In the description a reference to 'uterus' or 'uterus region' is intended to include the uterus, uterine horns and fallopian tubes.
Description and Preferred Embodiments of the Invention:
The present invention is now described with reference to accompanying figures, which are incorporated only to illustrate the invention and are not intended to limit scope of present invention, and are not to the scale.
The reproductive system of female dog [Figure 1 ] comprises vagina 1 , cervical canal 2, uterine horn'3, mucous plug 4 which blocks the cervical canal, cervix-vaginal junction 5, and uterine horn-cervical canal junction' 6.
The right angle (90° angle) turn at joining point 5 of cervical canal 2 and vagina 1 , and second right angle (90° angle) turn at joining point 6 of cervical canal 2 and uterine horn 3, poses difficulty for flow of sterilant particularly if it is semi-liquid or liquid of high viscosity.
Further, the mucus plug 4 causes total blockage in cervical canal 2, and hence, blocks way of sterilant from vagina to uterus.
Further, the muscles of cervical canal 2 are very tighten and continuously cause spasmodic contraction which have been found to cause reversal of sterilant if injected in the vagina, and hence, the sterilant does not flow from vagina to uterus.
The above problems have been overcome by judiciously constructed device of present invention.
Accordingly, the present invention relates to a device for delivery of sterilant in uterus of female dog for transcervical sterilization comprising a cylindrical tube 1 1 having a hole 12 running through its length and provided with one or more vertical holes 13 and at least one projection 14 provided with inflatable and vibratable cuffs 15 [Figure ?].
In accordance with one of the preferred embodiments of the present invention, the device comprises at least two projections 14 each provided with corresponding inflatable and vibratable cuffs 15, wherein at least one vacuum suction ring 16 is provided in-between two projections 14 [Figure 3].
In accordance with one of the preferred embodiments of the present invention, the device further comprises an inflating means 21 , a vibrating means 22, a vacuum means 23, and a sterilant injecting (or supply) means 24 [Figure 4].
The inventor confirms that the presently disclosed devices of above embodiments have overcome above-described difficulties caused by two right angles (90° angle) turns 5 and 6, mucous plug 4 in the cervix 2, as well as tighten muscles of cervical canal 2, and spasmodic contraction of muscles of cervical canal 2 in flow of sterilant, and hence, have demonstrated advantages of causing free flow of sterilant in reproductive system of female dog.
In accordance with present invention, the cuffs 15 are inflatable cuffs which have been found to have advantage of causing seal in vagina, and hence, avoiding reversal of sterilant once injected.
In accordance with one of the embodiments of the present invention, the cuffs are connectable to inflating means for causing inflation thereof. In accordance with present invention, the inflating means may be provided at any location which is convenient for construction of device, preferably the inflating means are provided on one end of the device or may also be provided on or around the cuffs as illustrated in accompanying Figure 4.
In accordance with present invention, the cuffs 15 are inflatable cuffs as well as capable of vibrating which not only have advantage of causing seal in vagina, but also have advantage of causing vibration in the vagina.
In accordance with one of the embodiments of the present invention, the cuffs are inflated by means of inflating means, and their vibration is caused by means of vibrating means. Accordingly, in one embodiment, the cuffs are connectable to inflating means for causing inflation thereof and vibrating means for causing vibration thereof. In accordance with present invention, the vibrating means may be provided at any location which is convenient for construction of device, preferably the vibrating means are provided on one end of the device or may also be provided on or around the cuffs as illustrated in accompanying Figure 4.
In accordance with one of the preferred embodiments of present invention, the vibrating means is electromechanical vibrator which not only causes vibration in cuffs, but also inflates the cuffs. Therefore, in accordance with this preferred embodiment of present invention, the inflating means and vibrating means are one means.
In accordance with one of the preferred embodiments of present invention, the vibrating means, which may also be capable of causing inflation of cuffs, is embedded within the sterilant or contraceptive source, or is provided on one end of the device or may also be provided on or around the cuffs.
It has been surprisingly observed that when cuffs inflate they cause seal in vagina, which surprisingly and unexpectedly has been found to have advantage of avoiding reversal of sterilant, and when cuffs vibrate they cause vibration in vagina, which surprisingly and unexpectedly has been found to have advantage of resulting in loosening of the mucous plug 4 which on continuous vibration of cuffs gets dislodged, and hence, problem of total blockage caused due to mucous plug 4 in cervical canal 2 is overcome^by present device, and thereby the sterilant easily flows from vagina to uterus through cervical canal. , ι ;
Additionally, it has been observed that vibration of cuffs also cause expansion of joining point 5 of cervical canal 2 and vagina 1, and joining point 6 of cervical canal 2 and uterus 3, which results in a comfortable and easy way for sterilant being injected through hole 12 and passing through holes 13 of present device to pass through the vagina 1 , cervical canal 2 and uterine horn 3 to yterus. Further, additionally, it has been observed that vibration of cuffs also results in loosening of muscles of cervical canal 2, and thereby effects of spasmodic contraction are greatly reduced, meaning thereby, reversal of sterilant injected in vagina is totally avoided, and hence, sterilant flows freely from vagina to uterus through cervical canal.
