Abstract

凝固や止血を含む治療、手術等の処置を良好に行うことのできる医療用処置具を提供する。医療用処置具1の処置部30は、第1、第2鉗子部材31,32を含む。これら鉗子部材31,32は、互いの間に電場を形成可能な一対の電極を構成する。第1鉗子部材31は、延び方向と直交する対向方向に対峙する一対の第1鉗子部31a,31aを含む。第2鉗子部材32は、上記対向方向に沿う回転軸線Lのまわりに、第1鉗子部31a,31aどうし間の間隙39に出入り可能な角度範囲で、第1鉗子部材31に対して相対回転可能になっている。

Description

医療用処置具及びその処置部
 本発明は、生体組織に治療や手術等の処置を施す医療用処置具及びその処置部に関し、特に、処置部が一対の鉗子部材を含んで開閉可能になっており、かつ、これら鉗子部材がバイポーラ型電極となってマイクロ波等の高周波電場を生成する医療用処置具及びその処置部に関する。
 例えば、特許文献1、2等に記載の医療用処置具によれば、挿入部の先端部に処置部が設けられている。処置部は、固定の鉗子部材と、回転可能な鉗子部材を含み、開閉可能になっている。一対の鉗子部材は、金属にて構成されることによってバイポーラ型電極となり、これら鉗子部材どうしの間に高周波電場が形成される。この高周波電場によって生体組織を焼いて止血、凝固等を施すことができる。また、一対の鉗子部材をハサミのように使うことで生体組織を切断(切開、切除等を含む)することができる。

 一対の鉗子部材を開いて生体組織を挟んだ状態で、これら鉗子部材どうし間に高周波電場を形成すると、生体組織における、鉗子部材どうし間に挟まれた部位が最も焼かれて凝固するものと考えられる。引き続いて、一対の鉗子部材を閉じると、上記最も焼かれて凝固した部位が切断される。そのため、凝固が不十分であると、周辺のあまり凝固していない部位が切断面に現れて、そこから出血するおそれがある。
 本発明は、上記に鑑み、生体組織に対して凝固や止血を含む治療、手術等の処置を良好に行うことのできる医療用処置具を提供することを目的とする。
 上記課題を解決するために、本発明に係る医療用処置具は、挿入部の先端に設けられるとともに、第1、第2鉗子部材を含んで開閉可能な処置部を備え、かつ前記第1、第2鉗子部材が、互いの間に電場を形成可能な一対の電極を構成する医療用処置具において、
 前記第1鉗子部材が、延び方向と直交する対向方向に対峙する一対の第1鉗子部を含み、
 前記第2鉗子部材が、前記対向方向に沿う回転軸線のまわりに、前記一対の第1鉗子部どうし間の間隙に出入り可能な角度範囲で、前記第1鉗子部材に対して相対回転可能に設けられていることを特徴とする。
 また、本発明に係る医療用処置具の処置部は、医療用処置具の挿入部の先端に設けられるとともに、第1、第2鉗子部材を含んで開閉可能になっており、かつ前記第1、第2鉗子部材が、互いの間に電場を形成可能な一対の電極を構成する処置部において、
 前記第1鉗子部材が、延び方向と直交する対向方向に対峙する一対の第1鉗子部を含み、
 前記第2鉗子部材が、前記対向方向に沿う回転軸線のまわりに、前記一対の第1鉗子部どうし間の間隙に出入り可能な角度範囲で、前記第1鉗子部材に対して相対回転可能に設けられていることを特徴とする。
 前記第2鉗子部材を前記第1鉗子部材に対して相対回転可能させることで、前記処置部を開閉できる。すなわち、前記第2鉗子部材を前記一対の第1鉗子部どうし間の間隙から出た状態にすることで、前記処置部が開いた状態になる。前記第2鉗子部材を前記一対の第1鉗子部どうし間の間隙に入り込んだ状態にすることで、前記処置部が閉じた状態になる。
