Definitions
- the invention relates to a hip socket for use in a hip joint prosthesis, which consists of a shaft on which a ball head can be fastened, the ball head is rotatably inserted into a socket insert, and the socket insert can be inserted and fixed in the acetabulum, wherein the shaft in the femur and the acetabulum are implantable in the pelvic bones.
- Acetabular cups are coupled to the pelvic bone by various techniques during the primary fixation operation.
- a common fixation is the clamping of the pan by means of Pressfit. This represents a form of coupling, which is to be assigned to the oversize connections in the technical sense. Ie. the
- the outer geometry of the acetabulum is greater than that of the doctor by milling the
- Acetabulum created pancake. The normal forces acting on the pan after the introduction of the cup through the bone and consequently
- Frictional forces ensure the primary anchoring of the acetabulum.
- the inhomogeneity of the pelvic bone in terms of stiffness is usually in the same direction, so that the doctor the direction of the highest load for the acetabulum in press fit with reasonable accuracy on the patient can estimate.
- the invention has the object of developing a hip socket according to the Oberbe-handle of claim 1 such that damage to the pan insert and / or a restriction of the function of the pairing ball head / socket insert are avoided by local reduction of the calotte diameter of the socket insert.
- this object is achieved in that the acetabulum is designed asymmetrically with respect to its rigidity and / or geometry in two directions orthogonal to one another and to the axis of symmetry of the acetabulum.
- the asymmetrical external loading in the pressfit situation can be counteracted by this targeted asymmetrical design of the acetabular cup.
- the acetabulum thus has different stiffnesses in two orthogonal directions.
- the higher stiffness axis of the acetabulum may be aligned colinearly with respect to the direction of maximum stiffness of the pelvic bone during implantation.
- the acetabular cup has a minimum wall thickness in a first direction and a maximum wall thickness in a second direction.
- This asymmetrical configuration of the wall thickness of the acetabular cup is achieved, for example, in such a way that a minimum wall thickness and orthogonal thereto a maximum wall thickness is realized in the region of the end surface in one direction.
- the acetabulum also has different stiffness in the two directions mentioned.
- stiffening or weakening elements are introduced into the acetabulum in one direction with respect to the other direction. By introducing stiffening elements - elements of a material with higher rigidity - in the jacket of the acetabulum also different stiffnesses of the acetabular cup can be achieved at different loading directions.
- An embodiment of the invention is characterized in that the stiffening or weakening elements of one direction steadily reduce or increase their rigidity towards the other direction.
- the debilitating elements can also be recesses in the acetabulum.
- the stiffness of the acetabular cup is weakened at the location of the lower stiffness material, with the degree of stiffness decrease being directional.
- the hip socket also has different stiffnesses with respect to different load directions. In extreme cases, no elements are introduced, but only corresponding material is left out in order to reduce the rigidity depending on the direction.
- the geometry of the acetabulum is asymmetrical in such a way that, in the case of asymmetric loading, the result is a possibly circular-symmetrical geometry.
- the cross section of the hip cup is advantageously oval perpendicular to the symmetry axis in the unloaded state.
- the loaded Pfannenqueritess can be achieved that in the presence of an asymmetric load also asymmetric deformation occurs, which leads to the formation of a symmetrical cup geometry.
- an oval ladle cross section is deformed as a result of the asymmetric load in Preßfit until the cross section has a nearly circular geometry.
- the acetabular cup is preferably made of at least one metal.
- a hip joint prosthesis generally consists of a shaft 1 coupled to a ball head 2 and a hip socket 4 coupled to a socket insert 3.
- Shaft 1 and acetabulum 4 are connected to the patient's body by ingrowth into the femoral and pelvic bones 21, respectively and are carriers for the ball head 2 and socket insert 3.
- the ball head 2 is rotatably mounted in the cap 5 of the socket insert 3.
- the acetabular cup 4 is rotationally symmetrical with respect to its axis of symmetry z, whereby the wall thicknesses a 1 , b 2 of the acetabulum 4 are equal in all directions x, y and an asymmetric loading of the acetabular cup 4 in the pressfit situation causes an asymmetrical deformation of the acetabular cup 4 May have consequences.
- FIG. 2 a, b shows an acetabulum 4 according to the invention, which is not constructed symmetrically in the plane of the end surface 7 (see FIG. 2 a).
- the wall thickness a in a direction orthogonal to the axis of symmetry z is maximal and the wall thickness b likewise in a direction y orthogonal to the axis of symmetry z is minimal.
- the directions x, y are orthogonal, i. arranged at right angles to each other.
- the acetabulum 4 Due to the different wall thicknesses a, b, the acetabulum 4 has different rigidity in the two directions x, y mentioned.
- FIG. 3 shows an acetabulum 4 according to the invention in which elements 6 that reinforce or weaken in a direction x with respect to the other orthogonal direction y are introduced into the acetabular cup.
- elements 6 that reinforce or weaken in a direction x with respect to the other orthogonal direction y are introduced into the acetabular cup.
- the stiffening or weakening elements 6 of a direction x comprise an angle range ⁇ on the end face of approximately 90 degrees (see FIG. 3 b). Importantly, the stiffness in a direction x differs from the stiffness in a direction y orthogonal to the direction x. In order to achieve a homogeneous change in rigidity between the two extreme values with respect to the direction x and y, the stiffening or weakening elements can comprise an angle ⁇ of up to 180 °, the wall thickness of the respective element being from one end to the middle of the element decreases continuously to the other end as well as continuously.
- the stiffness of the acetabular cup 4 is weakened at the location of the lower stiffness material, with the degree of decrease in stiffness being directionally dependent.
- the acetabular cup 4 also has different stiffnesses at different loading directions. In extreme cases, no elements are introduced, but only recessed in accordance with material to reduce the stiffness depending on the direction.
- FIG. 4 b shows a hip socket 4 whose geometry is asymmetrical in such a way that the cross section of the acetabular cup 4 is oval in the unloaded state (solid line) perpendicular to the axis of symmetry z.
- the acetabular cup 4 deforms (see dashed line in FIG. 4 b), so that as a result a symmetrical geometry occurs.
- FIG. 4 a shows the hip socket 4 in the loaded state.
- the forces which occur selectively in a simplified model are denoted by F 1 , F 2 . It is to be ensured in this embodiment that results in asymmetrical loading (in the direction of F 1 ) as circular as possible symmetrical geometry (see Figure 4 a).
- This asymmetrical design of the geometry of the acetabular cup 4 can also lead to an asymmetrical deformation, which can lead to the formation of a symmetrical cup geometry.
- an oval ladle cross section is deformed as a result of the asymmetric load in Preßfit until the cross section has a nearly circular geometry.