In accordance with one of the preferred embodiments, the vacuum suction ring
16 preferably provided in-between two projections 14 has been found to have advantage of further enhancing or improving the stability of the seal generated by inflatable cuffs 15 provided on projections 14.
The opposite end 18 of present device is closed which has advantage of avoiding flow of sterilant without having caused dislodgement of mucous plug and expansion of junctions 5 and 6, and relaxing of muscles of cervical canal, because any such advance flow of sterilant causes hindrance in functioning of present device.
It has been found that present device is capable of delivering a sterilant selected from a group comprising liquid sterilant, semi-solid liquid sterilant and liquid sterilant of high viscosity upto uterus for transcervical sterilization by overcoming difficulties posed by two right angle turns and mucous plug in; the female dog cervix, as well as by very tighten muscles of cervical canal, and spasmodic contraction of muscles of cervical canal.
It may be noted that present device has been described for application in delivering a liquid sterilant in uterus of a female dog. However, as can be understood from foregoing description, the present device may also be employed to deliver a contraceptive selected from a group comprising a solid contraceptive, wire form contraceptive and a contraceptive capable of forming a plug in uterus.
It may be noted that present device may be employed to deliver any contraceptive in desired region of any animal or human being provided the dimensions of hole 12, holes 13 and cuffs 14 are made in accordance with animal and human being and the contraceptive to be injected. In case a wire form or plug form of contraceptive, the opposite end 18 may be kept open.
Further, in one embodiment, the device further comprises an endoscope so that its insertion and movement can be controlled for efficient delivery of sterilant.
In accordance with one of the preferred embodiments of present invention, the sterilant (contraceptive) is a liquid polymer sterilant which is styrene maleic anhydride (SMA) complexed with dimethyl sulfoxide (DMSO) [RISUG™], which is in Phase III trials as a human male contraceptive and already tested for toxicology and during the animal trials has also been found to be suitable as female contraceptive, or its modified form having diluted styrene maleic anhydride complexed with dimethyl sulfoxide (DMSO), or further modified form having magnetic particles.
It has been observed that present device works more efficiently with pulsed alternating pressure, wherein pressure and vacuum suction are applied alternatively.
Accordingly, in one embodiment, once present device has been inserted in vagina and it reaches junction 5 in such a manner that at least one projection 14 is before junction 5 so that seal caused due to inflation of cuff 15 can block reversal of sterilant and if device comprises at least two projections 14 then second projection is preferably after junction 5 so that zone between two projections 14 accommodates opening of cervical canal. Once device is accommodated in this position, the cuffs 15 are inflated by applying pressure through inflating means so as to cause seal in vagina, and thereafter, vibrating means is switched on so as to cause vibration of inflated cuffs, which results in relaxing of muscles of cervical canal, and expansion of junctions 5 and 6, and loosening of mucous plug, which slowly gets dislodged. Thereafter, the vacuum suction and pressure are applied alternately which causes further relaxation of muscles of cervical canal, and complete dislodgement of mucous plug, and hence " results in a comfortable and easy way for the sterilant. At this moment the sterilant is injected through hole 12 which comfortably passes , through holes 13 and then passes through cervical canal, uterine horn and reaches uterus. Deposition of radiopaque sterilant in uterus region as confirmed by X-ray imaging [Figure 5, wherein relevant area has been encircled] confirms that sterilant has reached the uterus region which also confirmsjthat mucous plug was dislodged. Light Ketamine tranquilization was required for the procedure, but recovery time was minimal and complications were nil due to low dose and nonsurgical nature of entire procedure.
Accordingly, when present device is operated with pulsed alternating pressure it comfortably, easily and successfully forced the sterilant through the cervical canal. This embodiment permits delivery of even thicker fluids than with continuous pressure. Alternating pulsed pressure with pulsed suction from the vacuum seal managed to dislodge the mucous plug more conveniently, which is a vital step in transcervical sterilization and has been achieved by present invention.
In the. above description, a reference to 'uterus' or 'uterus region' is intended to include the uterus, uterine horns and fallop an tubes. The sterilant passes via uterus to the fallopian tubes. The long term contraceptive action is achieved when sterilant reaches and lodges in the fallopian tubes. The sterilant delivered through present device has been found to reach and lodge in the fallopian tubes as it is observed from the X-ray scan [Figure 5].
Accordingly, it may be noted that with cervical barrier surmounted in female dogs and a permanently-retained spermicidal and antimicrobial polymer available, transcervical delivery of sterilant is now possible.