 この医療用処置具によれば、処置部を開いて、第1、第2鉗子部材の間に生体組織を挟んだ状態で、これら鉗子部材どうし間に高周波電場を形成すると、生体組織における近接する二箇所の部位が最も焼かれて凝固するようにできる。すなわち、一方の第1鉗子部と第2鉗子部材との間、及び他方の第1鉗子部と第2鉗子部材との間の二箇所において、最も焼かれて凝固するようにできる。引き続いて、処置部を閉じていくと、第2鉗子部材が一対の第1鉗子部どうしの間に入り込むことで、上記最も焼かれて凝固した二箇所の間の部位が切断(切除、切開等を含む)される。したがって、切断箇所の両側の切断面には、それぞれ上記最も焼かれて凝固した部位が面する。よって、切断面から出血するのを防止でき、又は出血量を少なく抑えることができる。この結果、凝固や止血を含む治療、手術等の処置を良好に行うことができる。
 前記第2鉗子部材の基端部が、前記一対の第1鉗子部の基端部どうし間の間隙部分(前記間隙の基端部)の内部に配置されて、前記第1鉗子部材に対して前記回転軸線のまわりに相対回転可能に連結されており、前記第2鉗子部材の前記基端部より先端側部分が、前記一対の第1鉗子部の前記基端部より先端側部分どうし間の間隙部分(前記間隙の前記基端部より先端側部分)に出入り可能であることが好ましい。
 これによって、処置部をコンパクトにできる。
 前記第1、第2鉗子部材が、前記挿入部に収容された同軸ケーブルを介して、マイクロ波を出力するマイクロ波電源と接続されていることが好ましい。これによって、第1、第2鉗子部材どうし間にマイクロ波電場を形成することによって、処置対象部位にマイクロ波エネルギーを供給して、処置できる。
 本発明によれば、凝固や止血を含む治療、手術等の処置を良好に行うことができる。

 以下、本発明の実施形態を図面にしたがって説明する。
 図1~図8は、本発明の第1実施形態を示したものである。図1に示すように、この実施形態の医療用処置具1は、内視鏡(図示せず)のチャンネルに挿入して使用される鉗子であり、手元操作部10と、挿入部20と、処置部30とを備えている。手元操作部10は、操作本体部11と、スライドツマミ12とを含む。スライドツマミ12が操作本体部11にスライド可能に設けられている。
 図5に示すように、挿入部20は、ルーメンチューブ21(挿入本体部)と、同軸ケーブル22(マイクロ波エネルギー供給線)と、操作ワイヤ23とを含み、長く延びている。図1に示すように、挿入部20の基端に手元操作部10が設けられている。挿入部20の先端に処置部30が設けられている。この挿入部20が、内視鏡のチャンネルに挿入される。
 図2(a)及び同図(b)に示すように、ルーメンチューブ21は、ポリテトラフルオロエチレン(PTFE)等の絶縁樹脂にて構成されている。ルーメンチューブ21の内部には、相対的に大きな断面の大径ルーメン21aと、相対的に小さな断面の小径ルーメン21bとが軸線に沿って互いに並行するように形成されている。図3に示すように、ルーメンチューブ21の先端部分における大小のルーメン21a,21bどうしの間の部分には、ルーメン連通部21cが形成されている。ルーメン連通部21cを介して2つのルーメン21a,21bどうしが連通されている。
 図2に示すように、大径ルーメン21aには同軸ケーブル22が収容されている。ひいては、挿入部20に同軸ケーブル22が収容されている。図5に示すように、同軸ケーブル22は、線状の内部導体22aと、管状の編組からなる外部導体22bと、これら内外の導体22a,22b間の管状の誘電体層22cと、外部導体22bを覆う管状の保護被覆22dとを含み、挿入部20のほぼ全長にわたって延びている。図1において簡略的に図示するが、同軸ケーブル22の基端部がマイクロ波電源2に接続されている。これによって、内部導体22aの基端部がマイクロ波電源2の高圧側端子に電気的に接続されている。また、外部導体22bの基端部がマイクロ波電源2の接地側端子に電気的に接続されている。マイクロ波電源2は、マイクロ波を出力する。ここで、マイクロ波とは、数百MHz~数百GHzの電磁波を言い、好ましくは1GHz~10GHzの電磁波を言う。
 図5に示すように、同軸ケーブル22の先端側(処置部30の側)の部分は、より中心側の部材ほど、より先端方向へ突出されている。後述するように、内部導体22aの先端部は、処置部30の後記第1鉗子部材31に電気的に接続され、外部導体22bの先端部は、処置部30の後記第2鉗子部材32に電気的に接続されている。
