frame 3 by capping the positioning strips respectively proximal 5, median 7 and distal 6. These compression bands 17, 18 and 19 consist of strips of flexible fabric, sewn in their middle part to the central frame 3, the free ends of each of the compression bands 17, 18 and 19 comprising closure means such as a loop 20 and respectively complementary closure means such as a hook fabric strip 21 adapted to cooperate with loops positioned on the face external of each of the compression bands 17, 18 and 19.
Incidentally, to allow immobilization of the thumb of the patient's hand on which the orthosis is placed, the latter comprises with reference to Figures 4 and 5, a band 22 elastically deformable longitudinally, the faces of the band 22 having loops , capable of being surrounded around the thumb of the patient so that said band 22 follows the abductor short muscle of the thumb, the free ends of said band 22 being secured to pieces of hook fabric 23 and 24 positioned on the outer face 3a of the central frame 3 at the level of the median positioning 7 and contention 18 and respectively on the outer face of the median compression band 18 near the central armature 3, on either side of the latter.
It is obvious that the hooked fabric strips capable of cooperating with loops may be substituted by any fastening means capable of cooperating with complementary fastening means without departing from the scope of the invention.
Moreover, the thumb immobilization band 22 may also be substituted by any other equivalent means such as a piece of annular compression fabric from which extend two elastic bands longitudinally adapted to be fixed on the support 1 so that that said bands follow the short abductor muscle of the thumb.
On expliquera maintenant la mise en place de l'orthèse sur le poignet d'un patient en référence aux figures 1 , 4 et 5.We will now explain the implementation of the orthosis on the wrist of a patient with reference to Figures 1, 4 and 5.
The central armature 3 is first positioned under the forearm and on the palmar surface of the patient's hand so that the bent distal portion 2b of the splint 2 housed in the sheath 4 of the central frame 3 marries the thenar eminence of the patient's hand and that the trapezius of the patient's hand extends between the medial 7 and distal 6 positioning bands. The distal positioning strips 6, proximal 5 and medial 7 are then closed around the metacarpal of the patient's hand respectively to the lower cubital radiology joints, the patient's forearm and the wrist at the distal end of the patient. ulna of the patient by overlapping the end of each of the strips 5, 6 and 7 comprising the hook fabric strip 9 on the outer face of the other end of said strip 5, 6 and 7 respectively.
The dorsal sheath 13 in which the dorsal splint 12 is positioned is then fixed on the proximal positioning strips 5, 6 and medial 7 7 by coinciding the strips of hook fabric 16 Velcro ® type integral with one of the faces said sheath 13 with the positioning strips 5, 6 and 7 and exerting pressure. In order to ensure complete immobilization of the dorsal splint 12, and thus complete immobilization of the wrist, the proximal compression band 17, distal 19 and medial band 18 which, in addition to the proximal positioning strips 5, distal 6 and respectively median 7, the dorsal splint 12, are properly tightened before being blocked by applying the free ends of said strips respectively provided with a band of tissue hooks on the loops of the outer faces of the proximal compression bands 17, distal 19 and median 18, said free ends of s band s having previously passed in the ring secured to the other free end of each band.