 図2に示すように、ルーメンチューブ21の小径ルーメン21bには、操作ワイヤ23が収容されている。図1に示すように、操作ワイヤ23は、挿入部20のほぼ全長にわたって延びている。操作ワイヤ23の基端部は、スライドツマミ12に連繋されている。操作ワイヤ23の先端部は、後述する第2鉗子部材32に連繋されている。
 図4に示すように、処置部30は、一対の鉗子部材31,32を含み、開閉可能になっている。鉗子部材31,32は、ステンレス等の金属にて構成され、導電性を有している。
 図5に示すように、第1鉗子部材31は、基筒部31bと、一対の第1鉗子部31a,31aとを一体に有している。基筒部31bは、挿入部20と同軸の円筒形になっている。図3に示すように、この基筒部31bが、ルーメンチューブ21の先端部の外周に嵌め込まれている。これによって、第1鉗子部材31が、挿入部20の先端部に対して固定されている。
 第1鉗子部31aは、基筒部31bから先端方向へ延びる板状になっている。一対の第1鉗子部31a,31aが、互いに平行をなし、かつ延び方向と直交する対向方向に対向している。第1鉗子部31a,31aどうしの間にスリット状の間隙39が形成されている。図4及び図9に示すように、第1鉗子部31aの外面は、基筒部31bの外径より少し小径の部分円筒面になっている。第1鉗子部31aの内面(他方の第1鉗子部31aとの対向面)は、平坦であるが、凹曲面又は凸曲面であってもよい。第1鉗子部31aの基端部31d(基筒部31b側の部分)には軸穴31hが形成されている。図4に示すように、第1鉗子部31aの基端部31dよりも先端側の部分31fの幅方向(上記延び方向及び対向方向の何れとも直交する方向)の一縁には、凹設部31gが形成されている。凹設部31gの内面は、平坦な受け面31iになっている。この凹設部31gによって、第1鉗子部31aの先端側部分31fが、基端部31dよりも幅細になっている。さらに、先端側部分31fの先端部には突起部31eが設けられている。突起部31eは、先端側部分31fから幅方向の一側へ突出されている。したがって、第1鉗子部31aの先端部は、L字状になっている。
 図5に示すように、第1鉗子部31a,31aどうし間の間隙39は、基端側の間隙部分39bと、それよりも先端側の間隙部分39fとを含む。間隙部分39bは、一対の第1鉗子部31a,31aの基端部31d,31dどうしの間に形成されている。間隙部分39fは、一対の第1鉗子部31a,31aの先端側部分31f,31fどうしの間に形成されている。
 図6に示すように、第2鉗子部材32は、細長い板状になっている。第2鉗子部材32の基端部32dには、軸穴32hが形成されている。第2鉗子部材32の基端部32dよりも先端側の部分32fの一縁には、平坦な受け面32iが形成されている。第2鉗子部材32の先端部には、突起部32eが受け面32iから突出するように形成されている。これによって、第2鉗子部材32の先端部が、L字状になっている。図4に示すように、突起部32eの突出方向は、第1鉗子部材31の突起部31eとは逆側へ向けられている。
 図7(b)に示すように、第2鉗子部材32の基端部32dが、一対の第1鉗子部31a,31aどうし間の間隙39の基端の間隙部分39bの内部に配置されるとともに、後述する連結部5を介して、第1鉗子部材31に対して回転軸線Lのまわりに回転可能に連結されている。第2鉗子部材32の先端側部分32fは、先端側の間隙部分39fに出入り可能になっている。つまり、第2鉗子部材32は、間隙部分39fひいては間隙39に出入り可能な角度範囲で、第1鉗子部材31に対して相対回転可能になっている。
 これによって、処置部30が、開位置(開状態)と閉位置(閉状態)との間で開閉動作可能になっている。図7(b)の実線にて示すように、開位置では、第2鉗子部材32の先端側部分32fが第1鉗子部材31から斜め外側へ突出されるとともに、回転軸線Lに沿う方向から見て、受け面31i,32iどうし及び突起部31e,32eどうしが離れて対向する。図7(b)のニ点鎖線にて示すように、閉位置では、第2鉗子部材32の全体が、第1鉗子部材31の一対の第1鉗子部31a,31aどうしの間に挟まれるようにして、間隙39内に収容される。
 回転軸線Lは、鉗子部材31,32の延び方向及び幅方向と直交するとともに、第1鉗子部31a,31aどうしの対向方向に沿い、かつ鉗子部材31,32の基端部どうしの対向方向に沿っている。鉗子部材31,32の基端部どうしが、回転軸線Lに沿って重ねられている。
 図6に示すように、第2鉗子部材32の基端部32dには、連繋突起32bが一体に形成されている。連繋突起32bは、第2鉗子部材32の延び方向に対して斜めかつ後方へ突出されている。また、連繋突起32bと同一形状の金属製の連繋押さえ35が、第2鉗子部材32の基端部32dに溶接等にて接合されている。図4に示すように、連繋突起32bと連繋押さえ35とが、回転軸線Lと平行な方向に対峙している。
 なお、第2鉗子部材32が一対の連繋突起32b,32bを一体に有していてもよい。或いは、第2鉗子部材32に一対の連繋押さえ35,35を溶接等にて接合してもよい。
 図4に示すように、操作ワイヤ23の先端部には、金属製の連繋ピース33が溶接等にて接合されている。この連繋ピース33が、連繋突起32b及び連繋押さえ35どうしの間に挿し入れられるとともに、金属製の連繋ピン34を介して連繋突起32b及び連繋押さえ35と回転可能に連結されている。連繋ピン34の軸線ひいては連繋ピース33の回転軸線は、回転軸線Lと平行である。
 これによって、手元操作部10のスライドツマミ12を前後にスライド操作すると、操作ワイヤ23が挿入部20の延び方向に沿って押し引きされ、第2鉗子部材32が回転軸線Lのまわりに回転される。これによって、処置部30を開閉できる。第1鉗子部材31は固定であり、第2鉗子部材32が可動であるから、処置部30は片開き式の鉗子である。
 処置部30の第1、第2鉗子部材31,32は、同軸ケーブル22を介してマイクロ波電源2と接続されることによって、処置部30の一対の電極を構成している。詳しくは、第1鉗子部材31は、同軸ケーブル22の内部導体22aを介してマイクロ波電源2の高圧側端子と接続されている。また、第2鉗子部材32は、同軸ケーブル22の外部導体22bを介してマイクロ波電源2の接地側端子と接続されている。したがって、第1鉗子部材31が高圧極であり、第2鉗子部材32が接地極である。マイクロ波電源2の出力が同軸ケーブル22にて伝送されることによって、第1鉗子部材31及び第2鉗子部材32の間にマイクロ波電場(高周波電場)が形成される。
 第1鉗子部材31は、内部導体22aと次のようにして接続されている。
 図4に示すように、内部導体22aの先端の剥き出し部分の外周には、金属製の内側接続パイプ41が嵌め込まれている。内側接続パイプ41の外周面と誘電体層22cの外周面とが面一に連なっている。図5に示すように、内側接続パイプ41の周側部には、スリット41aが形成されている。このスリット41aを通して内側接続パイプ41と内部導体22aとがハンダ付けにて一体化されている。
 また、図3に示すように、基筒部31bの内部には、金属製の外側接続パイプ42が嵌め込まれている。基筒部31bと外側接続パイプ42とは、溶接等によって一体化されている。外側接続パイプ42は、第1鉗子部材31よりも基端側(図3において左側)へ突出されている。図4に示すように、外側接続パイプ42は、大径ルーメン21aに挿入されるとともに、内側接続パイプ41の外周に嵌め込まれ、さらには誘電体層22cの先端の剥き出し部分の外周に嵌め込まれている。内側接続パイプ41と外側接続パイプ42とは、レーザ溶接等によって一体化されている。これによって、内部導体22aが、内側接続パイプ41及び外側接続パイプ42を順次介して、第1鉗子部材31と電気的に接続されている。外側接続パイプ42の外周には熱収縮チューブからなる絶縁被覆層44が設けられている。
 なお、図5に示すように、外側接続パイプ42の先端面は、斜めにカットされることで、連繋突起32b及び連繋押さえ35との干渉を避けている。また、外側接続パイプ42の基端部(図5において左端部)と外部導体22bの先端部(図5において右端部)との間の誘電体層22cの外周には、絶縁チューブ43が設けられている。
 第2鉗子部材32は、外部導体22bと次のようにして接続されている。
 図5に示すように、同軸ケーブル22の先端部には、接続リング24が設けられている。接続リング24は、大径リング部24aと、小径リング部24bと、リング接続部24cとを有している。大小のリング部24a,24bの軸線は互いに平行になっている。これらリング部24a,24bがリング接続部24cを介して一体に連なっている。図4に示すように、大径リング部24aは、外部導体22bにおける先端の剥き出し部分の外周に嵌め込まれて固定されるとともに、大径ルーメン21a内に収容されている。リング接続部24cが、ルーメン連通部21cに通されている。小径リング24bは、小径ルーメン21b内に収容されている。この小径リング部24bの内部に、操作ワイヤ23が摺動可能に通されている。これによって、外部導体22bと操作ワイヤ23とが、接続リング24を介して電気的に接続されている。更には、外部導体22bが、接続リング24、操作ワイヤ23、連繋ピース33を順次介して、第2鉗子部材32と電気的に接続されている。
 なお、図4に示すように、操作ワイヤ23における接続リング24よりも先端側の部分には、熱収縮性チューブからなる絶縁被覆層25が被覆されている。この絶縁被覆層25と外側接続パイプ42の絶縁被覆層44とによって、操作ワイヤ23と外側接続パイプ42とがルーメン連通部21cを介して短絡するのが防止されている。
 次に、連結部5の詳細構造を説明する。
 図6及び図7(a)に示すように、連結部5は、ホルダ50と、軸ピン53を備えている。ホルダ50は、一対のホルダ部材51,51にて構成されている。これらホルダ部材51,51は、ポリエーテルエーテルケトン(PEEK)等の絶縁体にて構成され、互いに同一形状になっている。図8(a)及び同図(b)に示すように、各ホルダ部材51は、挟持板部51aと、半割筒部51bとを一体に含む。挟持板部51aは、回転軸線Lと直交する円形の板形状になっている。一対のホルダ部材51,51の挟持板部51a,51aどうしが、回転軸線Lに沿う対向方向(図8(a)において紙面と直交する方向)に対向している。挟持板部51aの中心部には穴部51hが形成されている。穴部51hは、大径半円部51dと、小径半円部51eを一体に有している。大径半円部51dの内周には、突起51fが形成されている。
 図6に示すように、半割筒部51bは、円筒を半割にした形状になっており、挟持板部51aの内側面(他方の挟持板部51aを向く面)から回転軸線L(上記対向方向)に沿って突出されている。図8(b)に示すように、半割筒部51bの内周面は、小径半円部51eの内周面と面一に連続している。半割筒部51bの半割端面51c(他方の半割筒部51bとの対向面)は、平坦になっている。半割筒部51bの先端面には、凹部51gが形成されている。
 図4に示すように、挟持板部51aは、鉗子部材31,32の基端部どうしの間に介在されている。すなわち、一方のホルダ部材51の挟持板部51aが、一方の第1鉗子部31aの基端部31dと第2鉗子部材32の基端部32dどうしの間に介在されている。他方のホルダ部材51の挟持板部51aが、他方の第1鉗子部31aの基端部31dと第2鉗子部材32の基端部32dどうしの間に介在されている。したがって、一対の挟持板部51a,51aが、第2鉗子部材32の基端部32dを両側から挟んでいる。各挟持板部51aが、鉗子部材31,32どうしを絶縁する絶縁板を構成している。
 図6に示すように、各ホルダ部材51の半割筒部51bが、第2鉗子部材32を挟んで両側から軸穴32hに挿し入れられている。図8(c)及び図9に示すように、2つの半割筒部51b,51bどうしが、軸穴32hの内部において組み合わさり、絶縁軸筒52を構成している。
 言い換えると、ホルダ50は、一対の挟持板部51a,51a(絶縁板)と、絶縁軸筒52とを備えている。絶縁軸筒52は、挟持板部51a,51aと直交する筒形状になっており、これら挟持板部51a,51aどうしの間に架け渡されている。この絶縁軸筒52が、軸穴32hに挿通されることによって、第2鉗子部材32の基端部32dを貫通している。絶縁軸筒52ひいてはホルダ50は、鉗子部材31,32に対して回転軸線Lのまわりに相対回転可能になっている。
 図8(c)及び図9に示すように、各半割筒部51bの先端部は、他方のホルダ部材51の大径半円部51dに嵌め込まれている。さらに、一方の半割筒部51bの凹部51gに他方の突起51fが嵌め込まれている。これによって、一対のホルダ部材51,51どうしが、相対回転不能に嵌合されている。また、一対の半割筒部51b,51bの平坦な半割端面51c,51cどうしがぴったり当接している。
 図7(a)に示すように、軸ピン53は、剛性金属にて構成され、細い円柱形状になっている。図9に示すように、この軸ピン53が、回転軸線L上に配置されて、絶縁軸筒52に挿通されるとともに挟持板部51aの穴部51hを貫通している。軸ピン53の中間部は、ホルダ50にて被覆されることによって、第2鉗子部材32から絶縁されている。さらに、軸ピン53の両端部は、第1鉗子部31a,31aの軸穴31h,31hにそれぞれ挿通され、カシメや溶接等によって第1鉗子部31a,31aの基端部に連結されている。したがって、軸ピン53は、第1鉗子部材31によって両持ち状態にて支持されている。
 医療用処置具1は、次のようにして使用される。
 挿入部20を内視鏡のチャンネルに挿入し、このチャンネルの先端から処置部30を生体内に突出させる。そして、スライドツマミ12を操作することによって、操作ワイヤ23を介して処置部30を開閉させて、処置対象部位に宛がうとともに、マイクロ波電源2からマイクロ波を出力することによって、鉗子部材31,32間にマイクロ波(高周波)電場を形成する。これによって、処置対象部位を焼いて止血する等の処置を行なうことができる。鉗子部材31,32の先端に突起部31e,32eを設けることによって、処置部30を処置対象部位に係止し易くなるだけでなく、マイクロ波電場を突起部31e,32eどうし間に特に集中させることができる。
 マイクロ波電源からの電力は、挿入部20の先端部近くまでは同軸ケーブル22によって伝送される。操作ワイヤ23は、先端部分だけが導線としての役目を担っている。これによって、伝送ロスを十分に抑えることができる。したがって、鉗子部材31,32どうし間に形成されるマイクロ波エネルギーを十分に大きくできる。この結果、処置時間を短縮することができる。
 操作ワイヤ23は、専ら処置部30を開閉操作するのに適した鋼材(例えばステンレス)を用いることができるから、処置部30を確実に開閉させることができる。
 医療用処置具1の具体的な使用方法の一例としては、生体組織を第1、第2鉗子部材31,32によって挟んで上記マイクロ波にて凝固止血した後、切断(切除)する。詳しくは、まず、図7(b)の実線にて示すように、処置部30を開くことで、第1、第2鉗子部材31,32の間に生体組織を挟み付ける。開位置では、第2鉗子部材32の先端側部分32fが、第1鉗子部材31に対して斜め外側へ突出し、先端の突起部31e,32eどうしの間が開くため、生体組織を確実に挟むことができる。挟んだ状態で、鉗子部材31,32どうし間にマイクロ波電場を形成することで、生体組織における近接する二箇所の部位が最も焼かれて凝固するようにできる。すなわち、一方の第1鉗子部31aと第2鉗子部材32との間、及び他方の第1鉗子部31aと第2鉗子部材32との間の二箇所において、最も焼かれて凝固するようにできる。
 引き続いて、処置部30を閉操作する。すると、図7(b)のニ点鎖線にて示すように、第2鉗子部材32の先端側部分32fが第1鉗子部31b,31bどうし間の間隙部分39fに入り込む。これによって、上記最も焼かれて凝固した二箇所の間の部位が切断(切開、切除を含む)される。したがって、切断箇所の両側の切断面には、それぞれ上記最も焼かれて凝固した部位が面する。よって、切断面から出血するのを防止でき、又は出血量を僅少に抑えることができる。この結果、凝固や止血を含む治療、手術等の処置を良好に行うことができる。
 更に、医療用処置具1によれば、第1鉗子部材31及び第2鉗子部材32の基端部どうしを回転軸線Lに沿って重ねるように配置することで、処置部30をコンパクトにして小径化できる。
 第1鉗子部材31及び第2鉗子部材32の基端部どうし間には挟持板部51aを介在させることによって、これら第1鉗子部材31及び第2鉗子部材32どうしを確実に絶縁できる。挟持板部51aの直径や厚みを大きくすることによって、沿面距離をかせぐことができ、第1鉗子部材31と第2鉗子部材32との短絡を確実に防ぐことができる。これによって、エネルギーロスを一層抑えることができる。
 また、一対の挟持板部51a,51aを第2鉗子部材32の両側面に宛がうことによって、第2鉗子部材32のねじれを抑制でき、ひいては鉗子部材31,32どうしの短絡を確実に防止することができる。
 さらに、第2鉗子部材32の基端部32dをホルダ50によって保護できる。
 回転軸線L上には金属製の軸ピン53を設けることによって、回転軸としての強度を十分に確保できる。また、一対の鉗子部31a,31aによって軸ピン53の両端部を両持ち状態にすることで、軸ピン53にかかる曲げモーメント等の負荷を軽減できる。
 この軸ピン53を絶縁軸筒52によって囲むことによって、第2鉗子部材32の軸穴32hの内周面と軸ピン53との間を確実に絶縁できる。
 絶縁軸筒52を一対の半割筒部51b,51bに分割しておき、第2鉗子部材32の軸穴32hの内部で組み合わせることによって、一対のホルダ部材51,51どうしを簡単に連結できる。また、例えば一方のホルダ部材51に大径筒を設け、他方のホルダ部材51に小径筒を設け、これら大径筒と小径筒とを嵌め合わせるようにした場合と比べて、これら大径筒及び小径筒の厚みよりも各半割筒部51bの厚みを大きくできる。したがって、絶縁軸筒52の剛性を確保できる。
 また、一対のホルダ部材51を互いに同一形状とすることによって、製造コストを低減できるとともに部品管理を容易化できる。
 次に、本発明の他の実施形態を説明する。以下の実施形態において既述の形態と重複する構成に関しては図面に同一符号を付して説明を省略する。
 図10及び図11は、本発明の第2実施形態を示したものである。なお、図10及び図12では、処置部30が第1実施形態(図1~図9)とは上下逆さまに図示されている。第2実施形態では、一対の第1鉗子部31aの受け面31i及び第2鉗子部材32の受け面32iがそれぞれ鋸刃状になっている。詳しくは、各第1鉗子部31aの受け面31iには、複数の突歯部31k,31k…が形成されている。各突歯部31kは、鉗子部31aの厚み方向(回転軸線Lに沿う方向)から見て、小さい三角形状ないしは山形状になっている。複数の突歯部31k,31k…が、第1鉗子部31aの延び方向に連続して配置されている。
 また、第2実施形態の第2鉗子部材32は、中央部が開方向(図10(b)において上方)へ向かって凸になるように弓形に湾曲している。第2鉗子部材32の受け面32iについても弓形に湾曲している。この受け面32iには、複数の突歯部32k,32k…が形成されている。各突歯部32kは、第2鉗子部材32の厚み方向(回転軸線Lに沿う方向)から見て、小さい三角形状ないしは山形状になっている。複数の突歯部32k,32k…が、第2鉗子部材32の延び方向に連続して配置されている。
 第2実施形態の医療用処置具1を使用する際は、図11(a)及び(d)に示すように、第2鉗子部材32の先端側部分32fを第1鉗子部31a,31a間の間隙部分39fから突出させることで、処置部30を開状態にする。そして、鋸刃状の受け面31i,32iどうしの間に生体組織を挟む。すると、これら受け面31i,32iの各突歯部31k,32kが生体組織に係止される。これによって、生体組織をしっかりと掴むことができ、鉗子部材31,32の滑りを抑制又は防止することができる。
 上記掴んだ状態で、鉗子部材31,32どうし間にマイクロ波電場を形成することで、生体組織における近接する二箇所の部位を焼いて凝固させる。
 続いて、図10(b)及び図11(b)~(c)に示すように、処置部30を閉じていくことで、第2鉗子部材32の先端側部分32fが第1鉗子部31a,31a間の間隙部分39fに入り込む。これによって、生体組織における上記凝固した二箇所の間の部位を切断(切開、切除を含む)することができる。よって、切断面から出血するのを防止でき、又は出血量を僅少に抑えることができる。
 さらに、図11(c)及び(f)に示すように、処置部30を閉方向の限界位置にすると、第2鉗子部材32が第1鉗子部31aとクロスするようになるために、生体組織を確実に切断することができる。この結果、凝固や止血を含む治療、手術等の処置を良好に行うことができる。
 図12は、本発明の第3実施形態を示したものである。第3実施形態では、第1鉗子部31a,31aの受け面31i,31iが平坦になっている。第2鉗子部材32の受け面32iは鋸刃状になっている。
 本発明は、上記実施形態に限定されるものではなく、その趣旨を逸脱しない範囲内において種々の改変をなすことができる。
 例えば、第1鉗子部材31が可動側であり、第2鉗子部材32が固定側であってもよい。或いは、両方の鉗子部材31,32が回転して開閉する両開き式であってもよい。
 第1鉗子部材31が接地極であり、第2鉗子部材32が高圧極であってもよい。
 電源2は、マイクロ波に限られず、ミリ波、ラジオ波、その他の高周波を出力するものであってもよい。
 本発明は、例えば医療用の鉗子に適用可能である。

Claims (4)

  1.  挿入部の先端に設けられるとともに、第1、第2鉗子部材を含んで開閉可能な処置部を備え、かつ前記第1、第2鉗子部材が、互いの間に電場を形成可能な一対の電極を構成する医療用処置具において、
     前記第1鉗子部材が、延び方向と直交する対向方向に対峙する一対の第1鉗子部を含み、
     前記第2鉗子部材が、前記対向方向に沿う回転軸線のまわりに、前記一対の第1鉗子部どうし間の間隙に出入り可能な角度範囲で、前記第1鉗子部材に対して相対回転可能に設けられていることを特徴とする医療用処置具。
  2.  前記第2鉗子部材の基端部が、前記一対の第1鉗子部の基端部どうし間の間隙部分の内部に配置されて、前記第1鉗子部材に対して前記回転軸線のまわりに相対回転可能に連結されており、
     前記第2鉗子部材の前記基端部より先端側部分が、前記一対の第1鉗子部の前記基端部より先端側部分どうし間の間隙部分に出入り可能であることを特徴とする請求項1に記載の医療用処置具。
  3.  前記第1、第2鉗子部材が、前記挿入部に収容された同軸ケーブルを介して、マイクロ波を出力するマイクロ波電源と接続されていることを特徴とする請求項1又は2に記載の医療用処置具。
  4.  医療用処置具の挿入部の先端に設けられるとともに、第1、第2鉗子部材を含んで開閉可能になっており、かつ前記第1、第2鉗子部材が、互いの間に電場を形成可能な一対の電極を構成する処置部において、
     前記第1鉗子部材が、延び方向と直交する対向方向に対峙する一対の第1鉗子部を含み、
     前記第2鉗子部材が、前記対向方向に沿う回転軸線のまわりに、前記一対の第1鉗子部どうし間の間隙に出入り可能な角度範囲で、前記第1鉗子部材に対して相対回転可能に設けられていることを特徴とする医療用処置具の処置部。
